Thiazide diuretics are widely used to treat hypertension, but their use is associated with impaired glucose tolerance. We propose that the diabetogenic action of thiazides may be due to their ability to open calcium-activated potassium (KCa) channels in pancreatic beta-cells.

A previously unreported association between bilateral abductor paralysis and hypokalaemia is reported in a patient presenting with stridor. Both the stridor and the bilateral abductor paralysis gradually resolved as the serum potassium was corrected. A casual link is suggested though the mechanism is unclear.

Cold exposure increases feeding, thermogenesis, and energy expenditure to maintain body temperature and energy balance, but the underlying neural mechanisms remain unclear. Here, we identify cold-responsive neurons in the dorsal posterior periventricular hypothalamus that coordinate these adaptations. Activation of GABAergic or cold-activated neurons increased food intake, body temperature, and brown adipose tissue thermogenesis, whereas their inhibition attenuated cold-induced hyperphagia and thermogenesis. Single-nucleus RNA sequencing identified Lef1 as a marker of cold-activated neurons in

We present the case of a 63-year-old male who sustained a penetrating soft tissue injury to the right thigh from a commercial firework. Following uncomplicated surgical debridement and discharge, the patient returned within hours exhibiting rapidly progressive ascending paralysis, bulbar weakness, and respiratory failure requiring intubation. Laboratory evaluation revealed profound hypokalemia (1.4 mmol/L), hypophosphatemia, and rhabdomyolysis. The clinical presentation, coupled with the patient's report of the firework emitting a green flare, is most consistent with acute barium toxicity. For

Enzalutamide resistance remains a critical obstacle in the treatment of castration-resistant prostate cancer (CRPC), with potassium calcium-activated channel subfamily N member 4 (KCNN4) emerging as a key mediator of therapeutic failure. Here, we demonstrate that KCNN4 is significantly upregulated in enzalutamide-resistant PCa cells and clinical tissues, correlating with poor prognosis. Mechanistically, p300, a histone acetyltransferase, directly binds to KCNN4 and mediates its acetylation at lysine 16, which competitively inhibits ubiquitination-mediated degradation, thereby stabilizing KCNN4

Super-bulky bis(anilido)acridanide pro-ligands (NNNAr)H3 (NNNAr = 4,5-bis(anilido)-2,7,9,9-tetramethylacridanide, anilido = NAr, Ar = DCHP, 2,6-dicyclohexylphenyl; TCHP, 2,4,6-tricyclohexylphenyl) have been prepared in high yields. The reaction of (NNNTCHP)H3 with trimethylaluminium affords bimetallic complex [Al2Me3(NNNTCHP)] which reacts with iodine to form the corresponding monometallic complex [Al(NNNTCHP)(I)2]. The reduction of [Al(NNNTCHP)(I)2] with sodium and potassium containing reducing agents was explored. From potassium reductions, only complexes K[Al(NNNTCHP)(I)] (from K/KI) and [A

The inhibitory effect of exogenous nitric oxide (NO) on the open state of ATP-sensitive potassium channels (K ATP) and its underlying mechanism remain unclear. In this study, patch-clamp and molecular biology techniques are used to investigate this issue. In acutely isolated rat mesenteric artery smooth muscle cells and human embryonic kidney 293 cells (HEK293) expressing inwardly rectifying potassium channel 6.1 subunit/sulfonylurea receptor 2B subunit (Kir6.1/SUR2B), sodium nitroprusside (SNP) was found to significantly inhibit the activity of open K ATP channels. Detection using biotin-labe

Cardiomyopathies are a major category of cardiovascular disease, significantly contributing to global morbidity and mortality. Their pathogenesis involves a complex interplay of genetic and environmental factors, with multiple gene families playing critical roles in disease progression. Among these, the Solute Carrier 12A (SLC12A) family encodes membrane cation-coupled chloride cotransporters (CCCs), which are essential for ion transport and cellular homeostasis. By regulating intracellular chloride and potassium concentrations, SLC12A/CCC transporters may directly influence myocardial electro

Spironolactone is a mineralocorticoid receptor antagonist with potassium-sparing effects used in the management of heart failure to minimize morbidity and mortality. It is typically given in combination with an angiotensin-converting enzyme inhibitor (ACEI) as part of standard management of pediatric heart failure but has limited literature regarding safety and efficacy in this population. We report a case of probable concomitant enalapril and spironolactone-induced hyperkalemia in a 2-month old female. The patient presented with Class III congestive heart failure and was initiated on enalapri

Gastrointestinal amyloidosis is a rare condition with diverse clinical and endoscopic manifestations. Among these subtypes, localized gastric AL amyloidosis is rare and associated with lymphoproliferative disorders. Here, we report a case of gastric mucosa-associated lymphoid tissue (MALT) lymphoma diagnosed during follow-up of localized gastric AL (κ) amyloidosis. A 65-year-old woman who underwent periodic endoscopic screening was found to have a localized erythematous lesion with erosion in the lower gastric body. Histopathological examination revealed amyloid deposits. Although potassium pe

