Hemodynamic and Metabolic-Inflammatory Phenotyping Across the Cardiac Index Spectrum in Moderate and Severe Tricuspid Regurgitation: Prognostic Implications.

BACKGROUND: Moderate and severe tricuspid regurgitation (TR) is associated with poor outcomes, yet current grading systems do not fully capture circulatory heterogeneity. We investigate the relationship of cardiac index (CI) with rest-exercise hemodynamics, metabolic and inflammatory profiles, and clinical outcomes in moderate and severe TR. METHODS: We prospectively enrolled 300 outpatients with atrial secondary, nonatrial secondary, and lead-associated moderate and severe TRs without ≥moderate left-sided valve disease. All underwent comprehensive laboratory profiling and ultrasound evaluati

Value of Fluorodeoxyglucose Positron Emission Tomography/Computed Tomography in Identifying Osteoarticular Septic Grafts in Suspected Infective Endocarditis: Results from a Large Monocentric Cohort.

Background: 18F-fluorodeoxyglucose positron emission tomography-CT (FDG-PET/CT) is useful for identifying infective endocarditis (IE) but also the detection of other concomitant septic foci. Previously, we found that FDG-PET/CT identified an osteoarthritic septic graft (OASG) in 19.1% of IE patients, frequently asymptomatic. These preliminary results encouraged us to extend our analyses to a larger population, including all patients initially explored for suspected IE, to assess the prevalence, characteristics, and OASG locations brought out by FDG-PET/CT and to identify predictive factors. Prognostic Importance and Predictors of Survival in Isolated Tricuspid Regurgitation: A Growing Problem.

OBJECTIVE: To define mortality associated with isolated tricuspid regurgitation (TR) and identify risk factors associated with decreased survival. PATIENTS AND METHODS: We conducted a retrospective cohort study of residents of southeastern Minnesota with moderate-severe or more severe isolated TR diagnosed between January 1, 2005, and April 15, 2015. Isolated TR was defined as TR in the absence of left-sided heart disease or pulmonary hypertension. Patients with an ejection fraction of less than 50%, right ventricular systolic pressure greater than 45 mm Hg, moderate or more severe left-sided

Progressive right ventricular enlargement due to pulmonary regurgitation: Clinical characteristics of a "low-risk" group.

BACKGROUND: The optimal interval between serial cardiac magnetic resonance imaging (CMRI) scans for monitoring right ventricular (RV) enlargement in the setting of severe pulmonic valve regurgitation (PR) is unknown. The purposes of this study were to (1) determine the annual change in RV volume on serial CMRI scans and (2) identify the risk factors for rapid progression of RV enlargement. METHODS: A retrospective study of adults with postintervention native valve PR and ≥2 CMRI scans at Mayo Clinic Rochester from 2000 to 2015 was conducted. Rapid progression of RV enlargement was defined as

Valvular disease in patients requiring long-term left ventricular assist devices: pathophysiology and therapeutic options.

It is becoming clear that valvular pathology in patients requiring long-term mechanical ventricular support is an evolving, poorly understood and potentially dynamic problem. While strict guidelines are lacking, there is a growing experience in the understanding and management of the growing spectrum of valvular problems in this complex patient population. While the management of tricuspid and aortic regurgitation is of significant concern and may potentially have significant adverse impact on outcomes, other valvular pathologies can be problematic and require management in a manner unique to

Hepatocellular carcinoma in an adult with repaired tetralogy of fallot.

Liver fibrosis is a growing concern among adults with congenital heart disease, particularly for those who have undergone a Fontan operation. Liver fibrosis leads to cirrhosis, a precursor of hepatocellular carcinoma. A few cases of hepatocellular carcinoma in patients with prior palliative surgery for congenital heart disease have been identified in the literature. The current case reports the first known case of hepatocellular carcinoma in a 45-year-old male with repaired tetralogy of Fallot.

Quadrivalvar replacement in infantile Marfan syndrome.

Marfan syndrome (MS) is a connective tissue disease involving the cardiovascular, ocular, and the musculoskeletal systems. MS has variable phenotypic expression and is most often diagnosed in adult life. Infantile-onset MS is rare and is associated with severe cardiovascular manifestations; there is an extremely high mortality during the first 2 years of life. We present a case of a child with severe infantile MS who, during the course of infancy and early childhood, developed aortic root dilatation and polyvalvar insufficiency requiring subsequent successful replacement of the aortic root and

Assessment of valvular regurgitation with Doppler echocardiography.

