Aspergillus Sternomyelitis Developed from Chronic Pulmonary Aspergillosis as a Late Complication to Lobectomy for Lung Cancer.

Progressive fibrobullous changes in the residual lobes are sometimes observed after lobectomy. Aspergillus osteomyelitis is an uncommon infection that rarely occurs sternally. A 70-year-old man who had undergone lobectomy 12 years earlier was admitted to our hospital for chest pain. He was diagnosed with Aspergillus sternomyelitis based on sternal bone culture after an ultrasound-guided percutaneous needle biopsy. The fibrosis and right residual lung apex volume loss had gradually progressed over 12 years, and therefore, chronic pulmonary aspergillosis (CPA) with direct invasion sternal from t

MR Imaging of the Diabetic Foot.

Abnormalities of the peripheral nervous, vascular, and immune systems contribute to the development of numerous foot and ankle pathologies in the diabetic population. Although radiographs remain the most practical first-line imaging tool, magnetic resonance (MR) is the tertiary imaging modality of choice, allowing for optimal assessment of bone and soft tissue abnormalities. MR allows for the accurate distinction between osteomyelitis/septic arthritis and neuropathic osteoarthropathy. Furthermore, it provides an excellent presurgical anatomic road map of involved tissues and devitalized skin t

Circular External Fixation as a Primary or Adjunctive Therapy for the Podoplastic Approach of the Diabetic Charcot Foot.

Numerous techniques have been described for surgical management of the diabetic Charcot foot. External fixation has become a main surgical tool for the reconstructive foot and ankle surgeon when dealing with the ulcerated diabetic Charcot foot. In the presence of an open wound and/or osteomyelitis, staged reconstruction with circular external fixation becomes ideal for salvage of the diabetic lower extremity. Also, circular external fixation can provide simultaneous compression and stabilization, correct the underlying osseous or soft tissue deformities, and surgically offload the diabetic Cha

Soft Tissue Reconstruction Pyramid for the Diabetic Charcot Foot.

Foot and ankle ulcerations in patients with diabetic Charcot neuroarthropathy (DCN) occur frequently and can be challenging to address surgically when conservative care fails. Patients with acute or chronic diabetic foot ulcers (DFU) are at continued risk for development of osteomyelitis, septic arthritis, gas gangrene, and potential lower extremity amputation. Concurrent vasculopathy and peripheral neuropathy as well as uncontrolled medical comorbidities complicate the treatment approach. In addition, pathomechanical forces left untreated may contribute to DFU recurrence in this patient popul

Empyema thoracis due to intrapleural migration of retained vascular catheter.

Intravascular retention and embolization of fragment of central venous catheters is a rare but well-documented complication of in-dwelling vascular access devices and ports. We describe the case of a 39-year-old male with fracture of the central venous catheter during removal of subcutaneous vascular access port. Over the next 3 years, the catheter fragment embolized through the pulmonary circulation and subsequently migrated into the left pleural space. He presented with empyema thoracis associated with thoracolumbar vertebral osteomyelitis. He required video-assisted thoracoscopy for retriev

Incidental findings of massive heel spurs in a veteran with a variant of psoriatic arthritis.

A middle-aged man presented for left foot diabetic ulcer care. Pedal radiographs were negative for signs of osteomyelitis. However, asymptomatic incidental osseous findings demonstrated significant plantar and posterior calcaneal spurring possibly consistent with diffuse idiopathic skeletal hyperostosis (DISH). A differential of DISH, psoriatic arthritis, Reiter's, and ankylosing spondylitis was developed. Subsequent spinal imaging and laboratory work-up did not satisfy the diagnostic criteria for DISH. This case illustrates radiographic changes characteristic of multiple seronegative arthropa

Extranodal natural killer/T cell lymphoma, nasal type presenting as a palatal perforation and naso-oral fistula.

