Update on the genetics of androgenetic alopecia, female pattern hair loss, and alopecia areata: implications for molecular diagnostic testing.

Источник пометил публикацию как отозванную

Androgenetic alopecia, female pattern hair loss, and alopecia areata are among the most common forms of nonscarring hair loss encountered in clinical practice. Although the exact pathogenesis of these forms of alopecia remains to be clarified, genetic factors appear to have a significant contribution to their pathogenesis. Current treatment strategies are limited and their effectiveness remains modest at best. This review summarizes the current purported pathogenesis and recent genetic discoveries relating to these forms of alopecia. The role of molecular diagnostic testing is also discussed i

Metastatic urachal cancer responding to FOLFOX chemotherapy.

Metastatic urachal cancer is a rare disease and subsequently, does not have a defined systemic treatment. Although urachal cancer is most commonly adenocarcinoma and histologically similar to colon cancer, treatment selection is usually based upon location (the proximity of the urachus to the bladder) with bladder cancer regimens the most commonly prescribed. We report a case of metastatic urachal cancer where the immunohistochemical profile's similarities to colon cancer led to treatment with colon cancer specific chemotherapy. Our case is the first to report urachal cancer treated with and r

Radiation Therapy for Meningiomas - Where Do We Stand and What's on the Horizon?

Radiation therapy, including conventionally fractionated external beam radiation therapy, stereotactic radiosurgery, and fractionated stereotactic radiation therapy, is a cornerstone in the interdisciplinary management of meningiomas. Recent advances in radiation oncology and also in other fields, such as neuropathology and imaging, have various implications for meningioma radiation therapy. This review aims to summarize current and anticipated developments, as well as active clinical trials related to the use of radiation therapy for meningiomas. In imaging, positron emission tomography has p

Targeting cell death in NAFLD: mechanisms and targeted therapies.

Nonalcoholic fatty liver disease (NAFLD) is a group of chronic liver disease which ranges from simple steatosis (NAFL) to non-alcoholic steatohepatitis (NASH) and is characterized by lipid accumulation, inflammation activation, fibrosis, and cell death. To date, a number of preclinical studies or clinical trials associated with therapies targeting fatty acid metabolism, inflammatory factors and liver fibrosis are performed to develop effective drugs for NAFLD/NASH. However, few therapies are cell death signaling-targeted even though the various cell death modes are present throughout the progr

Pathobiology-guided diagnostic advances, biomarker discovery, and emerging therapeutic strategies in progressive multiple sclerosis.

Progressive multiple sclerosis (PMS) represents a severe and disabling form of MS characterized by a gradual accumulation of neurological impairment and limited responsiveness to conventional disease-modifying therapies. Unlike the relapsing forms of the disease, progressive MS is driven not only by peripheral immune dysregulation but also by persistent neuroinflammation, microglial activation, mitochondrial dysfunction, and progressive neurodegeneration within the central nervous system. These complex and overlapping pathogenic mechanisms contribute to irreversible axonal loss and pose signif

Molecular pathologies and therapies for Pelizaeus-Merzbacher disease.

Pelizaeus-Merzbacher disease (PMD) is the most common and representative disorder among hypomyelinating leukodystrophies, affecting myelin in the central nervous system. PMD is caused by various mutations in the PLP1 gene, including the most common duplications, point mutations (which often lead to severe forms), deletions/null mutations (resulting in milder forms), and deep intron mutations associated with hypomyelination of early myelinated structures (HEMS), a mild variant with characteristic MRI findings. Each mutation type is known to trigger distinct cellular and molecular mechanisms. Un

Molecular Pathways and Potential Therapeutic Targets of Refractory Asthma.

Asthma is a chronic inflammatory lung disease. Refractory asthma poses a significant challenge in management due to its resistance to standard therapies. Key molecular pathways of refractory asthma include T2 inflammation mediated by Th2 and ILC2 cells, eosinophils, and cytokines including IL-4, IL-5, and IL-13. Additionally, non-T2 mechanisms involving neutrophils, macrophages, IL-1, IL-6, and IL-17 mediate a corticosteroid resistant phenotype. Mediators including alarmins (IL-25, IL-33, TSLP) and OX40L have overlap between T2 and non-T2 inflammation and may signify unique pathways of asthma

Mutational spectrum of dystrophinopathies in Singapore: Insights for genetic diagnosis and precision therapy.

Duchenne and Becker muscular dystrophies (DMD/BMD) are X-linked recessive disorders caused by mutations in the DMD gene. Emerging therapies targeting patients with specific mutations are now becoming a reality for many of these patients. Precise molecular diagnosis is essential to facilitate the identification of possible new treatments for patients in the local context. In this study, we screened 145 dystrophinopathic patients in Singapore and assessed their molecular status for eligibility to current emerging genetic therapies. Overall, 140 (96.5%) of all patients harbored pathogenic DMD mut

Non-communicable inflammatory skin diseases comprise clinically meaningful distinct endotypes as identified by non-hypothesized integration of phenotypic and transcriptome data.

BACKGROUND: Non-communicable inflammatory skin diseases (ncISDs) comprise over 100 conditions with overlapping clinical features. Current classification systems are largely based on morphology and do not adequately reflect underlying molecular mechanisms, limiting the implementation of precision therapies. OBJECTIVE: To establish a data-driven, molecular framework for stratifying ncISDs independent of conventional diagnostic categories. METHODS: We integrated 727 skin transcriptomes with clinical metadata from 390 patients across 22 ncISDs and applied unsupervised and machine-learning approa

Management of Pulmonary Arterial Hypertension: Current Strategies and Future Prospects.

