Thyroid eye disease: What's the latest?

Thyroid eye disease (TED) is an inflammatory autoimmune condition now recognized as being associated not only with Graves hyperthyroidism but also with euthyroidism and hypothyroidism. A high index of suspicion is required to diagnose TED and to recognize sight-threatening disease. Care is best provided in a multidisciplinary setting among primary care clinicians, endocrinologists, and ophthalmologists. This review summarizes the pathophysiology, risk factors, clinical presentation, and activity and severity evaluation of TED as well as the most recent advances in its treatment.

[Vibration disease in miners of the Far North].

The article covers a specified concept model of vibration disease pathogenesis. The leading mechanisms underlying the disease are nervous and hormonal dysfunction presenting hypercortisolemia and low serum levels of thyroid hormones, associated tunnel compression of upper limb nerves and immune disorders resulting in autoimmune aggression. Informative diagnostic methods are suggested and diagnostic values are justified. The authors proved efficiency of therapeutic measures aimed to relieve compression and ischemic changes in peripheral nerves, to correct immune disorders.

Factors influencing polyautoimmunity in systemic lupus erythematosus.

OBJECTIVE: Since characterization of the extent to which particular combinations of autoimmune diseases (ADs) occur in excess of that expected by chance may offer new insights into possible common pathophysiological mechanisms, polyautoimmunity (i.e., ADs co-occurring within patients) in systemic lupus erythematosus (SLE) and its associated factors were investigated. METHODS: This was a cross-sectional study in which 335 consecutive patients with SLE and the history of 22 ADs were investigated. A multivariate analysis was performed. A systematic literature review was done and results were gro

STAT1 and STAT3 gain of function: clinically heterogenous immune regulatory disorders.

PURPOSE OF REVIEW: The identification of STAT1 gain-of-function (GOF) in 2011 and STAT3 GOF in 2014 has advanced our understanding of the host immunity along the JAK/STAT pathway and allowed targeted treatment approaches. We review the clinical features and pathogenesis of STAT1 and STAT3 GOF and how this has shaped new approaches to therapy. RECENT FINDINGS: STAT1 GOF, initially described in patients with chronic mucocutaneous candidiasis (CMC) and autoimmune thyroid disease, is now recognized to cause early-onset multisystem autoimmunity and a range of infections. STAT3 GOF comprises mostly

Failure of CD25+ T cells from lupus-prone mice to suppress lupus glomerulonephritis and sialoadenitis.

The development of organ-specific autoimmune diseases in mice thymectomized on day 3 of life (d3tx mice) can be prevented by transferring CD4(+)CD25(+) T cells from syngeneic, normal adult mice. Using a d3tx model, we asked whether CD4(+)CD25(+) T cell deficiency contributes to glomerulonephritis (GN) in lupus-prone mice. New Zealand Mixed 2328 (NZM2328) mice spontaneously develop autoantibodies to dsDNA and female-dominant, fatal GN. After d3tx, both male and female NZM2328 mice developed 1) accelerated dsDNA autoantibody response, 2) early onset and severe proliferative GN with massive mesan

What happens to the normal thyroid during pregnancy?

Hormonal changes and metabolic demands during pregnancy result in profound alterations in the biochemical parameters of thyroid function. For the thyroidal economy, the main events occurring during pregnancy are: a marked increase in serum thyroxine-binding globulin levels; a marginal decrease in free hormone concentrations (in iodine-sufficient conditions) that is significantly amplified when there is iodine restriction or overt iodine deficiency; a frequent trend toward a slight increase in basal thyrotropin (TSH) values between the first trimester and term; a direct stimulation of the mater

Serum interleukin-6 in amiodarone-induced thyrotoxicosis.

Amiodarone, an iodine-rich cardiac drug, may induce thyrotoxicosis (AIT), which can occur in patients with preexisting thyroid abnormalities and in subjects with apparently normal thyroid glands. The pathogenesis of AIT is often due to iodine-induced excessive thyroid hormone synthesis, especially in patients with underlying thyroid disease. In some instances, however, AIT may be related to a destructive process due to amiodarone-induced thyroiditis, resulting in thyroid cell damage and thyroid hormone release into the circulation. Another thyroid inflammatory process, subacute thyroiditis, ha

Beyond dermatology: the role of allied health professionals in early recognition and reassurance of vitiligo.

Vitiligo is a chronic depigmentary disorder affecting 0.5% to 2% of the population, caused by autoimmune destruction of melanocytes. Nondermatology clinicians, such as physical therapists, occupational therapists, chiropractors, and dentists, frequently observe patients' skin and are well positioned to detect early lesions on the face, extremities, trunk, and back. Early recognition is important due to associations with autoimmune comorbidities (e.g., thyroid disease, type 1 diabetes) and psychosocial impact, including anxiety and psychiatric conditions. With brief training and clear referral

Reticular erythematous mucinosis: a systematic review of a controversial entity.

INTRODUCTION: Reticular erythematous mucinosis (REM) is a rare chronic dermatosis, predominantly affecting middle age women, characterized by net-like macules and patches on the chest and back. The etiology of REM is uncertain, though associations with environmental, hormonal, and autoimmune factors have been suggested. EVIDENCE ACQUISITION: This systematic review of 62 studies, involving 129 patients, analyzes the clinical, histopathological, and therapeutic aspects of REM. EVIDENCE SYNTHESIS: Findings indicate consistent mucin deposition in the dermis, a predominant lymphocytic infiltrate,

SCHMIDT'S SYNDROME IN A 32 YEARS OLD FEMALE.

