Autosomal recessive 'optic atrophy' with late onset and evidence of ganglion cell dysfunction: a sibship of two females.

Two sisters aged about 40 years presented with a recent moderate reduction in visual acuity and pale optic discs. The nonconsanguineous parents, 7 other siblings and all other family members have normal vision. This strongly indicates a hereditary, autosomal recessive origin. A primary ganglion cell pathology is strongly suspected because of the significant attenuation of the early component of the onset/offset visual-evoked potential in the presence of little change in the potential to a pattern reversal stimulus.

Ocular enlargement following infantile corneal opacification.

Congenital hereditary endothelial dystrophy (CHED) is not generally thought to be associated with other ocular abnormalities. Ultrasonography in a series of twenty eyes (ten patients) with CHED shows ocular enlargement similar to that occurring in uncomplicated axial myopia. There was an inverse relationship between the degree of enlargement and the visual acuity or visual result following penetrating keratoplasty suggesting that infantile corneal oedema sufficient to cause stimulus deprivation may result in abnormal enlargement of the globe.

Common problems in pediatric ophthalmology.

Because in children the neurological connections of sight are still plastic, any condition that is able to cause a reduction of visual acuity will also cause cortical suppression of the input from that eye. Early management of amblyopia is essential to its effective treatment. Prompt recognition of strabismus, nystagmus, dyslexia, retinoblastoma and albinism will also lead to an improved visual prognosis for the affected child. In the child with retinoblastoma, it will also vastly improve the prognosis for life.

The value of preoperative electro-ophthalmological examination before cataract extraction.

In 203 senile cataractous eyes preoperative results of photopic flash ERGs and VERs were compared with the optimum visual acuity after cataract extraction. Because preoperatively for some reason a good function of the retina or the conductive system was questionable, patients were referred to the electro-ophthalmological department. This encompassed approximately 10% of patients with a senile cataract. The relationships found indicated that the photopic flash ERGs and VERs are a good support for estimating post-operative visual function.

Optic disc drusen and tumours of the chiasmal region.

Two patients with optic disc drusen and tumour of the chiasmal region are presented. The association of optic disc drusen and intracranial space-occupying lesion is probably a chance occurrence. In spite of the recognition of the optic disc drusen, certain findings indicate further examinations. Progressive loss of central visual acuity unexplained by retinal pathology is an indication for a neurological investigation. Bitemporal and homonymous hemianopic visual field defects as well as evidence of papilloedema warrant a neurological evaluation.

Elucidating postoperative dynamics in tractional retinal detachment: a systematic review and meta-analysis of structural and functional outcomes following diabetic vitrectomy, including an analysis of postoperative complications.

This meta-analysis aims to evaluate the efficacy and safety of diabetic vitrectomy for treating tractional retinal detachment (TRD) by assessing visual acuity improvements, retinal reattachment success rates, and the frequency of postoperative complications. A comprehensive literature search identified 1,211 studies, 30 of which met the inclusion criteria and were analyzed. The meta-analyses were conducted via random effects models to account for heterogeneity. The primary outcomes were visual acuity improvement and retinal reattachment success, whereas postoperative complications were analyze
