Shoulder arthrodesis for treatment of flail shoulder in children with polio.

BACKGROUND: Poliomyelitis in children can cause paralysis of shoulder girdle muscles leading to a flail shoulder. Shoulder arthrodesis is indicated as a possible treatment for these children in order to stabilize the shoulder. This retrospective study reviewed all shoulder arthrodesis surgeries owing to complications of polio performed at a major medical institution between 1981 and 1996 to assess position of fusion, radiographic evidence of fusion, complications, and patient satisfaction. METHODS: A review of medical records identified 11 patients undergoing 13 shoulder arthrodesis procedure

Procedure-related readmissions following endoscopic retrograde cholangiopancreatography in a liver transplant cohort.

BACKGROUND AND AIM: Data on post-endoscopic retrograde cholangiopancreatography (ERCP) adverse events and readmission rates in liver transplantation (LT) patients remain scarce. This study determined the 30-day procedure-related readmission rate following ERCP in an LT cohort at an Australian tertiary academic center. METHODS: All unplanned readmissions within 30 days following ERCP in orthotopic LT patients between December 2012 and August 2021 were retrospectively identified. Demographic data, procedure variables, and readmission characteristics were also collected. RESULTS: Forty-five pro

A national audit of sinus surgery. Results of the Royal College of Surgeons of England comparative audit of ENT surgery.

The results of a national audit of sinus surgery are presented. Forty-six consultant ENT surgeons reported on over 2500 sinus procedures. There has been an exponential rise in the number of surgeons in England and Wales performing functional endoscopic sinus surgery (FESS) in recent years. Sixty-five per cent of surgeons in our study used the FESS technique and 81% had formal or other training. Outpatient sinus endoscopy and CT scans have become more routine pre-operative investigations, whilst the use of plain films has waned. There was a wide variation in the numbers of FESS procedures perfo

Cervical video-assisted thoracoscopic surgery using a flexible endoscope for bilateral thoracoscopy.

Video-assisted thoracoscopic surgery (VATS) has become the standard of care for pleural evaluation, drainage, and pleurodesis. The major limitations to standard VATS techniques include intercostal pain and the unilateral nature of the procedure. We report on a cervical VATS approach for bilateral thoracoscopy, pleural biopsy, and talc pleurodesis using a flexible video endoscope without any intercostal incision. A 64-year old male with peritoneal carcinomatosis was noted to have significant bilateral pleural effusions. A cervical video-assisted thoracoscopic surgery (C-VATS) procedure was perf

Analgesia considerations in orthopaedic surgery: the role of magnesium sulfate infusions.

Orthopaedic surgical operations are associated with significant post-operative pain, often managed with opioids, which carry risks of adverse effects and dependency. Magnesium sulfate, a NMDA receptor antagonist with analgesic and muscle relaxant properties, has emerged as a potential adjunct to improve pain control and reduce opioid consumption in orthopaedic procedures. Current evidence supports magnesium sulfate as a valuable adjunct in orthopaedic pain management, particularly in reducing opioid consumption and enhancing muscle relaxation. However, heterogeneity in study design, administra

Complex Regional Pain Syndrome-Like Symptoms Following Herpes Zoster Infection With Distal Extremity Involvement: A Case Report.

BACKGROUND: Complex regional pain syndrome (CRPS)-like symptoms are rarely observed after herpes zoster (HZ) infection. Especially in the presence of a rash spreading to the distal extremity, CRPS-like symptoms are more common. CASE REPORT: In this report, we present a 67-year-old patient who developed CRPS-like symptoms along with persistent severe pain after HZ infection, whose symptoms resolved after sympathetic ganglion blockade. CONCLUSIONS: CRPS-like symptoms are rarely to occur after HZ infection, especially in patients with HZ infection affecting the distal extremity. In these cases,

A comparison of parenteral and electronic dental anesthesia during operative procedures in children.

The effectiveness of electronic dental anesthesia (EDA) for pain control during restorative procedures was compared with local anesthesia (LA) in 108 pediatric patients aged 6-12. One tooth was treated with LA and another with EDA at the same appointment. The pain levels during restorative treatment were assessed using a color scale. Behavior also was assessed using sound, eye, and motor (SEM) scale. Although the success rate of EDA was less than that of LA, there was no significant difference between the two methods. On the other hand, 63% of the patients preferred EDA while 27.8% preferred L

Streptococcus viridans osteomyelitis with endocarditis presenting as acute onset lower back pain.

