Digital markers of motor speech impairments in natural speech of patients with ALS-FTD spectrum disorders.

BACKGROUND AND OBJECTIVES: Patients with ALS-FTD spectrum disorders (ALS-FTSD) have mixed motor and cognitive impairments and require valid and quantitative assessment tools to support diagnosis and tracking of bulbar motor disease. This study aimed to validate a novel automated digital speech tool that analyzes vowel acoustics from natural, connected speech as a marker for impaired articulation due to bulbar motor disease in ALS-FTSD. METHODS: We used an automatic algorithm called Forced Alignment Vowel Extraction (FAVE) to detect spoken vowels and extract vowel acoustics from 1 minute audio

Visual feedback therapy for restoration of upper limb function of stroke patients.

OBJECTIVE: To investigate the effects of mirror neuron theory-based visual feedback therapy (VFT) on restoration of upper limb function of stroke patients and motor-related cortical function using functional magnetic resonance imaging (fMRI). METHODS: Hemiplegic stroke patients were randomly divided into two groups: a VFT group and a control (CTL) group. Sixteen patients in the VFT group received conventional rehabilitation (CR) and VFT for 8 weeks, while 15 patients in the CTL group received only CR. The Barthel Index (BI) was used to assess the activities of daily living at baseline and the

Cross-sectional and longitudinal assessment of the upper cervical spinal cord in motor neuron disease.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive neuromuscular disease characterized by both upper and lower motor neuron degeneration. While neuroimaging studies of the brain can detect upper motor neuron degeneration, these brain MRI scans also include the upper part of the cervical spinal cord, which offers the possibility to expand the focus also towards lower motor neuron degeneration. Here, we set out to investigate cross-sectional and longitudinal disease effects in the upper cervical spinal cord in patients with ALS, progressive muscular atrophy (PMA: primarily lower mo

Evaluating the Effectiveness and Safety of the Electroencephalogram-Based Brain-Machine Interface Rehabilitation System for Patients With Severe Hemiparetic Stroke: Protocol for a Randomized Controlled Trial (BEST-BRAIN Trial).

BACKGROUND: We developed a brain-machine interface (BMI) system for poststroke patients with severe hemiplegia to detect event-related desynchronization (ERD) on scalp electroencephalogram (EEG) and to operate a motor-driven hand orthosis combined with neuromuscular electrical stimulation. ERD arises when the excitability of the ipsi-lesional sensorimotor cortex increases. OBJECTIVE: The aim of this study was to evaluate our hypothesis that motor training using this BMI system could improve severe hemiparesis that is resistant to improvement by conventional rehabilitation. We, therefore, plan

Molecular and cellular processes disrupted in the early postnatal Down syndrome prefrontal cortex.

Down syndrome is a genetic condition that causes intellectual disability and is characterized by early-onset delays in motor, cognitive, and language development. The molecular mechanisms underlying these neurodevelopmental impairments remain poorly understood. We used single-nucleus multiomic sequencing to simultaneously profile gene expression and chromatin accessibility in the Down syndrome prefrontal cortex during early postnatal development, a critical period for synaptogenesis, neural maturation, and developmental neuroimmune interactions. Our findings reveal widespread dysregulation of

Sculptors of cerebellar fissures and their potential as therapeutic targets for cerebellar dysfunction.

The cerebellum plays an important role in both motor control and cognition. The cerebellar cortex is neuron-rich and composed of characteristic folia and fissures. Defective cerebellar development leads to movement disorders and developmental delay. During early morphogenesis, cellular signaling programs orchestrate simultaneous cerebellar growth and foliation. Aberrant signaling causes various degrees of cerebellar hypoplasia. Based on mouse genetic studies, we discuss several developmental signaling pathways that drive cerebellar morphogenesis. Notably, hypoplasia of vermal lobules VI-VII ha

Concussed Neural Signature is Substantially Different than Fatigue Neural Signature in Non-concussed Controls.

Traumatic brain injuries can result in short-lived and long-lasting neurological impairment. Identifying the correct recovery timeframe is challenging, as balance-based metrics may be negatively impacted if testing is performed soon after exercise. Thirty-two healthy controls and seventeen concussed individuals performed a series of balance challenges, including virtual reality optical flow perturbation. The control group completed a backpacking protocol to induce moderate fatigue. Concussed participants had lower spectral power in the motor cortex and central sulcus when compared to fatigued

The Effects of Age-Related Hearing Loss on the Brain and Cognitive Function.

Age-related hearing loss (ARHL) is a common problem for older adults, leading to communication difficulties, isolation, and cognitive decline. Recently, hearing loss has been identified as potentially the most modifiable risk factor for dementia. Listening in challenging situations, or when the auditory system is damaged, strains cortical resources, and this may change how the brain responds to cognitively demanding situations more generally. We review the effects of ARHL on brain areas involved in speech perception, from the auditory cortex, through attentional networks, to the motor system.

Transcranial magnetic stimulation in clinical practice.

Transcranial magnetic stimulation allows a non-invasive and painless stimulation of the human brain and cranial nerves. The method is in use since 1985. Transcranial magnetic stimulation can use single stimuli, pairs of stimuli separated by different intervals (to the same or to several brain areas), or trains of repetitive stimuli at various frequencies. Single stimuli give rise to motor evoked potentials that have clinical use and serve diagnostic and prognostic purposes. Repetitive transcranial magnetic stimulation can modify excitability of cerebral cortex. Repetitive transcranial magnetic

Microcephaly with agenesis of corticospinal tracts and arthrogryposis, hypospadias, single umbilical artery, hypertelorism, and renal and adrenal hypoplasia--previously undescribed syndrome.

