The purpose of this study was to determine the indications for contemporary revision hip surgery in a consecutive series of patients. We retrospectively reviewed the clinical records and radiographs of 439 revision hip surgeries done between 1996 and 2003. Fifty-five percent of the surgeries were for aseptic loosening, 14% were for instability, 13% were for osteolysis around a well-fixed implant, 7% were for infection, 5% were for periprosthetic fracture, 3% were for conversion of a hemiarthroplasty, 1% was for psoas impingement, 1% was for loose recalled implants, and 1% was for implant fract

INTRODUCTION: The optimal bearing surface for the young, active patient undergoing total hip arthroplasty (THA) is unknown. We sought to examine the clinical results of ceramic-on-ceramic (CoC) THA in patients under 50 years of age with a diagnosis of osteonecrosis.
MATERIALS AND METHODS: Fifty-three hips underwent cementless THA with a CoC articulation. Clinical and functional outcomes, including preoperative and postoperative WOMAC and UCLA scores, were recorded. Serial radiographs were studied for component loosening or evidence of osteolysis. Revision surgery for any reason was the primar

BACKGROUND: First-generation modular titanium fiber-metal-coated acetabular components had high rates of wear, pelvic osteolysis, and liner dissociation. Second-generation components were designed to reduce the incidence of these problems but it is unclear whether the changes achieved these goals.
QUESTIONS/PURPOSES: We asked the following questions: (1) Is the risk of revision surgery for loosening, wear, or liner dissociation low with the second-generation acetabular component? (2) Is the rate of pelvic osteolysis low? (3) Can the liner be exchanged without bone cement?
METHODS: We retrosp

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is a successful treatment option for rotator cuff tear arthropathy (CTA). Since its introduction, the range of indications has been expanded while implant designs have been enhanced. The purpose of this study is to evaluate the long-term clinical, radiographic, and patient-reported outcomes of the Delta Xtend rTSA.
MATERIALS AND METHODS: In this prospective cohort study, 108 consecutive patients who underwent implantation of rTSA using the DePuy Delta Xtend in 2008 were included. Patients were divided into 4 indication groups: CTA (60%),

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) in the presence of significant glenoid bone loss remains a challenge. This study presents preliminary clinical and radiographic outcomes of primary and revision rTSA using a patient-matched, 3-dimensionally printed custom metal glenoid implant to address severe glenoid bone deficiency.
METHODS: Between September 2017 and November 2018, 19 patients with severe glenoid bone deficiency underwent primary (n = 9) or revision rTSA (n = 10) using the Comprehensive Vault Reconstruction System (VRS) (Zimmer Biomet, Warsaw, IN, USA) at a single ins

AIMS: Our intention was to investigate if the highly porous biological fixation surfaces of a new 3D-printed total knee arthroplasty (TKA) achieved adequate fixation of the tibial and patellar components to the underlying bone.
PATIENTS AND METHODS: A total of 29 patients undergoing primary TKA consented to participate in this prospective cohort study. All patients received a highly porous tibial baseplate and metal-backed patella. Patient-reported outcomes measures were recorded and implant migration was assessed using radiostereometric analysis.
RESULTS: Patient function significantly impr

PURPOSE: We initiated a retrospective study on the long-term survival of cemented endoprostheses for bone tumours around the knee to answer the following questions: (1) What was the survival of these patients? (2) What was the overall survival of cemented prostheses around the knee? (3) What types of failures were observed in these reconstructions? (4) Did the survival and complications vary according to the site of the implant? (5) What was the functional result after cemented prosthesis replacement around the knee?

BACKGROUND: Staphylococcus species are major pathogens of peri-prosthetic joint infection (PJI). Coagulase-positive staphylococci and coagulase-negative staphylococci have different intrinsic virulences. However, few studies have specifically compared the clinical manifestations and two-stage revision outcomes of PJI caused by these two species.
METHODS: We retrospectively collected 260 arthroplasty patients who underwent a two-stage revision because of PJI from January 2003 to June 2015 in our institute because of PJI. Sixty-four patients (36 hips and 28 knees) and 23 patients (13 hips and 1

INTRODUCTION: The principle of implanting a calcar-guided short stem consists of an individual alignment alongside the medial calcar providing the ability of reconstructing varus and valgus anatomy in a great variety. However, still, there are broad concerns about the safety of extensive varus and valgus positioning in regard to stability, bony alterations, and periprosthetic fractures.
MATERIALS AND METHODS: 216 total hip arthroplasties using a calcar-guided short stem (optimys, Mathys Ltd.) in 162 patients were included. Depending on postoperative CCD angle, hips were divided into five grou

BACKGROUND: Because Asian populations have different lifestyles, such as squatting and sitting on the floor, from those of Western populations, it is possible that the clinical results and survival rate of unicompartmental knee arthroplasty (UKA) for Asian patients may be different. This study described outcomes of mobile bearing medial UKA for Korean patients.
METHODS: A total of the 164 knees treated with mobile bearing UKAs in 147 patients (14 males and 133 females) were reviewed. The mean follow-up period was 12.1years (range 10.1-14).
RESULTS: The clinical outcomes, such as the Hospital

