Duodenal gastric-type neoplasm with uncertain malignant potential in a lipoma.

The pathologic diagnosis of duodenal tumors is a developing field; however, its overview remains unclear. We describe a rare case of a duodenal gastric-type neoplasm in a 50-year-old woman. She visited her primary care doctor with complaints of upper abdominal pain, tarry stools, and shortness of breath on exertion. She was admitted owing to a stalked polyp with erosion and hemorrhage in the descending part of the duodenum. Endoscopic mucosal resection (EMR) was performed on the polyp. Histologically, the resected polyp was a lipomatous lesion in the submucosal layer, composed of mature adipos

Comment on "Artificial intelligence in gastroenterology: A state-of-the-art review".

Colon capsule endoscopy (CCE) was introduced nearly two decades ago. Initially, it was limited by poor image quality and short battery time, but due to technical improvements, it has become an equal diagnostic alternative to optical colonoscopy (OC). Hastened by the coronavirus disease 2019 pandemic, CCE has been introduced in clinical practice to relieve overburdened endoscopy units and move investigations to out-patient clinics. A wider adoption of CCE would be bolstered by positive patient experience, as it offers a diagnostic investigation that is not inferior to other modalities. The shor

Molecular and biochemical approach for understanding the transition of amorphous to crystalline calcium phosphate deposits in human teeth.

Calcium phosphate (CaP) deposition during bone mineralization starts with the aggregation of Posner's clusters Ca9(PO4)6 into amorphous Ca-phosphate (ACP), which then transforms into crystalline CaP and finally maturates to hydroxyapatite (HA). Using dentin/enamel of human teeth as a model system, we show that the physiological inorganic polymer polyphosphate (polyP), a phosphate donor in mineralization, prevents the transition from amorphous to crystalline CaP at concentrations> 15 wt%. Stabilization of the amorphous phase of CaP by polyP is reversed by hydrolysis of the polymer by alkaline p

Laparoscopy-assisted endoscopic mucosal resection in the colon: a preliminary report.

Endocopic mucosal resection of flat villous tumours or giant polyps (> 3 cm) may give rise to local complications such as haemorrhage or perforation because of the very thin wall of the colon, above all in the right half. Our aim was to evaluate whether laparoscopy-assisted endoscopic excision of flat villous tumours or giant polyps (> 3 cm) can be safely performed, avoiding critical septic complications and can also help in the selection of patients to be submitted to colonic resection without increasing morbidity or mortality. The procedure is a new minimally invasive therapeutic approach in

Peroral sustained-release indomethacin treatment for rectal adenomas in familial adenomatous polyposis: a pilot study.

BACKGROUND/AIMS: The efficacy of peroral sustained-release indomethacin on rectal adenomas in colectomized patients with familial adenomatous polyposis and its toxicity were evaluated preliminarily. METHODOLOGY: A total of seven colectomized patients with familial adenomatous polyposis were treated with peroral sustained-release indomethacin at the usual clinical doses. The numbers of rectal polyps before and after treatment and approximately one year after cessation of treatment were counted using a flexible colonoscope and compared. RESULTS: After treatment, reductions of rectal polyp size

Drinking water and the prevalence of colorectal adenomas: an epidemiologic study in Telemark, Norway.

This study was based on an endoscopic screening study for detection of polyps in the rectum and sigmoid colon in a randomized, normal population sample of 400 individuals aged 50-59 years. Family disposition for cancer and indicators of lifestyle (including dietary registration) were recorded. The 310 individuals received domestic drinking water from one out of four public water supplies. The participants were categorized according to the water supply connected to their house of residence. Drinking water was analysed monthly during 2 years for chloroform, total organic carbon, colour index, ca

Ultrasound-Guided Endometrial Polypectomy Performed in an Office Setting Without Hysteroscopy.

Endometrial polyps are localized overgrowths of the endometrium that protrude into the endometrial cavity. Symptomatic persistent endometrial polyps warrant removal. Hysteroscopic polypectomy is the gold standard for endometrial polyp removal, with office-based procedures gaining popularity in recent years. We report an alternative technique of endometrial polypectomy in the office under transabdominal ultrasound guidance using an atraumatic 5-mm laparoscopic grasper, which is particularly useful when a hysteroscope is unavailable. This procedure could be successfully completed in more than 95

Surgical Treatment of a Severe Electrolyte Imbalance: A Case Report of an Elderly Patient With McKittrick-Wheelock Syndrome.

McKittrick-Wheelock is a rare syndrome characterized by a severe, difficult to correct electrolyte imbalance, chronic mucus diarrhea, and a large rectal polyp. In this case report, we describe an elderly patient diagnosed with a large nonmalignant rectal polyp during a routine colonoscopy 10 years prior to admission. For years, the patient has suffered from diarrhea, causing episodic life-threatening hypokalemia and hyponatremia with several unsuccessful attempts at endoscopic polyp removal. Due to symptomatic cholelithiasis, the patient was transferred to the surgical ward and diagnosed with

Progressive dysphagia from an esophageal liposarcoma, a complex problem in a challenging setting.

