Presence and associations of retinal pigment epithelium and outer retinal atrophy: the Beijing eye study.

The goal of the study was to assess the presence of an incomplete (iRORA) and complete (cRORA) retinal pigment epithelium (RPE) and outer retinal atrophy in a general population and explore their associations with other parameters. Participants of the population-based Beijing Eye Study with age-related macular degeneration (AMD), polypoidal choroidal vasculopathy (PCV) or those without retinal diseases, underwent optical coherence tomography of the macula. Among 5894 eyes, presence of iRORA and cRORA increased from 0/5117 (0%) and 0/5117 (0%) in the normal group and 0/457 (0%) and 0/457 (0%) i

Predictors of Progression to Villous Atrophy in Children with Type 1 Diabetes and Potential Celiac Disease.

BACKGROUND: Children with type 1 diabetes (T1DM) and positive celiac serology but normal duodenal mucosa (potential celiac disease-CeD) often have spontaneous normalization of serology despite a gluten-containing diet. This results in either repeated biopsies or an upfront advice for gluten-free diet (GFD) if TTG is >10ULN. OBJECTIVES: To find the predictors of villous atrophy in children with T1DM and potential CeD. DESIGN: Ambispective observational study: Children with T1DM and potential CeD on a gluten-containing diet were included. Clinical, serological, histological, and immunohistoche

Gastric atrophy and gastric cancer: a meta-analytical assessment of risk and the influence of topographical distribution.

Gastric atrophy is a precursor in the Correa cascade leading to gastric cancer, yet the precise magnitude of risk remains uncertain, particularly in low-incidence Western populations. We performed a systematic review and meta-analysis of observational studies published between 2010 and 2024 to evaluate gastric cancer risk in gastric atrophy, with stratification by anatomical distribution. Ten studies comprising 21 006 patients were included. Pooled analysis demonstrated a markedly increased risk of gastric cancer in gastric atrophy, with a standardised incidence ratio of 20.7 (95% confidence i

Newborn Screening for Spinal Muscular Atrophy in the Republic of Moldova: A Feasibility Study and First Steps.

Spinal muscular atrophy (SMA) is a severe neuromuscular disorder in which presymptomatic treatment substantially improves survival and motor outcomes, yet newborn screening for SMA remains unevenly implemented across Europe, and evidence from lower-resource health systems is needed to guide scale-up. In this study, we assessed the feasibility, diagnostic performance, and public health implications of implementing neonatal SMA screening in the Republic of Moldova within an established national newborn screening framework. A pilot genetic screening program was conducted using dried blood spot (D

Adherence, Persistence, and Safety of Risdiplam in Spinal Muscular Atrophy: A Population-Based Cohort Study.

INTRODUCTION: Spinal muscular atrophy (SMA) is a rare neuromuscular disorder caused by biallelic SMN1 variants, partially modulated by SMN2 copy number. Risdiplam, an oral SMN2 splicing modifier, has demonstrated efficacy in SMA. However, long-term adherence and persistence are key to sustaining benefit. We evaluated real-world adherence, persistence, and safety of risdiplam in a population-based cohort. METHODS: This was a retrospective observational study including all genetically confirmed SMA type 1-3 patients treated with risdiplam in Spain between January 2020 and October 2025. Adherenc

Synaptic alterations are preceding the axonal loss in optic atrophy of Wolfram syndrome mouse model.

BACKGROUND: Wolfram syndrome is a rare autosomal recessive disorder characterized by antibody-negative early-onset diabetes mellitus, optic atrophy, sensorineural hearing loss, arginine-vasopressin deficiency, and progressive neurodegeneration of the brainstem and cerebellum. It is caused primarily by pathogenic variants in the WFS1 gene, which encodes a transmembrane endoplasmic reticulum-resident protein involved in the unfolded protein response and cellular calcium homeostasis. Although multiple rodent models of Wolfram syndrome have been developed and shown to exhibit visual defects, some

Efficacy of acetylcholinesterase inhibitors on reducing hippocampal atrophy rate: a systematic review and meta-analysis.

