The clinical approach to obesity in pregnancy.

Over one third of reproductive age women are obese, and this marked prevalence is impacting pregnancy. Obese women face many challenges from preconception to postpartum. They are at increased risk for both maternal and fetal complications including gestational diabetes, hypertension, preeclampsia, congenital anomalies, stillbirth, fetal macrosomia, cesarean delivery, venous thromboembolism, wound complications, breast-feeding difficulty, postpartum depression, postpartum weight retention, and neonatal death. This discussion is designed to help clinicians understand how obesity affects pregnanc

Adolescent substance use and unplanned pregnancy: strategies for risk reduction.

Substance use among adolescents increases the risk of unplanned pregnancies, which then increases the risk of fetal exposure to addictive, teratogenic substances. Specific interventions are necessary to target pregnancy planning and contraception among reproductive-age substance users. Screening for substance use using the CRAFFT is recommended in all health care settings treating adolescent patients. Screening for tobacco and nicotine use is also recommended along with the provision of smoking cessation interventions. Using motivational interviewing style and strategies is recommended to enga

Role of Non-Steroidal Anti-Inflammatory Drugs in Gynecology.

This review summarizes the current use of non-steroidal anti-inflammatory drugs (NSAIDs) in obstetrics, gynecology and infertility. These medications are commonly used in different fields of reproductive medicine, for pain management after operative procedures and to relieve dysmenorrhea. In addition to their analgesic effect, NSAIDs are helpful in the management of menorrhagia by decreasing menstrual blood loss. NSAIDs alleviate pain associated with medical abortion, assist in undertaking natural cycle in-vitro fertilization by preventing follicular rupture and reducing premature ovulation, a

[Genital chlamydial infection].

Genital chlamydial infection is a sexually transmitted disease (STD) of public health importance. Infection with Chlamydia trachomatis can cause urethritis, cervicitis, pharyngitis, or epididymitis, although asymptomatic infections are quite common. Chlamydial infection remains asymptomatic in approximately 50% of infected men and 70% of infected women. Genital infection with C. trachomatis markedly enhances the risk for reproductive tract sequelae in women, including tubal factor infertility and ectopic pregnancy. A proper detection of C. trachomatis and its treatment is of importance to prev

Maternal smoking and undescended testes: reaching a tipping point.

In this issue, Jensen et al (Epidemiology. 2007;18:220-225) publish new evidence linking maternal smoking with cryptorchidism. These new data make the cumulative evidence for a true association persuasive enough to be seriously considered. Possible biologic mechanisms may include endocrine disruption by cigarette smoke, although the role of endocrine disruptors in cryptorchidism more generally has been inconsistent. There are important research questions suggested by the link between mother's smoking and cryptorchidism, including a possible role of father's smoking, the role of genetic suscept

Female sexuality, social reproduction, and the politics of medical intervention in Niger: Kel Ewey Tuareg perspectives.

This essay explores connections between political institutions, forms of power, and women's health care concerns from a cultural anthropological perspective. I focus on the roles of different medical establishments among the Kel Ewey Tuareg of Niger--Western-European sponsored, central state, traditional herbalism and Islamic scholarship--in creating, maintaining, and disputing these constructs, through the invention and elaboration of disease categories and through the selective application of medical and reproductive models and technology to women. I also explore women's attempts to manage t

[Vascular risk of oral contraceptive agents: realities and mechanisms. I. Risk evaluation].

The suspected risk of oral contraception was confirmed when large scale epidemiological studies became available. The present work recalls the methodology of a good evaluation, the pros and cons of retrospective and prospective studies, the different appreciations provided by measuring "relative risk" or "attributable risk". In terms of public health, the data obtained supported the necessity to include mortality related to oral contraception in an evaluation of reproductive mortality. This work compares the incidence of vascular complications evaluated through different studies, according to

Socio-economic and demographic predictors of unmet need for contraception among young women in sub-Saharan Africa: evidence from cross-sectional surveys.

