Familial coaggregation of major psychiatric disorders among first-degree relatives of patients with obsessive-compulsive disorder: a nationwide study.

BACKGROUND: Whether the first-degree relatives (FDRs) of patients with obsessive-compulsive disorder (OCD) have an increased risk of the major psychiatric disorders, namely schizophrenia, bipolar disorder, OCD, major depressive disorder (MDD), autism spectrum disorder (ASD), and attention deficit hyperactivity disorder (ADHD), remains unclear. METHODS: Using the Taiwan National Health Insurance Research Database with the whole population sample size (n = 23 258 175), 89 500 FDRs, including parents, offspring, siblings, and twins, of patients with OCD were identified in our study. The relative

[Current concept of mood disorders. Impact on the therapeutic management].

The unipolar-bipolar distinction underlines the recurrence of the disorder (40 to 80% of unipolars and 95% of bipolars) and also the transition from one subtype to the other (10 to 15% of unipolars will become bipolars). Some semiological forms of depressive illness may give clues concerning the required management modalities. Depressions with psychotic features have a good response to ECT or to TCAs+neuroleptics. As other authors, Quitkin et al. find a good response of atypical depression to MAOIs. The comorbidity of mood disorders with personality disorders may be of poor prognosis. Akiskal

Recurrent unipolar mania: A comparative, cross-sectional study.

INTRODUCTION: A significant number of patients experience recurrent episodes of mania without depressive episodes. Evidence from the available literature suggests that these patients differ from typical "bipolar" or "manic-depressive" patients, but results have been inconsistent. The current study aims to add to this literature by comparing the demographic, clinical and risk factor profiles of patients with recurrent mania with and without depression. METHODS: 66 patients with a diagnosis of bipolar I disorder were divided into "unipolar mania" (recurrent mania alone, MA) and "bipolar" (both

Corpus callosum morphology in major mental disorders: a magnetic resonance imaging study.

Mental disorders diagnosis is based on specific clinical criteria. However, clinical studies found similarities and overlapping phenomenology across a variety of disorders, which suggests a common neurobiological substrate. Thus, there is a need to measure disease-related neuroanatomical similarities and differences across conditions. While structural alterations of the corpus callosum have been investigated in obsessive-compulsive disorder, schizophrenia, major depressive disorder and bipolar disorder, no study has addressed callosal aberrations in all diseases in a single study. Moreover, re

Insight in bipolar mania: evaluation of its heterogeneity and correlation with clinical symptoms.

BACKGROUND: Studies on insight in bipolar mania are not numerous and usually consider insight as a unitary construct. OBJECTIVE: Evaluate how different facets of insight are affected in bipolar mania and investigate correlations between insight for each specific object in bipolar disorder and manic symptomatology. METHOD: A group of 165 bipolar patients were followed during a year, with 51 patients having manic episodes according to DSM-IV-TR criteria. Patients underwent a clinical assessment and insight was evaluated through the Insight Scale for Affective Disorders. RESULTS: The study fou

Comorbidity issues in the pharmacological treatment of Pathological Gambling: a critical review.

BACKGROUND: Pathological Gambling (PG) is an impulse control disorder often comorbid with other psychopathology, particularly bipolar spectrum disorders, attention deficit/hyperactivity disorder, obsessive-compulsive disorder (OCD) and substance abuse. This paper reviews the published literature on the pharmacological management of PG, highlighting how clinical and subclinical comorbid psychopathology influences the choice of pharmacological treatment. METHODS: Using Medline, the authors reviewed relevant articles published on this topic from 1995 to 2005, focusing on the best-designed studie

Untangling late-onset first-episode mania: A case series.

BACKGROUND: Late-onset bipolar disorder (LOB) presents unique diagnostic and management challenges as various symptom overlaps with neurocognitive disorders, medical comorbidities, and age-related physiological changes. CASES: We report four elderly patients presenting with first-episode mania, highlighting clinical trajectories, comorbidities, and treatment responses. Three patients had metabolic comorbidities, while two had family history of psychiatric illness. Neuroimaging revealed small vessel ischemic changes in two cases, suggesting a vascular contribution. All patients required inpati

Treatment-Refractory Mania with Psychosis in a Post-Transplant Patient on Tacrolimus: A Case Report.

Bipolar affective disorder type I imparts significant morbidity and disease burden in the population. It is characterized by occurrence of one or more manic episodes which may be preceded or followed by a depressive or hypomanic phase. About half of these manic episodes are characterized by the presence of psychotic features. The condition is further complicated when the patient has multiple comorbid conditions. We report here the case of a Caucasian woman, aged 66 years, previously diagnosed with Bipolar disorder who developed treatment refractory mania with psychotic feature after being on t

Emotion circuits differentiate symptoms of psychosis versus mania in adolescents.

The diagnostic boundary between schizophrenia and bipolar disorder can be unclear, particularly with early onset. We assessed if emotion brain circuits differentiate psychosis versus mania symptoms in a series of six early onset patients. Symptoms were dissociated by direction, awareness condition, and brain regions. Greater psychosis symptoms were correlated with greater prefrontal, anterior cingulate, amygdala, and fusiform face area activation during masked fear processing. By contrast, greater mania symptoms were correlated with less amygdala activation during unmasked fear and happy proce

Quetiapine-induced hyperventilation and dyspnea.

