Neurological sciences : official journal of the Italian Neurological Society and of the Italian Society of Clinical Neurophysiology2011eng
This report describes the clinical, radiological and autopsy findings of a young male suffering from acute, monophasic demyelinating disease presenting with acute onset of altered sensorium and a short past history of progressive multiple neurological deficits. The radiological findings revealed acute infarcts in the territory of anterior cerebral artery. The antemortem diagnosis centred on an infective (tubercular) aetiology leading to demyelination based on presence of tubercle bacilli in bronchoalveolar lavage (BAL) fluid. The autopsy revealed multiple subacute and acute plaques showing dem

The neural centers in the cerebral hemispheres, both cortex and basal ganglia, involved in the generation of saccadic and smooth pursuit eye movements have been well delineated in terms of their location and function. For the generation of saccades these include the frontal eye fields, the supplementary eye field, and the intraparietal sulcus, and in the basal ganglia the caudate nucleus and the substantia nigra, pars compacta. The generation of pursuit eye movements involves the middle temporal (area V5) and medial superior temporal areas and the frontal eye field. These centers and their con

A two-day-old, full-term male infant was admitted to the neonatal intensive care unit with heart failure. He was found to be non-dysmorphic, with no clinical evidence of sepsis. Physical examination was significant for hepatomegaly, active precordium, pansystolic murmur and hypotension requiring ionotropic support. A cranial bruit was detected on auscultation. Chest radiography revealed cardiomegaly and pulmonary oedema due to heart failure. Electrocardiogram and two-dimensional echocardiography were normal. Magnetic resonance imaging of the brain showed a large vein of Galen malformation, ext

INTRODUCTION: Infectious vascularitis is an unusual cause of ischemic stroke (IS). We report a case of Lyme meningovascularitis complicated with multiple IS.
CASE REPORT: A 64-year-old man, without any cardiovascular risk factor, was admitted for a right hemiparesia with a left thalamic hypodensity on the initial cerebral CT scan. No cause for this presumed IS could be identified. Later, the patient developed cognitive impairment and a bilateral cerebellar syndrome. Multiple infarcts and multiple intracranial stenosis were seen on cerebral MRI with magnetic resonance angiography (MRA). Cerebr

The brain lesions could lead to impairments of the comprehension and production of written language. This acquired inability is named alexia. It is a significant problem for neurologists and ophthalmologists. Our study presents a classification of the alexias, whose pathology was describe first by Dejerine (1891; 1892). There are two varieties of alexias: central alexias and peripheral alexias (especially agnozic alexia and attentional alexia). In agnozic alexia, the patient cannot read, but can write, understand and speak. It results from a type of cerebral disconnection in which the angular

We report a case of a dissecting aneurysm of the middle cerebral artery (MCA) and its treatment strategies. A 50-year-old male patient presented with occipital and neck pain for 2 days. CT scan revealed subarachnoid hemorrhage (SAH) in the region of the right sylvian fissure. A left carotid angiogram showed a dissecting aneurysm of the left MCA (M1). He was treated surgically by a pterional trans-sylvian approach. Clipping was done along with additional reinforcement by wrapping to completely obliterate the neck of the aneurysm. On reviewing the literature, we think that dissecting aneurysms s

BACKGROUND: Research into the causes of small-vessel stroke has been hindered by technical constraints. Cases of intracerebral hemorrhage occurring in unusual clinical contexts suggest a causal role for sudden increases in blood pressure and/or cerebral blood flow.
CASE DESCRIPTION: We describe a fatal primary thalamic/brain stem hemorrhage occurring in the context of sudden emotional upset. At autopsy, the brain harbored several perforating artery fibrinoid lesions adjacent to and remote from the hematoma as well as old lacunar infarcts and healed destructive small-vessel lesions.

To our knowledge we report the first case of meningitis from Coccidioides immitis associated with massive dural and cerebral venous thrombosis and with mycelial forms of the organism in brain tissue. The patient was a 43-year-old man with late-stage acquired immunodeficiency syndrome (AIDS) whose premortem and postmortem cultures confirmed C immitis as the only central nervous system pathogenic organism. Death was attributable to multiple hemorrhagic venous infarctions with cerebral edema and herniation. Although phlebitis has been noted parenthetically to occur in C immitis meningitis in the

A 30-year-old male and a 40-year-old female presented with Aspergillus fungal granuloma in the cerebral locations involving the gasserian-ganglion and its divisions in one case and was densely adherent to the lateral dural wall of the cavernous sinus in the other. Both patients were otherwise healthy with no evidence of immuno-suppression. The lesions resembled benign tumor on preoperative imaging and intraoperative consistency and vascularity. The lesions were successfully and completely resected. Both patients developed major cerebral arterial territory infarcts in the postoperative phase, r

We present an 18-year-old patient with Degos' disease who manifested the sudden onset of paraplegia. Neurological examination revealed a rt-hemianopsia, paraparesis (with a sensory level at Th12), and a neurogenic bladder. Brain MRI showed multiple cerebral infarctions accompanied by small hemorrhagic areas and GdDTPA enhancement of the dura. A cerebral angiogram displayed stenosis, ectasia, and aneurysms involving the peripheral branch of arteries. Stenoses also were observed in the celiac artery and small arteries in the 1t-kidney. Skin biopsy exhibited hyperkeratosis, atrophy of the epiderm

