Infants of diabetic mothers.

Advances in the management of the mother with diabetes have reduced the rate of morbidity and mortality for her infant. Aggressive control of maternal glycemic status is warranted, because most morbidities are epidemiologically and pathophysiologically closely linked to fetal hyperglycemia and hyperinsulinemia. The burgeoning public health problem of overweight and obesity in children will likely result in an increased incidence of metabolic syndrome X, characterized by insulin resistance and type II diabetes in adulthood. An early manifestation of this may be glucose intolerance during pregna

Homocystinuria presenting as psychosis in an adolescent.

Homocystinuria usually presents with ectopia lentis, mental retardation, thromboembolic complications, and skeletal abnormalities. Whereas neuropsychiatric abnormalities are often recognized in untreated homocystinuria, initial presentation with acute psychosis has only rarely been reported. We describe a previously well 17-year-old adolescent with an acute psychosis characterized by auditory and visual hallucinations and marked paranoia who was found to have pyridoxine-responsive homocystinuria. His mental state normalized within several weeks of inception of pyridoxine and antipsychotic ther

[Calculation of the "safety index" in clinical practice].

The paper is aiming to evaluate the SI calculation for children with ROP in clinical practice with respect to the prognosis and possible shortening of follow-up interval. The SI calculation and anamnestical evaluation was done in all preterm neonates referred for ROP screening in Faculty Hospital in Olomouc within 36 months interval with the stress to the relation of SI and ROP stage. Regarding to the prognosis SI corresponds well with ROP stage in children with higher risk, when asymmetrical so with the worse eye. Even above the threshold (SI = 1.0) ROP-3 was developed in some individual case

Hemolytic jaundice due to G6PD deficiency causing kernicterus in a female newborn.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an inherited deficiency of an enzyme necessary to protect the erythrocyte from oxidative stress and hemolysis. Without this enzyme, affected neonates are at risk for acute onset of hemolytic jaundice and severe sequelae, from hearing loss and mild retardation to kernicterus. In some populations, especially in blacks and those of Mediterranean ancestry, the incidence of G6PD deficiency has been reported to be as high as 10% to 14%. We describe a female newborn who had acute onset of hyperbilirubinemia leading to kernicterus in the first wee

Further delineation of spondylo-meta-epiphyseal dysplasia, short limb-abnormal calcification type, with emphasis on diagnostic features.

Further delineation of a generalized bone dysplasia which we call spondylo-meta-epiphyseal dysplasia, short limb-abnormal calcification type is presented. This dwarfing condition has several serious complications, with the most common cause of death being spinal cord damage secondary to atlantoaxial instability. It is a heritable condition with an autosomal recessive mode of transmission. Radiologic diagnostic criteria are developed on the basis of studies in 8 patients with the oldest being between 4 and 5 years old. The condition is clinically and radiographically apparent neonatally or in e

Results of a preventive program for congenital toxoplasmosis.

All pregnant women followed during the period 1982-87 were screened for toxoplasmosis, and 35 patients had documented seroconversion or doubtful toxoplasmosis titers. One patient opted for pregnancy termination. The remaining were followed with a protocol that included serial ultrasound examinations and prophylactic antibiotic treatment of the mother and neonate. No fetal abnormalities related to congenital toxoplasmosis were found. All the infants had negative toxoplasmosis test titers at birth; at follow-up only one was found to have developed a subclinical infection, at 2 months of age. Our

Prevalence of Gestational Diabetes Mellitus and associations with adverse pregnancy outcomes in the Faroe Islands: a population-based study.

BACKGROUND: Gestational diabetes mellitus (GDM) is associated with adverse maternal and neonatal outcomes. The Faroe Islands follow a selective screening strategy for GDM, and little population-based data exist. This study aimed to determine the prevalence of GDM in the Faroe Islands and its association with perinatal outcomes. METHODS: This nationwide, register-based study included all women with singleton pregnancies from 2021 to 2023 (n = 1,703). Under the national risk-based screening strategy, 1,017 (60%) women underwent a 2-h oral glucose tolerance test (OGTT) using the diagnostic cut-of

Association between bacterial presence in urinalysis and hypertension during pregnancy in women at Margono Hospital, Purwokerto: A single-center study in Indonesia.

