Associations of adolescent substance use and depressive symptoms with adult major depressive disorder in the United States: NSDUH 2016-2019.

BACKGROUND: Few studies have focused on the associations of adolescent substance use and depressive symptoms with adult major depressive disorder (MDD). METHODS: Data from 168,859 adults, among which, 15,959 had experienced MDD in the past year, as indicated by a major depressive episode (MDE) marked by MDD symptoms, were from the 2016-2019 National Surveys on Drug Use and Health. Weighted multivariable logistic regression (MLR) analyses were used to determine the associations. RESULTS: The overall MDD prevalence was 7.2 %, whereas the prevalence for adults without early onset depressive sym

Mutation of the CLN6 gene in teenage-onset progressive myoclonus epilepsy.

Progressive myoclonus epilepsies are severe, intractable, and neurodegenerative. They afflict patients of all ages, but more commonly adolescents, and comprise the main differential diagnosis of common juvenile myoclonic epilepsy. Genetic or minimally invasive pathologic diagnoses are available for many but not all teenage-onset progressive myoclonus epilepsies. We describe a multiplex family with autosomal recessive teenage-onset progressive myoclonus epilepsy that had remained undiagnosed despite extensive genetic and pathologic testing. We describe whole exome sequencing combined with homoz

Longitudinal Analysis of Somatic Mosaicism and Clonal Hematopoiesis in Cryopyrin-Associated Periodic Syndrome.

OBJECTIVE: Cryopyrin-associated periodic syndrome (CAPS) is an autoinflammatory disease caused by gain-of-function mutations in NLRP3. Although somatic NLRP3 mosaicism is increasingly recognized, it remains unclear how variant allele frequency (VAF) changes over time and whether age at disease onset reflects differences in somatic mutation dynamics. This study aimed to analyze the longitudinal VAF trends and their correlation with clonal hematopoiesis (CH) in patients with CAPS mosaicism. METHODS: We analyzed NLRP3 VAF in blood and various tissues, including dried umbilical cord (DUC), using

Long term disease burden post-transplantation: three decades of observations in 25 Hurler patients successfully treated with hematopoietic stem cell transplantation (HSCT).

BACKGROUND: Mucopolysaccharidosis type I-Hurler syndrome (MPSI-H) is a lysosomal storage disease characterized by severe physical symptoms and cognitive decline. Early treatment with hematopoietic cell transplant (HSCT) is critical to the survival of these patients. While survival rates and short-term outcomes are known to be improved by HSCT, the long-term cognitive, adaptive and psychosocial functional outcomes of children with (MPSI-H) post-HSCT are not well documented. This manuscript focuses on retrospective long-term follow-up (7-33 years) of 25 MPSI-H patients, transplanted between 1986

Investigating the Genetic Relationship Between Vitamin B12 Metabolism and Parkinson Disease.

BACKGROUND AND OBJECTIVES: Epidemiologic studies suggest that patients with Parkinson disease (PD) may have lower levels of vitamin B12 compared with healthy controls, and it was proposed that patients with PD could benefit from vitamin B12 supplementation. Functional studies have shown that B12 could modify LRRK2 activity and may directly interact with alpha-synuclein. The aim of this study was to investigate the role of common and rare variants in genes related to B12 metabolism and assess the potential causal relationships between B12 levels and PD risk, age at onset, and motor/cognitive pr

Epigenetic fingerprints link early-onset colon and rectal cancer to pesticide exposure.

The incidence of colorectal cancer (CRC) is rising rapidly in people younger than 50 years. Although this increase parallels shifts in lifestyle and environmental factors-collectively termed the exposome-whether these are indeed linked to the development of early-onset CRC (EOCRC) remains uninvestigated. Due to limited exposome data in most cancer cohorts, we constructed weighted methylation risk scores as proxies for exposome exposure to pinpoint specific risk factors associated with EOCRC compared to late-onset CRC (LOCRC) patients diagnosed at ≥70 years. Our analysis confirmed previously id

Associations of childhood exposure to interparental physical violence and verbal conflict with the risk of adult-onset diabetes.

