Herpesvirus pan encodes a functional homologue of BHRF1, the Epstein-Barr virus v-Bcl-2.

BACKGROUND: Epstein-Barr virus (EBV) latently infects about 90% of the human population and is associated with benign and malignant diseases of lymphoid and epithelial origin. BHRF1, an early lytic cycle antigen, is an apoptosis suppressing member of the Bcl-2 family. In vitro studies imply that BHRF1 is dispensable for both virus replication and transformation. However, the fact that BHRF1 is highly conserved not only in all EBV isolates studied to date but also in the analogous viruses Herpesvirus papio and Herpesvirus pan that infect baboons and chimpanzees respectively, suggests BHRF1 may

Modulation of human cutaneous reflexes during rhythmic cyclical arm movement.

The organization and pattern of cutaneous reflex modulation is unknown during rhythmic cyclical movements of the human upper limbs. On the assumption that these cyclic arm movements are central pattern generator (CPG) driven as has been suggested for leg movements such as walking, we hypothesized that cutaneous reflex amplitude would be independent of electromyographic (EMG) muscle activation level during rhythmic arm movement (phase-dependent modulation, as is often the case in the lower limb during locomotion). EMG was recorded from eight muscles crossing the human shoulder, elbow, and wrist

Loss of relative-motion sensitivity in the monkey superior colliculus after lesions of cortical area MT.

Many cells in the superficial layers of the monkey superior colliculus are sensitive to relative motion. The response to a small stimulus moving through a cell's receptive field is strongly modulated by the relative motion between the stimulus and a textured pattern moving through the surrounding visual field; modulation is independent of absolute direction and speed of the stimulus. To determine whether cortical visual area MT is essential for this type of relative-motion sensitivity, colliculus cells were studied in the anesthetized, immobilized preparation after ablation of area MT. Unilate

Genetic homogeneity of mutational spectrum of group-A xeroderma pigmentosum in Tunisian patients.

BACKGROUND: Xeroderma Pigmentosum (XP) is a rare autosomal recessive disorder characterized by cutaneous and ocular alterations. Eight genes, Xeroderma Pigmentosum group A (XPA) to Xeroderma Pigmentosum group G (XPG) and Xeroderma Pigmentosum group V (XPV), are known to be responsible for the disease and products of these genes are involved in the repair of deoxyribonucleic acid (DNA) lesions generated by UV radiation. Several XP patients suffer from neurological defects, found in the XPA (the most common form), D and G groups. The aim of this study was to investigate the mutational spectrum o

Portal vein embolization for arterio-portal shunt closure in hepatocellular carcinoma to facilitate selective internal radiation therapy.

BACKGROUND: Selective internal radiation therapy (SIRT) has been widely used in the treatment of hepatocellular carcinoma (HCC). However, marked intrahepatic shunt (IHS) secondary to arterio-portal shunt (APS) often precludes safe SIRT administration. AIM: To evaluate the efficacy and safety of portal vein embolization (PVE) in reducing IHS and enabling SIRT therapy in HCC patients with APS. METHODS: This retrospective study enrolled seven HCC patients with APS confirmed by hepatic angiography during mapping. PVE was performed to close the APS outlet and single photon emission computed tomog

Reliability and Agreement of Quantitative Pulmonary Imaging Biomarkers Between Ultra-Low-Dose and Low-Dose Chest CT: A Paired Intra-Individual Study.

Background/Objectives: Ultra-low-dose computed tomography (ULD-CT) enables substantial radiation reduction compared with routine low-dose computed tomography (LD-CT), but its quantitative reliability across lung imaging biomarkers remains insufficiently characterized. This study aimed to assess the agreement and reliability of quantitative pulmonary imaging biomarkers between paired ULD-CT and LD-CT examinations. Methods: In this prospective study, 48 patients who underwent paired LD-CT and ULD-CT on the same day were analyzed. Whole-lung quantitative biomarkers were categorized into density-d

KRAS Inhibition in Non-Small Cell Lung Cancer with Central Nervous System Involvement: Current Evidence and Future Directions.

