European guideline on obesity care in patients with gastrointestinal and liver diseases - Joint European Society for Clinical Nutrition and Metabolism / United European Gastroenterology guideline.

BACKGROUND: Patients with chronic gastrointestinal (GI) disease such as inflammatory bowel disease (IBD), irritable bowel syndrome (IBS), celiac disease, gastroesophageal reflux disease (GERD), pancreatitis, and chronic liver disease (CLD) often suffer from obesity because of coincidence (IBD, IBS, celiac disease) or related pathophysiology (GERD, pancreatitis and CLD). It is unclear if such patients need a particular diagnostic and treatment that differs from the needs of lean GI patients. The present guideline addresses this question according to current knowledge and evidence. OBJECTIVE: T

The role of body composition assessment in obesity and eating disorders.

Lack of a balanced diet can have a significant impact on most organs of the body. Traditionally, evaluation of these conditions relied heavily upon body mass index "BMI" measurements, which are limited and open to inaccurate interpretation or omission of critical data. Advances in imaging allow better recognition of these conditions using accurate qualitative and quantitative data and correlation with any morphological changes in organs. Body composition evaluations include the assessment of the bone mineral density (BMD), visceral fat, subcutaneous fat, liver fat and iron overload and muscle

Metabolic impairment in heart failure: the myocardial and systemic perspective.

Although bioenergetic starvation is not a new concept in heart failure (HF), recent research has led to a growing appreciation of the complexity of metabolic aspects of HF pathophysiology. All steps of energy extraction, transfer, and utilization are affected, and structural metabolism is impaired, leading to compromised functional integrity of tissues. Not only the myocardium, but also peripheral tissues and organs are affected by metabolic failure, resulting in a global imbalance between catabolic and anabolic signals, leading to tissue wasting and, ultimately, to cachexia. Metabolic feedbac

[The measureable clinical signs of malnutrition: the clinical significance of body composition - bioimpedance - analysis].

Undernutrition, or more precisely: malnutrition results in worse outcome in clinical practice. Recognition and treatment of different forms of malnutrition are not unconditionally easy in the daily routine, however, it could lead to significant improvement in the outcome. This review summarizes the most frequent forms of malnutrition and demonstrates the advantages of the systematic use of bioelectrical impedance analysis in patients undergoing chronic treatment either in hospitals or outpatient care. The importance of the method is still underestimated in Hungary as well as many other countri

Precision Medicine in Inflammatory Bowel Disease: The Emerging Role of Metabolic Dysfunction.

Inflammatory bowel disease (IBD), encompassing ulcerative colitis (UC) and Crohn's disease (CD), is a chronic inflammatory disorder of the gastrointestinal tract with a rapidly increasing global prevalence. In parallel, metabolic comorbidities including obesity, metabolic dysfunction-associated steatotic liver disease (MASLD), and sarcopenia are becoming increasingly common among patients with IBD and are now recognized as important modifiers of disease course and outcomes. As the therapeutic landscape of IBD continues to evolve, these intersecting trends create an opportunity to advance preci

Osteosarcopenia Revisited: From Mechanisms to Management.

Sarcopenia and osteoporosis are characterized by insufficient mass and function of skeletal muscle and bone, and often co-occur in older persons, a condition called osteosarcopenia. Loss of bone and skeletal muscle mass and function is driven by multiple risk factors that activate the hallmarks of aging. Recent genome-wide association studies in humans and animal models suggest pivotal roles for mitochondria, the nucleus, and nutrient anabolism, which are closely associated with the hallmarks of aging, as well as for osteokines and myokines, in the development of osteosarcopenia. Osteosarcopen

Risk factors and an optimized prediction model for urosepsis in diabetic patients with upper urinary tract stones.

To identify independent risk factors for urosepsis in diabetic patients with upper urinary tract stones (UUTS) and develop a prediction model to facilitate early detection and diagnosis, we retrospectively reviewed medical records of patients admitted between January 2020 and June 2023. Patients were divided based on the quick Sequential Organ Failure Assessment (qSOFA) score. The least absolute shrinkage and selection operator (LASSO) regression analysis was used for variable selection to form a preliminary model. The model was optimized and validated using the receiver operating characterist

Bovine colostrum-derived exosomes alleviate muscle degeneration by modulating gut microbiota and metabolic homeostasis in atrophy models.

