Early Intervention for Children With Cleft Palate: First Report on the Wechsler Preschool and Primary Scale of Intelligence at Age Four.

In recent years, the necessity of early intervention in speech therapy for children with cleft palate has become widely recognized. To achieve more effective treatment outcomes through comprehensive early intervention, we launched an Early Intervention Program (EI Program) in 2010 at the Department of Speech and Orofacial Function Development, Aichi Gakuin University Dental Hospital. As part of a series of studies to evaluate the effectiveness of the EI Program, the present study aimed to examine the relationship between participation in the EI Program and cognitive development in children wit

Scalable, sustainable cost-effective surgical care: a model for safety and quality in the developing world, part I: challenge and commitment.

BACKGROUND: With an estimated backlog of 4,000,000 patients worldwide, cleft lip and cleft palate remain a stark example of the global burden of surgical disease. The need for a new paradigm in global surgery has been increasingly recognized by governments, funding agencies, and professionals to exponentially expand care while emphasizing safety and quality. This three-part article examines the evolution of the Operation Smile Guwahati Comprehensive Cleft Care Center (GCCCC) as an innovative model for sustainable cleft care in the developing world. METHODS: The GCCCC is the result of a unique

Nasoalveolar molding in cleft care: is it efficacious?

In the era of evidence-based medicine, new treatment protocols and interventions should be routinely evaluated for their efficacy by reviewing the available evidence. In the cleft literature, nasoalveolar molding has garnered attention over the last decade as a new option for improving nasal form and symmetry before primary surgical repair. Systematic review of the evidence is, however, currently lacking. This review evaluates whether nasoalveolar molding can improve nasal symmetry and form toward the norm, as well as whether nasoalveolar molding demonstrates advantages over other protocols in

Are Mesenchymal Stem Cells Implanted in Lip Defect Capable of Returning Orbicularis Oris Muscle Function?

INTRODUCTION: This study aimed to evaluate the amount of orbicularis oris muscle formation and function after using scaffold-free cells and mesenchymal stem cells scaffold in repaired cleft lip of rats. METHODS: Forty male Wistar rats were used. A triangular cut (7 × 7 × 4  mm) was made on the upper lip of 30 rats and then divided into 3 equal groups. In the first group (control), the defected cuts were closed by routine surgical procedures. The second group consisted of cell scaffold and the third scaffold-free cells (5 × 5 × 3) in the area of the incision. Two months after restoration, muscl

Hemiscrotal agenesis with complete testicular descent in Van der Woude syndrome: a new phenotypic feature.

Van der Woude syndrome (VWS) and popliteal pterygium syndrome (PPS) spectrum are due to genetic variants in the IRF6 which phenotypically has been known to manifest with midline defects such as cleft lip and palate in VWS and additional nail, limb and genital anomalies in PPS. We report a case of VWS with the previously unrecognised phenotypic feature of hemiscrotal agenesis. While bifid scrotum has been reported in the more severe PPS, neither VWS nor PPS have previously noted hemiscrotal agenesis as part of the phenotypic picture. Hemiscrotal agenesis without evidence of any genetic anomaly

Early release of interalveolar synechiae under general anesthesia through fiberscopic nasal intubation.

This article presents a treatment strategy for early release of interalveolar synechiae, aiming to facilitate early oral feeding and prevent temporomandibular joint ankylosis. The treatment results of 2 patients with van der Woude syndrome were retrospectively studied. Both patients underwent early surgical release of interalveolar synechiae under general anesthesia through fiberscopic nasal intubation. The 2 patients were treated at the ages of 6 and 14 days, respectively. The interincisival distances increased from 5 and 6 mm preoperatively to 11 and 10 mm immediately after surgery. This was

Orofacial clefts, parental cigarette smoking, and transforming growth factor-alpha gene variants.

Results of studies to determine whether women who smoke during early pregnancy are at increased risk of delivering infants with orofacial clefts have been mixed, and recently a gene-environment interaction between maternal smoking, transforming growth factor-alpha (TGFa), and clefting has been reported. Using a large population-based case-control study, we investigated whether parental periconceptional cigarette smoking was associated with an increased risk for having offspring with orofacial clefts. We also investigated the influence of genetic variation of the TGFa locus on the relation betw

Nostril and alar reshaping.

The authors recommend, when necessary, the use of external excisions for correcting variations in the shape of the nostrils and alae in the cleft lip-nose deformity, in primary rhinoplasties, and in secondary rhinoplasties. Their experience in the use of external incisions was originally stimulated by Millard's paper of 1960 which recommended different types of remodeling external excisions in his analysis of how to handle variations in the shape of the nostrils and alae.

