Cholesterol efflux regulatory protein, Tangier disease and familial high-density lipoprotein deficiency.

Cellular cholesterol efflux, by which cholesterol is transported from peripheral cells to HDL acceptor molecules for transport to the liver, is the first step of reverse cholesterol transport. Two genetic disorders, Tangier disease and some cases of familial HDL deficiency, have defects of cellular cholesterol efflux. The recent discovery of mutations in the ABC1 gene, which encodes the cholesterol efflux regulatory protein, in both these disorders establishes cholesterol efflux regulatory protein as a rate-limiting factor in reverse cholesterol transport.

A boy with neurofibromatosis 1 and Poland anomaly.

Von Recklinghausen neurofibromatosis (NF1) is one of the most common autosomal dominant disorders and has one of the highest mutation rates of a single human locus. The NF1 gene has recently been mapped to proximal 17q. Poland anomaly consists of the combination of unilateral aplasia of the sternal head of the pectoralis major muscle and an ipsilateral anomaly of the hand. Although other defects may occur in patients with Poland anomaly, neurofibromatosis 1 has not been reported. We have seen a boy with these two disorders.

COBT: a gene-based rare variant burden test for case-only study designs using aggregated genotypes from public reference cohorts.

Rare disease gene discovery is limited by small cohorts and the frequent absence of matched controls. We present the Case-Only Burden Test (COBT), a gene-based burden test for case-only designs accounting for multiple variants per individual and additive effects. COBT uses a Poisson model to test for excess variants in a gene relative to expectations from population mutation rates. Simulations show high power and competitive performance versus case-control burden tests. Validation on 1000 Genomes data demonstrated good model fit and low false-positive rates. Applied to 478 ciliopathy patients,

Metastatic Colorectal Cancer Diagnosed Before Age 35: A Distinct Subset of Early-Onset Disease?

The incidence of early-onset colorectal cancer (CRC), commonly defined as diagnosis prior to age 50, is increasing. Studies of patients diagnosed prior to age 35 as a distinct subset of all early-onset patients have yielded inconsistent results. We extracted prospectively collected data from consecutive patients with metastatic colorectal cancer (mCRC) entered in the multi-site Treatment of Recurrent and Advanced Colorectal Cancer (TRACC) Australasian registry. We focused on comparing demographic and clinicopathologic characteristics of those diagnosed < 35Y with remaining early-onset patients

Dynamic Changes of ctDNA-Based MRD and Genomic Mutations Predict Prognosis in ESCC Treated With Definitive Radiotherapy.

This study characterized the genomic profile of esophageal squamous cell carcinoma (ESCC) via circulating tumor DNA (ctDNA) to identify predictive biomarkers for efficacy of definitive radiotherapy combined with systemic therapy. In this prospective cohort of 40 ESCC patients, 7 tumor tissues and 71 serial plasma samples were analyzed using a 340-gene panel. TP53 (45%) and PIK3CA (12.5%) were the most frequent SNVs, followed by NOTCH1/2 (7.5%). The 11q13.2-13.4 amplification (including CCND1/FGF19) occurred in 12.5% of cases. TP53 mutation rate was significantly higher in cT4 than cT2-3 stages

Poor diagnostic value of isocitrate dehydrogenase 1 R132H immunohistochemistry for determination of isocitrate dehydrogenase 1 status in patients with glioblastoma.

BACKGROUND: The World Health Organization (WHO) classification of central nervous system (CNS) tumors is a major advance toward improving the diagnosis of adult brain tumors. Despite the promise of isocitrate dehydrogenase (IDH) mutations as an important biomarker for glioblastoma, not all institutions have ready access to mutation detection polymerase chain reaction (PCR) methods, and deoxyribonucleic acid (DNA) sequencing may be problematic in very small biopsies. However, a simultaneous evaluation of IDH1 status by DNA sequencing and immunohistochemistry (IHC) to determine the sensitivity a

The promise of menin inhibitors: from approval to triplet regimens.