BACKGROUND: Neurodegenerative diseases (NDs) are conditions characterized by irreversible progressive degeneration to the nervous tissue and are usually associated with cognitive decline and functional deficits, especially in elderly. Acetylcholinesterase inhibitors (AChEIs) like donepezil, rivastigmine, and galantamine are commonly prescribed to alleviate cognitive symptoms associated with NDs. However, their long-term impact on slowing structural brain degeneration, particularly hippocampal atrophy, remains unclear. OBJECTIVE: This systematic review and meta-analysis assess the efficacy of

The Prevalence of Gastric Atrophy in Western and Northern European Populations: A Systematic Review and Meta-Analysis.

BACKGROUND: Gastric atrophy (GA) is a pre-neoplastic condition leading to gastric cancer (GC). Early GA detection is critical for guiding surveillance and preventing advanced GC. Histology is the current gold standard for GA diagnosis, but is considered not cost-effective for routine screening in Western populations. Serological methods offer a potentially affordable alternative. Understanding GA prevalence, symptom impact, and optimal detection strategies in low-risk Western populations is essential before integrating GA screening into GC prevention programs. METHODS: This systematic review

Extensive Macular Atrophy With Pseudodrusen Complicated by Macular Neovascularization in a Japanese Patient: A Case Report.

Recognizing the characteristic vertically oriented atrophy, pseudodrusen distribution, and retinal pigment epithelium-Bruch's membrane separation is critical for distinguishing extensive macular atrophy with a pseudodrusen-like appearance (EMAP) from age-related macular degeneration (AMD). Early identification of neovascular complications and prompt anti-vascular endothelial growth factor therapy can stabilize macular neovascularization (MNV) and help preserve residual vision in this rare retinal disorder. To the best of our knowledge, this is the first reported case of EMAP with MNV in an Asi

Development of a radiomics-based model for diagnosis of multiple system atrophy using multimodal MRI.

INTRODUCTION: Multiple system atrophy (MSA) is a rapidly progressive neuro-degenerative disorder characterized by autonomic dysfunction, levodopa- unresponsive parkinsonism, cerebellar ataxia, and corticospinal tract involvement. Early diagnosis remains challenging due to overlapping clinical manifestations and the absence of reliable biomarkers. This study aimed to develop a radiomics-based diagnostic model using multimodal MRI to improve MSA detection. METHODS: A retrospective cohort of 62 clinically probable MSA patients (per the 2022 Movement Disorder Society criteria), and 73 matched hea

Systemic delivery of anti-sense oligonucleotide targeting a-synuclein for the treatment of multiple system atrophy.

Multiple System Atrophy (MSA) is a rare, sporadic, age-related synucleinopathy characterized by Parkinson-like motor symptoms and ataxia. There is no therapy for MSA other than symptomatic treatment. MSA is characterized pathologically by glial cytoplasmic inclusions (GCI) of a-synuclein (aSyn) occurring in oligodendrocytes leading to loss of myelination in the brain. We recently utilized a peptide-mediated delivery method to systemically transport an anti-sense oligonucleotide (ASO) targeted to aSyn in a mouse model of MSA. We hypothesized that systemic delivery of aSyn ASO by peptide mediate

Real-world analysis of the efficacy and safety of nusinersen in pediatric patients with spinal muscular atrophy.

BACKGROUND: Spinal muscular atrophy (SMA) is a rare neurodegenerative disease that significantly affects multiple systems in children. Nusinersen, the first approved treatment for SMA, enhances SMN protein production by targeting the RNA splicing site of the SMN2 gene, thus improving motor function. However, the high cost of nusinersen treatment raises concerns about its economic feasibility. METHODS: This study retrospectively analyzed clinical data of 42 pediatric SMA patients treated with nusinersen from January 2022 to October 2024 at our hospital. We assessed the efficacy, safety, and ec

Paracetamol and its metabolites in children and adults with spinal muscular atrophy - a population pharmacokinetic model.

AIMS: The aim of the study was to investigate whether differences in paracetamol pharmacokinetics (PK) between spinal muscular atrophy (SMA) patients and healthy controls (HC) could be attributed to specific clinical covariates. METHODS: Nonlinear mixed-effects modelling (NONMEM 7.4) was used to develop a population PK model, explore covariates for paracetamol and its metabolites and perform simulations. RESULTS: With body weight as allometric scaling in the model, SMA disease resulted in a 58% (95% confidence interval [CI]: 20%-130%) increase in the volume of distribution for paracetamol an
