[A method for correcting dislocation in hemorrhages into the cerebral hemispheres].

Hypertension consequent upon increasing brain edema, and intercerebral pressure gradient which is the cause of transverse dislocation diminish with the use of a method which provides for hydrodynamic equilibrium. The method consists in connecting the ventricle of the intact cerebral hemisphere to a balloon located in the cavity of the removed hematoma according to the principle of communicating vessels. The craniospinal pressure gradient is removed by additional catheterization of the spinal subarachnoid space and its connection to the ventricle and balloon according to the same principle.

Thermodilution cardiac output measurement with a large left-to-right shunt.

Cardiac output was measured by the thermodilution method in a patient with a left-to-right shunt undergoing cardiac catheterization. It appeared that the thermodilution method measured systemic rather than pulmonary blood flow. This occurred because of the slow injection of injectate in the presence of a large left-to-right shunt. Theoretical thermodilution cardiac output curves are provided to illustrate the interaction of these two factors when three different durations of injection and four different shunt sizes are used.

[Iatrogenic injuries of the venous femoroiliac axis].

The occurrence of iatrogenous vascular accidents is underestimated. The authors describe 14 venous lesions to the femoro-iliac axis out of 60 iatrogenous vascular traumatisms. They consider the different aetiologies, underlining the disadvantages of femoral catheterisms and the main surgicalfactors contributing to venous traumatism. They emphasize the need for preventive measures in each discipline, and also the need for rapid diagnosis, thorough treatment, and careful post-operative after-care, bearing in mind secondary thrombo-embolic complications.

Sacral agenesis and neurogenic bladder dysfunction. A case report and review of the literature.

A case of partial sacral agenesis resulting in mixed upper and lower motor neurogenic bladder dysfunction is reported. Although the patient also has a concomitant congenital absence of one forearm she is being managed successfully on intermittent clean self-catheterization, illustrating the value of this modality in the management of neurogenic bladder dysfunction and that it can quite easily be managed by a young woman with only one functional hand. A review of the literature on this uncommon condition is also presented.

Echocardiography in congenital heart disease.

It has been demonstrated that echocardiography is a useful and accurate noninvasive technique for the study of congenital cardiovascular diseases. This method provides significant information in that invasive cardiac catheterization and angiocardiography can be planned with greater facility. This is especially important in severely ill babies in whom invasive diagnostic methods are risky. It is particularly attractive in neonates in whom the clinical differentiation of lethal heart disease from septic shock is difficult, since in the latter situation the echographic findings are normal.

Effects of 6-month customized home-based exercise on motor development, bone strength, and parental stress in children with simple congenital heart disease: a single-blinded randomized clinical trial.

BACKGROUND: New "noncardiac" problems in children with congenital heart disease (CHD), such as developmental delay or long-term neurodevelopmental impairments, have attracted considerable attention in recent years. It is hypothesized that exercise might attenuate CHD-associated neurodevelopmental impairments; however, this has not been thoroughly investigated. The objective of this prospective, single-blinded, randomized controlled experiment was to evaluate the impact of customized home-based exercise for children with CHD. METHODS: Children aged 0-5 years with echocardiography-confirmed sim

Technical Note: A procedure to place urinary catheters in 1- and 6-week-old preweaned Holstein heifer calves for the in vivo evaluation of intestinal permeability.

Oral administration of indigestible markers and subsequent urine collection is a useful method to determine in vivo gastrointestinal tract (GIT) permeability in cattle for research purposes. However, urine sampling techniques often rely on total waste collection, which reduces the ability to perform more frequent sampling and obtain accurate volumes and sterile samples. An alternative is urethral catheterization, though the feasibility of this technique has not been thoroughly tested in preweaned Holstein heifer calves. The study objective was to develop a urethral catheter placement procedure

Effect of a Musical Intervention During the Implantation of a Central Venous Catheter or a Dialysis Catheter in the Intensive Care Unit: A Prospective Randomized Pilot Study.

BACKGROUND: Listening to music may reduce anxiety during medical procedures. However, the magnitude of any effect may differ with respect to patient and procedure. We evaluated the effect of a musical intervention on patient anxiety during a central venous catheter or dialysis catheter implantation in an intensive care unit. METHODS: A prospective single-center controlled open-label 2-arm randomized trial was conducted in a medical intensive care unit (ICU) from February 2018 to February 2019. Patients undergoing central venous catheterization were randomized to listening to music or not duri

Immediate and Short-Term Outcome of Percutaneous Atrial Septal Defects Closure in Adult Patients.

Background Atrial septal defect secundum (ASD II) is the commonest of the congenital heart diseases in the adult population and the closure of ASD II causes a significant improvement in hemodynamics and the clinical status of the patient. However, it carries certain risks, especially the development of post-closure pulmonary edema and congestive cardiac failure, which may lead to death. Therefore, this study was designed to share our experience and to evaluate the immediate and short-term outcomes of percutaneous closure of ASD II in adult patients presenting at a tertiary care cardiac center

Use of Scaffolding Tissue Biografts To Bolster Vesicourethral Anastomosis During Salvage Robot-assisted Prostatectomy Reduces Leak Rates and Catheter Times.