In polyglandular autoimmune (PGA) syndromes, there is immune dysfunction of two or more endocrine glands. Immunity mediated disorders of non-endocrine organs may also be seen. These syndromes are of two main types: type I and type II. We are reporting this case of a 32 years old lady who presented initially with hypothyroidism for many years and received thyroid replacement therapy. After that she was married and had children. After an interval of about seven years of the initial diagnosis of hypothyroidism, she was hospitalized in first trimester of pregnancy for severe vertigo, syncopal epis

Impact of fetal-maternal microchimerism on women's health--a review.

Microchimerism is defined by the presence of circulating cells, bi-directionally transferred from one genetically distinct individual to another. It occurs either physiologically during pregnancy, or iatrogenically after blood transfusion and organ transplants. The migrated cells may persist for decades. Much controversy exists around the role of microchimeric cells in the pathogenesis of various diseases and around their role in tissue repair. Microchimerism has been investigated in different autoimmune disorders, such as systemic sclerosis, systemic lupus erythematosus, autoimmune thyroid di

Association scan of 14,500 nonsynonymous SNPs in four diseases identifies autoimmunity variants.

We have genotyped 14,436 nonsynonymous SNPs (nsSNPs) and 897 major histocompatibility complex (MHC) tag SNPs from 1,000 independent cases of ankylosing spondylitis (AS), autoimmune thyroid disease (AITD), multiple sclerosis (MS) and breast cancer (BC). Comparing these data against a common control dataset derived from 1,500 randomly selected healthy British individuals, we report initial association and independent replication in a North American sample of two new loci related to ankylosing spondylitis, ARTS1 and IL23R, and confirmation of the previously reported association of AITD with TSHR

Thyroglobulin antibody in differentiated thyroid cancer: international laboratory practice survey and the case for a dual-threshold model.

OBJECTIVES: Anti-thyroglobulin antibodies (TgAb) have a dual role during the follow-up of differentiated thyroid cancer (DTC): they may interfere with thyroglobulin (Tg) immunoassays and also serve as surrogate biomarkers of disease status. However, laboratory practices for TgAb measurement and reporting remain poorly standardised. We conducted an international survey to characterize current laboratory approaches and identify opportunities for harmonization. METHODS: A structured questionnaire was distributed to 120 laboratory medicine centres performing Tg and TgAb testing. The survey collec

Mendelian Randomization Analysis of Immune Cell Targets in Hashimoto's Thyroiditis Progression to Hypothyroidism.

INTRODUCTION: This study aimed to identify immune cell traits that may contribute to the progression of Hashimoto's Thyroiditis (HT) to autoimmune hypothyroidism (AIH) using Mendelian Randomization (MR) analysis. METHODS: We integrated publicly available Genome-Wide Association Study (GWAS) datasets and performed two-sample MR to evaluate the causal effects of 731 immune cell phenotypes on HT and AIH. Single-Nucleotide Polymorphisms (SNPs) were selected as Instrumental Variables (IVs). Multiple MR approaches, including Inverse Variance Weighting (IVW), MR-Egger, and weighted median methods, w

Immune tolerance induction using the thyrotropin receptor epitope 78-94 (p37) prevents Graves' disease in HLA-DR3 transgenic mice.

Graves' disease (GD) is an organ-specific autoimmune thyroid disorder characterized by anti-thyrotropin receptor (TSH-R) antibodies (TRAb), with strong genetic susceptibility conferred by the HLA-DRB1*03:01 (DR3) allele. We investigated whether pre-immunization with the immunodominant TSH-R-derived peptide spanning residues 78-94 (ISRIYVSIDVTLQQLES; p37) could induce immune tolerance and prevent GD in DR3 transgenic mice. GD was induced by intramuscular injection of adenovirus encoding human TSH-R (Ad-TSH-R289). Mice were pretreated with p37 either as a single 50 μg dose or by step-up escalati

Children with Hashimoto’s Thyroiditis Have Increased Intestinal Permeability: Results of a Pilot Study.

Increased intestinal permeability (IIP) precedes several autoimmune disorders. Although Hashimoto’s thyroiditis (HT) is the most common autoimmune disorder, the role of IIP in its pathogenesis had received little attention. Zonulin plays a critical role in IIP by modulating intracellular tight junctions. Rise of serum zonulin levels were shown to indicate IIP in human subjects. In this case-control study, we examined the hypothesis that patients with HT have IIP. We studied 30 children and adolescents with HT, and 30 patients with congenital hypothyroidism (CH) matched for age, gender and body

Demographic and clinical characterization of multiple sclerosis in Mexico: The REMEMBer study.

BACKGROUND: Multiple sclerosis (MS) is a chronic neurodegenerative disease of the central nervous system with high prevalence in young adults around the world. The vast majority of epidemiological studies and statistics are based on European and American data, so most clinical guidelines and medical consensus are based on this information. There is very limited evidence in Mexico regarding demographic and clinical aspects of MS. Therefore, this study comprehensively described the epidemiological and clinical features of MS in a large cohort of patients from eight tertiary-level centers in Mexi

Dehydroepiandrosterone sulfate does not prevent spontaneous and iodine-induced lymphocytic thyroiditis and diabetes mellitus in the BB/Wor rat.

Chronic feeding of dehydroepiandrosterone (DHEA) and its sulfated metabolite, dehydroepiandrosterone sulfate (DHEAS), has previously been reported to decrease hyperglycemia, obesity, cancer, and autoantibody generation in a number of animal models and to increase muscle mass and physiological and psychological well-being in elderly humans, although these latter studies remain controversial. The present study was carried out to determine whether large amounts of DHEAS given orally would prevent the occurrence of spontaneous and iodine-induced autoimmune lymphocytic thyroiditis (LT) and/or spont