Extranodal natural killer/T cell lymphoma (ENKL), nasal type, a rare disease presenting with vague non-specific symptoms, can impose great diagnostic difficulties and may masquerade several infectious, rheumatological or neoplastic conditions. Here, the authors report a case of ENKL presenting as a palatal perforation, naso-oral fistula and fever in a 21-year-old man, which was initially misdiagnosed as bacterial osteomyelitis, invasive fungal infection and Wegener's granulomatosis. The case report underlines the possibility of ENKL occurring at a young age, its rapidly progressive and locally

Primary frontal sinus carcinoma with extradural anterior cranial fossa involvement.

Paranasal sinus carcinoma is rare, with an estimated annual incidence of less than 1 per 100,000 population. Primary frontal sinus involvement is extremely rare, accounting for only 0.3% of all paranasal sinus malignancies. A frontal sinus cancer may be mistaken for a mucocele, pyocele, or osteomyelitis. We report the case of a 48-year-old woman with a carcinoma that originated in the frontal sinus. Computed tomography and magnetic resonance imaging demonstrated bony destruction with intracranial extension but no involvement of the bulbus. The patient underwent a frontal craniotomy, tumor exci

Septic rupture of the ascending aorta after aortocoronary bypass surgery.

We describe an exceptional case of non-fatal septic rupture of the ascending aorta in a patient with sternal dehiscence, deep sternal wound infection (DSWI) and pleural empyema after aortocoronary bypass surgery. Routine follow-up computed tomography (CT) detected a mediastinal pseudoaneurysm originating from the ascending aorta. Thereby, massive and irregular sternal bone defects and contrast-enhancing mediastinal soft tissue suggest osteomyelitis and highly-active and aggressive DSWI as initial triggers. Urgent thoracotomy 1 day later included ascending aorta reconstruction, total sternum re

Efficacy of composite allograft and demineralized bone matrix graft in treating tibial plateau fractures with bone loss.

Tibial plateau fractures with bone loss or significant comminution require grafting and stable fixation. We hypothesized a standardized protocol of internal fixation augmented with a mixture of demineralized bone matrix and corticocancellous allograft chips would result in high healing rates with minimal subsidence. Union was achieved in all 36 patients available for follow-up by a mean of 4.4 months. Mean range of motion was 2 degrees to 120 degrees. One patient developed osteomyelitis. Subsidence ranging from 2.5 to 5.7 mm occurred in 4 patients (11%). This treatment method provides sufficie

Acute septic arthritis of the hip joint in infancy and childhood.

Forty-three children with 45 hip joint infections were admitted to the hospital between 1983 and 1988. The duration of symptoms was the single most important prognostic feature. Almost all children who were treated within four days of symptoms had a satisfactory outcome. Concomitant osteomyelitis of the proximal femur produced by far a worse prognosis than if the infection was confined to the synovium of the hip joint. In addition, patients whose hips were infected with Staphylococcus aureus bacteria were more likely to have a poor prognosis than those whose hips were infected with non-S. aure

Clinical and pathologic studies of twenty-six patients with penetrating foreign body injury to the joints, bursae, and tendon sheaths.

Foreign body synovitis has been neglected in the rheumatology literature. We describe 26 patients in whom arthritis, bursitis, or tenosynovitis appeared within 1 day to 7 years after an initial injury by a penetrating foreign body. Twenty-two patients presented with acute synovitis, which was followed by chronic or recurrent inflammation mimicking septic arthritis, osteomyelitis, monarticular juvenile rheumatoid arthritis, bone tumor, or apatite deposition disease. Foreign bodies were not seen in 5 inflammatory synovial fluids studied, but were seen in the synovium or periarticular tissues of

A variant of the congenital dyserythropoietic anaemia type II with structural abnormalities in the granulocytic series.

Typical features of congenital dyserythropoietic anaemia (CDA) were found in a 13-year-old girl admitted for chronic recurrent multifocal osteomyelitis. The findings on light microscopy were in agreement with those described in CDA type II, whereas on electron microscopy, the ultrastructure findings were compatible with both types I and II. The acidified serum lysis test (Ham test) performed with eight normal sera was negative. The patient's red blood cells showed an increased agglutinability with anti-i antibodies. Morphological changes were also shown in the mature neutrophilic granulocyte s

Septic arthritis mimicking cellulitis: distinction using radionuclide bone imaging.

