Nafamostat Mesylate Protects Against Acute Renal Ischemia-Reperfusion Injury by Alleviating Mitochondrial Dysfunction, Inhibiting Ferroptosis, and Regulating Proximal Tubular Cells: A Multi-Omics Analysis.

OBJECTIVE: The present study aims to explore the role of nafamostat mesylate (NM) in ischemia-reperfusion (I/R)-induced acute kidney injury (AKI). METHODS: Twenty-one male rats were randomly divided into three groups: sham, I/R, and I/R + NM groups. Acute renal I/R injury models were created in the I/R and I/R + NM groups by right nephrectomy and clamping the left renal pedicle for 45 min. Rats in the I/R + NM group were intraperitoneally injected with 0.75 mg/kg of NM before modeling. Blood and kidney specimens were collected at 24 h after model establishment. RNA sequencing (RNA-seq), KEGG

Suture-free polyglactin 910 mesh repair of kidney graft rupture: A case report and review of literature.

BACKGROUND: Transplantectomy has long been considered the preferred treatment for spontaneous renal graft rupture, prioritizing patient safety over kidney salvage. In the last decade, there has been an increasing number of reports showing that, in selected scenarios, conservative management through graft repair represents a feasible option, challenging traditional approaches. CASE SUMMARY: We describe the case of a 37-year-old sensitized XY patient who experienced early spontaneous graft rupture after receiving his second deceased donor kidney transplant. Following temporary hemodynamic stabi

Kidney Injury Molecule-1 as a Biomarker for Renal Cancer: Current Insights and Future Perspectives-A Narrative Review.

Kidney injury molecule-1 (KIM-1) is a transmembrane protein that is significantly upregulated in renal cells following injury. It has considerable potential as a biomarker for diagnosing and monitoring renal cell carcinoma (RCC). This review examines KIM-1 expression across multiple biological sources-including tissue, blood, and urine-and highlights its strong association with RCC risk. Clinical studies have shown that KIM-1 levels decline within weeks after nephrectomy, underscoring its utility in assessing therapeutic response. Additionally, urinary KIM-1 levels correlate with histopatholog

Beyond Boundaries: Metastasis of Cervical Squamous Cell Carcinoma to the Kidney.

Cervical squamous cell carcinoma (SCC) is an aggressive gynecological malignancy that commonly metastasizes to the lungs, liver, and bones; renal involvement is extremely rare. We report the case of a 68-year-old woman with a history of high-grade cervical SCC, treated a decade earlier with hysterectomy and bilateral salpingo-oophorectomy, who recently presented with right flank pain and hematuria. Imaging revealed a mass involving the bladder and right distal ureter causing hydronephrosis. She underwent a cystoscopy with right ureteral stenting and transurethral resection of the bladder tumor

Identifying Hereditary Leiomyomatosis and Renal Cell Cancer through Unobtrusive Cutaneous Nodules: A Clinical Report.

Cutaneous small vessel vasculitis (CSVV) is an immune-mediated inflammatory disorder affecting the small dermal vessels and is often linked to autoimmune diseases, infections, or malignancies. Renal cell carcinoma (RCC), a common urologic malignancy, rarely presents with CSVV as a paraneoplastic manifestation. This case describes a 68-year-old woman with a history of left radical nephrectomy for clear cell RCC who presented with persistent flank pain and progressive purpuric, painful lesions on her lower limbs. Imaging revealed recurrent metastatic disease in the left retroperitoneum with infe

Wunderlich Syndrome as an unusual presentation of Microscopic Polyangiitis: A Case report with Review of Literature.

Wunderlich syndrome, characterized by spontaneous perinephric hematoma with subcapsular extension has been scarcely reported in microscopic polyangiitis (MPA). We report the case of a 45-year-old woman, who presented with constitutional symptoms, left-eye episcleritis, and rapidly progressive glomerulonephritis. She developed sudden, severe left flank pain with hemoglobin drop two days after admission. Both computed tomography (CT) and non-contrast magnetic resonance imaging revealed large left-sided perinephric hematoma. CT angiography failed to demonstrate intrarenal aneurysms. A remarkable

Paraneoplastic Trichomegaly as a Harbinger of Metastatic Renal Cell Carcinoma: A Case Report.

Renal cell carcinoma (RCC) is a common malignancy, often diagnosed incidentally on cross-sectional imaging. While classic symptoms such as flank pain, hematuria, and a palpable abdominal mass are well-known, RCC may present with more insidious findings. Here, we describe a case of paraneoplastic trichomegaly as the initial manifestation of metastatic RCC. A man in his 60s presented with progressive eyelash lengthening and mild hypertrichosis of the extremities and beard. Medication review and initial endocrine workup were unrevealing. Laboratory tests showed iron deficiency anemia, and subclin

Imaging Pitfalls of Multilocular Cystic Renal Neoplasm of Low Malignant Potential: A Case Report.

Multilocular cystic renal neoplasm of low malignant potential (MCRNLMP) is a rare subtype of renal tumor classified within the spectrum of clear cell renal tumors, and it is characterized by an excellent prognosis. Histologically, it is composed of multiple cysts lined by low-grade clear cells and separated by fibrous septa, with no solid expansile growth or significant nuclear atypia. Few reports have documented MCRNLMP mimicking a solid mass, especially in elderly patients. We report the case of an 80-year-old male in whom a left renal mass was incidentally detected during abdominal imaging

Acquired cystic disease-associated renal cell carcinoma: A case report.

