Dynamic External Distraction Device for Proximal Phalangeal Head Intra-articular Fracture.

Dynamic distraction external fixation (DDEF) is commonly used for proximal interphalangeal joint fractures based on the principle of ligamentotaxis. However, this mechanism may be limited when fracture fragments lack soft tissue attachment. We present a 14-year-old girl with a depressed intra-articular fracture of the proximal phalanx head and joint stiffness, treated 5 weeks after injury using DDEF. Despite delayed treatment and minimal ligamentous connection, the fragment was successfully elevated and healed. We propose the so-called "vacuum phenomenon" as a hypothesis-generating concept tha

[Femoral head osteotomy for complex deformity after Legg-Calvé-Perthes disease].

Complex hip deformities after Legg-Calvé-Perthes disease are characterized by an enlarged, asymmetrically deformed femoral head, a short femoral neck, a high-riding greater trochanter, and secondary acetabular dysplasia. These features lead to intra- and extra-articular impingement, instability, abductor weakness, and early osteoarthritis. Conventional osteotomies or isolated acetabular reorientations do not directly address the femoral head deformity and, therefore, show limited results. Detailed analysis of the vascular anatomy enabled the development of intracapital osteotomy with central s

Clinical Outcomes of Duodenum-Preserving Core Pancreatic Head Resection (DP-CoPHR).

BACKGROUND: Compared with pancreaticoduodenectomy, duodenum-preserving pancreatic head resection (DPPHR) is superior for resecting benign or low-grade malignant tumors located in the core area (adjacent to major ducts) of the pancreatic head. However, this procedure is technically complex. We developed a modified technique, duodenum-preserving core pancreatic head resection (DP-CoPHR), designed to simplify the procedure. This study evaluates its clinical outcomes. METHODS: We retrospectively analyzed 6 patients with benign or low-grade malignant tumors located in the core region of the pancre

Deep Optic Nerve Head Structural Alterations in Adults with Cystic Fibrosis.

Background: Cystic fibrosis (CF) is a systemic genetic disorder characterized by chronic inflammation, hypoxia, and metabolic imbalance that may affect neural and microvascular structures. While previous studies have evaluated superficial optic nerve head (ONH) parameters in CF, deep ONH structures, particularly the lamina cribrosa (LC), remain insufficiently explored. This study aimed to assess both superficial and deep ONH morphology in adults with CF using swept-source optical coherence tomography (SS-OCT). Methods: This observational case-control study included 34 CF individuals and 34 age

Smarcc1 drives optic stalk patterning and optic nerve head astrocyte differentiation.

The optic nerve develops from the neuroectodermal optic stalk, which undergoes coordinated morphogenesis and gives rise to optic nerve astrocytes that support retinal ganglion cell axons. Here, we define the progression of astrocyte formation from the optic stalk and identify stage-specific functions of the SWI/SNF scaffolding subunits Smarcc1 and Smarcc2. Both factors are co-expressed in retinal pigment epithelium (RPE) and optic stalk progenitors, with Smarcc2 persisting in differentiated RPE and astrocytes. Conditional deletion using Dct-Cre revealed compensatory activity in pigmented linea

Accidental Botulism Caused by Consumption of Vacuum-Packaged Rabbit-Head Meat.

With the increase of sporadic cases of foodborne botulism and the occurrence of several large incidents of botulinum contamination in food, botulism has received greater attention in clinical practice. Foodborne botulinum toxin poisoning is an acute foodborne toxic disease caused by ingestion of food contaminated with Clostridium botulinum exotoxin. We present two cases of foodborne botulism affecting a mother and daughter who were admitted to our department after consumption of vacuum-packed rabbit-head meat. The time to symptom onset was less than 24 h in the daughter (aged 8 years) and 1 we

Human head-neck model and its application thresholds: a narrative review.

There have been many studies on human head-neck biomechanical models in the last two decades, and the associated modelling techniques were constantly evolving at the same time. Computational approaches have been widely leveraged, in parallel to conventional physical tests, to investigate biomechanics and injuries of the head-neck system in fields like the automotive industry, orthopedic, sports medicine, etc. The purpose of this manuscript is to provide a global review of the existing knowledge related to the modelling approaches, structural and biomechanical characteristics, validation, and a

Intraventricular pressure and volume during conventional and automated head-up CPR.

BACKGROUND: Active compression-decompression (ACD) CPR, an impedance threshold device (ITD) and automated head and thorax elevation, collectively termed AHUP-CPR, increases cerebral and coronary perfusion pressures, brain blood flow, end-tidal CO2 (ETCO2) and cerebral oximetry (rSO2) in animal models compared with conventional (C) CPR. We tested the hypothesis that cardiac stroke volume (SV) is higher with AHUP-CPR versus C-CPR or ACD + ITD in a porcine cardiac arrest model. METHODS: Farm pigs (n = 14) were sedated, anesthetized, and ventilated. Hemodynamics, including biventricular pressure-

Noncanonical Pathways of Proteasome Inhibition in HPV-Negative Head & Neck Cancer.

