How important is functional status as a predictor of service use by older people?

In studies of older people, it is often assumed that biophysical, or functional, status is the primary determinant of formal service use. This article reports baseline data from a longitudinal study of a community-based, linked random sample of frail elders (n = 635) and their informal caregivers (n = 429) to investigate the relative contribution of social circumstances to the use of community-based formal services. Elder respondents were categorized into three groups defined by their primary source of care: (a) informal only, (b) mixed help with predominantly informal care, (c) mixed help wit

Perinatal mortality in rural India: a strategy for reduction through primary care. I Stillbirths.

In a prospective community based study of the distribution and determinants of stillbirths in a rural area of Maharashtra, India, that was carried out for two years, 3129 singleton and 22 twin births were recorded in a population of 47 000. Of the 3173 babies, 85 singletons and five of the twins were stillborn giving a stillbirth rate of 28.4/1000 births. The causes of stillbirths are analysed and the possibility of reducing the unacceptably high stillbirth rate by adequate training of grassroot level workers in screening pregnant women for detection of "at risk" mothers and their timely refer

Harambee! 2.0: Community resources and resilience factors to leverage for improving HIV testing behaviors among African immigrant communities in Seattle, Washington.

Significant challenges promoting positive HIV testing behaviors among African immigrant communities in the U.S. persist, though existing community resources may be leveraged to improving these behaviors and increasing testing uptake. We conducted 30 key informant interviews and five focus group discussions (n = total 72 participants) among members of the Ethiopian, Somali, and Eritrean communities in Seattle, WA to identify these resources. Our findings highlight the following three main themes for responsive interventions: (1) capitalize on religious leaders and institutions as key facilitato

Community Engagement in the Diagnosis and Control of a Bovine Paralytic Rabies Outbreak in Two Rural Communities of Mexico.

Rabies is a neglected zoonosis with adverse public health effects. We describe the community engagement in containing a bovine paralytic rabies outbreak in two rural communities of Mexico. We carried out a participatory rapid appraisal using different community participation techniques for 2 weeks, including information sharing, community meetings, prioritization of activities, and training. In addition to the animal census and vaccination, necropsy and immunofluorescence tests were performed to diagnose rabies. Cattle mortality during the outbreak of bovine paralytic rabies was 4.5% (15/331);

Assessing disease burden from pathogen release through untreated hospital wastewater to neighboring communities in Dhaka city.

Hospitals in low-resource settings discharge untreated wastewater containing fecal pathogens, posing substantial health risks to surrounding communities. This study aims to model fecal pathogen flows from a government hospital and neighboring communities in Dhaka, Bangladesh, and to quantify associated disease burdens across neighboring communities with varying sanitation conditions. Using a systems modeling approach integrating Quantitative Microbial Risk Assessment (QMRA) and Disability-Adjusted Life Years (DALYs), we estimated health risks from five key pathogens (Vibrio cholerae, Norovirus

Examining fear appeals and information seeking in tuberculosis health education campaigns: an experimental test of the extended parallel processing model with perceived barriers as a moderator.

Tuberculosis (TB) health education campaigns often struggle to motivate at-risk populations by employing one-size-fits-all fear appeals without considering how structural barriers shape audiences' ability to respond. Drawing on the Extended Parallel Processing Model (EPPM), this study tested how varying levels of threat and efficacy in TB messages influence information-seeking intentions, with perceived barriers as a moderator. In a 2 (threat: low versus high) × 2 (efficacy: low versus high) between-subject experiment with 225 Chinese participants, results revealed that high-threat messages in

How to eliminate hepatitis C between people who inject drugs in community services and prisons in Catalonia.

BACKGROUND: The Spanish National Health System, with devolved powers to autonomous communities such as Catalonia, faces significant challenges in controlling viral infections like hepatitis C (HCV) among vulnerable groups, particularly people who inject drugs (PWID), where prisons serve as crucial intervention sites. Catalonia's health authorities have implemented strategies to combat HCV, including direct-acting antiviral (DAA) treatments and harm reduction programmes within both community and penitentiary settings. However, substantial barriers persist in achieving full treatment uptake and

Potential roles of pharmacists in HIV/AIDS care delivery in Nepal: A qualitative study.

