Impact of awake prone positioning duration on intubation or mortality in COVID-19 patients with acute respiratory failure: secondary analysis of a randomized clinical trial.

BACKGROUND: Compared with shorter awake prone positioning (APP), prolonged APP (≥ 12 h daily) reduces the intubation rate in patients with COVID-19-related acute hypoxemic respiratory failure (AHRF). However, the optimal APP duration is uncertain. In this secondary analysis, we aimed to explore whether a longer APP duration is associated with improved outcomes and to identify the optimal duration of APP. METHODS: Data from a multicenter randomized controlled trial involving nonintubated COVID-19 patients with AHRF were analyzed. Daily APP duration over 7 days after randomization was recorded

Progression-Free and Overall Survival After KRd Consolidation in FDG PET/CT Positive Patients After ASCT: A Phase II Study (CONPET).

BACKGROUND: In multiple myeloma, 18F-Fluorodeoxyglucose (FDG) positron emission tomography (PET) positivity after first-line autologous stem cell transplantation (ASCT) is associated with reduced progression-free survival (PFS) and overall survival (OS). Intensified treatment may abrogate the adverse effects of a positive PET after ASCT, but this treatment strategy has not been evaluated in prospective clinical trials. PATIENTS AND METHODS: In this phase II clinical study, patients who were in at least very good partial response (VGPR) after ASCT and had a positive PET received four cycles of

HLA-B*15:01-positive severe COVID-19 patients lack CD8+ T cell pools with highly expanded public clonotypes.

Understanding host factors driving asymptomatic versus severe disease outcomes is of key importance if we are to control emerging and re-emerging viral infections. HLA-B*15:01 has been associated with asymptomatic SARS-CoV-2 infection in nonhospitalized individuals of European ancestry, with protective immunity attributed to preexisting cross-reactive CD8+ T-cells directed against HLA-B*15:01-restricted Spike-derived S919-927 peptide (B15/S919+CD8+ T-cells). However, fundamental questions remained on the abundance and clonotypic nature of CD8+ T-cell responses in HLA-B*15:01-positive patients

Steroid-responsive autoimmune psychosis with cerebrospinal fluid-restricted oligoclonal bands and positive tissue-based assay findings: a case series of three patients.

BACKGROUND: Partial psychotic disorders such as autoimmune psychosis do not respond adequately to conventional antipsychotic drugs or psychotherapy. Autoimmune psychosis presents with isolated psychiatric symptoms, responds to immunotherapy, and differs from typical autoimmune encephalitis. Previous study has proposed an approach to identifying autoimmune psychosis, however, further research remains necessary to enhance our understanding of this condition. CASE PRESENTATION: We describe 3 patients presenting with acute or recurrent psychiatric symptoms (patient 1, 2 and 3), accompanied by cog

Management of diabetes in patients hospitalized for acute cardiac events: Joint position paper from the French Society of Cardiology and the French-speaking Diabetes Society.

Patients with type 2 diabetes, but also older patients with type 1 diabetes, are at major risk of cardiovascular morbidity and death. After an acute cardiac event, the prognosis of patients with diabetes is impaired, with clear increases in in-hospital and long-term morbidity and deaths. Both hyper- and hypoglycaemia are deleterious after an acute cardiac event, and the decision to start intravenous insulin is often challenging. Moreover, some antidiabetic treatments have cardioprotective effects, and the onset of an acute cardiac event provides an opportunity to shift to these treatments. The

Safe and transparent introduction and evaluation of targeted axillary dissection in patients with node-positive breast cancer undergoing primary surgery: international consensus process.

BACKGROUND: Axillary node clearance is the current standard of care in patients with node-positive breast cancer undergoing primary surgery, despite a lack of evidence to demonstrate survival benefit and high rates of life-changing morbidity. Targeted axillary dissection (TAD) may be a safe alternative to axillary node clearance, but there is no agreement how primary TAD should be performed. TADPOLE-TOGETHER aimed to use international consensus methods to agree the key components of primary TAD to promote standardized introduction and evaluation of the technique within the TADPOLE trial. Can axillary lymph node dissection be omitted in breast cancer patients with 1-2 positive sentinel nodes? A systematic review and meta-analysis.

PURPOSE: Axillary lymph node dissection (ALND) has traditionally been recommended for breast cancer patients with positive sentinel lymph nodes (SLNs). Although omitting ALND is now widely accepted for patients undergoing breast-conserving surgery (BCS) with limited sentinel lymph node biopsy (SLNB) involvement, based on trials such as Z0011, evidence for patients undergoing total mastectomy (TM) remains limited and conflicting. This meta-analysis aimed to evaluate whether ALND can be safely omitted in TM patients with 1-2 positive SLNs by comparing survival outcomes between ALND and SLNB alon

False-positive results of adrenal venous sampling due to mild autonomous cortisol secretion in a patient with primary aldosteronism: a case report with review of literature.

We report the case of a 50-year-old female diagnosed with primary aldosteronism (PA) complicated with mild autonomous cortisol secretion (MACS). The patient had a 5-year history of hypertension, and screening revealed an elevated aldosterone-to-renin ratio (ARR). Although she demonstrated no clinical features of Cushing's syndrome, her serum potassium level was at the lower end of the normal range. The baseline plasma renin activity was <0.2 ng/mL/h, and the plasma aldosterone concentration was 165 pg/mL. Confirmatory tests supported the diagnosis of PA, and computed tomography (CT) revealed a

OBJECTIVE: Patients with Helicobacter pylori-positive gastric dysplasia are at a higher risk of progressing to gastric cancer (GC). Systemic inflammatory markers have been identified as predictors of poor prognosis in patients with GC. However, their role in gastric dysplasia remains elusive. The aim of this study is to evaluate the utility of systemic inflammatory markers as predictors in patients with H. pylori-positive gastric dysplasia. METHODS: This study included a total of 664 normal individuals and patients diagnosed with chronic gastritis from the Yangzhou screening cohort, as well a

A Case of Syndrome of Inappropriate Secretion of Antidiuretic Hormone Induced by Selpercatinib in a Patient With RET Fusion Gene-Positive Non-Small Cell Lung Cancer.

An 80-year-old woman with a lung nodule in the right lower lobe and pleural thickening was diagnosed with lung adenocarcinoma by a surgical lung biopsy. The oncogene panel test showed a positive RET-fusion gene mutation, and selpercatinib was administered as a first-line treatment. She developed severe hyponatremia and was subsequently diagnosed with the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). Discontinuing selpercatinib and initiating demethyltetracycline gradually improved SIADH. After confirming that sodium levels had returned to normal, we resumed and continued

Household Clustering of High-Risk Contacts in Smear-Positive TB Patient Families: Evidence for Hotspot Households and Risk Stratification in Rural Eastern Cape.

BACKGROUND: Household contacts of smear-positive tuberculosis (TB) patients face an elevated risk of infection and disease progression, particularly young children and individuals living in overcrowded households. Despite WHO recommendations for systematic contact screening and provision of TB preventive therapy (TPT), implementation remains suboptimal in high-burden rural areas. This study aimed to develop a practical framework for identifying and prioritizing high-risk families by examining demographic predictors, household clustering, and machine learning-based risk models.