INTRODUCTION: Acquired cystic kidney disease (ACKD) affects almost half of patients with chronic hemodialysis. ACKD is recognized as a predisposing condition for renal carcinoma, particularly in patients undergoing long-term dialysis. Acquired cystic disease-associated renal cell carcinoma (ACD-RCC) is a subtype of renal cell carcinoma (RCC) occurring solely in patients with ACKD. ACD-RCC was first introduced as a distinct entity in the 2016 World Health Organization (WHO) classification of renal tumors and retained in the 2022 update. However, it remains underdiagnosed due to histologic overl

Higher abdominal fat area associates with lower donor kidney function before and after living kidney donation.

Central body fat distribution affects kidney function. Abdominal fat measurements using computed tomography (CT) may prove superior in assessing body composition-related kidney risk in living kidney donors. This retrospective cohort study including 550 kidney donors aimed to determine the association between CT-measured abdominal fat areas and kidney function before and after donor nephrectomy. Donors underwent glomerular filtration rate measurements (125I-Iothalamate, mGFR) before and 3 months after donation. Linear regression analyses with body surface area (BSA)-standardized and crude mGFR

[Bilateral multifocal kidney abscess due to Candida albicans].

Renal abscesses are a rare complication of urinary tract infections and may be associated with increased morbidity and mortality. Most cases occur in patients with predisposing factors such as immunosuppression. Diagnosis requires high clinical suspicion and its treatment consists in the use of parenteral antibiotics and antifungals associated or not with surgical interventions such as nephrostomy and nephrectomy. Few cases have been published in the medical literature of multifocal bilateral renal abscesses and even fewer due to Candida albicans. We present the case of a 20-year-old woman wit

Adjuvant immunotherapy for locally advanced renal cell carcinoma.

INTRODUCTION: Locally advanced renal cell carcinoma (RCC) presents a therapeutic challenge due to 20-40% relapse risk post-nephrectomy. There has been substantial interest in utilizing immunotherapy interrupting the PD-1/PD-L1 axis in the perioperative space, especially in the adjuvant setting, in order to minimize such risk. AREAS COVERED: We conducted a PubMed search using the terms 'adjuvant' and 'RCC.' We begin by examining landmark studies in the postoperative space for locally advanced RCC, with special emphasis on immunotherapeutic biologics. Important considerations are outlined in an

Dietary Mg Supplementation Decreases Oxidative Stress, Inflammation, and Vascular Dysfunction in an Experimental Model of Metabolic Syndrome with Renal Failure.

BACKGROUND: Metabolic syndrome (MetS) and chronic kidney disease (CKD) are commonly associated with cardiovascular disease (CVD) and in these patients Mg concentration is usually decreased. This study evaluated whether a dietary Mg supplementation might attenuate vascular dysfunction through the modulation of oxidative stress and inflammation in concurrent MetS and CKD. METHODS: A rat model of MetS (Zucker strain) with CKD (5/6 nephrectomy, Nx) was used. Nephrectomized animals were fed a normal 0.1%Mg (MetS+Nx+Mg0.1%) or a supplemented 0.6%Mg (MetS+Nx+Mg0.6%) diet; Sham-operated rats with Met

Multi-omic analysis of human kidney tissue identified medulla-specific gene expression patterns.

The kidney medulla is a specialized region with important homeostatic functions. It has been implicated in genetic and developmental disorders along with ischemic and drug-induced injuries. Despite its role in kidney function and disease, the medulla's baseline gene expression and epigenomic signatures have not been well described in the adult human kidney. Here we generated and analyzed gene expression (RNA-seq), chromatin accessibility (ATAC-seq), chromatin conformation (Hi-C) and spatial transcriptomic data from the adult human kidney cortex and medulla. Tissue samples were obtained from ma

Bilateral renal mucormycosis following COVID-19 infection: A therapeutic challenge.

India witnessed an epidemic of mucormycosis during the second wave of the COVID-19 pandemic. Renal mucormycosis has been reported rarely, mostly from India, but only 2 cases have been reported following COVID-19 infection to date. We report a case of mucormycosis predominantly affecting kidneys in a young and previously healthy male following COVID-19 pneumonia, for which he had received corticosteroid, remdesivir, and tocilizumab. He presented with hematuria, progressive oliguria, and severe acute kidney injury (AKI) requiring dialysis. The diagnosis was made on kidney biopsy and contrast-enh

Endothelium-Specific Deficiency of Polycystin-1 Promotes Hypertension and Cardiovascular Disorders.

BACKGROUND: Autosomal dominant polycystic kidney disease is the most frequent hereditary kidney disease and is generally due to mutations in PKD1 and PKD2, encoding polycystins 1 and 2. In autosomal dominant polycystic kidney disease, hypertension and cardiovascular disorders are highly prevalent, but their mechanisms are partially understood. METHODS: Since endothelial cells express the polycystin complex, where it plays a central role in the mechanotransduction of blood flow, we generated a murine model with inducible deletion of Pkd1 in endothelial cells (Cdh5-CreERT2;Pkd1fl/fl) to specifi