BACKGROUND: One of the key factors contributing to morbidity associated with salvage radical prostatectomy is a significant vesicourethral anastomosis (VUA) disruption or postoperative tissue dehiscence in the region of the distal bladder neck that causes a large prolonged urinary leak, perineal pain, and delayed catheter removal. OBJECTIVE: To describe our surgical technique using a urinary bladder extracellular matrix (UB-ECM) scaffold incorporated into the base of the VUA and the distal bladder neck during salvage robot-assisted radical prostatectomy (sRARP) and to assess outcomes and safe

Management of early and late complications of ileocolonic continent urinary reservoir (Miami pouch).

PURPOSE: The object of this study is to review our experience and formulate a plan for early recognition and effective management of early and late complications seen in patients who have undergone construction of the ileocolonic continent urinary reservoir. METHOD: Charts of patients who underwent continent urinary diversion at the Division of Gynecologic Oncology, University of Miami School of Medicine, from 1988 to 1996 were reviewed. We analyzed our data in terms of early and late (beyond 6 weeks) complications resulting directly from the operation or from this form of urinary diversion.

New insights learned from the pulmonary to systemic blood flow ratio to predict the outcome in patients with hypoplastic left heart syndrome in the pre-Glenn stage: a single-center study.

BACKGROUND: To the best of our knowledge, no study has been made until now to determine whether the ratio between pulmonary and systemic blood flow (Qp/Qs) in the pre-stage II (PS2) or pre-Glenn stage can predict the outcome in patients with hypoplastic left heart syndrome (HLHS) who underwent Norwood (NW) palliation. PATIENTS AND METHODS: From January 2016 to August 2022, 80 cardiac catheterizations in 69 patients with HLHS in NW palliation stage with modified Blalock-Taussig shunt (MBTS) were retrospectively recruited. The Qp/Qs was measured under stable conditions using the Fick formula. N

Percutaneous Common Carotid Artery Access for Cardiac Interventions in Infants Does Not Acutely Change Cerebral Perfusion.

Pediatric cardiac interventions via percutaneous common carotid artery (CCA) access have been shown to be safe and effective. However, the impact of placement of a sheath in the carotid artery for interventions on cerebral perfusion is unknown. In this study we used cerebral near-infrared spectroscopy (NIRS) to analyze the effects of percutaneous CCA access for cardiac interventions on cerebral perfusion. This study is a retrospective chart review carried out at a tertiary care center on all pediatric patients who underwent percutaneous cardiac catheterization via carotid artery access from Ja

Recurrent catheter-related right atrial thrombosis in a hemodialysis patient: A case report and literature review.

Right atrial thrombus is an uncommon yet potentially life-threatening complication associated with central venous catheterization. We report an unusual case of catheter-associated right atrial thrombosis (CRAT) in a patient receiving maintenance hemodialysis, who presented with recurrent thrombus on the fifth day following surgical thrombectomy. This report summarizes the clinical presentation, diagnostic modalities, and management strategies for CRAT. Our intention is to improve clinicians' awareness of this complication and provide practical insights into optimal management in hemodialysis p

Transcatheter Management of Pulmonary Vein Stenosis in Children.

Pediatric intraluminal pulmonary vein stenosis (PVS) is a challenging condition where the diagnosis is confirmed via cardiac catheterization. The patient population is complex with inherent hemodynamic vulnerabilities, and the high rate of restenosis necessitates recurrent, technically demanding transcatheter interventions. Furthermore, decisions made during each procedure can have a lasting impact on future care. To address these challenges, we present a guideline for transcatheter interventions for PVS, informed by both published literature and the Authors' clinical experience.

Infarcts of a Cardioembolic Source Mimicking Lacunar Infarcts: Case Series With Clinical and Radiological Correlation.

Lacunar strokes are the hallmark of cerebral small vessel disease. There are several well-established mechanisms for the pathogenesis of lacunar stroke, but the cardioembolic mechanism is not well-established. Three cases of acute ischemic stroke following elective cardiac and cerebral catheterization are reported. These cases had typical lacunar-looking infarcts on neuroimaging despite strong evidence of an embolic source with temporal correlation. Awareness of such findings and pathogenesis may help investigational workup and management of these patients.

Right Heart Failure With Recurrent Pericardial Effusion Mimicking Effusive-Constrictive Pericarditis Several Years After Renal Transplantation.

Pericardial disease is a rare complication after renal transplantation. We present a patient who developed high-output cardiac failure from a large arteriovenous (AV) fistula with recurrent pericardial effusion resulting in a constrictive hemodynamic pattern that was revealed during cardiac catheterization. Pericardiectomy was considered for recurrent effusive pericarditis, but per cardiac surgery recommendations, closure of the AV fistula dramatically cured the patient's heart failure, and no recurrence of pericardial effusion was seen during follow-up almost a year later.
