The management of arrhythmias following an acute myocardial infarction.

Arrhythmias are extremely common early after AMI. An arrhythmia is defined by exclusion, either because the sequence of myocardial depolarisation is other than normal or because certain arbitrary limits are exceeded. It follows that the term "arrhythmia" encompasses a complex heterogenous group. Although arrhythmias are defined in electrical terms they are only important because of their immediate, delayed or potential haemodynamic consequences. These occur because of changes in heart rate, loss of atrial transport function, increased myocardial oxygen consumption, decreased myocardial blood f

Genotype and arrhythmic risk in dilated cardiomyopathy.

BACKGROUND AND AIMS: Dilated cardiomyopathy (DCM) presents with variable risk of life-threatening arrhythmias. Genetic diagnosis guides sudden death prevention strategies, but risk magnitude for arrhythmogenic genes remains unknown. This study leveraged the international Sarcomeric Human Cardiomyopathy Registry (SHaRe) to address this gap and improve risk stratification. METHODS: Effects of pathogenic/likely pathogenic (P/LP) variants in genes associated with DCM and ventricular arrhythmia on clinical events were analysed in DCM-SHaRe patients. Patients were stratified by presence of a P/LP v

Implantable Cardioverter-Defibrillator Therapy in Apical Hypertrophic Cardiomyopathy: Insights From a Multicenter Study.

BACKGROUND AND OBJECTIVES: Prognosis and risk markers of sudden cardiac death (SCD) remain incompletely defined in patients with apical hypertrophic cardiomyopathy (HCM). This study evaluated established SCD risk markers and compared long-term implantable cardioverter-defibrillator (ICD) therapy outcomes between apical and non-apical HCM. METHODS: We analyzed 96 patients with HCM who underwent ICD implantation at 3 tertiary care hospitals in Korea. All SCD risk markers recommended by the 2024 American Heart Association (AHA)/American College of Cardiology (ACC) and 2023 European Society of Ca

Prophylactic implantation of cardioverter-defibrillator in patients with advanced light-chain amyloidosis-A pilot study.

BACKGROUND: Ventricular arrhythmias (VAs) and electromechanical dissociation have been observed as the most common causes of sudden cardiac death (SCD) in patients with light chain (AL) amyloidosis. However, an implantable cardioverter-defibrillator (ICD) has rarely been implanted in patients with advanced AL amyloidosis due to very poor prognosis. METHODS: Between July 2021 and December 2022, 10 patients with advanced cardiac AL amyloidosis referred to our institute who received prophylactic ICD implantation were prospectively recruited. The primary endpoint was the prevalence of VAs and app

Risk factors and outcomes associated with failed implantable cardioverter-defibrillator shocks.

BACKGROUND: Implantable cardioverter-defibrillators (ICDs) deliver shocks to terminate ventricular tachycardia/ventricular fibrillation (VT/VF) to reduce the risk of sudden cardiac death. Failure of ICD shocks to terminate VT/VF, even if not resulting in death immediately, may signal patients at high risk of adverse outcomes. OBJECTIVE: We sought to identify predictors and outcomes associated with failed appropriate ICD shocks in patients with reduced left ventricular ejection fraction with a primary prevention ICD. METHODS: Our study cohort consisted of 5512 patients with an ICD or cardiac

Life-threatening arrhythmia in patients with suspected acute myocarditis.

BACKGROUND: Patients with acute myocarditis (AM) often undergo prolonged rhythm monitoring due to the risk for life-threatening arrhythmia. OBJECTIVE: To describe the occurrence, timing and potential early rule-out of life-threatening arrhythmia in patients with AM. METHODS: We included consecutive patients with suspected AM admitted to the ICU/IMC for continuous rhythm monitoring into a cohort study. We assessed the incidence and timing of life-threatening arrhythmia (sustained ventricular tachycardia, ventricular fibrillation, cardiac arrest). To rule-out arrhythmia, we evaluated left vent

Prognostic value of right ventricular involvement in hypertrophic cardiomyopathy: A systematic review and meta-analysis.

