Research progress on immunotherapeutics for triple-negative breast cancer from a single-cell perspective.

Triple-negative breast cancer (TNBC) is a highly heterogeneous subtype with no effective therapeutic targets. Recent advances in immunotherapy, particularly PD-1/PD-L1 inhibitors and CTLA-4 antibodies, have yielded promising clinical outcomes in some patients. Additionally, cell-based therapies and tumor vaccines show substantial potential. Evidence increasingly supports the superior efficacy of combination therapies integrating immunotherapy with antitumor agents over traditional monotherapies. Single-cell and spatial multi-omics technologies, including scRNA-seq and scATAC-seq, as well as sp

Selective activation of pro-anti-CTLA-4 antibody maintains therapeutic efficacy and reduces immune-related adverse events.

Immune checkpoint inhibitors (ICIs) have advanced significantly in immuno-oncology (IO). The first marketed ICI, Ipilimumab, targets cytotoxic T-lymphocyte-associated antigen-4 (CTLA-4) on T cells, promoting effective attack against cancer cells. While effective, this therapy is frequently associated with immune-related adverse events (irAEs) stemming from systemic immune overactivation. In a clinical trial of 131 patients with unresectable melanoma treated with ipilimumab, 61% experienced irAEs, most notably dermatologic, gastrointestinal, endocrine, and hepatic toxicities. To mitigate irAEs

Association of Circulating T Cell and Tumor Microenvironment Profiles with Immune Checkpoint Blockade Outcomes in Sarcoma.

PURPOSE: Immune checkpoint blockade (ICB) benefits only a subset of patients with sarcoma. Biomarkers of response and resistance are needed to help guide patient selection. EXPERIMENTAL DESIGN: We analyzed peripheral blood and tumor samples from patients with sarcoma treated in five ICB-based clinical trials. Baseline peripheral blood mononuclear cells (PBMC) underwent 11-color flow cytometry to define T-cell immunotypes. Baseline tumor tissue underwent RNA sequencing (RNA-seq) to classify tumors into four tumor microenvironment (TME) subtypes using consensus clustering of 29 functional gene

Optimal First-line Immune-related Therapy Selection for Advanced Gastric Cancer: A Systematic Review and Network Meta-analysis.

PURPOSE: Although immune checkpoint inhibitors combined with chemotherapy improve survival in advanced gastric cancer (AGC), comparative evidence across first-line regimens remains insufficient. This study aimed to systematically evaluate their relative efficacy and safety. METHODS: PubMed, Embase, the Cochrane Library, Web of Science, and major conferences were searched up to February 28, 2025. Eligible RCTs comparing ICI-related regimens as first-line AGC treatment were included. Primary outcomes were overall survival (OS) and progression-free survival (PFS). RESULTS: Eleven RCTs involving

Melanoma cell states shape spatial tumor-immune ecosystems to dictate the efficacy of anti-PD1 immunotherapy.

BACKGROUND: Melanoma shows one of the highest response rates to immune checkpoint inhibitors (ICIs), yet nearly half of patients experience primary or acquired resistance. While immune contexture strongly influences therapeutic efficacy, tumor cell-intrinsic features are increasingly recognized as key regulators of antitumor immunity. In particular, intratumoral heterogeneity driven by melanoma cell plasticity underlies diverse immune escape mechanisms. How this plasticity shapes ICI outcomes in patients remains poorly defined. METHODS: Tumor cell states and immune contexture were assessed in

Insight of immune checkpoint blockades in melanoma: mechanism and clinical translation.

Melanoma, a malignant tumor originating from melanocytes, accounts for only 4% of all skin cancers but is responsible for 75% of skin cancer-related deaths. In recent years, its incidence has steadily increased. Targeted therapies and immunotherapies have made significant strides, emerging as the most effective treatments for metastatic melanoma. Immune checkpoint blockades (ICBs) have become a central component of systemic therapy for advanced melanoma. However, their clinical use is limited by immune-related toxicities, primary and acquired resistance, and marked inter-patient heterogeneity

Modulating the tumor immune phenotypes by radiotherapy: formulating and validating the combination therapy of radiation, PD-L1, and TIM-3 blockade in colorectal cancer.