Primary pulmonary hypertension (PPH), now known as pulmonary arterial hypertension (PAH), has induced significant treatment breakthroughs in the past decade. Treatment has focused on improving patient survival and quality of life, and delaying disease progression. Current therapies are categorized based on targeting different pathways known to contribute to PAH, including endothelin receptor antagonists (ERAs), phosphodiesterase-5 inhibitors (PDE-5 inhibitors), prostacyclin analogs, soluble guanylate cyclase stimulators, and activin signaling inhibitors such as Sotatercept. The latest addition

Endometriosis and Endometrial Cancer-Association Between Biological Mechanisms and Its Clinical Implications.

Endometriosis and endometrial cancer are distinct gynecological conditions that share overlapping biological mechanisms with implications for clinical management. Endometriosis is a chronic, benign disorder characterized by the ectopic implantation of functional tissue lining the uterus, primarily affecting women of reproductive age. It commonly causes chronic pelvic pain, dysmenorrhea, dyspareunia, and infertility. The disease is marked by persistent inflammation, hormonal dysregulation, and alterations in cellular signaling, which mirror some neoplastic processes despite lacking malignant po

The Evolution of Treatment-Free Remission.

One of the most remarkable achievements of the TKI era has been the capacity to induce deep molecular remissions that are sustainable off therapy in chronic myeloid leukemia (CML) patients - treatment-free remission (TFR). TFR was first described in a handful of patients within 3-4 years of imatinib approval. In 2004 TFR was tested in a small French pilot study, followed soon after by the French STIM and Australasian TWISTER studies. These early trials demonstrated that TFR was achievable, but also showed that rapid relapse was equally likely. Perhaps the most critical observation was that rel

Detection of EGFR-SEPT14 fusion in cell-free DNA of a patient with advanced gastric cancer: A case report.

BACKGROUND: Gastric cancer is the fifth most diagnosed cancer worldwide and the third most common cause of cancer-related death. In recent decades, increasing application of next-generation sequencing has enabled detection of molecular aberrations, including fusions. In cases where tissue is difficult to obtain, cell-free DNA (cfDNA) is used for detecting mutations to identify the molecular profile of cancer. Here, we report a rare case of EGFR-SEPT14 fusion detected from cfDNA analysis in a patient with gastric cancer. CASE SUMMARY: A 49-year-old female diagnosed with advanced gastric cancer

Integrated computational framework for drug repurposing in metastatic prostate cancer using transcriptomic meta-analysis, machine learning, and molecular modeling.

BACKGROUND: Metastatic prostate cancer (mPCa) continues to be the primary cause of mortality associated with prostate cancer, after considerable progress in targeted therapy. The rise of therapeutic resistance, coupled with the protracted and expensive nature of new drug development, has heightened interest in computational drug repurposing methodologies. This study sought to create a comprehensive computational framework that integrates transcriptome meta-analysis, network biology, machine learning, and molecular modelling to discover FDA-approved medications with potential therapeutic effica

Decoding the Heterogeneity of Diffuse Large B-Cell Lymphomas: A Comprehensive Genetic, Transcriptomic, and Phenotypic Profiling of B-Cell Lymphoma Cell Lines.

Diffuse large B-cell lymphoma (DLBCL) is the most prevalent form of non-Hodgkin lymphoma, exhibiting significant molecular and clinical heterogeneity. Advances in classification integrating phenotypic, genetic, and transcriptomic features have improved diagnosis and prognosis. However, a comprehensive and integrated molecular characterization of DLBCL cell lines is still lacking, which limits their optimal use as reliable experimental models. We employed fluorescence in situ hybridization, immunohistochemistry, and targeted DNA and RNA sequencing to identify genetic subtypes and determine the

Progress towards genetic and pharmacological therapies for keratin genodermatoses: current perspective and future promise.

Hereditary keratin disorders of the skin and its appendages comprise a large group of clinically heterogeneous disfiguring blistering and ichthyotic diseases, primarily characterized by the loss of tissue integrity, blistering and hyperkeratosis in severely affected tissues. Pathogenic mutations in keratins cause these afflictions. Typically, these mutations in concert with characteristic features have formed the basis for improved disease diagnosis, prognosis and most recently therapy development. Examples include epidermolysis bullosa simplex, keratinopathic ichthyosis, pachyonychia congenit

Unmasking Bartonella henselae: A forgotten cause of hepatosplenic and marrow involvement.

Bartonella henselae is an uncommon but important cause of prolonged pyrexia of unknown origin (PUO) and hepatosplenomegaly. Conventional diagnostic tools often fail in disseminated disease, leading to delayed or missed diagnoses. We report a 60-year-old man with PUO, progressive weight loss, and hepatosplenomegaly. Clinical suspicion, guided by significant cat exposure, led to molecular confirmation of B. henselae infection after conventional workup proved inconclusive. Liver biopsy revealed pleomorphic rod-shaped organisms on Warthin-Starry staining, while real-time PCR targeting the rpoB gen

KIF5A downregulation in spinal muscular atrophy links axonal regeneration defects with ALS.

Spinal muscular atrophy (SMA) is a devastating neuromuscular disorder caused by mutations in the Survival Motor Neuron 1 (SMN1) gene, leading to decreased SMN levels and motor neuron dysfunction. SMN-restoring therapies offer clinical benefit, but the downstream molecular consequences of SMN reduction remain incompletely understood. Here, we demonstrate that SMN deficiency results in downregulation of KIF5A in human neurons and in a mouse model of SMA. We provide evidence that reduced SMN levels impair axon regeneration, which is rescued by KIF5A overexpression and that the RNA-binding protein
