Late onset lamellar keratitis and epithelial ingrowth following orbital cellulitis.

PURPOSE: To report a case of late onset lamellar keratitis and epithelial ingrowth associated with orbital cellulitis 1 month after LASIK surgery. METHODS: A 19-year-old patient presented with pain and subsequent blurry vision in the right eye. One month prior to presentation, he had undergone LASIK in both eyes. Orbital cellulitis was suspected. RESULTS: Under aggressive treatment, the patient recovered from orbital cellulitis and lamellar keratitis and he also regained his vision with topical steroid treatment. CONCLUSIONS: Lamellar keratitis does not always occur early following LASIK an

Preliminary study of positron emission tomography in the detection and management of orbital malignancy.

PURPOSE: To determine the clinical efficacy of positron emission tomography (PET) in the treatment of patients with orbital malignancy. METHODS: Prospective, consecutive case series to assess the ability of PET to (1) identify lesions previously demonstrated on CT or MRI; (2) characterize the metabolic activity of these lesions; and (3) determine the presence of metabolically active metastases elsewhere in the body. RESULTS: Eighteen patients with suspected orbital malignancies, ages 32 to 78 years, underwent PET in addition to CT and/or MRI. Sixteen of 18 also underwent orbital biopsy. Hist

High-flow orbital arteriovenous malformation in a child: current management and options.

Vascular malformations of the orbit cause significant morbidity such as chronic pain, diplopia, amblyopia, and cosmetic disfigurement. They are rare lesions which require multidisciplinary care, and in the modern era results of treatment have been greatly improved with the assistance of endovascular therapy. Other treatment options include laser therapy, percutaneous embolization, open surgery, or a combination of these modalities. Nevertheless some patients suffer poor results despite modern medical advances. A case of an orbital arteriovenous malformation (AVM) initially treated independentl

[Systemic enzyme therapy in the treatment of children with lower orbital wall fractures].

Wobenzim, a drug for systemic enzyme therapy, was used in 26 children with lower orbital wall fractures as monotherapy in early periods (on days 1 to 7) after brain injury to improve healing conditions and to minimize the formation of scar tissues in the fractural area. A control group comprised 24 patients who received systemic dehydration and vessel-strengthening therapy. Systemic enzyme therapy with Wobenzim could reduce the attenuation time of inflammatory manifestations of injury, prevent complications, such as strangulation of the musculus rectus inferior in the fractural area and develo

Persistent unilateral orbital and eyelid oedema as a manifestation of Melkersson-Rosenthal syndrome.

Melkersson-Rosenthal syndrome (MRS) is a complex neuromucocutaneous disorder characterized by localized orofacial oedema and cranial nerve dysfunction, frequently associated with minor signs, including furrowed tongue. Complete forms are rare whereas mono- and oligosymptomatic variants are more common. A 71-year-old man presented with a 2-year history of relapsing and progressively persistent oedema of the right eyelids and periorbital region. A fissured tongue and telangiectatic rosacea had been present since the age of 50 and 60 years, respectively. The patient was also affected by essential

Orbital metastasis as the initial finding of breast carcinoma: a ten-year survival.

A 50-year-old woman with a clinically negative breast examination was diagnosed with metastatic breast cancer only after pathologic evidence of an orbital metastasis. She presented with pressure and pain above the right eye associated with enophthalmos and pseudo-Brown syndrome. The diagnosis of breast cancer based on initial ophthalmic findings requires a strong level of suspicion. Multiple reports show that survival in these cases is limited, usually only 1 year after diagnosis. We present a case of 10-year survival after the diagnosis of breast cancer presenting as an orbital mass. These ne

Secondary orbital implants after evisceration: a new conjunctiva-sparing technique.

