BACKGROUND: Autoimmune sclerosing cholangitis (ASC) is a rare clinical entity characterized by overlapping features of autoimmune hepatitis and primary sclerosing cholangitis. It predominantly affects pediatric patients. Therapeutic management is often complex, requiring a multidisciplinary and individualized approach, especially in the context of associated autoimmune diseases.
CASE PRESENTATION: We present the case of a female patient diagnosed at the age of 10 with ASC, for which immunosuppressive therapy with prednisone, azathioprine (AZA), and ursodeoxycholic acid (UDCA) was initiated, w

BACKGROUND: Autoimmune blistering diseases (AIBDs) are characterized by autoantibody-mediated epithelial damage affecting the skin and mucous membranes. Among them, pemphigus vulgaris (PV) and mucous membrane pemphigoid (MMP) frequently involve the gingiva, most commonly presenting as desquamative gingivitis (DG). This chronic and painful gingival involvement may compromise oral hygiene and favor secondary inflammatory changes. Consequently, growing interest has focused on the periodontal status of patients with AIBDs and on associated oral microbiological findings.

OBJECTIVE: Autoimmune encephalitis (AE) is a disease with a potential for recurrence, and patients receive immunotherapy to prevent it. However, there is no consensus on the duration of immunotherapy. This study aimed to determine the recurrence rate and identify the risk factors for AE to provide guidance on the duration of immunotherapy.
METHODS: A comprehensive search of the Embase, Web of Science, Cochrane, and PubMed databases was conducted from database inception until January 18, 2025, to identify clinical studies and observational studies reporting the recurrence of AE. Data on recurr

Autoimmune hemolytic anemia (AIHA) is an acquired form of hemolytic anemia with several serological subtypes, including drug-induced immune hemolytic anemia (DIIHA). Due to the complex nature of diagnosis, many of the published reports rely on establishing a temporal relationship between drug exposure and symptom onset. To date, only four case reports have suggested a possible association between cephalexin and DIIHA. Here, we present a case of AIHA that occurred following exposure to cephalexin. An 85-year-old female presented with fatigue, dyspnea, and orange urine following initiation of ce

BACKGROUND: Autoimmune encephalitis (AE) encompasses a group of inflammatory disorders of the brain parenchyma mediated by autoimmune mechanisms. Currently, first-line immunotherapy for AE typically involves corticosteroids, intravenous immunoglobulin, and plasma exchange or immune adsorption. However, a subset of patients shows an inadequate response to these treatments, which are also associated with an increased risk of infection and other potential adverse effects. Efgartigimod, a neonatal Fc receptor (FcRn) antagonist, is primarily approved for treating generalized myasthenia gravis. Prel

BACKGROUND: Autoimmune thyroiditis (AIT) ranks among the most common autoimmune disorders globally. Adherence to a healthy lifestyle may substantially offset the risk conferred by a high transcriptomic risk profile.
METHODS: Immune-related differentially expressed genes (IRDEGs) were first identified based on data from the Gene Expression Omnibus (GEO) database (NAIT=13, Nnon-AIT=10) and a cross-sectional dataset from Anhui Province (NAIT=5, Nnon-AIT=5). Feature selection was then performed using LASSO regression and random forest algorithms to pinpoint key IRDEGs. These key genes were subseq

Vitiligo is a dermatological disease characterized by focal disappearance of the melanin pigment on the skin and mucous membranes of a person. Despite the variety of proposed hypotheses for the development of this pathology, the central role belongs to the autoimmune mechanism of melanocyte damage.Analysis of the results of numerous international studies confirms a pronounced tendency towards an increase in the prevalence of endocrine autoimmune diseases among patients with vitiligo compared to the general population. Such diseases include, first of all, autoimmune thyroiditis, Graves' disease