Esophageal lipomatous tumors are extremely rare, and due to their nonspecific clinical manifestations, distinguishing them is nearly impossible without the aid of histopathology, immunohistochemistry, and molecular analysis. Complete resection with clear margins is the treatment of choice to avert metastasis, improve prognosis, and prevent complex complications due to the polyp growth and location. We present the case of a 70-year-old male who presented in 2023 with dysphagia due to an esophageal polyp; surgery was recommended. However, he did not accept any treatment due to fear. One year lat

Low-grade appendiceal mucinous neoplasm penetrating sigmoid colon: A case report.

Low-grade appendiceal mucinous neoplasm (LAMN) is principally characterized by low-grade cytology without evidence of invasion to other organs. We report a LAMN surgical case whose appendiceal tumor penetrated the sigmoid colon wall. An 87-year-old man was referred for endoscopic resection (ER) of a colon polyp. Despite four ERs over 5 years, the polyp recurred at the same site. Laparoscopic surgery revealed a dilated appendix firmly attached to the sigmoid colon. We performed en bloc resection of both the sigmoid colon and appendix without tumor exposure. The histopathological evaluation show

Use of the full-thickness resection device (FTRD) to prevent bleeding after colonic polypectomy in a patient with a left ventricular assist device.

Background and study aims  Treatment of colorectal polyps in patients undergoing permanent anticoagulation is associated with a significantly increased risk of peri-interventional bleeding. This specifically applies to polyps > 1 cm where endoscopic full-thickness resection (EFTR) with the full-thickness-resection device (FTRD) can be taken into consideration as an alternative approach to classical snare polypectomy. For these indications, EFTR appears to be superior to conventional techniques especially in terms of sufficient hemostasis due to the mechanical tissue compression achieved by the

Jejunal intussusception caused by a huge Vanek's tumor: a case report.

Inflammatory fibroid polyp (known also as Vanek's tumor) is a type of localized, non-neoplastic inflammatory pseudotumor or inflammatory myofibroblastic tumor that occurs most commonly in the stomach but also in the small and large bowel. It is a documented cause of intussusception in adults. We report a case of a 40-year-old woman who presented with severe, postprandial abdominal pain followed by projectile vomiting over a period of three days. Ultrasonography demonstrated a solid and echogenic mass surrounded by the typical mural layers of an invaginated jejunum. She underwent urgent laparot

A case of polypoid cavernous hemangioma of the sigmoid colon excised by colonoscopic polypectomy.

A 28-year-old Japanese man was admitted to our institute because of sharp anal pain and a mass that prolapsed from the anus after defecation. After spinal anesthesia for emergency operation, the mass spontaneously withdrew into the anus and the pain disappeared. Surgery was postponed. Barium enema and colonoscopy revealed a pedunculated polyp in the sigmoid colon, which was removed by snare polypectomy with electrocautery. The resected polyp was granular and reddish, and measured 33 x 22 x 14 mm. Histological examination of the polyp revealed a cavernous hemangioma. Hemangioma of the colon is

[Validity of hysteroscopy in clinical setting: single centre analysis of 605 consecutive hysteroscopies].

OBJECTIVE: Hysteroscopy represent standard diagnostic and therapeutic method in the treatment of endometrial pathology, where patient selection for this procedure depends in majority on preoperative uterine ultrasound scan. Hysteroscopy can be used for removal of polyps or myomas, endometrial tumor resection, synechiolysis, sterilisation or removal of remnants from pregnancy. Hysteroscopic surgery can be also an option for patients who wish to preserve the uterus for the treatment of recurrent bleeding. We aimed to evaluate the validity, complication rate and accuracy of hysteroscopy in correl

Gastric polypoid intramucosal carcinoma and an adjacently located leiomyoma at the cardia.

We report a 65-year-old patient with a gastric polyp of 2.5 cm in diameter located at the cardia on upper gastrointestinal (GI) endoscopy. Pathological examination of the excised polyp showed intramucosal carcinoma. Endoscopic ultrasonography (EUS) reported the lesion as early gastric carcinoma with probable submucosal involvement. On serial sections of the gastrectomy material, the lesion was an intramucosal carcinoma and surprisingly there was a leiomyoma located adjacently.

[Diagnosis and treatment of eosinophilic rhinosinusitis].

Chronic rhinosinusitis (CRS), a chronic inflammation of the nasal sinus mucosa, is based on a simplified classification of a single clinical phenotype (with or without nasal polyps) that does not adequately reflect the heterogeneity of the pathogenesis of chronic rhinosinusitis complexity. Currently, according to the lesion mucosa or polyps eosinophil infiltration, this type of chronic rhinosinusit is known as eosinophilic chronic rhinosinusitis (ECRS). The curative effect of ECRS is poor than non-eosinophilic rhinosinusitis. This article summarizing the diagnosis and treatment of eosinophilic

Peutz-Jeghers syndrome and family survey: a case report.

Peutz-Jeghers syndrome (PJS) is a rare inherited autosomal dominant disease characterized by mucocutaneous pigmentation and multiple polyps in the gastrointestinal tract. We report on an 18-year-old Chinese male who complained with pigmentation on face and extremities for over 10 years. Colonoscopy revealed more than ten polyps from transverse colon to rectum. The family survey included 15 family members from three generations, and 6 PJSs (4 males and 2 females) were found. This case is reported because of its rarity of Peutz-Jeghers syndrome and the survey of family history.
