Chronic HPA axis response to stress in temporomandibular disorder.

PURPOSE: Perceived stress is associated with temporomandibular disorder (TMD), but whether cortisol levels are elevated in individuals with TMD is unknown. We hypothesized that cortisol concentration, a biomarker of hypothalamic-pituitary-adrenal (HPA) axis function, was elevated in TMD cases relative to controls, and that perceived stress was positively correlated with cortisol concentration. METHODS: In this case control study, TMD case status was determined by examiners using TMD Research Diagnostic Criteria. Participants (n=116) aged 18 to 59 years were recruited from within a 50 mile rad

Safety of noninvasive brain stimulation in children and adolescents.

BACKGROUND: Noninvasive brain stimulation (NIBS) techniques such as transcranial magnetic stimulation (TMS) and transcranial current stimulation (tCS) have the potential to mitigate a variety of symptoms associated with neurological and psychiatric conditions, including stroke, cerebral palsy, autism, depression, and Tourette syndrome. While the safety of these modalities has been established in adults, there is a paucity of research assessing the safety of NIBS among children. OBJECTIVE: To examine the existing literature regarding the safety of NIBS techniques in children and adolescents wi

T-helper and regulatory T-cell cytokines in the peripheral blood of patients with active alopecia areata.

BACKGROUND: Alteration in T-lymphocyte function and cytokines secreted by T-cell subsets has been proposed in the immunopathogenesis of alopecia areata (AA). The role of T-helper and regulatory T-cell cytokines in the pathogenesis of active AA has not been established. OBJECTIVES: To assess the role of hallmark cytokines of T-helper cells (Th1, Th2 and Th17) and regulatory T cells (Tregs) in the pathogenesis of AA, and its clinical correlation. METHODS: Fifty-one patients with AA and 45 age- and sex-matched healthy control subjects were included in the study. Serum interleukin (IL)-2, interf

Diffuse alopecia areata is associated with intense inflammatory infiltration and CD8+ T cells in hair loss regions and an increase in serum IgE level.

BACKGROUND: Mechanism leading to an abrupt hair loss in diffuse alopecia areata (AA) remains unclear. AIMS: To explore the characteristics of diffuse AA and possible factors involved in its pathogenesis. METHODS: Clinical and laboratory data of 17 diffuse AA patients and 37 patchy AA patients were analyzed retrospectively. Serum IgE level was evaluated in all diffuse and patchy AA patients, as well as 27 healthy subjects without hair loss to serve as normal control. Univariate analysis was performed using Fisher's exact test and Wilcoxon rank-sum test. Associations between inflammatory cell

Incidence and clinical predictors of psoriatic arthritis in patients with psoriasis: a population-based study.

OBJECTIVE: To determine the incidence and disease-specific predictors of clinically recognized psoriatic arthritis (PsA) in patients with psoriasis. METHODS: We identified an incidence cohort of psoriasis subjects age >/=18 years diagnosed between January 1, 1970 and December 31, 1999 in a population-based setting. Psoriasis diagnoses were validated by confirmatory diagnosis in the medical record. Incident and clinically recognized PsA subjects were classified according to the Classification of Psoriatic Arthritis (CASPAR) criteria. Cox proportional hazards models were used to identify predic

Pregnancy in patients with idiopathic thrombocytopenic purpura: assessing the risks for the infant at delivery.

The objective of this review is to estimate the risk of severe thrombocytopenia at birth and minor or major morbidity for the infant of a pregnancy complicated by immune thrombocytopenic purpura. English language publications on immune thrombocytopenic purpura in pregnancy from January 1980 to December 1990 were used. Manuscripts considered for infant risk assessment fulfilled the following criteria: 10 or more entrants, reported fetal platelet samples (cordocentesis, scalp sampling or cord sample at birth), and provided all infant platelet data. All other data not fulfilling these criteria ar

Two Atypical Cases of Adams-Oliver Syndrome With DOCK6 Genetic Variants.

Adams-Oliver syndrome (AOS) is characterized by scalp aplasia cutis congenita (ACC) and terminal transverse limb defects (TTLD). Autosomal recessive AOS type 2 caused by DOCK6 variants is usually accompanied by central-nervous-system and ocular anomalies. We report two unrelated children who each carried compound heterozygous DOCK6 variants. Both presented with variable degrees of global developmental delay and visual impairment, but neither showed ACC or TTLD. Our observations highlight the phenotypic variability in DOCK6-related AOS and a possible ascertainment bias in screening patients onl

Isolation of Epidermal and Hair Follicle Melanocytes.

Here, we describe the isolation of epidermal melanocytes and hair follicle melanocytes from human skin tissue. Epidermal and hair follicle melanocytes are two distinct populations of melanocytes that are contained within the skin and the hair follicle, with differing yet overlapping roles. Epidermal melanocytes are normally isolated from the epidermis of vellus-haired skin tissue, for example, face, breast, abdomen, while hair follicle melanocytes are derived from the outer root sheath (ORS) of the middle/lower terminal anagen hair follicles of the scalp. These methods utilize ethically source

The Cost of the Curl: Wig-Induced Hair Loss in Pediatric Irish Dancers.

