Principal diagnoses in psychiatric outpatients with borderline personality disorder: Implications for screening recommendations.

BACKGROUND: In the Rhode Island Methods to Improve Diagnostic Assessment and Services project, we examined how often borderline personality disorder (BPD) is the principal diagnosis in patients presenting for outpatient psychiatric treatment. To inform clinicians when it is most productive to screen for BPD, we examined the prevalence of BPD in psychiatric outpatients with different principal diagnoses. METHODS: In this study, 3,674 psychiatric outpatients were evaluated with a semi-structured diagnostic interview for DSM-IV BPD. RESULTS: Slightly ≥10% of the sample was diagnosed with BPD (n

Can the GAD-7 be used to create valid, reliable, feasible and actionable outcome-focused paediatric anxiety quality measures for use across specialty and paediatric treatment settings?

INTRODUCTION: Paediatric anxiety disorders are among the most common psychiatric disorders of childhood and adolescence, affecting between 15% and 30% of youth. Rates rose sharply during the COVID-19 pandemic, and untreated anxiety is associated with impaired functioning and increased risk of depression, suicidality and substance use in adulthood. Evidence-based treatments such as cognitive-behavioural therapy and selective serotonin reuptake inhibitors are highly effective yet remain underused, while non-recommended treatments, including benzodiazepines, are often prescribed. Despite the publ

Systemic inflammatory indices, cardiac conduction disorders, and atrial fibrillation: Phenotype-specific associations and incremental discrimination in the China National Health Survey (CNHS).

BACKGROUND: Systemic inflammation may influence cardiac electrophysiology, but evidence on phenotype-specific links with ECG-defined conduction disorders and atrial fibrillation (AF) in China is limited. METHODS: We performed a cross-sectional analysis of 10,303 adults in the China National Health Survey. Nine inflammatory markers (neutrophils, lymphocytes, monocytes, platelets, hsCRP, NLR, SII, SIRI and PIV; log-transformed and standardized) were related to ECG-defined right bundle branch block (RBBB), intraventricular conduction delay (IVCD), left bundle branch block (LBBB), left anterior f

Integration of specialised mental health services in an HIV clinic in a low resource setting.

Neuropsychiatric complications associated with HIV have been extensively studied, given the significant impact they have on the global disease burden. Despite an abundance of research focusing on HIV's influence on mental health, there remains a dearth of investigation into the integration of comprehensive mental health services and the exploration of a broader spectrum of mental health conditions within this population, especially for low-resource settings. This programme aimed to integrate specialised mental health services through the introduction of routine screening, internal referral path

Understanding patient, caregiver, and healthcare provider perspectives of the management of long-chain fatty acid oxidation disorders.

Long-chain fatty acid oxidation disorders (LC-FAODs) are a group of rare, inherited, metabolic disorders that can lead to a wide range of symptoms that predominantly affect organ systems with high energy needs, such as the heart, liver, skeletal muscle, and nervous system. Clinical management primarily consists of close attention to and monitoring of diet and activity and avoidance of prolonged fasting. In addition, patients and caregivers must be alert for signs of life-threatening metabolic decompensation. As a result, LC-FAODs can have significant and wide-ranging impacts on the lives of pa

The association of pre-operative biomarkers of endothelial dysfunction with the risk of post-operative neurocognitive disorders: results from the BioCog study.

INTRODUCTION: Endothelial dysfunction (ED) promotes the development of atherosclerosis, and studies suggest an association with age-related neurocognitive disorders. It is currently unclear whether ED is also associated with the risk of perioperative neurocognitive disorders. METHOD: We included 788 participants aged ≥ 65 years of the BioCog study. Patients were scheduled to undergo elective surgery with expected duration > 60 min. Blood was collected before surgery for measurement of 5 biomarkers of ED: asymmetric and symmetric dimethylarginine (ADMA; SDMA), intercellular and vascular adhesi

Neonatal sex and maternal factors associated with small-for-gestational-age neonates: A nationwide population-based study.

BACKGROUND AND AIMS: Small-for-gestational-age (SGA) newborns have a higher risk of morbidity and mortality. Recognizing the risk factors for SGA helps raise early awareness of the issue and provides valuable insights for both healthcare providers and pregnant women. We aimed to identify determinants of SGA using population-based databases in Taiwan. METHODS: Data were retrieved from the National Health Insurance, Birth Reporting, and Maternal and Child Health databases for this nationwide case-control study. Live births between 20 and 44 weeks of gestation from 2005 to 2014 were enrolled and
