[A Study of Local Anesthesia Surgery Cases for Elderly Breast Cancer Patients].

The standard treatment for breast cancer surgery is for the patient to be under general anesthesia, but many elderly patients are at high risk under general anesthesia. In some cases, it is possible to do surgery under local anesthesia, but there have been few reports evaluating the usefulness of such surgery. Therefore, we investigated this in 17 patients at our hospital. The mean age was 78 years old(median 83, 42-98), and mean tumor diameter 14 mm(median 13, 3-40). The reasons why local anesthesia was selected were due to old age, comorbidities, diagnostic treatment, and difficulty of hospi

Frailty Focus: Empowering Rural Health With Advanced Nurse Practitioners: A Discussion Paper.

BACKGROUND: Frailty is a multidimensional condition with symptoms relating to falls, immobility, incontinence, impaired memory and medication side-effects. With increasing numbers of frailty, particularly in rural areas, healthcare systems are being challenged globally. Moreover, frailty may be more common in rural communities as a consequence of transportation issues, limited access to healthcare services and health promotion activities. Advanced Nurse Practitioners are ideally placed to undertake comprehensive geriatric assessments and identify frailly syndromes. AIMS: Explore the function

The effects of remimazolam and propofol on delirium following anesthesia and sedation in elderly patients: a systematic review and meta-analysis.

BACKGROUND: Delirium is a prevalent and serious postoperative complication in elderly patients, with an incidence reaching up to 70%, particularly in intensive care settings. It is associated with prolonged recovery, increased healthcare costs, and heightened mortality risk. While multifactorial etiologies involving neurotransmitter dysregulation and inflammatory cascades underline its pathogenesis, anesthetic agent selection has emerged as a modifiable risk factor requiring rigorous evaluation. OBJECTIVE: This meta-analysis aims to compare the effects of remimazolam and propofol on the incide

Clinical Value of Nutritional Assessment Tools in Older Adults with Chronic Obstructive Pulmonary Disease: An Integrative Review.

Chronic Obstructive Pulmonary Disease (COPD) is a progressive respiratory disorder frequently observed in older adults and is often accompanied by aging-related conditions such as sarcopenia, cognitive impairment, and immune dysfunction. Malnutrition is a major yet underrecognized contributor to poor outcomes in this population, accelerating disease progression and functional decline. This integrative review critically examines the clinical value of three widely used nutritional assessment tools-the Mini Nutritional Assessment (MNA), the Patient-Generated Subjective Global Assessment (PG-SGA),

Factors associated with delirium in a real-world acute-care setting: analysis considering the interdependence of clinical variables with the frailty syndrome.

PURPOSE: Delirium risk assessment in the acute-care setting generally does not account for frailty. The objective of this retrospective study was to identify factors associated with delirium, considering the interdependency of clinical variables with frailty syndrome in complex older patients. METHODS: The clinical records of 587 participants (248 M, median age 84) were reviewed, collecting clinical, anamnestic and pharmacological data. Frailty syndrome was assessed with the Clinical Frailty Scale (CFS). Delirium was the main study endpoint. The correlations of the considered anamnestic and c

Musculoskeletal health and life-space mobility in older adults: Findings from the Hertfordshire Cohort Study.

This study explores the relationship between musculoskeletal conditions of ageing and life-space mobility (LSM) in 1110 community-dwelling older adults from the Hertfordshire Cohort Study. LSM is a novel measure which captures ability to mobilise within the home, locally and more widely. Among men, older age, care receipt, not driving a car, lower wellbeing, and reduced physical function were associated with lower LSM, while in women only driving status and physical function were associated with LSM. Osteoporosis, arthritis, and fractures had no significant associations with LSM in either gend

Eyeball Test for the Assessment of Frailty in Elderly Patients With Cardiovascular Disease: A Prospective Study.

Frailty has been associated with poor outcomes in patients with cardiovascular diseases (CVDs). We aimed to assess the accuracy of the Eyeball test for frailty assessment in elderly patients with CVD. This is a prospective study including stable patients ≥75 years old who were followed-up in a cardiology clinic. Frailty assessment was performed separately through the Eyeball test and the Fried test in a blinded way. Cardiologists were asked to rate the frailty status of participants based on their routine clinical assessment and grade frailty on a Fried-type scale (1 to 5, with frailty defined

Untangling the Complex Interplay between Social Isolation, Anorexia, Sarcopenia, and Mortality: Insights from a Longitudinal Study.

BACKGROUND: Social isolation is a pervasive and debilitating condition that has adverse prognostic impacts. This condition often co-occurs with other geriatric syndromes, further exacerbating negative health outcomes. Given these considerations, the present study aims to elucidate the roles of social isolation in older adults with anorexia of aging and/or sarcopenia with respect to long-term mortality using a nationally representative cohort study. METHODS: Data were obtained from the Taiwan Longitudinal Study on Aging (TLSA), with a sample size of 3,762 study participants aged 50 years and o
