Hot Topics in Primary Care: Update on the Recognition and Management of Gout: More Than the Great Toe.

Gout is a chronic inflammatory condition that is increasing in prevalence and commonly associated with other chronic diseases such as obesity, diabetes mellitus, hypertension, hypercholesterolemia, chronic kidney disease, cardiovascular disease, and thromboembolic disorders. These associations make the management of patients with gout more complex. Although identification of MSU crystals in synovial fluid is diagnostic, a presumptive diagnosis of gout can be made clinically based on the presence of hyperuricemia, rapid development of pain, tenderness, and swelling in a single toe (male) or elb

Urticaria and angioedema.

Urticaria (hives) is a common disorder that often presents with angioedema (swelling that occurs beneath the skin). It is generally classified as acute, chronic or physical. Second-generation, non-sedating H1-receptor antihistamines represent the mainstay of therapy for both acute and chronic urticaria. Angioedema can occur in the absence of urticaria, with angiotensin-converting enzyme (ACE) inhibitor-induced angioedema and idiopathic angioedema being the more common causes. Rarer causes are hereditary angioedema (HAE) or acquired angioedema (AAE). Although the angioedema associated with thes

Catheter burden following urogynecologic surgery.

BACKGROUND: Data on the experience that women who undergo urogynecologic surgery have with postoperative catheterization are severely limited. As the importance of our patients' perioperative experience becomes more valued, assessment of the burden of postoperative catheterization, which has not yet been performed, is increasingly needed. OBJECTIVE: The aim of this study was to compare catheter burden in women who self-selected use of an indwelling Foley catheter vs clean intermittent self-catheterization for voiding dysfunction after reconstructive pelvic surgery. STUDY DESIGN: This is a ne

A randomized controlled trial of a money management-based substance use intervention.

OBJECTIVE: Money management has been implemented, often in bundled interventions, as a strategy to counteract spending of public support checks and other funds on drugs and alcohol. The authors conducted a randomized controlled trial of a voluntary money management program as an adjunctive treatment for patients in treatment for mental illness, substance use disorders, or both. In the advisor-teller money manager (ATM) intervention, a money manager stores patients' checkbooks and automated bank cards, trains patients to manage their own funds, and links spending to activities related to their

Becoming the boss.

Even for the most gifted individuals, the process of becoming a leader is an arduous, albeit rewarding, journey of continuous learning and self-development. The initial test along the path is so fundamental that we often overlook it: becoming a boss for the first time. That's a shame, because the trials involved in this rite of passage have serious consequences for both the individual and the organization. For a decade and a half, the author has studied people-particularly star performers-making major career transitions to management. As firms have become leaner and more dynamic, new managers

Associations Between 24-Hour Physical Behavior, Self-Perceived Stress, and Coping Self-Efficacy in Everyday Life: Ambulatory Assessment Study.

BACKGROUND: Psychological stress poses a risk to mental and physical health and has become a major public health challenge. As physical behaviors (ie, physical activity, sedentary behavior (SB), and sleep) play a key role in mental well-being, their targeted modification could be an approach to coping with stress in everyday life. Previous studies have primarily either analyzed the associations between isolated physical behaviors and stress-related outcomes or employed cross-sectional designs. Accordingly, there is a need for deeper insights into the within- and between-person associations bet

Bridging the Gap: The Roles of Legacy Leadership and Psychological Safety in Nurse Interns' Readiness for Practice.

AIM: To examine the relationship between legacy leadership and nurse interns' readiness for clinical practice and to test the moderating role of psychological safety. BACKGROUND: The transition from nursing education to clinical practice is a critical period marked by gaps in readiness, limited self-regulation, and increased patient safety risks. Leadership within clinical settings remains a key modifiable factor shaping this transition. Legacy leadership, grounded in mentorship, ethical modeling, long-term professional development, and psychological safety have been proposed as facilitators

Aligning training with clinical reality: linking case-mix and self-reported confidence to entrustable professional activities in physician-staffed EMS.

BACKGROUND: Emergency medical services (EMS) physicians must manage a broad spectrum of high-acuity prehospital emergencies. However, current training pathways in Germany are not explicitly competency-based and provide limited structured exposure to low-frequency, high-stakes scenarios. Entrustable professional activities (EPAs) offer a framework to align training with real-world clinical demands. This study aimed to identify key prehospital tasks suitable for an EPA-based curriculum. METHODS: We conducted a multi-method descriptive study combining (1) a retrospective analysis of 4,078 prehos

The Impact of Stuttering on Development of Self-Identity, Relationships, and Quality of Life in Women Who Stutter.

PURPOSE: The experiences of women who stutter have been underresearched. Clinicians have little guidance from the research literature on issues specific to women who stutter and are likely to have less clinical contact with this group than with men who stutter because of the higher prevalence of stuttering in men. This study explored the experiences of a small group of women who stutter with a particular focus on what the main current issues are and how gender may have influenced experiences with stuttering. METHOD: This qualitative study involved recruitment of 9 women who stutter (aged 35-8