PURPOSE: To describe a new conjunctiva-sparing technique for secondary orbital implantation after evisceration. METHODS: Two patients with conjunctival cicatrization and a volume-deficient anophthalmic socket had implantation of an intraconal biointegratable implant. This was placed through a lateral canthal approach, after temporary disinsertion of the lateral rectus, thereby avoiding further injury to the conjunctiva. RESULTS: A good surgical outcome was achieved in both patients. There were no intraoperative or postoperative complications, and both have remained stable for nearly 2 years.

Bilateral exudative retinal detachments as the presenting features of idiopathic orbital inflammation.

Idiopathic orbital inflammation, also known as orbital pseudotumour, is a term describing a spectrum of idiopathic, non-neoplastic, non-infectious, space-occupying orbital lesion without identifiable local or systemic cause. This disease occurs mainly in young adults and typically presents with acute proptosis, chemosis and limited extraocular movement. Herein an unusual case of idiopathic orbital inflammation presenting with bilateral exudative retinal detachment in a 9-year-old girl is described. It demonstrates that prompt diagnosis and corticosteroid treatment can yield good clinical respo

Expanding MIRAgel scleral buckle simulating an orbital tumor in four cases.

PURPOSE: To describe four patients with an enlarging orbital mass from a swollen MIRAgel scleral buckle that simulated an orbital neoplasm. METHODS: In a retrospective, single-center case series at the Ocular Oncology Service at Wills Eye Hospital of Thomas Jefferson University, 4 eyes of 4 patients were referred for evaluation and treatment of a suspected orbital tumor. RESULTS: The initial presenting features were orbital mass (case 1), strabismus (case 2), and conjunctival mass with orbital extension (cases 3 and 4). Each patient vaguely recalled previous uncomplicated retinal detachment

Cavernous hemangioma extending to extracranial, intracranial, and orbital regions. Case report.

Extraaxial cavernous hemangiomas are rare intracranial lesions that can be located in different cranial compartments. Extension across different tissue planes such as the subcutaneous tissue, skull, orbital cavity, intracranial dura mater, and extracranial trigeminal divisions within the same patient has not been previously reported. This 32-year-old woman suffered left exophthalmos, left sixth nerve palsy, and trigeminal neuropathy. Magnetic resonance imaging studies revealed an extensive multicompartmental lesion, with enhancement following Gd administration. A left orbitopterional approach

Orbital apex syndrome after ethmoidal artery ligation for recurrent epistaxis.

A 34-year-old man with severe, recurrent epistaxis underwent external anterior and posterior ethmoidal artery ligation on the right side. Visual loss from optic neuropathy and complete ophthalmoplegia developed after surgery. Computed tomography revealed surgical clips within the right orbital apex. Emergent removal of the surgical clips and medial wall decompression were performed. Four weeks later, his visual acuity and extraocular motility were unchanged, and diffuse pallor of the right optic disc was observed. Orbital apex syndrome can be a complication of ethmoidal artery ligation. Severe

Seesaw modification of the lateral orbital wall in Le Fort III osteotomy.

OBJECTIVE: The traditional treatment for patients with syndromic craniosynostosis and midfacial retrusion has consisted of Le Fort III osteotomy and advancement. Distraction with rigid external systems allows advancement of the midface segment much more than the conventional methods. This excessive advancement resulted in the superiormost margin of the advancement segment becoming prominent. It can be felt easily with palpation and may influence the appearance of the patient negatively. This article presents a procedure osteotomy designed to modify the osteotomy lines at the lateral orbital ri

Reconstruction of orbital floor fracture using solvent-preserved bone graft.

The orbital floor is one of the most frequently damaged parts of the maxillofacial skeleton during facial trauma. Unfavorable aesthetic and functional outcomes are frequent when it is treated inadequately. The treatment consists of spanning the floor defect with a material that can provide structural support and restore the orbital volume. This material should also be biocompatible with the surrounding tissues and easily reshaped to fit the orbital floor. Although various autografts or synthetic materials have been used, there is still no consensus on the ideal reconstruction method of orbital
