Evolution of Pancreatic Endotherapy.

In the last half century, endotherapy for pancreatic diseases has changed considerably. Although endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic ultrasound (EUS) were introduced initially as diagnostic tools, they quickly evolved into therapeutic tools for preventing and managing complications of pancreatitis. More recently, therapeutic endoscopy has shown potential in palliation and cure of pancreatic neoplasms. This article discusses the changing landscape of pancreatic endotherapy as therapeutic ERCP and EUS were introduced and because they have evolved to treat differe

Salivary gland disease in the era of COVID-19 pandemic.

Coronavirus disease 2019 (COVID-19) pandemic forced significant changes in current approach to outpatient evaluation of common otolaryngology complaints as hospitals around the world are trying to limit the spread of the virus and to preserve health care resources. These changes raise a lot of questions regarding patient triage and treatment decisions in clinical situations when it is unclear if the workup and management can be postponed. In this communication, we present our approach to evaluation and triage of new patients with complaints concerning for salivary gland disease.

Technique for Resonance® stent exchange in patients with extrinsic obstruction: description of a novel approach and literature review.

Malignant ureteral obstruction is a morbid diagnosis and can be difficult to manage. Ureteral stents often fail as the disease progresses. Metallic stents have provided a longer-term option for decompression as they inherently resist compression and require changes left often. However, changing metallic stents is challenging as it cannot be done over a wire as is done with plastic stents. We present a novel technique for safe and efficient exchange of the Resonance(®) metallic stent. We also review the literature on metallic stents.

Gynecologic and other infections in pregnancy.

The treatment of gynecologic and other infections in obstetric patients involves consideration of the physiologic changes of pregnancy, the clinical implications of the infection for the patient as well as the fetus, and the safety of antimicrobials available for therapy. This article highlights the treatment of infections of the vagina, uterus, and urinary tract, with a focus on how therapy changes in obstetric patients. In addition, the emergency department management of other clinically important infections in pregnancy, such as those caused by the human immunodeficiency virus, influenza vi

Chronic hypoxemia in children with cyanotic heart disease.

Hypoxemia, visible as cyanosis, is present in infants and children with congenital cardiac defects that result in arterial desaturation. Although many of these defects can be surgically repaired, some children remain cyanotic. Chronic cyanosis causes adaptive changes such as polycythemia and potentially damaging changes in other organ systems. This article reviews the assessment and management of patients with cyanotic congenital heart disease. The physiologic consequences and associated medical problems of chronic cyanosis are highlighted. Nursing implications in caring for cyanotic children

Management of fibrosing alveolitis with polymyositis dermatomyositis.

A 38-year-old man with polymyositis-dermatomyositis developed acute life-threatening fibrosing alveolitis. We report a two year follow-up of this patient, during which control of alveolitis was obtained using prednisolone, and was maintained with the addition of cyclophosphamide. Relapse of myositis responded to increased prednisolone. Serial measurements of vital capacity and transfer factor assisted management of alveolitis, but changes in vital capacity preceded changes in transfer factor, and were more convenient to perform during frequent outpatient review.

Rapidly fatal respiratory failure and angioimmunoblastic lymphadenopathy: possible contributions of immunoblastic leukaemia, chemotherapy, and multiple antibodies directed against mature blood cells.

A patient with angioimmunoblastic lymphadenopathy, immunoblastic leukaemia, pulmonary immunoblastic infiltration, and multiple antihaemocytic antibodies in his serum deteriorated rapidly after chemotherapy due to severe progressive respiratory of dysfunction. The haematological and immunological changes that accompanied this are described and discussed in the light of the pulmonary changes observed at necropsy of pulmonary oedema, fibrinous thrombi within venules, and immunoblastic infiltration of these thrombi and the venule walls. A pathophysiological mechanism is postulated in an attempt to

Concussion and the Sleeping Brain.

BACKGROUND: Emerging research has suggested sleep to be a modifier of the trajectory of concussion recovery in adolescent and adult populations. Despite the growing recognition of the relationship between sleep and concussion, the mechanisms and physiological processes governing this association have yet to be established. MAIN BODY: Following a concussion, a pathophysiologic cascade of events occurs, characterized by numerous factors including microglia activation, ionic imbalance, and release of excitatory neurotransmitters. Importantly, each of these factors plays a role in the regulation

Kinetic proteomics identifies targeted changes in liver metabolism and the ribo-interactome by dietary sulfur amino acid restriction.