Pharmacologic targeting of the menin-KMT2A protein-protein interaction has emerged as a therapeutic breakthrough for acute leukemias harboring KMT2A rearrangements or NPM1 mutations. The first-in-class menin inhibitor (revumenib) achieved accelerated regulatory approval in November 2024 for monotherapy of relapsed/refractory KMT2A rearranged leukemia. Next-generation agents (ziftomenib, bleximenib, enzomenib, BMF-219) have displayed similar composite complete remission rates (20-35%) and overall response rates (45-65%) in heavily pretreated KMT2Ar and NPM1m acute myeloid leukemia (AML) with me

Exemestane plus everolimus and palbociclib in metastatic breast cancer: clinical response and genomic/transcriptomic determinants of resistance in a phase I/II trial.

The landscape of cyclin-dependent kinase 4/6 inhibitor (CDK4/6i) resistance is still being elucidated and the optimal subsequent therapy to overcome resistance remains uncertain. Here we present the final results of a phase Ib/IIa, open-label trial (NCT02871791) of exemestane plus everolimus and palbociclib for CDK4/6i-resistant metastatic breast cancer. The primary objective of phase Ib was to evaluate safety and tolerability and determine the maximum tolerated dose/recommended phase II dose (100 mg palbociclib, 5 mg everolimus, 25 mg exemestane). The primary objective of phase IIa was to det

Right ventricular strain rate during exercise accurately identifies male athletes with right ventricular arrhythmias.

AIMS: Athletes with right ventricular (RV) arrhythmias, even in the absence of desmosomal mutations, may have subtle RV abnormalities which can be unmasked by deformation imaging. As exercise places a disproportionate stress on the right ventricle, evaluation of cardiac function and deformation during exercise might improve diagnostic performance. METHODS AND RESULTS: We performed bicycle stress echocardiography in 17 apparently healthy endurance athletes (EAs), 12 non-athletic controls (NAs), and 17 athletes with RV arrhythmias without desmosomal mutations (EI-ARVCs) and compared biventricul

Genotype-Phenotype Correlations of Dystrophic Epidermolysis Bullosa in India: Experience from a Tertiary Care Centre.

Recent advances in the field of genomics have seen the successful implementation of whole exome sequencing as a rapid and efficient diagnostic strategy in several genodermatoses. The aim of this study was to explore the potential of molecular studies in dystrophic epidermolysis bullosa (DEB) in India. Whole exome sequencing was performed using genomic DNA from each case of epidermolysis bullosa, followed by massively parallel sequencing. Resulting reads were mapped to the human reference genome hg19. Sanger sequencing subsequently confirmed the potentially pathogenic mutations. Whole exome seq

Small cell carcinoma of the gallbladder: a clinicopathologic, immunohistochemical, and molecular pathology study of 12 cases.

Small cell carcinomas of the gallbladder are unusual neoplasms that have been characterized only recently. The authors describe the clinical, histopathologic, immunohistochemical, and molecular features of 12 small cell carcinomas of the gallbladder. The mean age at diagnosis was 69 years, and the male-to-female ratio was 5:7. The neoplasms had an average size of 3 cm, and 90% showed invasion of the muscularis propria and perimuscular connective tissue. Seventy-five percent of the carcinomas had metastasized or extended locally beyond the gallbladder at surgery. Survival was uniformly poor, wi

Placental mitochondrial DNA mutation-related oxidative stress underlies subsequent risk of adverse prognosis in sFGR twins.

BACKGROUND: Selective fetal growth restriction (sFGR), a serious complication of monochorionic diamniotic (MCDA) twin pregnancies, typically results from unequal placental sharing, especially in type II sFGR. This condition is associated with an increased risk of intrauterine death and adverse neurological outcomes in one of the twin fetuses. Mitochondria play an important role in fetal development and mitochondrial DNA (mtDNA) mutations have been implicated in the pathogenesis of singleton FGR. Mutations in the mtDNA can lead tomitochondrial dysfunction, oxidative stress, and apoptosis. Howev