Carfilzomib, a second-generation proteasome inhibitor approved for the treatment of multiple myeloma, is a highly potent and selective inhibitor of the CT-L activity of the i20S proteasome. Several studies have shown that carfilzomib (CFZ) can bypass resistance to bortezomib; however, its impact on squamous cell carcinoma of the head and neck is not well understood. This study aimed to evaluate the anticancer potential of CFZ in head and neck cancer cells (HNSCC) by examining its effects on proliferation, apoptosis, and the underlying mechanisms in both HPV-positive and HPV-negative HNSCC. Repetitive head impacts induce neuronal loss and neuroinflammation in young athletes.

Repetitive head impacts (RHI) sustained from contact sports are the largest risk factor for chronic traumatic encephalopathy (CTE). Currently, CTE can only be diagnosed after death and the multicellular cascade of events that trigger initial hyperphosphorylated tau (p-tau) deposition remain unclear. Further, the symptoms endorsed by young individuals with early disease are not fully explained by the extent of p-tau deposition, severely hampering development of therapeutic interventions. Here, we show that RHI exposure associates with a multicellular response in young individuals (<51 years old

Uncovering chromatin factor landscapes in head and neck squamous cell carcinoma.

Chromatin factors, defined here as proteins that chemically modify the DNA and histones, remodel chromatin and regulate nucleosome occupancy, play central roles in the transcriptional regulation of genes and are implicated in cancer initiation and progression in multiple cancer types. To systematically investigate the genomic and expression alterations of chromatin factors in head and neck squamous cell carcinoma (HNSCC), we utilized the molecular profiles from 530 HNSCC tumor samples in the Cancer Genome Atlas (TCGA), and characterized the mutational, copy number and transcriptional alteratio

Incidence of postoperative delirium among patients who underwent head & neck surgery.

OBJECTIVE: Postoperative delirium is a state of confusion and cognitive impairment that can be observed after extensive surgery. This serious complication can lead to longer hospital stays and higher costs. As the global population ages, postoperative delirium has also become a concern for patients undergoing head and neck surgery. In this study, we investigated whether the preoperative recognition of cognitive impairment could help with the prediction of postoperative delirium among patients undergoing head and neck surgery. METHODS: We conducted a three-year prospective study in our clinic

Sex Differences in Head Acceleration Events in Law Enforcement Corrections Cadets.

PURPOSE: Law enforcement cadets (LECs) undergo subject control technique training that may expose them to repetitive head impacts recorded as head acceleration events (HAEs) using instrumented mouthguards. Prior research suggests that sex and/or gender differences in HAE frequency and magnitude vary by sport. This study aimed to examine sex differences in HAE exposure among LECs during training. METHODS: We collected HAEs from 82 civilian LECs (16 females, mean age = 30 ± 9 years) using instrumented mouthguards. We compared peak linear acceleration (PLA) and peak rotational velocity (PRV) of

Tiny Infants, Positional Head Deformity, Developmental Positioning and Neonatal Nursing Practice.

BACKGROUND: Low-birth weight infants are at risk for neurodevelopmental complications. Prolonged mechanical ventilation and endotracheal tube stability can compromise developmentally supportive positioning and result in positional head deformity (PHD). Nationally, PHD prevalence is 22%-66%, yet an internal audit of 93 neonatal intensive care unit (NICU) discharge summaries found normal head assessments. PURPOSE: To evaluate PHD prevalence among NICU preterm infants and neurodevelopmental positioning practice. METHODS: A convenience sample of 50 NICU preterm infants >72 hours of life discharg

Temporary Cemented Spacer for Complex Radial Head Fractures: A Case Report.

BACKGROUND The management of comminuted and displaced radial head fractures (Mason III), particularly when associated with dislocation (Mason IV) or Essex-Lopresti injury, presents a complex surgical challenge. Treatment options include conservative management, surgical fixation, arthroplasty, or radial head resection; however, no consensus exists regarding the optimal approach for all injury types. Radial head resection is often preferred in complex cases with multiple fragments where reconstruction is unfeasible. While functional and radiological outcomes vary widely, there is a growing tren

Glycolytic reprogramming impairs chondrocyte function in broilers with femoral head necrosis.

Cartilage extracellular matrix (ECM) destruction is a hallmark of femoral head necrosis (FHN) in broilers. Chondrocytes undergo metabolic reprogramming under stress to maintain function. However, the metabolic alterations in FHN chondrocytes remain unclear. This study aims to investigate the overall changes of metabolic state in FHN chondrocytes and its functions. Femoral head cartilage of healthy and FHN broilers was collected for non-targeted metabolome and transcriptome analyses. Additionally, primary chondrocytes were isolated from femoral head cartilage of control (CON) and FHN broilers f

Tumor subtype classification tool for HPV-associated head and neck cancers.

BACKGROUND: Molecular subtypes of HPV-associated Head and Neck Squamous Cell Carcinoma (HNSCC), named IMU (immune strong) and KRT (highly keratinized), are well-recognized due to distinct molecular features, tumor microenvironments, clinical outcomes, and potentially differing optimal treatment strategies. Currently, no standardized method exists to subtype a new HPV + HNSCC tumor. Our paper introduces a machine learning-based classifier and webtool to reliably subtype HPV + HNSCC tumors using the IMU/KRT paradigm and highlights the importance of subtype in HPV + HNSCC. METHODS: We conducted