The cerebral hemisphere contralateral to the preferred hand is generally dominant for learned representational motor acts, including those involving buccofacial muscles. It is generally also language-dominant. This buccofacial apraxia has, with rare exceptions, been associated with left hemispheric lesions in right-handers. We describe two patients with severe buccofacial apraxia caused by large middle cerebral artery territory infarcts in the hemisphere ipsilateral to the preferred hand and nondominant for language. Neither patient had aphasia or major limb apraxia. Computed tomographic scans

BACKGROUND AND OBJECTIVE: Cerebral ischemia-reperfusion injury (CIRI) is a major cause of poor outcome after ischemic stroke, and effective therapeutic targets are lacking. This study aimed to investigate the role of polypeptide N-acetylgalactosaminyltransferase 3 (GALNT3) in CIRI.
METHODS: The mouse transient middle cerebral artery occlusion/reperfusion (tMCAO/R) model was established, and transcriptomic analysis of the peri-infarct cortex was performed to screen target genes. In vivo experiments, neurological scoring and TTC staining were used to assess the effects of GALNT3 on brain injury

Background/Objectives: Senescence has been recently described in brain cells following ischemic stroke. The potential of targeting senescence as an effective therapeutic approach in the treatment of ischemic stroke requires further investigation. This study evaluated the effects of the senolytic drug navitoclax after experimental ischemic stroke. Methods: Navitoclax was injected into male young Wistar rats at doses of 10 and 30 mg/kg (i.p.). to evaluate its pharmacokinetics, cerebral levels and potential to cause thrombocytopenia. Subsequently, a second group of rats underwent 60 min of transi

BACKGROUND: Atrial fibrillation (AF) and cerebral small vessel disease (CSVD) are prevalent in older adults and may synergistically contribute to cognitive decline and stroke risk. This study investigates the association between AF and CSVD markers, including white matter hyperintensities (WMHs), cerebral microbleeds (CMBs), lacunes, and silent brain infarcts, focusing on sex-specific patterns, anticoagulation status, and predictive modeling.
METHODS: In a cross-sectional study of 923 individuals aged ≥70 years undergoing 3.0-Tesla brain MRI, AF was identified via ECG, self-reports, and hospi

BACKGROUND: Ischemic stroke remains a major cause of mortality and disability, with few therapeutic options outside the narrow time window for reperfusion. Collateral circulation critically sustains penumbral viability, yet no pharmacological agent is approved to enhance collateral flow. Salvia (S.) miltiorrhiza and Panax (P.) notoginseng possess complementary vascular, anti-inflammatory, and neuroprotective actions, suggesting potential synergy in acute ischemia.
METHODS: A combined extract of S. miltiorrhiza and P. notoginseng (SM/PN) was tested in a permanent middle cerebral artery occlusi

BACKGROUND: Ischemic stroke is a leading cause of death and disability, with reperfusion injury-driven oxidative stress representing a key therapeutic target. Selenium nanoparticles (SeNPs) show neuroprotective potential, yet the influence of nanoparticle morphology remains poorly understood.
OBJECTIVE: To compare the neuroprotective effects of spherical (Sph) versus rod-shaped (Rods) SeNPs in models of cerebral ischemia/reperfusion, focusing on redox modulation, mitochondrial bioenergetics, and functional outcomes.
METHODS: Primary rat cortical neuron-astrocyte co-cultures were exposed to S

Background The diagnosis of tuberculous meningitis (TBM) is challenging, as it requires a high level of clinical suspicion and robust efforts to manage complications. Aim The study focussed on the early detection of seizures and their functional and final outcome with etiological workup radiologically and on lab investigations. Design and methods This was a prospective observational study, in which 100 suspected TBM patients were enrolled based on Lancet consensus criteria at the Postgraduate Institute of Medical Education and Research (PGIMER), Chandigarh, from August 2022 to December 2023. BACKGROUND: Acute infectious meningoencephalitis is a severe condition caused by bacterial, viral, fungal, or parasitic infections of the central nervous system (CNS). A rare but severe complication is cerebral vasculitis with ischemic infarcts. The available literature is sparse and mainly restricted to case reports, further challenging an accurate and timely diagnosis.
METHODS: We provide a detailed retrospective analysis of five cases of fulminant infectious vasculitis accompanied by ischemic strokes treated at the neurological intensive care unit (ICU) of the Department of Neurology, Medi

Multiple consensus statements have called for preclinical randomized controlled trials to improve translation in stroke research. We investigated the efficacy of an interleukin-17A neutralizing antibody in a multi-centre preclinical randomized controlled trial using a murine ischaemia reperfusion stroke model. Twelve-week-old male C57BL/6 mice were subjected to 45 min of transient middle cerebral artery occlusion in four centres. Mice were randomly assigned (1:1) to receive either an anti-interleukin-17A (500 µg) or isotype antibody (500 µg) intravenously 1 h after reperfusion. The primary end