BACKGROUND: Nepal is facing escalating infection rates of HIV/AIDS, a major global public health threat. Continuum of services is an identified strategic component of Joint United Nations Programme on HIV/AIDS (UNAIDS) commitment to end this public health crisis by 2030 and achieve the Sustainable Development Goal 6 (SDG 6). Pharmacists are integral members of the continuum of care in HIV/AIDS but the idea is novel to Nepal. Realizing need to explore and identify potential roles of pharmacists in HIV/AIDS care delivery, this study aimed to gain an insight into the views of stakeholders on the

Lived Experience, Research Leadership, and the Transformation of Mental Health Services: Building a Researcher Pipeline.

In recent years, investment in participatory research methods within mental health services research has grown. Participatory efforts are often limited in scope, however, and attention to research leadership is largely absent from discourse about stakeholder involvement in the United States. This Open Forum calls for investment in building a pipeline of researchers with significant psychiatric disabilities and intersecting lived experiences frequently studied in public sector services research, including homelessness, incarceration, comorbid health problems, structural racism, and poverty. Admission and discharge criteria for adolescents requiring inpatient or residential mental health care: a scoping review.

OBJECTIVE: This scoping review sought to locate and describe criteria relating to admission to and discharge from inpatient mental health care for adolescents aged 11 to 19 years in the literature. INTRODUCTION: In the United Kingdom (UK) and internationally, it is estimated that one in 10 children and adolescents has a diagnosable mental health problem. Children and adolescents with the highest levels of need are cared for in hospital, but there is a high demand for beds and a general lack of agreement regarding the criteria for admission to, and discharge from, such units. Stakeholder perspectives on antenatal depression and the potential for psychological intervention in rural Ethiopia: a qualitative study.

BACKGROUND: Psychological interventions for antenatal depression are an integral part of evidence-based care but need to be contextualised for respective sociocultural settings. In this study, we aimed to understand women and healthcare workers' (HCWs) perspectives of antenatal depression, their treatment preferences and potential acceptability and feasibility of psychological interventions in the rural Ethiopian context. METHODS: In-depth interviews were conducted with women who had previously scored above the locally validated cut-off (five or more) on the Patient Health Questionnaire durin

Childrens' and Parents' Willingness to Join a Smartphone-Based Emergency Response Community for Anaphylaxis: Survey.

BACKGROUND: Medical emergencies such as anaphylaxis may require immediate use of emergency medication. Because of the low adherence of chronic patients (ie, carrying anti-anaphylactic medication) and the potentially long response time of emergency medical services (EMSs), alternative approaches to provide immediate first aid are required. A smartphone-based emergency response community (ERC) was established for patients with allergies to enable members to share their automatic adrenaline injector (AAI) with other patients who do not have their AAI at the onset of anaphylactic symptoms. Transitioning across professional boundaries in midwifery models of care: A literature review.

BACKGROUND: High-risk pregnancy, or one with escalating complexities, requires the inclusion of numerous health professions in care provision. A strategy of midwife navigators to facilitate the smooth transition across models of care and service providers has now been in place in Queensland, Australia, for over twelve months, and a formal review process will soon begin. Navigators are experienced nurses or midwives who have the expertise and authority to support childbearing women with chronic or complex problems through the health system so that it is co-ordinated and they can transition to s

Co-occurring Disordered Gambling Among Treatment-Seekers at a Community Outpatient Addiction Clinic.

BACKGROUND: Parallel to the ongoing expansion of legalized gambling activities is a growing concern about rising occurrence of uncontrollable gambling. People with preexisting gambling and/or chemical addictions may be particularly vulnerable, but the extent of such co-occurring conditions and their demographic and clinical characteristics have not been sufficiently elucidated. To that end, the present study attempted to both, quantify the presence and to characterize co-occurring pathological or problem gambling (ie, respectively, at least 1- or at least 5 pathological gambling criteria from