INTRODUCTION: Urinary tract infections are prevalent among pregnant women and can lead to serious maternal and neonatal complications. Hypertensive disorders of pregnancy, a leading cause of maternal morbidity, may be associated with urinary tract infections. This study investigates whether bacteriuria detected via routine urinalysis, a standard screening in Indonesia, contributes to hypertension risk during pregnancy, aiming to enhance clinical management and screening protocols. AIM OF STUDY: To evaluate the association between bacteriuria detected through routine urinalysis and hypertensio

Ophthalmic Changes in the Offspring of Pregnant Women with Gestational Diabetes Mellitus or Diabetes Mellitus - A Systematic Review.

BACKGROUND: Gestational diabetes mellitus (GDM) affects 5.8% to 25.1% of pregnant women and is associated with a range of adverse perinatal outcomes, including intrauterine growth restriction, prematurity, neonatal respiratory distress, and adiposity. Offspring of mothers with GDM or diabetes mellitus (DM) are also at elevated risk for long-term metabolic complications, such as obesity, dyslipidemia, and type 2 DM. While systemic and structural anomalies-including congenital heart disease, skeletal malformations, and renal agenesis-have been well documented, limited attention has been paid to

Case Report: Congenital neurosyphilis presenting as post-hemorrhagic hydrocephalus in a preterm infant and a review of literature.

BACKGROUND: Congenital syphilis (CS) remains a global public health concern, with rising incidence even in high-income countries. In Italy, a higher risk has been reported among primigravidae and younger mothers with late or missed prenatal screening. While neurological involvement in CS is well known, it is rarely reported in preterm infants, where it may be severe and atypical. CASE PRESENTATION: A male preterm infant, delivered at 32 weeks via emergency cesarean due to abnormal fetal monitoring and breech position, was admitted to our NICU with intraventricular hemorrhage, midline shift, a

[Congenital cytomegalovirus infection, is more prevalent in our country in newborns exposed to HIV?].

OBJECTIVE: Congenital cytomegalovirus infection (cCMV) has been considered more prevalent among HIV-exposed children during pregnancy. Spanish national guidelines recommend the cCMV screening in these newborns. Nowadays, pregnant women have a better control of HIV infection compared to previous decades. We aim to analyze the prevalence and associated risk factors to cCMV in these children. METHODS: A retrospective cross-sectorial study was performed. All newborns exposed to HIV were assisted in a third-level hospital (2014-2020). Epidemiological and clinical data of the mother and newborn wer

Two new variants of G6PD deficiencies in Singapore.

Identification of mutations in G6PD gene is performed as an epidemiologic investigation of G6PD deficiency in many countries. In order to understand the hereditary background of G6PD deficiency in a population, screening of mutations is required not only in exonic regions but also for intron and promoter regions. One hundred male neonatal samples diagnosed as with G6PD deficiency by newborn screening in Singapore were used in this study. The multiplex PCR using the multiple tandem forward primers and common reverse primer (MPTP) method was carried out to detect the common 11 mutations in south

Predictors of early-onset permanent hearing loss in malnourished infants in Sub-Saharan Africa.

The objective of this study was to determine the predictors of early-onset permanent hearing loss (EPHL) among undernourished infants in a low-income country where routine screening for developmental disabilities in early childhood is currently unattainable. All infants attending four community-based clinics for routine immunization who met the criteria for undernutrition by the Growth Standards of the World Health Organization (WHO) based on weight-for-age, weight-for-length and body-mass-index-for-age were enlisted. EPHL was determined after two-stage screening with transient-evoked otoacous

[Evaluation of the detection of phenylketonuria and hypothyroidism at the Lille Regional Center].

During the past 20 years, neonatal screening for metabolic diseases has been developed and implemented in the Nord-Pas de Calais region. The regional centre (established in 1972) has screened more than 1,000,000 babies for phenylketonuria (PKU) and more than 700,000 for congenital hypothyroidism (CH). Ninety-two PKU and 182 CH patients were discovered and treated. The recent introduction of a computerised system for the clerical works in the centre has allowed a systematic survey of some parameters linked to the screening practices: collection, handling and follow-up of samples. The analysis o