Sarcopenia, a condition characterized by chronic systemic inflammation and a significant decline in the quality of life, is primarily associated with aging and degenerative diseases. Several studies have shown that milk exosomes contain a substantial number of miRNAs that are involved in immunity, inflammation, osteoporosis, and gut microbiota regulation. This study aimed to evaluate the potential functional role of bovine colostrum-derived exosomes (BCE) in reducing muscle atrophy. BCE treatment enhanced the viability of C2C12 myotube and stimulated myogenic differentiation, while inhibiting

The Diagnostic Potential of Urinary Titin Fragment in Neuromuscular Diseases.

Biomarkers are important for the diagnosis and follow-up of neuromuscular diseases. Creatine kinase (CK) is a widely used marker of active muscle damage; however, it is not suitable for assessing muscle mass loss. Therefore, additional biomarkers are required to monitor skeletal muscle damage and loss. Titin plays an essential role in the structure and function of muscle fibers. It provides stability and elasticity to the sarcomeres. During sarcomere damage, fragments of titin and other proteins are released from muscle fibers and can be detected in blood and urine. Urinary titin-N fragment (U

The role of bile acid metabolism in bone and muscle: from analytics to mechanisms.

Osteoporosis and sarcopenia are both common age-related disorders that are associated with increased morbidity and mortality. Bone and muscle are metabolically very active tissues that require large amounts of energy. Bile acids (BAs), a group of liver-derived steroid compounds, are primarily known as emulsifiers that facilitate the resorption of dietary fat and lipids. In addition, they have pleiotropic metabolic functions in lipoprotein and glucose metabolism, inflammation, and intestinal bacterial growth. Through these effects, they are related to metabolic diseases, such as diabetes, hyper

Is ChatGPT an Effective Tool for Providing Dietary Advice?

The chatbot Chat Generative Pretrained Transformer (ChatGPT) is becoming increasingly popular among patients for searching health-related information. Prior studies have raised concerns regarding accuracy in offering nutritional advice. We investigated in November 2023 ChatGPT's potential as a tool for providing nutritional guidance in relation to different non-communicable diseases (NCDs). First, the dietary advice given by ChatGPT (version 3.5) for various NCDs was compared with guidelines; then, the chatbot's capacity to manage a complex case with several diseases was investigated. Characterization of Proteome Changes in Aged and Collagen VI-Deficient Human Pericyte Cultures.

Pericytes are a distinct type of cells interacting with endothelial cells in blood vessels and contributing to endothelial barrier integrity. Furthermore, pericytes show mesenchymal stem cell properties. Muscle-derived pericytes can demonstrate both angiogenic and myogenic capabilities. It is well known that regenerative abilities and muscle stem cell potential decline during aging, leading to sarcopenia. Therefore, this study aimed to investigate the potential of pericytes in supporting muscle differentiation and angiogenesis in elderly individuals and in patients affected by Ullrich congenit

A case report of pancreatic exocrine insufficiency in a patient with Parkinson's disease: A coincidence or is there more to it than meets the eye?

Pancreatic exocrine insufficiency (PEI) is an under-diagnosed condition. Untreated PEI can result in developing gastrointestinal symptoms and long-term complications including weight loss, nutrient deficiencies, sarcopenia and osteoporosis. Current best practice recommends testing for PEI in certain disorders including chronic pancreatitis, cystic fibrosis, pancreatic cancer and post-pancreatic surgery. However, there is increasing evidence that PEI is associated with a number of conditions in addition to the aforementioned diseases. These 'at-risk' conditions are a heterogeneous group of dise

Effect of Muscle Forces on Femur During Level Walking Using a Virtual Population of Older Women.

Aging is associated with a greater risk of muscle and bone disorders such as sarcopenia and osteoporosis. These conditions substantially affect one's mobility and quality of life. In the past, muscles and bones are often studied separately using generic or scaled information that are not personal-specific, nor are they representative of the large variations seen in the elderly population. Consequently, the mechanical interaction between the aged muscle and bone is not well understood, especially when carrying out daily activities. This study presents a coupling approach across the body and the

Exploring the impact of protein intake on the association between oxidative balance score and lean mass in adults aged 20-59: NHANES 2011-2018.

BACKGROUND: Previous studies have established a correlation between the pathogenesis of oxidative stress and sarcopenia. The Oxidative Balance Score (OBS) is an integrated measure that reflects the overall balance of antioxidants and pro-oxidants in dietary components and lifestyle. However, there are limited reports on the association between OBS and lean mass and the impact of protein intake on the association between OBS and lean mass. METHODS: Using data from the National Health and Nutrition Examination Survey from 2011 to 2018, multivariate linear and logistic regression analyses were c
