Experimental and CFD analysis of flow impediments and encrustation in ureteral stents using in vitro urinary tract model.

A significant challenge for urological treatment is caused by encrustation that can obstruct urinary drainage. We analyzed the rate of encrustation formation for the initial three weeks in Conventional Double J Stent (CDJS), Renovated Double J Stent (RDJS), and Single J Stent (SJS) under patient-specific control conditions (37 °C, pH 7.8, 60 mL/hr., 50% stenosis). A conceptually designed in vitro urinary tract model representing the kidney, ureter and bladder model is used where the inlet flow rate is controlled with a peristaltic pump, and when the bladder is filled with artificial urine (AU)…

Severe Hydrops of an Idiopathic Solitary Kidney and Ureter:Report of One Case.

Hydronephrosis is a common urological disease, and pregnancy with hydronephrosis is also common. However,it is extremely rare that patients suffering from hydronephrosis after delivery cannot recover on their own. Moreover, due to the no specificity of clinical manifestations,it is easy to be ignored by clinicians. This paper reports a solitary kidney patient with severe dilatation and hydronephrosis of the kidney and ureter that were caused by late pregnancy, and the hydrops could not recover spontaneously after delivery. In addition, the methods of open surgery, ureteroscopy, and ureteral stent placem…

Rethinking ureteral stent design: Do more side holes improve performance?

Ureteral stents, particularly double-J stents, are widely used to maintain urinary flow in the presence of ureteral obstruction. These stents are designed with side holes along their length to promote urine drainage and prevent complications. Despite their effectiveness in restoring urinary flow, ureteral stents are prone to complications such as encrustation (mineral buildup) and biofilm formation, which can lead to infections and blockages. Vesicoureteral reflux (VUR), described as urine flowing backwards from the bladder to the kidney, is another common complication, raising the risk of inf…

Ureteral complications during colorectal surgery.

Ureteral complications occur rarely during colorectal surgery (0.3 to 1.5%), are mainly diagnosed postoperatively (50-70%), and result in both short- and long-term morbidity. The objective of this update was to report on prevention, diagnosis, and treatment options for ureteral injuries that occur during colorectal surgery. For prevention, it is essential to identify at-risk patients preoperatively. Routine prophylactic insertion of a double-J catheter is not recommended. Intra-operatively, non-invasive techniques to aid in locating the ureters based on intravenous injection of methylene blue …

Safe pressures for suction ureteric access sheaths: in vitro conversion of pressures to flow.

OBJECTIVE: To convert suction pressure settings into practical, size-specific operating limits for suction ureteric access sheaths (sUAS). MATERIALS AND METHODS: Using an 8.7-F flexible ureteroscope with a standard 3.9-F working channel, we measured irrigation inflow at various pressures with no instrument, 270-μm laser fibre, and 1.9-F basket. Outflow was measured using 10/12-, 11/13-, and 12/14-F sUAS with suction applied via Neptune 3™ (Stryker, Kalamazoo, MI, USA) at 30-s intervals and pressures ranging from 50 to 300 mmHg. Inflow and outflow were matched by linear interpolation deriving …

Natural biopolymers in the fabrication and coating of ureteral stent: An overview.

Ureteral stents are indwelling medical devices that are most commonly used in treating different urinary tract complications like ureteral obstruction, kidney stones, and strictures, and allow normal urine flow from the kidney to the bladder. Tremendous work has been done in ureteral stent technology to meet the clinical demands, however, till-date a gold standard material for ureteral stents has not yet been developed. Many materials such as metal, and synthetic polymers have been published, however, the role of natural biopolymers has not yet been summarized and discussed. There is no detail…

Utility of ureteral diameter ratio for clinical decision-making in children with vesicoureteral reflux: a systematic review and meta analysis.