Traction alopecia is a well-described cause of hair loss associated with high-tension hairstyles but is rarely reported in children related to wig use. We describe three pediatric patients aged 7-8 years who developed focal vertex scalp alopecia after wearing traditional Irish dancing wigs during high-energy dances. Clinical findings and trichoscopy were consistent with traction alopecia, with partial or complete regrowth following wig avoidance. These cases highlight an underrecognized form of pediatric alopecia caused by prolonged wig use during Irish dancing, emphasizing the importance of e

Profile and Management of Traumatic Orbital Hematoma in a Child With Hemophilia C: A Case Report.

Acute orbital compartment syndrome (AOCS) is a rare condition needing prompt surgical intervention to avoid irreversible visual loss. We present a two-year-old with decreased activity, head edema, pain, restricted ocular movement, proptosis, and downward displacement of the left eyeball after a minor fall. Upon thorough evaluation, hemophilia C was diagnosed. An MRI scan revealed a circumferential scalp hematoma and subperiosteal hematoma, causing proptosis of the left orbit. Surgical evacuation of the blood collection resulted in complete recovery and resolution of symptoms. In AOCS, bleeding

Combined Corticosteroid and Minoxidil Therapy for Early Recovery in Radiation-Induced Alopecia: A Case Report.

This case report describes a 42-year-old man who developed radiation-induced alopecia after an endovascular embolization procedure for a cerebral arteriovenous fistula. The alopecia appeared as a well-defined patch of hair loss on the right scalp, confirmed by biopsy showing lymphocytic infiltration and increased catagen and telogen follicles. The patient was treated with intramuscular corticosteroid injections and topical 5% minoxidil, leading to complete hair regrowth after three months. Radiation-induced alopecia, though rare and typically self-limiting, requires early diagnosis. This case

An Atypical Presentation of Giant Cell Arteritis in an Elderly Patient: Diplopia and Fever Masked by Pneumonia.

Giant cell arteritis (GCA) is a chronic vasculitis typically affecting individuals over 50 years of age, often presenting with headache, jaw claudication, scalp tenderness, or visual symptoms. We report an 86-year-old male with psoriasis and hypertension who presented with cough, fever, night sweats, headache, and binocular diplopia, initially managed as pneumonia and possible stroke. Despite antibiotics, his fever and ocular symptoms persisted. Elevated inflammatory markers and temporal artery ultrasound confirmed GCA. Prompt corticosteroid therapy led to clinical improvement. This case highl

[Translated article] Acne Keloidalis Nuchae: A Therapeutic Challenge for Dermatologists.

Acne keloidalis nuchae (AKN) is a chronic inflammatory dermatosis of the scalp that causes scarring alopecia. The etiology of AKN has been associated with an immune response vs. follicular antigens. AKN is characterized by fibrotic papules and plaques that can converge in tumor-like lesions; subclinical disease has been reported in perilesional areas, which could impact its development. Early diagnosis and treatment are essential to reduce morbidity and preserve and minimize healing. Various treatments have been established including optimal medical therapy, surgical excision, and light source

Vorasidenib-Induced Trichomegaly and Hypertrichosis: a New Side Effect in a Patient with Diffuse Astrocytoma.

Vorasidenib, an oral dual inhibitor targeting mutant enzymes isocitrate dehydrogenase 1 and 2, is utilized in the management of diffuse low-grade gliomas. Despite limited documentation of its adverse events, we present the case of a 44-year-old male who exhibited trichomegaly and hypertrichosis of body hair, eyebrows, and eyelashes following one month of vorasidenib treatment. Notably, the patient experienced diffuse hair regrowth on the scalp, including in areas affected by severe androgenetic alopecia. This report holds significance as it highlights a previously unreported side effect, there

Alopecia associated with the use of selective serotonin reuptake inhibitors: Systematic review.

We aimed to systematically review published cases of alopecia (hair loss) associated with selective serotonin reuptake inhibitors (SSRIs). Four electronic databases were searched up to November 16, 2021. Thirty-eight publications describing 71 patients with a total of 81 episodes of alopecia met inclusion criteria. Patients' age ranged from 7 to 85 years and 80.3% were female. Alopecia most commonly affected scalp (98.6%). Reported time to onset ranged from 3 days to 5 years (median: 8.6 weeks). Discontinuation of the suspected SSRI led to recovery in 63.0% of episodes. Clinicians should be aw

Darier disease: the use of dermoscopy in monitoring acitretin treatment.

Darier disease is an uncommon autosomal dominant inherited disease, caused by a mutation in the ATP2A2 gene. The clinical findings are hyperkeratotic papules on the trunk, scalp, face, and neck, maceration of intertriginous areas, palmar pits, whitish papules on the oral mucosa and nail abnormalities. The main histopathologic findings are acantholysis and dyskeratotic keratinocytes. Dermatoscopic features are comedo-like openings with a central polygonal yellowish/brownish structure, surrounded by a whitish halo. First-line treatment includes acitretin.