OBJECTIVES: Quetiapine is an atypical antipsychotic with favorable properties in the treatment of bipolar disorder, but it has recently been associated with respiratory dysfunction. METHODS: In this case report, we describe a patient who developed quetiapine-induced hyperventilation and dyspnea after being treated with quetiapine. RESULTS: To our knowledge, this is the first case presented with quetiapine-induced hyperventilation in a patient with bipolar disorder. CONCLUSIONS: We recommend a quick reduction of the quetiapine dosage and a change to an alternative antipsychotic for patients

Psychotic mania associated with mefloquine in a bipolar patient.

A 63-year-old male patient with a history of bipolar I disorder presented to the emergency department in an acutely psychotic state. The patient had just returned from vacation in a malarial high-risk area and, as a result, had taken five weekly doses of mefloquine for prophylaxis. The patient's bipolar disorder was being treated with lithium, and he had been stable for about 8 years. All organic causes of psychosis were ruled out. The patient was admitted to the inpatient psychiatric unit and was treated with quetiapine until he was stable enough to return home.

Unipolar or bipolar depression? Improving diagnostic confidence with the adult patient.

PURPOSE: The purpose of this article is to inform advanced practice nurses in primary care about the differential diagnosis of bipolar disorder (BD), when depression is identified in the adult patient. DATA SOURCES: Selected research and clinical articles. CONCLUSIONS: Adult patients with BD are much more likely to seek treatment for depression than for mania or hypomania. Recognition of BD is improved when the primary care clinician is alerted to the factors indicating bipolarity and utilizes available screening tools. IMPLICATIONS FOR PRACTICE: Misdiagnosis of a bipolar spectrum disorder

50 Years with lithium treatment in affective disorders: present problems and priorities.

Lithium has been used as a treatment for various psychiatric- and somatic-illnesses for more than 50 years. Today the main use of lithium is for the prevention of episode recurrences in bipolar disorder. The main emphasis of this review will be on the efficacy and effectiveness of lithium prophylaxis in bipolar disorder but the review will also discuss other indications for lithium treatment, the historical development, pharmacokinetic and -dynamic issues, unwanted effects of lithium and the organisation of treatment services. Finally, although not the main purpose of this review, a short desc

Comorbidity of migraine and major affective disorders.

Two epidemiologic studies of young adults from the Detroit metropolitan area and from Zurich, Switzerland, focused on the relationship between migraine and affective disorders, such as major depression, dysthymia, bipolar disorder, and cyclothymia. Through application of current definitions of migraine and psychiatric disorders in structured diagnostic interviews, data have been obtained that strongly support clinical observations on migraine-major depression comorbidity. The findings to support a link between migraine and bipolar disorder are less consistent. Physicians caring for patients wi

[The Unitary Psychosis Theory].

Since E. Kraepelin isolated schizophrenia and bipolar disorder as heterogeneous diseases, attempts to categorize mental illnesses have continued to the DSM-5. Meanwhile, cases of psychosis occurring as a result of neurosyphilis have been reported. Whilst in some cases it is useful to divide mental illnesses, in others imposing such classifications may be not be feasible. Since 2008 numerous papers have been published showing that the same genes are related to an increased incidence of several psychiatric diseases including intellectual disorder, autism, ADHD, schizophrenia, bipolar disorder, a

Suicide attempts in 107 adolescents and adults with kleptomania.

Suicide attempts in kleptomania have received little investigation. This study examined rates, correlates, and predictors of suicide attempts in kleptomania. A total of 107 adolescent and adult subjects (n = 32 [29.9%] males) with DSM-IV kleptomania were assessed with standard measures of symptom severity, psychiatric comorbidity, and functional impairment. Subjects had high rates of suicide attempts (24.3%). The suicide attempt in 92.3% of those who attempted suicide was attributed specifically to kleptomania. Suicide attempts were associated with current and life-time bipolar disorder (p = .

Childhood parental loss and bipolar spectrum in obese bariatric surgery candidates.

BACKGROUND: Early adverse experiences such as sexual, verbal, or physical abuse and parental neglect have been associated with a higher risk of weight or eating problems in adulthood, and death or prolonged separation from parents during childhood has been implicated as a correlate of adult psychiatric disorders that are widely represented in obese subjects seeking bariatric surgery. OBJECTIVE: The aim of this study was to examine the rate of childhood parental loss in obese bariatric surgery candidates and explore its association with a psychiatric diagnosis and clinical and weight/eating-re

Early health economic assessment of eLi12, a new method to estimate 12-h lithium levels when blood sampling deviates from 12 h.

OBJECTIVE: Early economic evaluations (EEE) can evaluate the economic potential of new innovative healthcare solutions. We present a methodological framework for EEE in bipolar disorder and use eLi12 as an illustrative case, a new method to estimate 12-h lithium blood levels when blood sampling deviates from the 12-h timing, enabling more flexibility for patients and better data on 12-h lithium levels. METHODS: A decision-analytic model evaluated the costs and consequences of eLi12 for the treatment of bipolar disorder from a Danish national healthcare payer perspective, assessing the minimum

Alcohol Use Disorder Is Differently Associated With Psychotic Symptoms According To Underlying Psychiatric Disorders: A General Population Study.

AIMS: Psychotic symptoms can occur in the general population, and alcohol use disorder (AUD) is an identified vulnerability factor. However, it remains unclear how AUD is associated with psychotic symptoms, depending on the underlying psychiatric condition. We aimed to compare the prevalence of psychotic symptoms among subjects with different types of psychiatric disorders, i.e. unipolar or bipolar disorders, anxiety disorders, psychotic disorders or no psychiatric disorder, depending on whether or not there was an underlying AUD. METHODS: In a 38,694-subject general population study, we compa