We describe a small, term, male infant with corticospinal tract aplasia secondary to motor cortex dysplasia from a neuronal proliferation and/or migrational defect. The infant also had microdolichocephaly, sloping forehead, hypertelorism, flat nose, apparently low-set ears, micrognathia, arthrogryposis without muscle wasting, cortical thumbs, rocker-bottom feet, scoliosis, single umbilical artery, and hypospadias with chordee. Oligohydramnios was present prenatally. Neurologic examination showed a comatose state, seizures, minimal spontaneous movement, minimal response to pain, and absent prim

[Neuropsychological findings of functional cortical mapping in patients with brain tumors].

In 3 patients with brain tumors involving language areas, we investigated functional cortical regions and epileptogenic foci using subdural electrode stimulation before operation. These stimulation studies were useful in preserving postoperatively their language function. The following neuropsychological findings were obtained; (1) Stimulation of Broca area (Brodmann 44 and 45) did not produce anarthria (phonetic disintegration), but semantic and phonemic parahasias and word finding difficulties. (2) Stimulation of lower motor cortex elicited anarthria. (3) Buccofacial apraxia was not produced

Nicotine-sensitive paresis.

Immediately after a patient with myoclonus epilepsy smoked a nicotine-containing cigarette, tetraparesis and hyperreflexia with ankle clonus developed, but disappeared within several minutes. During paresis, the H-reflex size of the soleus muscle increased, EEG showed more slow waves than before smoking, and the cerebral perfusion increased around the motor cortex as shown by single photon emission CT. A similar effect occurred when the patient chewed nicotine gum, and smoking a cigarette with a high nicotine content induced severe positive and negative myoclonus after the development of tetra

Functional neuroanatomy of musical object processing in Alzheimer's disease and frontotemporal dementia.

Music, besides its emotional and social resonance, models a complex sensory environment exemplifying auditory objects corresponding to sources (musical instruments) and information streams (melodies). These musical dimensions can be variably preserved or blighted by neurodegenerative disease. Music is, therefore, an attractive way to investigate the neural mechanisms of sensory object processing in these diseases. Here, we assessed the functional neuroanatomy underlying the perceptual, semantic, and apperceptive processing of musical objects in Alzheimer's disease and temporal variant frontote

Decreased gamma band power and increased betagamma phaseamplitude coupling are characteristic of brain activity in patients with chronic spinal cord injury.

Neurophysiological biomarkers are needed to characterize the condition of patients with spinal cord injury (SCI), for which effective symptomatic biomarkers are lacking. We recorded the resting-state magnetoencephalography data of 22 patients with SCI and 29 healthy controls. Power spectral density and phase-amplitude coupling (PAC) were assessed for six frequency bands using source-reconstructed cortical currents. Compared with controls, SCI patients exhibited significantly reduced gamma band power and increased beta-gamma PAC in the frontal cortex, including the primary motor area (q < 0.05,

Bilateral lingual epilepsy following traumatic brain injury.

Lingual seizures are an uncommon form of focal motor epilepsy. Although classically associated with infective, neoplastic or vascular causes, it may follow traumatic brain injury. A 37-year-old man had sustained a severe brain injury when aged 2 years. Following a generalised tonic-clonic seizure at the age of 9 years, he subsequently had brief and recurrent episodes of bilateral lingual twitching that persisted into adulthood. These events were refractory to carbamazepine, lacosamide and valproate. MR brain imaging showed cystic encephalomalacia of the left frontal operculum. Electroencephalo

Single-cell transcriptomic landscape of the neuroimmune compartment in amyotrophic lateral sclerosis brain and spinal cord.

Development of therapeutic approaches that target specific microglia responses in amyotrophic lateral sclerosis (ALS) is crucial due to the involvement of microglia in ALS progression. Our study identifies the predominant microglia subset in human ALS primary motor cortex and spinal cord as an undifferentiated phenotype with dysregulated respiratory electron transport. Moreover, we find that the interferon response microglia subset is enriched in donors with aggressive disease progression, while a previously described potentially protective microglia phenotype is depleted in ALS. Additionally,

A mouse model for cerebral/cortical visual impairment (CVI) impairs vision and disrupts the spatial frequency tuning of neurons in visual cortex.

Cerebral/cortical visual impairment (CVI) is a visual disorder associated with perinatal hypoxic injury. The pathophysiology of CVI is poorly understood in part because of the lack of an animal model. Here we developed a murine model of CVI from existing rodent early postnatal hypoxia models for periventricular leukomalacia. Exposure to hypoxia during the equivalent to the human third trimester did not perturb motor function but caused severe impairments in binocular depth perception and visual acuity. Impaired vision was associated with normal retinal function, but reduced size of the visual

Mental rotation ability and spontaneous brain activity: a magnetoencephalography study.

We performed experiments using magnetoencephalography to clarify the relationship between three-dimensional visuospatial abilities and spontaneous visual thinking characteristics. Subjects were divided into two groups based on the rate of correct answers to mental rotation tasks: those with good performances (Group G) and those with bad performances (Group B). We found the followings: (1) in the mental rotation tasks, the 25-35 Hz lower γ band activities in the superior parietal lobule/intraparietal sulcus regions and in the occipitotemporal region were significantly larger in Group G than in

Amblyopia: neural basis and therapeutic approaches.

Abnormalities in visual processing caused by visual deprivation or abnormal binocular interaction may induce amblyopia, which is characterized by reduced visual acuity. Occlusion therapy, the conventional treatment, requires special attention as occlusion of the fellow normal eye may reduce its visual acuity and impair binocular vision. Besides recovering visual acuity, some researchers have recommended restoration of stereoacuity and motor fusion and reverse suppression in order to prevent diplopia. Recent studies have documented that the amblyopic visual cortex has a normal complement of cel