Two patients originally diagnosed as having cellulitis involving the dorsum of the foot actually had bacterial arthritis of an underlying joint. In both patients, even after pyarthrosis was suspected, the wrong joint was aspirated. The arthropathies were located by subsequent 99mTc-phosphate bone imaging at a time when roentgenograms were normal. Early diagnosis and aggressive therapy of septic arthritis are essential to prevent joint destruction and osteomyelitis. Radionuclide bone imaging can identify inflammatory joint disease but it cannot specify etiology. In our patients, however, the di

Staphylococcus aureus sepsis in children with cancer.

Seventy episodes of Staphylococcus aureus sepsis occurring over a nine-year period in pediatric cancer patients are reviewed. Prominent findings at the time of diagnosis included fever, granulocytopenia, and active malignancy. Probable or suspected sites of primary infection were present in 40 episodes (57%). Serious direct complications of staphylococcal sepsis included only three cases of pneumonia and one of myositis. However, second infections by other organisms developed in 16 episodes (24%), resulting in nine nonstaphylococcal infectious deaths during therapy. Endocarditis and osteomyeli

Magnetic resonance imaging of a pediatric osteoarticular complication of sickle cell disease: Bone infarction associated with soft tissue collection.

Sickle cell disease is a hereditary hemoglobinopathy characterized by hemoglobin S polymerization, leading to recurrent vaso-occlusive episodes and multiorgan complications. Osteoarticular involvement is a major cause of morbidity in children. We report the case of a 10-year-old girl with homozygous sickle cell disease (HbSS) who presented with acute right leg pain, fever, and localized swelling. Magnetic resonance imaging (MRI) demonstrated a metaphyseal bone infarction associated with an adjacent soft tissue collection. This case highlights the pivotal role of MRI in evaluating bone marrow a

Disseminated Scytalidium philadelphianum infection in a Belgian Malinois.

We report disseminated mycosis in a Belgian Malinois caused by Scytalidium philadelphianum. The dog presented for progressive hindlimb weakness after diagnosis of suspected bacterial discospondylitis. Magnetic resonance imaging combined with radiographs revealed multifocal discospondylitis, osteomyelitis, and spondylitis with epidural empyema. Scytalidium philadelphianum was identified on culture and DNA sequencing of urine and fine needle aspirates of affected disc spaces. Aggressive therapy including amphotericin B liposomal complex, micafungin, and azoles were initiated, but the dog was eut

Daptomycin and acute lung injury: recognising drug-induced eosinophilic pneumonia.

Daptomycin-induced eosinophilic pneumonia (DIEP) is a rare medication adverse event that can lead to severe respiratory distress and clinical decompensation. Herein, we report a man in his 70s who was being treated with outpatient intravenous daptomycin for foot osteomyelitis and presented with fevers and shortness of breath. His clinical exam and imaging favoured an infectious pneumonia. However, the patient's respiratory and clinical status continued to decline despite broad-spectrum antibiotics. A diagnosis of DIEP was established by elevated peripheral eosinophil count and >25% eosinophils

Pulmonary aspiration thrombectomy with Lightning Flash catheter in a young patient with fibrosing mediastinitis.

We present the case of a 33-year-old patient with atypical pulmonary anatomy secondary to fibrosing mediastinitis who underwent pulmonary thrombectomy with the Lightning Flash aspiration catheter following a pulmonary embolism triggered by tibial osteomyelitis. Despite an inconclusive computed tomography angiography report, the diagnosis was made through clinical evaluation associated with electrocardiographic and echocardiographic changes. Angiography was crucial for diagnosis. The procedure was technically successful, resulting in immediate improvement in pulmonary artery systolic pressure a
