Radiographic and clinical outcomes after instrumented reduction and transforaminal lumbar interbody fusion of mid and high-grade isthmic spondylolisthesis.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: The objective of this study is to document and review the surgical technique and the clinical and radiographic outcomes after instrumented reduction and transforaminal lumbar interbody fusion of mid and high-grade adult isthmic spondylolisthesis. SUMMARY OF BACKGROUND DATA: Although high-grade slips make up a distinct minority of all patients afflicted with spondylolisthesis, approximately 2% in most series, the treatment of this small cohort of patients has been the topic of on going, and often heated debate. Recently, there has been a tr

Postoperative deep wound infections in adults after spinal fusion: management with vacuum-assisted wound closure.

OBJECTIVE: Vacuum-assisted wound closure (VAC) exposes the wound bed to negative pressure, resulting in removal of edema fluid, improvement of blood supply, and stimulation of cellular proliferation of reparative granulation tissue. It has been used to treat open wounds in the extremities, open sternal wounds, pressure ulcers, and abdominal wall wounds. This study retrospectively reviewed instrumented spine fusions complicated by surgical wound infection and managed by a protocol including the use of VAC in order to evaluate the efficacy of applying vacuum therapy on patients with deep spine i

Treatment Guideline for Patients with Native Culture-negative Pyogenic Vertebral Osteomyelitis.

BACKGROUND: With the development of evidence-based guidelines for the diagnosis and antibiotic treatment of patients with pyogenic vertebral osteomyelitis, various nonsurgical and surgical treatment strategies have achieved favorable results. However, sufficient administration of appropriate antibiotics is a prerequisite for treatment success, which cannot be guaranteed in patients with culture-negative pyogenic vertebral osteomyelitis. Unfortunately, previous studies on culture-negative pyogenic vertebral osteomyelitis were limited by small patient groups, short follow-up periods, varied trea

Delayed hemothorax in adolescent idiopathic scoliosis 5-year post-spinal fusion.

PURPOSE: Adolescent idiopathic scoliosis (AIS) is commonly managed with posterior spinal fusion (PSF) and thoracic pedicle screw fixation. While vascular complications are typically acute, delayed hemothorax occurring years after surgery is exceedingly rare. This report describes a unique case of delayed hemothorax secondary to thoracic pedicle screw proximity to the descending thoracic aorta. METHODS: An 18-year-old female presented with chest pain and dyspnea 5 years after T4-L3 posterior thoracic fusion for AIS. Computed Tomography Angiography (CTA) revealed a large left pleural effusion,

Perioperative Strategies to Reduce Occupational HIV Exposure in Head and Neck Microsurgical Reconstruction: A Case Report.

Complex head and neck reconstructive surgeries in patients living with human immunodeficiency virus (HIV) present unique risks of occupational bloodborne pathogen exposure. We report the case of a 63-year-old man with well-controlled, long-standing HIV infection who underwent hemiglossectomy and microvascular free flap reconstruction for advanced tongue cancer. He was receiving antiretroviral therapy and had a CD4+ T-cell count of 361 cells/µL-above the threshold for acquired immunodeficiency syndrome (AIDS)-consistent with partial immune recovery. To minimize occupational exposure risk, we im

Transoral resection of a symptomatic odontoid process aneurysmal bone cyst: illustrative case.

BACKGROUND: Aneurysmal bone cysts (ABCs) are slow-growing, expansile bone tumors most often observed in the long bones and lumbar and thoracic spine. Anterior column ABCs of the spine are rare, and few cases have described their surgical management, particularly for lesions with extension into the odontoid process and the bilateral C2 pedicles. In the present case, the authors describe a two-stage strategy for resection of a symptomatic 2.3 × 3.3 × 2.7-cm C2 ABC with cord compression in a 13-year-old patient. OBSERVATIONS: Initial tumor debulking was completed via a transoral approach, and re

Complications of ankle arthroscopy: frequency, prevention, and treatment.

The complication rate of ankle arthroscopy (AA) ranges from 3.5% to 14%. To avoid such complications, it is essential to have a thorough understanding of the anatomy of the ankle, to perform the procedure very carefully and with appropriate instrumentation, and to use a non-invasive distraction technique. The most frequent complications are neurological (cutaneous nerve injuries), which are usually caused by direct injury during arthroscopic portals or by a distracting pin when using an invasive distraction technique. They usually resolve spontaneously within a few months. The iatrogenic forma

Belgian consensus guideline on the management of hemorrhoidal disease.

INTRODUCTION: Hemorrhoidal disease is a common problem that arises when hemorrhoidal structures become engorged and/or prolapse through the anal canal. Both conservative and invasive treatment options are diverse and guidance to their implementation is lacking. METHODS: A Delphi consensus process was used to review current literature and draft relevant statements. These were reconciliated until sufficient agreement was reached. The grade of evidence was determined. These guidelines were based on the published literature up to June 2020. RESULTS: Hemorrhoids are normal structures within the a

Imaging Assessment of the Postoperative Spine: An Updated Pictorial Review of Selected Complications.