BACKGROUND: Adverse childhood experiences have been associated with health across the life course. However, the associations of childhood exposure to interparental physical violence and verbal conflict with the risk of adult-onset diabetes remain unclear. METHODS: A total of 6769 participants without diabetes were enrolled. We assessed childhood exposure to interparental physical violence and verbal conflict through the standard questionnaire. The outcome was adult-onset diabetes, defined as the new occurrence of diabetes during follow-up. Diabetes was ascertained if any of the following crit

Incidence Trends of Early-Onset Colorectal Cancer in Germany: A Registry-Based Study From 2003 to 2023.

Motivated by studies on early-onset colorectal cancer (EO CRC, below 50 years) from the United States (US) and elsewhere, this study aimed to provide national incidence trends of EO CRC for Germany. We retrieved colorectal cancer cases (ICD-10 codes C18-C20) using pooled data from German cancer registries with high-quality data. Appendix (C18.1) was excluded from the main analyses and investigated separately. We calculated age-standardised incidence rates (ASR) by calendar year, age group, sex, tumour site, histological subtype, tumour size, grading and estimated corresponding average annual p

A retrospective study of differential prognostic factors in early-onset versus late-onset colorectal cancer: a comprehensive clinical and machine learning analysis.

BACKGROUND: The incidence of early-onset colorectal cancer (EO-CRC; age <50 years) has been increasing worldwide. This single-center retrospective study aimed to compare the clinical characteristics of EO-CRC and late-onset CRC (LO-CRC; age ≥ 50 years) and to identify age-specific prognostic factors for overall survival (OS). METHODS: A total of 1,148 CRC patients were retrospectively analyzed and categorized into EO-CRC (n = 247) and LO-CRC (n = 901) groups. Clinical characteristics were compared using the Mann-Whitney U test and Chi-square test. Prognostic factors associated with OS were id

Genomic Landscape of Early-Onset Colorectal Cancer: A Comparative Analysis With Average-Onset Metastatic Colorectal Cancer in a Real-World NGS Cohort.

PURPOSE: The incidence of early-onset colorectal cancer (EOCRC), defined as colorectal cancer (CRC) diagnosed before age 50 years, has been rising globally, in contrast to declining rates in older populations. However, despite its distinct clinical features, the genomic characteristics of EOCRC and its biological relationship to average-onset CRC (AOCRC) remain incompletely defined, particularly in the metastatic setting. METHODS: We conducted a genomic analysis of patients with metastatic CRC who underwent next-generation sequencing (NGS) as part of routine clinical practice. Using targeted

Evaluating Risk Factors for Early-Onset Colorectal Cancer in a Large, Prospective Cohort.

BACKGROUND: Despite the worrisome rise of early-onset colorectal cancer (EOCRC), risk factors have not been definitively established. We use a large, granular database to evaluate risk factors for EOCRC, investigate differences in associations with EOCRC and later-onset CRC (LOCRC), and compare metrics of accelerated aging, a hypothesized driver of EOCRC. METHODS: This was a case-control analysis within the UK Biobank. Risk factors for each cancer were identified and compared, including aging measures (chronological age, telomere length, PhenoAge, and homeostatic dysregulation). RESULTS: A t

Are there any differences between adult-onset cerebellitis and childhood cerebellitis?

Acute cerebellitis (AC), a rare inflammatory disorder of the cerebellum, is primarily associated with infectious, post-infectious, and autoimmune mechanisms. It predominantly affects children and typically follows a benign and self-limited course. This study, the largest case series to date focusing exclusively on adult-onset AC, reveals distinct clinical, laboratory, and imaging characteristics compared to the pediatric AC literature. We included 16 patients (mean age of 39.5 years) who met the clinico-radiological features of AC. The causes of AC in this study were varied, encompassing paran

The relationship between occupation and specific forms of idiopathic adult-onset dystonia.

BACKGROUND: The development of idiopathic adult-onset dystonia (IAOD) in different body parts is associated with specific demographic and clinical characteristics, as well as with specific risk factors. OBJECTIVE: To investigate whether specific occupations are associated with specific forms of IAOD at onset, namely blepharospasm (BSP), cervical dystonia (CD), and task-specific upper limb dystonia (TS-ULD). METHODS: Data from 905 IAOD patients enrolled in the Italian Dystonia Registry were analysed. Each patient was assigned to the corresponding occupational category by specialists in occupa