INTRODUCTION: Globally, sub-Saharan Africa (SSA) bears the highest proportion of women with unmet need for contraception as nearly 25% of women of reproductive age in the sub-region have unmet need for contraception. Unmet need for contraception is predominant among young women. We examined the association between socio-economic and demographic factors and unmet need for contraception among young women in SSA. METHODS: Data for this study obtained from current Demographic and Health Surveys (DHS) conducted between January 1, 2010 and December 31, 2018 in 30 sub-Saharan African countries. The

Development and application of a sensitive method for urinary biomonitoring of microcystin-LR: association with semen quality in adult men.

BACKGROUND: Microcystin-LR (MC-LR) is a cyanobacterial toxin that can impair male reproductive function. However, epidemiological investigations of human MC-LR exposure have been limited due to the lack of sensitive and reliable biomonitoring methods. OBJECTIVE: This study aimed to develop and validate a sensitive method for urinary biomonitoring of MC-LR and to investigate the association between urinary MC-LR exposure and semen quality in adult men. METHODS: A sensitive analytical method was developed and validated for the determination of MC-LR in human urine. The validated method was sub

Menstrual and reproductive factors and risk of Alzheimer's disease in elderly women: a cohort study in Eastern China.

Epidemiological studies suggest that proxies of lifetime estrogen exposure are associated with the risk of Alzheimer's disease (AD). However, results remain limited and inconsistent. This study aimed to examine whether menstrual and reproductive factors, proxies of estrogen exposure, are associated with the risk of AD in Chinese postmenopausal women. Information on menstrual and reproductive factors was obtained through the baseline survey of the Zhejiang Ageing and Health Cohort Study. During the three follow-up waves, AD diagnoses were conducted. We excluded participants with prevalent cogni

Future landscapes of women's hormone-sensitive cancers: unraveling global trends, age stratification, and regional inequities (1990-2035).

BACKGROUND: Hormone-sensitive cancers (HSCs) pose a significant threat to women's health, with global rising incidence rates. This study provides the most up-to-date assessment of the global burden of HSCs. METHODS: We retrieved HSC burden data from the Global Burden of Disease (GBD) study (1990-2021), categorized by age groups, and extracted risk factors linked to HSC mortality. Age-standardized incidence (ASIR), Age-standardized death (ASDR), and Age-standardized DALY rates (ASDiR), along with estimated annual percentage changes (EAPC), were used to evaluate HSC burden. Generational trends in reproductive factors among women in the US: implications for breast cancer incidence.

BACKGROUND: Reproductive factors are key breast cancer risk factors, yet contemporary generational patterns remain unclear. We aimed to evaluate trends in reproductive factors among US women born between 1910 and 2000. METHODS: We conducted a serial, cross-sectional analysis of reproductive factors using data from the US National Health and Nutrition Examination Survey (NHANES) collected between 1999 and 2020. Female participants were grouped by birth cohorts (1910-1930, then by 10-year to 2000). Self-reported data were used to derive age at menarche, prevalence of age at menarche < 12 years,

PHGDH restricts porcine reproductive and respiratory syndrome virus replication by sustaining cellular antioxidant capacity and redox homeostasis.

Porcine reproductive and respiratory syndrome virus (PRRSV) remains a major threat to the swine industry. Although redox imbalance and oxidative stress are known to favor PRRSV infection, the host determinants that preserve redox homeostasis during PRRSV replication remain insufficiently defined. Here, we identify phosphoglycerate dehydrogenase (PHGDH), the rate-limiting enzyme of the de novo serine synthesis pathway, as a novel restriction factor for PRRSV. We found that PRRSV infection downregulated PHGDH at both mRNA and protein levels. PHGDH knockdown significantly enhanced PRRSV replicati

Inconsistent medication recommendations for immune-mediated inflammatory diseases across pregnancy, lactation, and paternal preconception: a guideline-based review.

BACKGROUND: Immune-mediated inflammatory diseases (IMIDs) often affect women of childbearing age. Since active inflammatory disease is related with impaired reproductive outcomes, effective disease control is crucial. Clinical guidelines aim to support evidence-based shared decision-making. However, inconsistencies between guidelines across specialties may cause confusion, undermine adherence and lead to adverse health outcomes. This study aimed to assess consistency within and across perinatal IMID guidelines in gastroenterology, rheumatology and dermatology regarding medication use during pr