BACKGROUND: Right ventricular (RV) involvement has been reported in one out of three patients with hypertrophic cardiomyopathy (HCM), however its prognostic significance remains unknown. We aimed to assess the prognostic value of RV involvement in patients with HCM through a systematic review and meta-analysis. METHODS: A literature search was performed on PubMed, ClinicalTrials.gov and Cochrane Library databases from inception through November 15, 2023. Original articles enrolling HCM patients >18 years old and evaluating the association of RV parameters routinely assessed in clinical practi

The triglyceride-glucose index, ventricular arrhythmias and major cardiovascular events in patients at high risk of sudden cardiac death.

BACKGROUND: The association between the triglyceride-glucose (TyG) index and ventricular arrhythmias (VAs) is unclear. This study aimed to investigate the relationship between the TyG index, VAs, and major cardiovascular events in patients at high risk of sudden cardiac death (SCD). METHODS: We enrolled 1046 patients at high risk of SCD with an indication for implantable cardioverter-defibrillator (ICD) implantation at the Chinese National Center for Cardiovascular Diseases. The primary outcome was VAs, defined as sustained ventricular tachycardia and ventricular fibrillation documented by th

Modified-Bentall Single-Patch Konno Enlargement Technique for Aortic Stenosis and Prosthesis-Patient Mismatch.

BACKGROUND: Aortic stenosis and prosthesis-patient mismatch complicate surgery for patients with small left ventricular outflow tracts. We present outcomes of a modified-Bentall single-patch Konno enlargement (BeSPoKE) technique for complex left ventricular outflow tract obstruction in adults. METHODS: The BeSPoKE technique facilitates a true outflow tract enlargement through an anterior septoventriculoplasty, using a single pericardial patch, followed by composite aortic valve-graft root replacement. Postoperative outflow tract geometry and valvular physiology were compared against preoperat

Impact of Transcatheter Edge-to-Edge Mitral Valve Repair on Atrial Functional Mitral Regurgitation from the GIOTTO Registry.

Atrial functional mitral regurgitation (aFMR) has a peculiar pathophysiology that may have distinctive outcomes. We investigated the impact of transcatheter edge-to-edge repair in aFMR compared with other FMR etiologies. The GIOTTO (GIse registry Of Transcatheter treatment of MR) is a multicenter, prospective study enrolling patients with symptomatic MR treated with MitraClip up to 2020. We categorized patients with FMR as aFMR, ischemic FMR (iFMR), and nonischemic ventricular FMR (niFMR). The clinical end points were defined according to the Mitral Valve Academic Research Consortium. Of 1,153

KCCQ total symptom score, clinical outcome measures, and adverse events associated with omecamtiv mecarbil for heart failure with reduced ejection fraction: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Omecamtiv mecarbil (OM) is a direct myosin activator that augments left ventricular systolic function. This review compares OM to placebo by evaluating its effect on clinical outcomes and adverse events in patients with heart failure with reduced left ventricular ejection fraction. METHODS AND RESULTS: A literature search of multiple databases for randomized controlled trials (RCTs) investigating OM versus placebo was undertaken. Six RCTs comprising 9596 patients were included. Use of OM was associated with a reduced risk of stroke (RR: 0.69; 95% CI 0.52-0.92). There was no signif

Ventricular arrhythmia events in heart failure patients with cardiac resynchronization therapy with or without a defibrillator for primary prevention.

BACKGROUND: It is uncertain whether cardiac resynchronization therapy with a defibrillator (CRT-D) provides better survival benefits than a CRT-pacemaker (CRT-P) in heart failure patients with a reduced ejection fraction (≦35%, HFrEF) treated with contemporary HF therapy. METHODS: We retrospectively analyzed the ventricular arrhythmia (VAs; sustained ventricular tachycardia/fibrillation) events in HFrEF patients who underwent CRT without a prior history of VAs or aborted sudden cardiac death before the CRT implantation. Between January/2010 and December/2020, a CRT device was implanted in 79

INTRODUCTION: Foot-and-mouth disease (FMD) causes mortality in calves due to myocarditis; however, the effects of FMD virus on cardiac arrhythmogenesis and Purkinje cells are unknown. Identifying diagnostic and prognostic markers in FMD-affected calves may be useful in disease management in the endemic countries. MATERIALS AND METHODS: A total of 81 FMD-affected calves were prospectively monitored till death or recovery. Foot-and-mouth disease was diagnosed by serology and reverse transcriptase-polymerase chain reaction (RT-PCR). Electrocardiography was recorded and serum cardiac biomarkers w