Imaging of the postoperative spine requires the identification of several critical points by the radiologist to be written in the medical report: condition of the underlying cortical and cancellous bone, intervertebral disc, and musculoskeletal tissues; location and integrity of surgical implants; evaluation of the success of decompression procedures; delineation of fusion status; and identification of complications. This article presents a pictorial narrative review of the most common findings observed in noninstrumented and instrumented postoperative spines. Complications in the noninstrumen

A Case of Gastric Emphysema: Incidental Findings or Serious Illness.

Gastric emphysema (GE) or gastric pneumatosis is a rare entity defined as air within the gastric wall. Etiologies include pneumothorax, instrumentation, infection, gastric wall ischemia, and mechanical injury. Several theories exist as to how the air disrupts the integrity of the gastric wall. These include a bacterial infection with Clostridium species and other gas-forming aerobic colonic bacilli, instrumentation with direct submucosal gastric wall injury, mechanical injury following increased intra-abdominal pressure, penetrating air through the mediastinum from increased intrapulmonary pre

Use of a cutting stapler to excise a left atrial appendage in minimally invasive cardiac surgery.

PURPOSE: A cutting stapler is a well-established instrument in many surgical fields. However, its efficacy and safety have not been proven yet in resecting a left atrial appendage (LAA) in minimally invasive cardiac surgery (MICS). METHODS: A cutting stapler was used to resect the LAA in 98 consecutive patients who underwent MICS. Of these, 26 patients underwent aortic valve replacement, 72 mitral valve repair/replacement, 25 tricuspid annuloplasty, 7 closure of atrial septal defect, and 26 the Maze procedure (contains overlapping). The ascending aorta was elevated using a retractor, and a 12

Sublaminar fixation versus hooks and pedicle screws in scoliosis surgery for Marfan syndrome.

BACKGROUND: In patients with Marfan syndrome (MFS), surgical correction of spinal deformities with hooks and/or pedicle screws involves a higher rate of complications than in patients with adolescent idiopathic scoliosis. Therefore, sublaminar instrumentation is often a last resort option. This study wants to assess the ability of sublaminar fixation to achieve three-dimensional scoliosis correction and spine stabilization compared with hook and/or pedicle screw systems. METHODS: Twenty-one MFS patients who underwent posterior spinal fusion at a highly specialized medical center in 1995-2017

Amyand's hernia: role of CT for a correct diagnosis.

Residual giant thoracic disc herniations after open decompression are uncommon and present a significant surgical challenge, particularly in the setting of prior instrumentation and distorted anatomy. Here, the authors present a case of salvage full-endoscopic resection of a residual giant, centrally calcified thoracic disc herniation causing persistent spinal cord compression following prior transpedicular decompression and instrumented fusion. A 37-year-old woman presented with persistent thoracic pain, gait disturbance, and myelopathic symptoms after partial improvement from an initial open

Cervical spine fracture through a cervical disc replacement.

INTRODUCTION: We report a rare case of cervical spine trauma through a cervical disc replacement and adjacent multilevel disc fusions. Cervical disc replacement (CDR) is a viable option for the surgical treatment of degenerative disc disease however long term follow up data regarding this operative technique is poor specifically relating to traumatic complications. We know of no previous reports of bilateral cervical pedicle fractures occurring adjacent to CDR and anterior cervical spine instrumentation. PRESENTATION OF CASE: A 46 year-old with a history of C6C7 CDR and C4-6 anterior cervical

The steel pan sign of sigmoid volvulus-A case series.

INTRODUCTION: Signs in radiology are usually based on many common objects or patterns that are easily recognizable in everyday life. The objective behind this association is to aid in the understanding and diagnosis of the disease process. These signs can be seen in different imaging modalities such as plain radiograph and computed tomography. PRESENTATION OF CASE: 4 consecutive cases of sigmoid volvulus presented at our tertiary hospital between January 2016 and June 2017. 2 of these cases were managed surgically and others were managed conservatively. Evaluating the deflection of dexterous continuum manipulators with unevenly distributed compliant joints.

Dexterous continuum manipulators (DCMs) offer great potential for increasing instrument reach in minimally-invasive surgical procedures. We previously designed and fabricated a tendon driven DCM with a large instrument channel and evenly distributed compliant joints for minimally-invasive skull base surgery and the treatment of osteolysis during hip revision surgery. The evenly distributed compliant joints, in some cases, may limit the reach of the DCM during lesion removal. In this paper, we propose a finite element analysis (FEA) method for optimizing the distribution of the compliant joints

Sinus balloon dilators: one surgeon's experience and proposed indications for their use.

From October 2006 through September 2007, balloon sinusotomies were attempted on 89 sinuses in 45 patients with chronic sinus disease. Ninety-eight percent of sinuses were successfully dilated, 3.4% required revision surgery, and one complication (unlikely related to use of the balloon) occurred. Forty-four percent had previous conventional endoscopic sinus surgery (ESS), 87% were hybrid cases (combination of balloon and conventional ESS instruments used), 33% had nasal polyposis, and 1.98 sinuses per patient were dilated. Preoperative Lund-Mackay radiographic sinus-staging scores averaged 12.