An elderly male with a history of diabetes mellitus and a recent dental procedure presented to the emergency department with acute lumbosacral pain and low grade fever. Computerized tomography (CT scan) and magnetic resonance imaging (MRI) yielded a presumptive diagnosis of pyogenic vertebral osteomyelitis. A diagnosis of viridans Streptococcus vertebral osteomyelitis was confirmed by gallium scanning and blood culture. The literature has emphasized the occurrence of pyogenic vertebral osteomyelitis as a chronic process. A review suggests that viridans Streptococci, although an uncommon cause

Surgical lymphology. Therapy option for lymphoedema and lipohyperplasia dolorosa.

The typical therapy in lymphology is conservative. However, reconstructive and resective treatments for primary and secondary lymphoedema as well as resective procedures for lipohyperplasia dolorosa (LiDo) "lipedema" have been available for several decades. Each of these procedures has its clear indication and decades of successful history. These therapies represent a paradigm shift in lymphology. In reconstruction, the basic idea is to restore lymph flow, to bypass the obstacle to drainage in the vascular system. The combination procedures of two-stage application of resection and reconstruct

Prospective, Randomized, Multi-centered Clinical Trial Assessing Safety and Efficacy of a Synthetic Cartilage Implant Versus First Metatarsophalangeal Arthrodesis in Advanced Hallux Rigidus.

BACKGROUND: Although a variety of great toe implants have been tried in an attempt to maintain toe motion, the majority have failed with loosening, malalignment/dislocation, implant fragmentation and bone loss. In these cases, salvage to arthrodesis is more complicated and results in shortening of the ray or requires structural bone graft to reestablish length. This prospective study compared the efficacy and safety of this small (8/10 mm) hydrogel implant to the gold standard of a great toe arthrodesis for advanced-stage hallux rigidus. METHODS: In this prospective, randomized non-inferiorit

2026 American Society of Anesthesiologists Practice Guideline on Perioperative Pain Management Using Local and Regional Analgesia for Cardiothoracic Surgeries, Mastectomy, and Abdominal Surgeries.

This practice guideline addresses perioperative pain management using local and regional anesthesia for cardiothoracic, mastectomy, and abdominal surgery in adults and children. For adults, the American Society of Anesthesiologists (Schaumburg, Illinois) Task Force on Perioperative Pain Management strongly recommends fascial plane blocks to reduce pain and/or opioid requirements in the first 24 h postoperatively for open cardiothoracic, abdominal, retroperitoneal, and pelvic surgeries and mastectomy. Fascial plane blocks are also recommended in adults to reduce pain and/or opioid requirements

Rectus sheath block added to parasternal block may improve postoperative pain control and respiratory performance after cardiac surgery: a superiority single-blinded randomized controlled clinical trial.

BACKGROUND: The population undergoing cardiac surgery confronts challenges from uncontrolled post-sternotomy pain, with possible adverse effects on outcome. While the parasternal block can improve analgesia, its coverage may be insufficient to cover epigastric area. In this non-blinded randomized controlled study, we evaluated the analgesic and respiratory effect of adding a rectus sheath block to a parasternal block. METHODS: 58 patients undergoing cardiac surgery via median sternotomy were randomly assigned to receive parasternal block with rectus sheath block (experimental) or parasternal

Factors Associated With Concerns the Day After Office Hand Surgery.

PURPOSE: Pain after minor hand surgery can be misinterpreted as a problem. The sense that something may be wrong intensifies the pain. Some surgeons anticipate these feelings and call patients the evening or day after surgery to guide them through the recovery process. A study of routine, next day, postoperative phone calls can help determine the frequency of concerns and associated factors. We asked: 1) What factors are associated with concern the day after office hand surgery? 2) What factors are associated with pain intensity, satisfaction with care, and patient's perceived recovery traject

Assessment of pain and postoperative nausea and vomiting and their association in the early postoperative period: an observational study from Palestine.

BACKGROUND: Postoperative nausea and vomiting (PONV) and postoperative pain (POP) are most commonly experienced in the early hours after surgery. Many studies have reported high rates of PONV and POP, and have identified factors that could predict the development of these complications. This study aimed to evaluate the relationship between PONV and POP, and to identify some factors associated with these symptoms. METHODS: This was a prospective, multicentre, observational study performed at An-Najah National University Hospital and Rafidia Governmental Hospital, the major surgical hospitals i
