Muscle channelopathies.

Muscle channelopathies encompass a wide range of mainly episodic conditions that are characterized by muscle stiffness and weakness. The myotonic conditions, characterized predominantly by stiffness, include myotonia congenita, paramyotonia congenita, and sodium channel myotonia. The periodic paralysis conditions include hypokalemic periodic paralysis, hyperkalemic periodic paralysis, and Andersen-Tawil syndrome. Clinical history is key, and diagnosis is confirmed by next-generation genetic sequencing of a panel of known genes but can also be supplemented by neurophysiology studies and MRI. As

An Unusual Presentation of Thyrotoxicosis: Leg Weakness and Hypokalemia in a 21-Year-Old Male.

Patients with hyperthyroidism or thyrotoxicosis present with an unspecific constellation of signs or symptoms such as palpitations, tremors, weight loss, or diarrhea. In some severe cases, hyperthyroidism can predispose patients to metabolic abnormalities and arrhythmias. Thyrotoxic periodic paralysis (TTP) is a rare, life-threatening complication or variant of hyperthyroidism associated with hypokalemia and muscle weakness that affects young Asian or Hispanic males between 20-40 years. TTP is reversible, and the management consists of beta-blockers, antithyroid therapy, and conservative potas

Hoarseness of Voice in a Patient with Mitral Stenosis and Ortner's Syndrome.

Rheumatic mitral stenosis is prevalent in this part of the world and it gives rise to wide array of manifestations. However, hoarseness of voice secondary to recurrent laryngeal nerve paralysis (Ortner's syndrome) is an uncommon manifestation. This case illustrates an uncommon presentation in a common disease. A 29-year-old lady presented with a 2-year history of hoarseness of voice. Physical examination revealed a mid-diastolic murmur and left vocal cord paralysis. Echocardiography confirmed mitral stenosis with pulmonary hypertension. She underwent percutaneous mitral balloon valvotomy in 19

Nonspecific necrotizing petrositis: an unusual complication of otitis in children.

Three cases of an unusual form of suppurative chronic otitis media in children 2 and 3 years old are presented. The clinical feature common to all cases was otorrhea, beginning at the first months of life, with a silent evolution to a complete peripheral facial paralysis. In one case the facial paralysis was bilateral. The treatment applied was surgical: subtotal petrosectomy. The surgical findings were bloody granulation tissue, sequestering completely the otic capsule. The facial nerve was destroyed almost the total length of the Fallopian canal in all cases. The histopathologic examination

[Amipaque myeloradiculography in the diagnosis of brachial plexus root rupture].

Myelography with a water-soluble contrast medium amipak (metrisamide) was applied in a complex with other diagnostic methods of examination in 22 patients with injury to the roots and primary trunks of the brachial plexus. Five of these patients had a clinical picture of Erb-Duchenne paralysis, in 17 injury to all the primary trunks with predominance of Djerine-Klumpke paralysis in the clinical picture was found. The results obtained provide evidence that myelography with amipak is a reliable method of the diagnosis of preganglionic (intraforaminal) injury to the roots of the brachial plexus.

Experimental arthrogryposis caused by viral myopathy.

Immobilization of the embryo has been postulated to cause the joint deformities in arthrogryposis multiplex congenita (AMC). Experimental damage to the motor neurons or pharmacologic blockade of neuromuscular transmission has previously resulted in typical joint changes of AMC. In the present investigation, we have studied the effects of paralysis produced by a viral myopathy on joint development. Coxsackievirus A2 was injected intravenously into chick embryos on the seventh day of incubation. Within 48 hours, severe myositis and paralysis resulted. Electron microscopical and immunofluorescenc

Rare case of real-time observation of paralytic deterioration after cervical dislocation in the hyperacute phase.

BACKGROUND: There have been no prior reports of real-time detailed records leading to complete quadriplegia immediately after fracture dislocation in high-energy trauma. Here, we report a case of cervical dislocation in which the deterioration to complete motor paralysis (modified Frankel B1) and complete recovery (Frankel E) could be monitored in real time after reduction in the hyperacute phase. CASE PRESENTATION: A 65-year-old man was involved in a car accident and sustained a dislocation at the C5/6 level (Allen-Ferguson classification: distractive flexion injury stage IV). His paralysis

Excised larynx evaluation of wedge-shaped adjustable balloon implant for minimally invasive type I thyroplasty.

OBJECTIVES/HYPOTHESIS: To describe the method of inserting a wedge-shaped adjustable balloon implant (wABI) via a minithyrotomy for medialization thyroplasty and evaluate its effect on a range of phonatory parameters using the excised larynx bench apparatus. STUDY DESIGN: Repeated measures with each larynx serving as its own control. METHODS: A prototype wABI was deployed in six excised canine larynges of various sizes through a minithyrotomy and then filled with saline. Mucosal wave, aerodynamic, and acoustic parameters were measured for three conditions: normal, vocal fold paralysis, and p

[Surgical injury of the temporo-frontal branch of the facial nerve. Anatomical study and clinical consequences].