BACKGROUND: Relative young and more active patients with osteoarthritis (OA) of the isolated medial femorotibial compartment in conjunction with anterior cruciate ligament (ACL) deficiency are difficult to treat. The aim of this study was to explore the early clinical outcomes of combined Oxford unicompartmental knee arthroplasty (UKA) and ACL reconstruction for the patients presenting ACL deficiency and isolated OA of the medial compartment.
METHODS: Twenty-eight patients were included into the study. All patients were treated by combined Oxford UKA and ACL reconstruction. Plain radiographs

PURPOSE: Dislocation is the most frequent complication following revision total hip arthroplasty (THA). Although several risk factors for dislocation in revision THA have been described, many cannot be modified at the time of surgery. Identifying modifiable risk factors for subsequent dislocation after revision THA provides opportunity for orthopedic surgeons to decrease instability.
METHODS: A retrospective analysis of 203 consecutive revision THA procedures performed by a single surgeon with a minimum 2-year follow-up between May 2003 and June 2012 was performed. 2 (1.0%) died and 14 (6.9%)

BACKGROUND: Total hip arthroplasty (THA) in patients with small or abnormal proximal femoral anatomy is challenging as a result of complex anatomic deformities in the hip. It is unclear which stem is the most appropriate for these patients. One possible implant design that may help meet this need is the modified Wagner Cone prosthesis, whose design consists of monoblock cone with splines; however, to our knowledge, no clinical results have been published using this implant.
QUESTIONS/PURPOSES: We evaluated the hip scores and radiographic results (including signs of osseointegration and subsid

Our hypothesis was that a high-flexion rotating-platform posterior stabilized (RP-PS) design could maximize range of motion (ROM) and improve deep-flexion activities. Eighty-seven consecutive patients (109 knees) with high-flexion RP-PS design were prospectively followed up for a minimum of 3 years. Radiographic and clinical outcomes were analyzed using Knee Society Score criteria and Western Ontario and McMaster Universities Osteoarthritis Index. Good to excellent clinical scores were achieved in 96% of the knees. There were no cases of infection, malalignment, loosening, osteolysis, or spino

BACKGROUND: Metallic wires and cables are commonly used in primary and revision THA for fixation of periprosthetic fractures and osteotomies of the greater trochanter. These systems provide secure fixation and high healing rates but fraying, third-body generation, accelerated wear of the bearing surface, and injury to the surgical team remain concerning.
QUESTIONS/PURPOSES: We determined the rate of cable failure, union, and complications associated with a novel, nonmetallic cerclage cable in periprosthetic fracture and osteotomy fixation during THA.
METHODS: We retrospectively reviewed 29 p

Advanced osteonecrosis of the femoral head is increasingly treated with uncemented total hip arthroplasty (THA), particularly in the younger population. While early outcomes appear promising, little is known about the optimum bearing surface in this patient subpopulation. The goal of this study was to evaluate the clinical and radiological outcomes of uncemented ceramic-on-ceramic THA in young adults with osteonecrosis of the femoral head. Twenty-four consecutive patients (24 hips) with osteonecrosis of the femoral head and 24 patients (24 hips) with osteoarthritis were treated with an uncemen

Surgical reconstruction for limb salvage is an acceptable and often preferred treatment for patients who are facing potential amputation. Because of the invasive nature of these surgeries, complications such as infection are not uncommon. Osseous infection at the site of implantable hardware may result in a tremendous consequence to the patient. In an attempt to minimize infection with implantable hardware, a local delivery system involving the use of antimicrobial coating on medical devices has been developed. This system optimizes local administration of antibiotics to the sources of infecti

STUDY DESIGN: Randomized controlled study.
OBJECTIVE: To compare the rates of adverse events associated with disc arthroplasty versus those of anterior cervical discectomy and arthrodesis with allograft and plate.
SUMMARY OF BACKGROUND DATA: Cervical disc arthroplasty as a substitute for fusion has been developed to maintain motion and, theoretically, prevent adjacent segment degeneration. Currently, cervical arthroplasty devices are undergoing clinical testing for safety and efficacy. The evaluation of safety is performed by critical analysis of all adverse occurrences following surgery to

We examined the association between patient-related factors and the risk of initial, short- and long-term implant failure after primary total hip replacement. We used data from the Danish Hip Arthroplasty Registry between 1 January 1995 and 31 December 2002, which gave us a total of 36 984 patients. Separate analyses were carried out for three follow-up periods: 0 to 30 days, 31 days to six months (short term), and six months to 8.6 years after primary total hip replacement (long term). The outcome measure was defined as time to failure, which included re-operation with open surgery for any re
