A 40-Year-Old Man With 2-Year Chronic Epigastric Pain From a Retained 18-cm Toothbrush: Endoscopic Management and Clinical Decision-Making.

BACKGROUND Foreign body ingestion is common in children, whereas ingestion of non-food objects in adults is uncommon. Toothbrush ingestion is particularly rare. Because of their length and rigidity, toothbrushes are unlikely to pass spontaneously through the gastrointestinal tract and can cause pressure injury, ulceration, perforation, or fistula if retained. We report a case of chronic epigastric pain caused by long-term retention of a swallowed toothbrush and highlight the value of endoscopy when radiologic findings appear more severe than the clinical presentation. CASE REPORT A 40-year-old

Evolution of endoluminal treatment of Zenker's diverticulum and complete pharyngo-oesophageal obstruction.

PURPOSE OF REVIEW: Endoscopic approaches to surgery of the foregut, in particular, the upper oesophageal sphincter (UES), continue to develop, providing options for management that were previously unavailable. This review examines endoluminal surgical tunnelling approaches (third space endoscopy) to treat Zenker diverticulum and the more rare condition of complete oesophageal stenosis. RECENT FINDINGS: Endoluminal surgical approaches to the UES such as Z-POEM (per-oral endoscopic myotomy) that utilize gastroscopes and upper gastrointestinal endoscopic instrumentation, are increasingly being u

Real-time Use of Computer-aided Diagnosis in the Optical Diagnosis of Gastric Neoplasia: A Multicenter Randomized Controlled Trial.

BACKGROUND & AIMS: The multicenter, randomized, control trial was conducted to evaluate whether computer-aided diagnosis (CADx) improves the optical diagnosis of gastric neoplasia in nonexpert endoscopists. METHODS: Patients undergoing endoscopy prior to endoscopic resection (ER) of gastric neoplasia or for surveillance after ER were randomized to the CADx-assisted and non-CADx-assisted groups. Endoscopy was performed by a nonexpert endoscopist blinded to the patient information. The CADx system provides a confidence level for gastric neoplasia on a still white light image (WLI) captured duri

A Sequential Combination of Baveno VI Criteria With Spleen Stiffness Measurement Improves the Screening of High-Risk Esophageal Varices in Compensated Advanced Chronic Liver Disease.

BACKGROUND: Detecting high-risk esophageal varices (HRV) in cirrhotic patients is crucial for intervention and preventing variceal bleeding. While endoscopy is the gold standard for diagnosis, it is invasive and carries risks. Spleen stiffness measurement (SSM) may improve HRV screening and increase the spared endoscopy rate (SER). METHODS: We enrolled 129 patients with compensated advanced chronic liver disease (cACLD) who were candidates for variceal screening based on Baveno VI criteria (transient elastography ≥ 20 kPa or platelet count ≤ 150 × 109/L). We collected SSM50Hz, SSM100Hz, liver

Gastroenteropancreatic Neuroendocrine Tumors.

Neuroendocrine tumors (NETs) represent a heterogeneous group of neoplasms with diverse biological and clinical behavior, and gastroenteropancreatic neuroendocrine tumors (GEP-NETs) are the most common subtype. This review provides an overview of GEP-NETs, with a focus on incidence trends, pathologic classification, diagnostic strategies, therapeutic advances, and the role of endoscopy in diagnosis and management of GEP-NETs. Incidence rates of GEP-NETs have significantly increased over recent decades, largely due to improved diagnostic modalities and increased use of endoscopy, although enviro

Narrow band imaging for detection of gastric intestinal metaplasia and dysplasia: A systematic review and meta-analysis.

BACKGROUND AND AIMS: Gastric intestinal metaplasia (GIM), a precursor of gastric adenocarcinoma, is challenging to diagnose with white light endoscopy (WLE) and can be missed by random gastric biopsies. Narrowband imaging (NBI) may potentially improve the detection of GIM. However, pooled estimates from prospective studies are lacking. METHODS: Electronic databases were searched for studies comparing NBI and WLE alone for detection of GIM and synchronous dysplasia. Primary outcome was pooled detection rate of GIM by NBI compared with WLE in prospective studies. The secondary outcome was concu

Structured oropharynx, hypopharynx and larynx assessment during routine esophagogastroduodenoscopy improves detection of pre- and early cancerous lesions: a multicenter, comparative study.

Background and study aims  A structured assessment of the oropharynx, hypopharynx and larynx (OHL) may improve the diagnostic yield for the detection of precancerous and early cancerous lesions (PECLs) during routine esophagogastroduodenoscopy (EGD). Thus, we aimed to compare routine EGDs ± structured OHL assessment (SOHLA), including photo documentation with regard to the detection of PECLs. Patients and methods  Consecutive patients with elective EGD were arbitrarily allocated to endoscopy lists with or without SOHLA. All detected OHL abnormalities were assessed by an otolaryngologist-head &