Maternal Folic Acid, B12, and Zinc Serum Levels and Risk of Orofacial Cleft in Offspring in Tabriz.

BACKGROUND: Orofacial clefts (OFCs) are prevalent congenital malformations affecting approximately 1 in 700 live births. The etiology of OFCs is multifactorial, encompassing both genetic predispositions and environmental factors, with maternal nutritional status, particularly folic acid, vitamin B12, and zinc levels, playing a crucial role in fetal development. This study investigates the association between maternal serum micronutrient levels and the risk of OFCs in Tabriz. METHODS: A case-control study was conducted over six months at the Children's Hospital of Tabriz University of Medical

The orofacial cleft risk gene IRF6 is a target gene of SOX9 in cranial neural crest cells.

Orofacial clefts represent the most prevalent congenital anomalies affecting the craniofacial region. They can be evoked by a disturbed development of either oral epithelium or cranial neural crest-derived mesenchyme, or by disruptions of their interplay. IRF6 is a well-known risk gene associated with non-syndromic orofacial clefts and causative for Van der Woude syndrome, an autosomal dominant syndrome characterized by orofacial clefting and lower lip pits. Its expression in the oral epithelium depends on the transcription factor TFAP2A. We here show by immunofluorescence on mouse embryonic s

Prevalence and Associated Factors of Malnutrition among Children Under Five Years with Clefts Attending a Hospital in Rwanda.

BACKGROUND: Children with clefts face a high risk of malnutrition (undernutrition) with a range of serious life-threatening challenges. The objective of this study was to estimate the prevalence of malnutrition in children with clefts in Rwanda. METHODS: A retrospective cross-sectional study was conducted among children with clefts admitted at a Hospital in Rwanda between 2018 and 2022. Descriptive statistics were conducted, and WHO Z-scores were used to classify nutritional status (stunting, wasting, underweight, and overall malnutrition). Subsequently, chi-square and logistic regression ana

Oral clefts and maternal biomarkers of folate-dependent one-carbon metabolism in Utah.

BACKGROUND: Maternal folate intake and related biomarkers have been inconsistently associated with a risk of oral clefts. METHODS: Maternal concentrations of plasma folate (PF) and erythrocyte folate (EF), plasma pyridoxal-5'-phosphate (PLP; active vitamin B(6) ) and total plasma homocysteine (tHcy) were measured in a Utah study with 347 cases and 469 controls. RESULTS: Risk of all clefts combined, including cleft lip with or without cleft palate (CL/P) and cleft palate only (CP), was 65% lower in the highest versus lowest PF quartile (odds ratio [OR], 0.35; 95% confidence interval [CI], 0.2

Yunis-Varon Syndrome.

Yunis-Varon syndrome is a rare autosomal recessive disorder with characteristic facial features and limb anomalies. We report a neonate born to consanguineously married normal parents with typical clinical and radiologic features of Yunis-Varon syndrome along with complete cleft lip and palate: an infrequent association. The family had two previous babies with similar features who died in infancy. This is a first reported case of Yunis-Varon syndrome in Pakistan.

Diagnosis and management of patients with clefts: a comprehensive and interdisciplinary approach.

This article reviews the most current practice guidelines in the diagnosis and management of patients born with cleft lip and/or palate. Such patients frequently have multiple medical and social issues that benefit greatly from a team approach. Common challenges include feeding difficulty, nutritional deficiency, speech disorders, hearing problems, ear disease, dental anomalies, and both social and developmental delays, among others. Interdisciplinary evaluation and collaboration throughout a patient's development are essential.

Craniofacial anomalies: from development to molecular pathogenesis.

Advances in developmental biology combined with progress in human genetics are helping us decipher how the craniofacial region develops and how the consequences of misdirected development result in malformation. This review describes the molecular etiology of a number of craniofacial developmental anomalies. The more common craniofacial anomalies cleft lip and palate and craniosynostosis, as well as cleidocranial dysplasia, hemifacial microsomia, holoprosencephaly, enlarged parietal foramina, Treacher Collins syndrome and cherubism are discussed.

Earliest evidence for arthrogryposis multiplex congenita or Larsen syndrome?

A sixteenth-century illustrated pamphlet from Great Britain suggests that documentary evidence may permit accurate diagnosis of pathological conditions in earlier societies. The document is of particular importance, since the presented congenital abnormalities, including cleft lip, spina bifida cystica, genu recurvatum, and talipes deformity are reported rarely in archaeological skeletal material. It is suggested that the combination of abnormalities may represent the earliest case of arthrogryposis multiplex congenita or Larsen syndrome.