BACKGROUND: KRAS mutations, particularly G12C, frequently cause non-small cell lung cancer (NSCLC). Due to the protective nature of the blood-brain barrier and the paucity of clinical data specifically about CNS involvement, brain metastases - which occur in up to 40% of patients with advanced KRAS-mutant NSCLC - present unique therapeutic challenges. SUMMARY: The changing role of KRAS-targeted treatments in NSCLC with metastases to the central nervous system (CNS) is examined in this review. It highlights new clinical findings on KRAS G12C inhibitors, including sotorasib and adagrasib, and d

Impact of neoadjuvant chemotherapy on the rate of clinically significant postoperative pancreatic fistula.

BACKGROUND: While numerous studies have evaluated predictors of postoperative pancreatic fistula (POPF) after pancreatectomy, limited data exist regarding the impact of neoadjuvant chemotherapy (NAC) on POPF among patients undergoing surgery for pancreatic ductal adenocarcinoma (PDAC). This study aimed to assess the association between NAC and POPF using a contemporary national cohort. METHODS: We queried the American College of Surgeons National Surgical Quality Improvement Program pancreatectomy-targeted datasets for 2022 and 2023. NAC was defined as chemotherapy within 90 days before surge

Dysphagia Rehabilitation Outcomes in Head and Neck Cancer Patients With Radiation Associated Dysphagia: Does Time Since (Chemo)radiation Matter?

BACKGROUND: Since radiation-associated dysphagia may evolve in the months to years following (chemo)radiotherapy (C/RT), it is important to determine whether swallowing-related outcomes vary according to the time elapsed since cancer treatment. This may help to determine the time window in which a patient might benefit most from dysphagia rehabilitation. METHODS: Fifty-four patients were stratified into three groups based on the time since completion of the C/RT: < 1 year; 1-5 years; > 5 years post-C/RT. Differences in swallowing-related outcomes were compared at baseline and after 8 weeks of

Hemispherotomy for atypical teratoid rhabdoid tumor-associated drug-resistant epilepsy: illustrative case.

BACKGROUND: Drug-resistant epilepsy (DRE) following treatment for atypical teratoid rhabdoid tumor (ATRT) is rare. The authors report a case in which hemispherotomy was performed to palliate new-onset DRE after ATRT therapy. OBSERVATIONS: A 6-year-old boy with a large left thalamic ATRT underwent subtotal resection, chemotherapy, and radiation therapy. His neurological baseline remained stable, with right hemiparesis, right hemifield neglect, and apraxia of speech. Two years after tumor recurrence and treatment, he developed frequent tonic seizures of left hemispheric origin. Despite trials o

Unveiling Hidden Abscesses: The Clinical Utility of Diffusion-Weighted Whole-Body Imaging with Background Suppression (DWIBS) in Metastatic Abscess Screening.

A 74-year-old man with type 2 diabetes presented with fever, urinary retention, and urinary difficulties. Initial abdominal Computed Tomography (CT) suggested acute pyelonephritis, but a low-density area in the prostate was overlooked. Following the confirmation of methicillin-resistant Staphylococcus aureus (MRSA) in blood and urine cultures, comprehensive screening for metastatic abscesses was necessitated. Diffusion-weighted whole-body imaging with background suppression (DWIBS) was utilized and clearly identified a prostatic abscess (PA), nephritis, urethritis, and subcutaneous cysts. Thes

Layer by Layer: The Anatomical 'Onion Peel' Approach to Surgery Beyond Total Mesorectal Excision.

BACKGROUND: The global incidence of rectal cancer is rising, accompanied by the increasing use of neoadjuvant chemoradiotherapy. Although this approach improves oncological outcomes, it frequently increases operative difficulty during rectal resection. Achieving a negative circumferential resection margin (CRM) remains critical to reducing local recurrence. While clear margins are often attainable utilising the standard total mesorectal excision (TME) plane, dissection beyond TME is increasingly required following neoadjuvant radiation or for locally advanced or recurrent disease. Such surgery

A case report of carotid body tumor with rare thoracic vertebral metastasis and literature review.

BACKGROUND: Malignant carotid body tumor (CBT), a type of carotid body paraganglioma (PGL), is defined by the presence of distant metastasis; histology alone cannot reliably distinguish benign from malignant CBT. We present a 70-year-old female patient who had been suspected of having CBT for more than 10 years without specific treatment, and complained of abdominal pain for 6 months. Contrast-enhanced MRI and CT revealed a T11 vertebral lesion, followed by percutaneous biopsy of the lesion, which showed atypical cell nests. Immunohistochemical staining was positive for CD56, CgA, Syn and S-10