Dietary sulfur amino acid restriction (SAAR) protects against diet-induced obesity, extends healthspan, and coincides with an overall reduction in hepatic protein synthesis. To explore the underpinnings of SAAR-induced slowed growth and its impact on liver metabolism and proteostasis, we resolved changes in hepatic mRNA and protein abundances and compared synthesis rates of individual liver proteins. To achieve this, adult male mice were provided deuterium-labeled drinking water while freely consuming either a regular-fat or high-fat diet that was SAA restricted. Livers from these mice and the

Changes in the use of psychotropic drugs during the course of Alzheimer's disease: A large-scale longitudinal study of French medical records.

INTRODUCTION: We aim to understand how patients with Alzheimer's disease (AD) are treated by identifying in a longitudinal fashion the late-life changes in patients' medical history that precede and follow AD diagnosis. METHODS: We use prescription history of 34,782 patients followed between 1996 and 2019 by French general practitioners. We compare patients with an AD diagnosis, patients with mild cognitive impairment (MCI), and patients free of mental disorders. We use a generalized mixed-effects model to study the longitudinal changes in the prescription of eight drug types for a period 15

Characterizing Sleep and Wakefulness in the Acute Phase of Concussion in the General Population: A Naturalistic Cohort from the Toronto Concussion Study.

Growing literature links concussion to changes in sleep and wakefulness in humans and in rodent models. Sleep has been linked with synaptic reorganization under other conditions; however, the characterization and role of sleep after acute concussion remains poorly understood. While much research has focused on insomnia among patients with chronic or persistent concussion symptoms, there is limited understanding of sleep and acute concussion, its potential role in recovery, and associated risk factors for the development of chronic sleep disturbance. Studies to date are limited by small sample

Oral health and menopause: a comprehensive review on current knowledge and associated dental management.

The menopause is physiological changes in women that give rise to adaptive changes at both systemic and oral level. As we all begin to reach an older age, dental health and hygiene becomes a major concern. The dentist is often the first person to appreciate numerous changes that are experienced throughout the body during menopause. The teeth and gums are extremely susceptible to any hormonal changes that take place just before menopause and readily decrease body's ability to fight off minor infections or maintain a healthy balance of useful and harmful bacteria within the oral environment. Impact of capsule endoscopy on management of inflammatory bowel disease: a single tertiary care center experience.

BACKGROUND: Capsule endoscopy (CE) is performed to assess inflammatory bowel disease (IBD). We aimed to define the results of CE in subtypes of IBD and to determine whether CE results in management changes. METHODS: A retrospective cohort study was performed of all CEs for IBD at a tertiary care center from 2003-2009. Descriptive statistics were used to compare IBD-specific medications, surgeries, and imaging studies in the 3 months prior and 3 months after CE. RESULTS: Of 907 CEs performed from 2003-2009, 128 were for an indication of symptomatic IBD and 124 capsules left the stomach (86 fo

Development of a Novel Non-Invasive Blood Pressure Measurement Technique Using Near-Infrared Spectroscopy and Deep Neural Networks.

Non-invasive monitoring of blood pressure is essential for health management in daily life, particularly for elderly individuals and patients with autonomic dysfunction. However, conventional cuff-based sphygmomanometers are uncomfortable and provide discontinuous monitoring. In this study, we propose a novel non-invasive technique for blood pressure change assessment based on prefrontal near-infrared spectroscopy (NIRS) and deep neural networks. Unlike existing approaches that rely primarily on electrocardiography or photoplethysmography signals, the proposed method independently utilizes pre

Influence pattern and utility evaluation of connected vehicle warning information on driving behavior: A case study of freeway debris incident.

OBJECTIVE: Debris incidents pose a severe threat to freeway traffic safety; however, research on how debris affects driver behavior and on the design and evaluation of corresponding warning measures remains insufficient. This study employs an interconnected driving simulation platform to analyze the effects of illumination conditions, driver attributes (sex, age, and driving experience), and warning measures (voice prompt, traffic-cone warning, and variable message sign [VMS] prompt) on driving behavior in typical freeway debris scenarios. METHODS: Using driving simulation techniques, five ke

Remote Therapeutic Monitoring Enhances Adherence to a Home Exercise Program in Chronic Stroke.

BACKGROUND: Poor adherence to home exercise programs is a persistent challenge in physical rehabilitation and beyond. Data suggest that follow-up telehealth visits featuring behavior change techniques can improve adherence. However, the optimal frequency of behavioral support for effective self-management of technology-enabled home exercise programs remains unclear. This paper investigates how adherence to home exercise varies with the number of follow-up visits holding the type of exercise constant and measuring adherence objectively. METHODS: A secondary analysis of adherence was conducted