The aim of this study was to investigate the utility of ureteral diameter ratio (UDR) as a tool to prognosticate and manage vesicoureteral reflux (VUR). Four scientific databases (PubMed, EMBASE, Web of Science, and Scopus) were systematically searched. Inclusion criteria were all studies in which UDR was used in prognostication and/or management of VUR. An independent assessment of the methodological quality was performed by two authors using the Newcastle Ottawa Quality scale. The statistical analysis was performed using a random-effects model. Thirteen studies (all retrospective) were inclu…

Technical feasibility and safety of the alternative snare technique using a 0.018-inch guide wire and 5-French catheter for double-J ureteral stent removal.

To evaluate the technical feasibility of the alternative snare technique using a 0.018-inch guide wire and 5-French (Fr) catheter for double-J ureteral stent (DJUS) removal. In this retrospective study, 11 DJUS were removed in 9 consecutive patients between July 2023 and October 2023. We evaluated patient characteristics, DJUS characteristics, and procedure characteristics. Out of 11 cases, 8 (72.7%) were successful in removing the DJUS using the alternative snare technique without major complications. The average time between DJUS insertion and removal was 47.4 ± 50.0 days. The most common DJ…

Systematic use of magnetic double J stent in pediatric kidney transplantation: A single-center experience.

BACKGROUND: The intraoperative insertion of a double J stent (DJS) is known to reduce urological complications and is broadly accepted in kidney transplant (KTx) patients. The magnetic ureteral DJS (mDJS) represents a valid alternative device as it can be removed without cystoscopy, using a transurethral magnet. This is of particular importance in the pediatrics, allowing us to avoid cystoscopy requiring general anesthesia (GA) in this population. To date, few data are available on the systematic use of mDJS in pediatric patients undergoing KTx. METHODS: We report a retrospective analysis of …

Stent Omission in Pre-stented Patients Undergoing Ureteroscopy Decreases Unplanned Health Care Utilization.

INTRODUCTION: Despite AUA guidelines providing criteria for ureteral stent omission after ureteroscopy for nephrolithiasis, stenting rates in practice remain high. Because pre-stenting may be associated with improved patient outcomes, we assessed the impact of stent omission vs placement in pre-stented and non-pre-stented patients undergoing ureteroscopy on postoperative health care utilization in Michigan. METHODS: Using the MUSIC (Michigan Urological Surgery Improvement Collaborative) registry (2016-2019), we identified pre-stented and non-pre-stented patients with low comorbidity undergoin…

Ureteric stent associated spondylodiscitis.

This case report describes a clinical presentation of spondylodiscitis, following an emergency ureteric stent placement for an infected and obstructed kidney in a woman in her late 70s who presented with right flank pain, raised inflammatory markers and an acute kidney injury. Non-contrast CT kidney, ureters and bladder (KUB) revealed a 9 mm obstructing stone and prompt decompression with a JJ stent was performed. Although the urine culture showed no growth at first, an extended spectrum beta-lactamase Escherichia coli was found in a subsequent urine culture after discharge. Postoperatively, t…

Allium® ureteral stent, a new player in the treatment of ureteral stenosis: a prospective cohort study.

INTRODUCTION AND OBJECTIVES: Ureteral stricture is a chronic condition that can result in the obstruction of urinary drainage from the affected renal unit, leading to loss of renal function. Treatment methods can be categorized into two main headings: endourological and reconstructive procedures. We aimed to investigate the efficacy and safety of the self-expandable Allium® ureteral stent, which has been used in the minimally invasive treatment of ureteral stenosis in recent years. MATERIALS AND METHODS: Twenty patients who were applied Allium® ureteral stent between 2017 and 2021 included in…

[Evaluation of the Effects of Long-Term Indwelling Ureteral Stents on Preserving Renal Parenchyma and Function in Patients with Ureteral Stenosis].

Although ureteral stenting is a common conservative treatment for ureteral stricture, it is unclear whether a long-term indwelling ureteral stent protects the kidney against parenchymal atrophy and functional deterioration. In this study, we evaluated the changes in renal parenchymal thickness (RPT) and estimated the glomerular filtration rates (eGFR) in patients with indwelling ureteral stents for one year or more. As a control, we also evaluated changes in RPT associated with indwelling percutaneous nephrostomy (PNS) for one year or more. Polymer ureteral stents were used and replaced every …