BACKGROUND: Most colorectal cancers (CRCs) are mismatch repair-proficient (pMMR) and microsatellite stable (MSS), and they respond poorly to immune checkpoint inhibitors (ICIs). Radiotherapy (RT) can promote antitumor immunity but may also trigger adaptive immune suppression through checkpoint upregulation, providing a rationale for combination therapies. METHODS: We integrated transcriptomic analyses from The Cancer Genome Atlas and Gene Expression Omnibus cohorts-annotated for microsatellite instability (MSI)/MMR status-with single-cell RNA-sequencing data from the Human Colorectal Cancer A

Presentation and management of tislelizumab induced toxic optic neuropathy.

Immune checkpoint inhibitors (ICIs) have revolutionised the management of advanced malignancies, but can cause immune-related adverse events (irAEs), including vision-threatening complications such as optic neuropathy. Ocular irAEs have been described with several programmed death-1/programmed death-ligand 1 and cytotoxic T lymphocyte-associated antigen-4 inhibitors, but ocular toxicities of tislelizumab have so far been limited to Vogt-Koyanagi-Harada (VKH)-like uveitis, panuveitis and retinal vasculitis. We describe a woman with metastatic lung adenocarcinoma who was treated with second-line

Aging is associated with the outcomes of immune checkpoint blockade in renal cell carcinoma.

Aging causes gradual and significant declines in immune functions and responses. However, the impact of aging on the immunotherapy outcomes in advanced renal cell carcinoma is largely unknown. Here, we conducted a pooled analysis of individual participant data from two regulatory-approved randomized controlled trials: CheckMate 214 and JAVELIN Renal 101. The overall population (n=1926) included 964 individuals treated with immune checkpoint inhibitors (ICIs) and 962 subjects treated with sunitinib. The optimal age to separate young from old was found to be 60. For young patients, immunotherapy

PD-L1 Expression in Prostate Cancer: Anatomopathological Features, Methodological Pitfalls, and Therapeutic Potential.

Programmed death-ligand 1 (PD-L1) has become a central biomarker and therapeutic target across multiple solid tumors, yet its clinical meaning in prostate cancer (PCa) remains unsettled. PCa is commonly described as an immunologically 'cold' malignancy, characterized by limited baseline cytotoxic T-cell infiltration and a tumor microenvironment (TME) shaped by myeloid-driven suppression and low neoantigen load in many cases. Against this background, PD-L1 expression in PCa is typically low in untreated primary tumors but can increase in aggressive variants, advanced stages, and metastatic cast

Characterization of aberrant alternative splicing landscape in patients with metastatic renal cell carcinoma.

PURPOSE: Aberrant alternative splicing (AS) events have been implicated in cancer progression; however, their role in metastatic renal cell carcinoma (mRCC) remains underexplored. This study aims to identify AS events associated with clinical benefits from immune checkpoint inhibitors and targeted therapies in mRCC. MATERIALS AND METHODS: We conducted a retrospective analysis on 101 patients with mRCC who received systemic therapy and underwent RNA sequencing. Patients were divided into subgroups based on ICIs (alone or in combination) and targeted therapies. Responders and non-responders wer

A Rare Case of Early-Onset Steroid-Refractory Fulminant Hepatitis Following Dual Checkpoint Inhibition in Melanoma Initially Presenting With Hemorrhagic Brain Metastasis.

Hemorrhagic central nervous system (CNS) metastases are well recognized in melanoma yet seldom represent the initial manifestation, and fulminant, steroid-refractory hepatitis from immune checkpoint inhibitors (ICIs) remains exceedingly rare. Here, we present a 54-year-old man with no prior medical history who was brought to the emergency department with acute confusion and aphasia and was found to have a large right frontal hemorrhagic mass with midline shift. Urgent craniotomy and hematoma evacuation revealed high-grade melanoma, BRAF V600-negative. Imaging identified stage IV disease with p

Impact on survival of severe immuno-related adverse events in deficient mismatch repair/microsatellite instable-high digestive cancers treated with immunotherapy.