The frequency of frontal paralysis due to surgical trauma of the temporo-frontal nerve seems to be increasing due to the growing number of operations in the temporo-frontal area and the complexity of surgical techniques. A series of 20 anatomical dissections demonstrates the course of the temporo-frontal nerve. Frontal myomectomy of the healthy side to allow facial equilibration seems to us to be the best technique of reconstructive surgery for definitive frontal paralysis.

Diabetic oculomotor mononeuropathy: involvement of pupillomotor fibres with slow resolution.

The third and sixth cranial nerves are most frequently affected in diabetic extraocular mononeuropathies. In diabetic oculomotor involvement the pupil is undisturbed. Of the 21 patients of diabetic oculomotor paralysis 4 patients (19 percent) had dilated unreactive pupil suggestive of pupillomotor involvement. No other cause could be attributed to oculomotor paralysis. These patients showed complete but slow resolution as compared to those without pupil involvement.

Bilateral vocal cord palsy during carotid endarterectomy under cervical plexus block.

We describe a case of vocal cord palsy leading to respiratory obstruction during carotid endarterectomy under cervical plexus block in a patient who had preexisting contralateral vocal cord paralysis subsequent to a previous thyroidectomy. The patient required immediate tracheal intubation and subsequent tracheostomy to maintain the airway postoperatively. Care must be given to avoid contralateral vocal cord paralysis in the presence of a preexisting vocal cord palsy.

Anconeus Muscle Transfer to Correct or Prevent Wrist Radial Deviation in Radial and Posterior Interosseous Nerve Injuries.

PURPOSE: Wrist radial deviation is a possible complication of tendon transfer for restoration of wrist extension in cases of radial nerve paralysis. In posterior interosseous nerve (PIN) injury, this is because of the imbalance caused by the intact extensor carpi radialis longus and paralysis of the extensor carpi ulnaris (ECU). This deformity may also occur following transfer of the pronator teres (PT) to the extensor carpi radialis brevis (ECRB) for radial nerve palsy. To address wrist radial deviation, we propose transferring the anconeus muscle, extended by the intermuscular septum between

Does Herpes 6 Infection Have a Role in Bell's Palsy Among Children and Adolescents?

BACKGROUND: Bell's palsy is a peripheral paralysis of the seventh cranial nerve, whose etiology is unknown. Using polymerase chain reaction technology, it is possible to sample accessible body fluids and identify possible viral factors. The purpose of this research is to investigate its connection to the herpes virus family by testing for the presence of the virus in the saliva and tear fluid of Bell's palsy patients. METHODS: Saliva and tears were collected from 42 children and adolescents suffering from idiopathic facial nerve paralysis. Polymerase chain reaction was used to test for the pr

Role of nitric oxide in the onset of facial nerve palsy by HSV-1 infection.

IMPORTANCE: Although herpes simplex virus type 1 (HSV-1) is a causative agent of Bell palsy, the precise mechanism of the paralysis remains unknown. It is necessary to investigate the pathogenesis and treatment of Bell palsy due to HSV-1 infection. OBJECTIVE: This study elucidated the role of nitric oxide (NO) in the incidence of facial nerve paralysis caused by HSV-1 in mice and to evaluate the possible role of edaravone, a free radical scavenger, in preventing the paralysis. DESIGN, SETTING, PARTICIPANTS: Sixty-two mice served as animal models of Bell palsy in this laboratory study conduct

Anosognosia in parietal lobe syndrome.

Patients with right parietal lesions often deny their paralysis (anosognosia), but do they have "tacit" knowledge of their paralysis? I devised three novel tests to explore this. First, the patients were given a choice between a bimanual task (e.g., tying shoe laces) vs a unimanual one (e.g., threading a bolt). They chose the former on 17 of 18 trials and, surprisingly, showed no frustration or learning despite repeated failed attempts. I conclude that they have no tacit knowledge of paralysis (or, if such knowledge exists, it is not available for this particular task). Second, I used a "virtu

An unusual complication of dengue infection.

We present an unusual complication of dengue infection resulting in postviral phrenic neuropathy and diaphragmatic paralysis in a 34-year-old man. There is a paucity of literature on this condition, with postviral neuropathies previously reported to be associated commonly with herpes zoster, poliovirus, and rarely, West Nile virus and human immunodeficiency virus infections. To our knowledge, this is the first reported case of flavivirus causing isolated postviral phrenic neuropathy and diaphragmatic paralysis.