Hypokalemic periodic paralysis (HPP) is an uncommon but reversible cause of acute flaccid paralysis. It can clinically resemble neurological emergencies such as Guillain-Barré syndrome (GBS), leading to potential misdiagnosis and delays in appropriate treatment. We report a 30-year-old man who presented with sudden-onset quadriparesis following an acute febrile illness with gastrointestinal symptoms characterized by fever, myalgia, arthralgia, and vomiting. Initial neurological assessment demonstrated proximal and distal weakness without sensory involvement. Laboratory tests revealed severe hy

Hypertension is a major risk factor for cardiovascular (CV) disease and progression in chronic kidney disease (CKD). Blood pressure (BP) measurement should be standardised and supplemented by ambulatory and home BP monitoring. BP goals are generally <140/90 and <130/80 mmHg for those at higher risk of CV disease or CKD progression. Lifestyle modification includes restricting salt intake to <5 g/day. Most people with hypertension and CKD, especially those with a urine albumin:creatinine ratio ≥3 mg/mmol, should be treated with a renin-angiotensin system inhibitor to which, in selected individua

We report a man in his mid-30s who presented with persistent hypokalaemia and was initially suspected to have Gitelman syndrome because of hypokalaemic metabolic alkalosis, hypocalciuria and normal blood pressure. However, the absence of hypomagnesaemia was atypical, and a urine osmolality that was low relative to a concurrent serum osmolality of 285 mOsm/kg prompted further evaluation, which confirmed a partial urinary concentrating defect on formal testing. Genetic testing identified a novel homozygous frameshift variant in CLDN10 NM_006984.5(CLDN10):c.113del, p.(Thr38Argfs*46) confirming HE

Hyponatremia in neonates is a rare but not uncommon finding, especially among preterm neonates, and can be life-threatening, requiring careful diagnostic evaluation and management. We describe a two-month-old infant born at 33 weeks of gestation presenting with severe hyponatremia and failure to thrive (FTT) to highlight the diagnostic and management approach and challenges in such cases. A two-month-old male child presented with abdominal distension and poor weight gain. Admission weight was 2.72 kgs (<3rd percentile). Lab results revealed a serum sodium of 108 mEq/L and potassium of 2.6 mEq/

Distal renal tubular acidosis (dRTA) is a significant clinical expression of Sjögren's syndrome (SS). While SS-related dRTA is traditionally linked to impaired H+-ATPase, we report a unique case demonstrating selectively decreased anion exchanger 1 (AE1) expression with preserved H+-ATPase expression. A 16-year-old girl with SS presented with muscle weakness, difficulty in ambulation, and severe hypokalemia. Laboratory studies revealed non-anion gap metabolic acidosis, elevated urinary potassium excretion, and overt proteinuria. Renal histology identified a notably reduced expression of AE1 bu

This study aimed to evaluate the nutritional enhancement of wheat-oat blends by incorporating turnip powder at varying concentrations (10%, 20%, 30%, 40%, and 50%). The research assessed the impact of these blends on key nutritional components, including moisture, protein, fat, fiber, calcium, phosphorus, iron, and potassium. The wheat-oat-turnip blends were prepared and analyzed using standard nutritional methods, including proximate analysis and mineral content measurement. Results showed that increasing the concentration of turnip powder led to significant changes in moisture, protein, fat,

Considering that potassium hydroxide (KOH) is well known for its strong carbon dioxide (CO2) capture capability, a comparative study was conducted on low-temperature heterogeneous hydrogenation catalysts operable under CO2/hydrogen (H2)/KOH conditions. A series of Ru-based heterogeneous catalysts, supported on commercial activated carbon or coordinated with NN, PP, and PNP ligands were tested under identical conditions. Among them, the PNP ligand-based catalyst, Ru-MACHO-POMP (porous organometallic polymer), exhibited the highest activity, selectively producing potassium formate (HCO2K) with 1

Despite significant advancements in ester hydrogenation using homogeneous metal catalysts over the past two decades, the catalytic hydrogenation of sterically hindered α-trisubstituted carboxylic esters remains a formidable challenge. Herein, we present a class of imidazole-based NNP-pincer manganese (Mn) catalysts capable of efficiently hydrogenating α-trisubstituted carboxylic esters to their corresponding β-trisubstituted primary alcohols, which are widely applied in the pharmaceutical and fine-chemical industry. Under catalytic conditions, the imidazole moiety is deprotonated by tBuOK to g

This article is part of a review series on water and electrolyte disorders, based on the annual "Electrolyte Winter Seminar" for early-career nephrologists in Japan. The seminar features interactive case-based discussions, some of which are included as self-assessment questions. The fifth installment addresses the management of hyperkalemia. Hyperkalemia frequently occurs in patients with chronic kidney disease (CKD) and can become life-threatening when severe, necessitating prompt treatment regardless of its underlying cause. Renin-angiotensin system inhibitors (RASi) are a recognized risk fa

Nitriles are important organic compounds that are frequently found in pharmaceuticals, natural products, agrichemicals, and functional materials. In addition, nitriles serve as versatile building blocks in synthetic chemistry, as the nitrile moiety can be readily transformed into a variety of valuable functional groups, such as carbonyl derivatives, amines, and heterocycles. However, conventional methods for nitrile synthesis often rely on highly toxic metal cyanides, such as copper(I) cyanide (CuCN) and potassium cyanide (KCN). In this review, I introduce our recent works on a cyanide- and tr