BACKGROUND: Deficient DNA mismatch repair/microsatellite instability-high (dMMR/MSI-H) cancers are very sensitive to immune checkpoint inhibitors (ICIs), yet their use is frequently complicated by immune-related adverse events (irAEs). Our study analyzed the impact of grade ≥3 irAEs (immune-related severe adverse events (irSAEs)) on survival in patients with dMMR/MSI-H digestive cancers treated with ICIs. METHODS: We conducted an international, multicenter, ambispective study involving 1,175 patients from 34 centers. The primary endpoint was the correlation between the occurrence of irSAEs an

Immunotherapy resistance in triple-negative breast cancer: Mechanisms and emerging therapeutic strategies.

Triple-negative breast cancer (TNBC) is an aggressive subtype of breast cancer with limited treatment options and a poor prognosis. Immunotherapies, particularly immune checkpoint inhibitors (ICIs), have expanded the therapeutic options for TNBC. Despite the revolutionary breakthroughs in the treatment of TNBC, some patients still exhibit primary or acquired resistance to immunotherapy. A comprehensive understanding of the mechanisms of immunotherapy resistance is crucial for developing new strategies to overcome the resistance. In this review, we discuss the multifactorial resistance mechanis

Monitoring Early Efficacy of the Checkpoint Immunotherapy Based on Fluorescence Lifetime Imaging Microscopy of the Patient-Derived Lymphocyte Subsets.

Despite the success of immune checkpoint inhibitor (ICI) therapy, the problem of resistance makes the search for early response biomarkers critical. This study investigates the potential of monitoring autofluorescence of the NAD(P)H coenzyme in the T-lymphocyte subsets to assess early treatment efficacy. The ability of NAD(P)H FLIM to reflect sensitivity to ICI therapy was tested on the B16F0 mouse melanoma model and validated in the pilot study of the samples from melanoma patients during treatment. The mean fluorescence lifetime of NAD(P)H, τm, showed the best correlation with subsequent cli

Immunotherapy plus chemotherapy in advanced biliary tract cancers: pros and cons.

INTRODUCTION: Biliary tract cancers (BTC) are highly immunosuppressive 'cold' tumors with limited treatment options. In unresectable or advanced-stage disease, the combination of immune checkpoint inhibition with chemotherapy resulted in a modest survival benefit over chemotherapy. AREAS COVERED: In this review, current obstacles and strategies to improve the benefit of immunotherapy are highlighted. This includes challenges in optimizing patient selection, understanding the tumor microenvironment and interactions with the gut microbiome, and improving combination treatment strategies, whethe

Mesothelioma location influences the tumour microenvironment and immune checkpoint therapy response in preclinical models.

Mesothelioma is a cancer derived from mesothelial cells, most commonly arising from the pleura or the peritoneum. Immune checkpoint therapy (ICT) has shown survival benefit for pleural mesothelioma, but little is known about the response in peritoneal mesothelioma. Most preclinical mesothelioma models involve subcutaneous cancer cell implantation, which lacks the relevant tumour microenvironment of peritoneal mesothelioma and does not resemble the clinical presentation. We therefore set out to explore the influence of location on the mesothelioma tumour microenvironment, comparing pleural, per

Dermatomyositis predominates among checkpoint inhibitor-associated inflammatory myositis phenotypes: a pharmacovigilance disproportionality analysis identifies a class-wide signal.

ICI-induced inflammatory myositis (IIM) is a recognized irAE with significant morbidity and potential for life-threatening overlap syndromes. Whether polymyositis (PM) and dermatomyositis (DM) carry differential pharmacovigilance signals across ICI drug classes has not been systematically examined. A disproportionality analysis of FAERS was conducted to quantify reporting odds ratios across twelve approved ICI agents. Adverse event reports were extracted from FAERS through March 2026 using OpenVigil 2.1. RORs with 95% CIs were calculated for twelve ICI agents spanning PD-1, PD-L1, CTLA-4, and
