Preoperative prediction of the selection of the NOTES approach for patients with symptomatic simple renal cysts via an interpretable machine learning model: a retrospective study of 264 patients.

BACKGROUND: There are multiple surgical approaches for treating symptomatic simple renal cysts (SSRCs). The natural orifice transluminal endoscopic surgery (NOTES) approach has gradually been applied as an emerging minimally invasive approach for the treatment of SSRCs. However, there are no clear indicators for selecting the NOTES approach for patients with SSRCs. We aimed to investigate the preoperative clinical determinants that influence the selection of the NOTES approach in patients with SSRCs and to construct a prediction model to assist the surgeons in selecting the NOTES approach. X-linked lymphoproliferative disease with initial onset of neurological symptoms: a case and literature review.

X-linked lymphoproliferative syndrome type 1 (XLP-1) is a life-threatening X-linked recessive immunodeficiency classically characterized by susceptibility to Epstein-Barr virus (EBV), hypogammaglobulinemia, and lymphoma. While neurological involvement can occur, it is exceptionally rare as the initial and predominant manifestation. This case report details a novel presentation of XLP-1 in a 4-year-old boy who presented with acute, initial neurological symptoms (sudden fever, headache, and vomiting) in the absence of typical immune dysregulation features. Whole-exome sequencing (WES) identified

Value of Optical Genome Mapping (OGM) for Diagnostics of Rare Diseases: A Family Case Report.

Optical genome mapping (OGM) is a novel method enabling the detection of structural genomic variants. The method is based on the laser image acquisition of single, labeled, high-molecular-weight DNA molecules and can detect structural genomic variants such as translocations, inversions, insertions, deletions, duplications, and complex structural rearrangements. We aim to present our experience with OGM at the Clinical Institute of Genomic Medicine, University Medical Centre Ljubljana, Slovenia. Since its introduction in 2021, we have used OGM for the testing of facioscapulohumeral muscular dys

Unused Opioids Following Palliative Care for Children at Home: A Missed Opportunity for Opioid Stewardship?

CONTEXT: Opioids are often a key component of compassionate end-of-life care for children at home. However, after a child dies, unused opioids pose a risk for misuse or diversion, thereby contributing to the ongoing opioid crisis. OBJECTIVES: This exploratory study aimed to identify the prevalence and context of unused opioids in bereaved families' homes to inform interventions to remove medications in a compassionate and timely manner. METHODS: Bereaved families whose child received palliative care from a tertiary hospital in Ontario, Canada, were invited to participate in an online survey

The role of sclerostin in the development and progression of cardiovascular diseases- a potential biomarker?

Sclerostin, a glycoprotein primarily synthesized by osteocytes, is a key regulator of bone remodeling through its inhibition of the Wnt/β-catenin signaling pathway. Beyond its established role in bone metabolism, sclerostin has been identified in vascular tissues, including smooth muscle cells of blood vessels, suggesting a potential role in cardiovascular diseases (CVD). Research on sclerostin's role in CVD has yielded conflicting results, highlighting its complex involvement in vascular calcification and atherosclerosis. Some studies associate elevated sclerostin levels with a reduced risk o

Association of fast foods and dairy products in acne severity and scarring: a cross-sectional study.

INTRODUCTION: The pilosebaceous unit is impacted by the prevalent chronic inflammatory skin condition known as acne vulgaris. The formation of comedones, papules, and pustules are among the clinical signs of acne. Acne is usually thought to be harmless, yet it can leave disfiguring scars and have psychological effects. METHODS: In our cross-sectional study with 386 participants, we examined acne severity and scarring in relation to variables such as age, gender, family history of acne, and dietary habits (especially fast food and dairy products). The assessments were conducted by a trained me

Ultrasound, Doppler and other signs during drug-free transition from subclinical hypothyroidism to euthyroidism: a case report.

BACKGROUND: It is known that subclinical hypothyroidism (SCH) often converts to euthyroidism. However, the mechanism of such changes is unclear and can only be speculated. This difficulty is likely due to limitations in diagnostic guidelines and their interpretation. Therefore, in the presented case, important diagnostic criteria that can improve understanding of the essence and pathogenesis of primary hypothyroidism are shown and explained. CASE DESCRIPTION: A 15-year-old female patient experienced a transition from SCH to euthyroidism against the background of highly normal values of thyroi

Cochlear clefts in children: prevalence, characteristics, and clinical implications.

BACKGROUND: Temporal bone computed tomography (CT) is essential for evaluating ear diseases but requires awareness of normal variations to avoid misdiagnosis. The cochlear cleft, seen as pericochlear CT hypoattenuation, reflects incomplete endochondral ossification and mainly occurs in children. Although a normal variant, it can mimic conditions such as otosclerosis. Data on its prevalence and extent in children, especially in relation to hearing status and other temporal bone findings, remain limited. OBJECTIVE: The aim of our study is to evaluate the overall prevalence of cochlear clefts in