The reemergence of the mpox virus (MPXV) as a global health threat, with >100 000 cases reported across 115 countries since the WHO's 2022 alert, has intensified research into its pathophysiology. MPXV exists in two main clades with distinct geographic and clinical profiles. Despite progress, the impact of MPXV infection in patients with autoimmune diseases remains underexplored. Emerging data highlight immunomodulatory MPXV proteins that suppress innate immunity and dysregulate adaptive responses, through inhibition of type I interferons, nuclear factor kappa-B, and tumor necrosis factor-α. Autoimmune encephalitis is an inflammatory disorder of the central nervous system frequently associated with antibodies directed against neuronal surface or intracellular antigens. However, a substantial subset of patients remains seronegative, posing diagnostic challenges. We report the case of a 65-year-old man who presented to the emergency department with acute confusion, prosopagnosia, psychomotor agitation, and generalized tonic-clonic seizures, followed by coma requiring intubation and intensive care admission. Extensive microbiological findings, including CSF PCR for herpes simplex vir

BACKGROUND: Although Bartonella henselae is primarily known for causing self-limiting cat scratch disease in immunocompetent individuals, it can also lead to severe disseminated infections, particularly in immunocompromised patients.
CASE PRESENTATION: We present a rare case of disseminated B. henselae infection in a 70-year-old man with multiple comorbidities, including a recent aortic valve replacement and pacemaker implantation. The patient initially presented with purpuric skin lesions, progressive renal impairment, and pancytopenia, leading to a preliminary diagnosis of autoimmune glomer

INTRODUCTION: Autoimmune hemolytic anemia (AIHA) is rare but potentially life-threatening complication of leptospirosis, a zoonotic infection caused by the Leptospira species. Autoimmune hemolytic anemia occurs when the immune system targets red blood cells, resulting in hemolysis and anemia. The relationship between leptospirosis and AIHA is uncommon and often under-recognized, which can delay diagnosis and management. This case highlights the importance of considering hematologic complications in leptospirosis.
CASE PRESENTATION: A 23-year-old male presented with low-grade fever, productive

The metabolic activities of T cells play a pivotal role in regulating their activation, differentiation, and effector functions. In recent years, it has emerged as a key focus of research in the maintenance of immune homeostasis and the modulation of inflammatory responses. T cells not only rely on metabolic reprogramming to meet their energy and biosynthesis demands, but also utilize intermediate metabolites to regulate epigenetic modifications and then affect gene expression and cell fate. More importantly, T cell metabolism faces adaptive pressures in tissue-specific microenvironments, whic

In recent years, the incidence and complexity of autoimmune diseases (AIDs) have been steadily increasing, posing grim challenges to clinical management. These diseases often involve multi-organ dysfunction and impose a pronounced burden on patients' physical and mental health. Current therapeutic strategies remain suboptimal, frequently limited by poor specificity and severe systemic side effects. With the rapid advancement of nanotechnology, nanodrugs have emerged as a potential approach on account of their enhanced targeting capability, high therapeutic efficacy, and reduced toxicity. In pa

Microglia critically influence multiple sclerosis (MS) pathophysiology through debris clearance, myelin repair, and modulation of neuroinflammation. These processes are partly regulated by ATP-gated ion channel P2X4, predominantly expressed in microglia. We previously reported that ivermectin (IVM), a positive allosteric modulator of P2X4, modulates microglia activation and function in myelin phagocytosis and promotes following lysolecithin-induced demyelination, and ameliorates neurological symptoms in experimental autoimmune encephalomyelitis (EAE). Here, we dissected the molecular and cellu

Cannabinoids are traditionally recognized for their effect on the nervous system. Emerging evidence suggests that cannabinoids mitigate inflammation driven by Th1/Th17 responses, which are linked to autoimmune diseases. In addition to their symptomatic and analgesic effects, cannabinoids suppress the immune response by modulating regulatory T-cell activity and reducing microglial activation, and help to maintain the integrity of the epithelial barrier. These findings suggest that cannabinoids may be involved in immune and metabolic regulatory pathways. Despite the promising preclinical data, t

Autoimmune diseases arise from breakdown of immune tolerance and are commonly treated with broad immunosuppression that limits efficacy and increases toxicity. For drug discovery, mRNA therapeutics offer a programmable, modular platform to develop antigen-specific and mechanism-driven autoimmune therapies. mRNA-based approaches enable transient, controllable expression of self-antigens, immunoregulatory proteins and transcription factors to induce tolerance, reshape local immune microenvironments and enhance regulatory immune pathways. Preclinical studies across models of multiple sclerosis, t
