Preparing surgery for Generation Z - Motivation and protective factors for pursuing a surgical career among medical students and early-career physicians: a cross-sectional study.

BACKGROUND: Surgery is facing a substantial shortage of young professionals. In particular, it has become increasingly difficult to attract medical students, especially women, to pursue a surgical career, although women currently represent the majority of medical students. The specialty is often associated with a high workload, hierarchical leadership structures, and limited compatibility between professional and private life. Against this background, the aim of this study was to identify the factors that motivate medical students, early-career physicians and board-certified surgeons to pursue

Intraoperative complications and prevention strategies in robot-assisted breast surgery.

Robot-assisted breast surgery (RAS) improves cosmetic outcomes, but poses anatomical challenges-such as limited working space and lack of tactile feedback-that may increase intraoperative complication risk. To investigate the types and frequencies of intraoperative complications in RAS and assess preventive strategies employed by robotic breast surgeons based on their clinical experience, this Web-based survey was distributed to members of the Korea Robot-Endoscopy Minimal Access Breast Surgery Study Group. Twenty-six breast surgeons actively performing RAS responded to the questionnaire. The

When Benchmarks Fail Us: A Case Study in Cytoreductive Surgery.

INTRODUCTION: Relative Value Units (RVUs) are utilized to measure physician work effort and create national benchmarks. Physicians are often measured against national benchmarks to determine compensation. Using a case study in cytoreductive surgery, we explored variability in coding that can impact national benchmarks. METHODS: A survey was conducted amongst surgeons in the peritoneal surface malignancies consortium (PSM). Data was collected on clinical experience, clinical full time equivalent, wRVUS and institutional coding practice. RESULTS: Coding of the same procedure resulted in signif

Pregnancy and infertility in orthopedics: A review of the current state.

BACKGROUND: Orthopedic surgery continues to have one of the lowest rates of female trainees among all medical specialties in the United States. Barriers to pursuing a surgical residency include the challenges of family planning and work-life balance during training. METHODS: A systematic literature search of articles published between June 2012 and December 2022 in the MEDLINE, EMBASE, and Cochrane databases was performed in January 2023 according to PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis). Studies were included if they evaluated pregnancy and peripartum ex

Robotic assisted surgery reduces ergonomic risk during minimally invasive colorectal resection: the VOLCANO randomised controlled trial.

PURPOSE: Minimally invasive surgery benefits patients but poor operating ergonomics causes musculoskeletal injuries in surgeons. This randomised controlled trial aims to assess whether robotic-assisted surgery with the open-console Versius® system can reduce surgeons' ergonomic risks during major colorectal resections. METHODS: Prospectively registered at ClinicalTrials.gov (NCT05262296) in March 2022. Adult patients requiring a minimally invasive colorectal resection were potentially eligible. Photographs taken at 2-min intervals were analysed using the objective Rapid Entire Body Assessment

Overcoming a Hostile Work and Learning Environment in Academic Surgery-Tools for Change at Every Level.

Mistreatment has been documented as a negative factor in the learning environment for the past 30 y but little progress has been made to determine an effective way to significantly improve these interactions. Faculty may also be victims of a hostile work environment as well, although frequency has not been well-measured or reported. In fact, it may be difficult to identify and address mistreatment and hostility in the work place within the commonly established surgical culture. Thus, efforts to define, identify, and address workplace mistreatment or hostility are crucial to the success of the

Surgical Treatment Strategy and Perioperative Analysis of Patients With Intracranial Mirror Aneurysms.

OBJECT: Intracranial mirror aneurysm is a type of multiple aneurysms, which is rare in clinical and literature. There is no unified conclusion on the treatment strategy of mirror aneurysms. Therefore, our study evaluated the surgical approach and prognosis of a series of mirror aneurysms, and explored the clinical features and treatment strategies of mirror aneurysms. METHODS: This study retrospectively analyzed the clinical features, imaging data, treatment methods and post-hospital results of 67 cases of mirror aneurysms admitted to our department from December 1997 to April 2018. RESULTS:

Tissue diagnosis during colorectal cancer surgery using optical sensing: an in vivo study.