Echocardiography is routinely performed for the evaluation of valvular regurgitation. Different applications of Doppler echocardiography have been successfully applied to detect and quantify valvular regurgitation. Recent advances in color Doppler made possible the study of the dynamic behavior of the regurgitant orifice and, along with continuous wave Doppler, can provide data on the regurgitant volume and fraction. Doppler echocardiography can also be used to follow serial changes in these hemodynamically important parameters after medical or surgical therapy.

Aortopulmonary shunt with replacement of a common atrioventricular valve in a patient with double inlet ventricle.

Replacement of a common atrioventricular valve with a St. Jude medical prosthesis, together with construction of a central aortopulmonary shunt using a polytetrafluoroethylene graft, were performed on a patient with double inlet ventricle complicated by severe regurgitation across an atrioventricular valve and hypoplastic pulmonary arteries. No complications were noted during a postoperative follow-up of 58 months; indeed, a considerable increase was noted in postoperative exercise capacity. This simple surgical procedure should be considered as a mode of treatment in patients with this combin

[Significance of some coefficients resulting from dye photometry in the functional characterization of mitral defects].

One hundred and eighty six patients with mitral defects were examined and subdivided into four groups according to the basic clinical and instrumental tests: 59 with pure, 46 with prevailing mitral stenosis, 37 with prevailing and 44 with pure mitral insufficiency. The patients, within the pointed group were subdivided into subgroups, depending on the character of the auricular rhythm, the presence of accompanying tricuspid insufficiency with hemodynamic significance and cardiac decompensation. The circilatory times, TA, TD and TC were calculated for all the patients and especially the coeffic

Lifetime management of valvular heart disease in younger and low risk patients.

Valvular heart disease in younger and low-risk patients presents unique therapeutic challenges because treatment decisions must account not only for immediate procedural success but also for the durability of the index intervention, the feasibility of future reinterventions, and the preservation of subsequent treatment options. Consequently, lifetime management has emerged as a longitudinal treatment paradigm in which every intervention influences the sequence, complexity, and outcomes of future procedures. Over the past decade, transcatheter valve therapies have expanded rapidly from a treatm

Surgical Interventions for Late Aortic Valve Regurgitation Associated with Continuous Flow-Left Ventricular Assist Device Therapy: Experience Gained and Lessons Learned.

This study aimed to investigate the outcomes of surgical interventions for symptomatic moderate-to-severe aortic regurgitation (AR), including aortic valve replacement (AVR) and repair (AVP), in 184 patients who underwent continuous flow-left ventricular assist device (Cf-LVAD) implantation as a bridge-to-transplant (BTT) between November 2007 and April 2020. Ten patients (median age, 34 (25-41) years; 60% men) underwent surgical interventions (AVR, n = 6; AVP, n = 4) late after cf-LVAD implantation. The median duration after the device implantation was 34 (24-44) months. Three patients requir

Differences between diabetic and non-diabetic nephropathy patients in cardiac structure and function at the beginning of hemodialysis and their impact on the prediction of mortality.

OBJECTIVES: To characterize differences in cardiac structure and function in hemodialysis (HD) patients with diabetic nephropathy (DN) and in those without using echocardiography and to determine their impact on the prediction of mortality using echocardiographic parameters. METHODS: Clinical, laboratory, and echocardiographic data were collected from patients commencing HD. RESULTS: Compared with those without DN, patients with DN had lower peak velocity of the early diastolic wave (e'), larger left atria, and higher peak early diastolic velocity (E)/e' and peak velocity of tricuspid regurg

A novel approach to eliminate intraventricular lead placement in patients with congenital heart disease.

PURPOSE: We describe two patients with congenital heart disease who were referred for implantable cardioverter-defibrillator (ICD) placement. Both patients possessed factors causing the conventional transvenous approach to be undesirable. We therefore devised a technique which circumvented both intravascular ICD lead placement, as well as placement of leads across the tricuspid valve. METHODS: For both patients, a bipolar pace-sense lead was successfully placed in a branch of the coronary sinus. A shocking coil was then tunneled from the posterolateral margin of the device pocket, being posit