BACKGROUND: In colorectal cancer surgery there is a delicate balance between complete removal of the tumor and sparing as much healthy tissue as possible. Especially in rectal cancer, intraoperative tissue recognition could be of great benefit in preventing positive resection margins and sparing as much healthy tissue as possible. To better guide the surgeon, we evaluated the accuracy of diffuse reflectance spectroscopy (DRS) for tissue characterization during colorectal cancer surgery and determined the added value of DRS when compared to clinical judgement. METHODS: DRS spectra were obtaine

Robot-assisted microsurgical vasovasostomy: the learning curve for a pure microsurgeon.

Vasovasostomy success rates improved with the application of the operative microscope in 1975. The robotic platform offers potential advantages including: a stable, ergonomic, scalable control system with three-dimensional visualization and magnification, the elimination of physiological tremor, and simultaneous control of three instruments and a camera. A previous publication revealed a fellowship-trained microsurgeon (PKK) could transition to robot-assisted microsurgical vasovasostomy (RAVV) with comparable outcomes. The objective of this current study was to evaluate the learning curve for

Locating the ulnar nerve during elbow arthroscopy using palpation is only accurate proximal to the medial epicondyle.

PURPOSE: Knowledge of ulnar nerve position is of utmost importance to avoid iatrogenic injury in elbow arthroscopy. The aim of this study was to determine how accurate surgeons are in locating the ulnar nerve after fluid extravasation has already occurred, and basing their localization solely on palpation of anatomical landmarks. METHODS: Seven cadaveric elbows were used and seven experienced surgeons in elbow arthroscopy participated. An arthroscopic setting was simulated and fluids were pumped into the joint from the posterior compartment for 15 min. For each cadaveric elbow, one surgeon wa

Risk preferences and attitudes to surgery in decision making.

Background and purpose - There is increasing evidence that several commonly performed surgical procedures provide little advantage over nonoperative treatment, suggesting that doctors may sometimes be inappropriately optimistic about surgical benefit when suggesting treatment for individual patients. We investigated whether attitudes to risk influenced the choice of operative treatment and nonoperative treatment. Methods - 946 Swedish orthopedic surgeons were invited to participate in an online survey. A radiograph of a 4-fragment proximal humeral fracture was presented together with 5 differe

Randomized controlled trial comparing dynamic simulation with static simulation in trauma.

BACKGROUND: Current general surgery residents have limited exposure to open trauma operative cases. Simulation supplements variable rotation volume and provides experience with critical but rarely performed procedures. Open simulation classically focuses on static models with anatomic accuracy but lacks practicality when hemorrhage control is the lifesaving maneuver. We sought to evaluate whether training on a dynamic simulator, while much less expensive than training on a static cadaver, might be at least as effective in training surgery residents to expeditiously place temporary vascular shu

A multidisciplinary approach to reducing morbidity and operative blood loss during resection of carotid body tumor.

Seven carotid body tumors in six patients were successfully managed using a multimodality approach that included the vascular surgeon, head and neck surgeon and angiographer. Five tumors were managed with preoperative subselective embolization of tumor vessels. Two required vascular reconstruction. The mean operative blood loss was 332 milliliters. All of the patients survived, and the only morbidity was one instance of transient vocal cord paresis. Surgical resection remains the treatment of choice for carotid body tumors. After angiographic embolization, a combined surgical approach by both

From Laparoscopy to Robotics: Navigating the Learning Curve in Colon Cancer Surgery Within a High-Volume East London District General Hospital.

BackgroundRobotic-assisted surgery (RAS) is widely adopted, and understanding the learning curve (LC) is crucial for surgeons self-assessment and credentialling. We characterised the transition from laparoscopy to robotic approach for three experienced surgeons based on analysis of colectomies performed for malignancy.MethodsWe analysed retrospectively all consecutive robotic colon cancer surgeries performed between February 2020 and April 2025. We excluded rectal cancer resections, palliative, multivisceral and beyond TME cases. The cumulative summation (CUSUM) methodology was utilised to gra

An anatomic checklist for accurate staging of grossly invasive thyroid cancer.

OBJECTIVE: The final surgical pathology report follows the patient throughout their cancer journey. For locoregionally advanced cancers, lack of surgeon-pathologist communication can lead to understaging, adversely impacting management. Our study aims to improve the accuracy of staging grossly invasive thyroid cancer by introducing an anatomic checklist, enhancing surgeon-pathologist communication. METHODS: We studied 35 consecutive patients with either gross extrathyroidal or extranodal extension, 29 of whom underwent primary resections requiring AJCC staging. Surgeon A initially only dictat
