Tirzepatide non-response with subsequent semaglutide response in class 2 obesity.

We report a paradoxical case of a 40-year-old man with class 2 obesity (body mass index 39.8 kg/m2) and insulin resistance (homeostatic model assessment of insulin resistance [HOMA-IR] 5.0; reference <2.0) who achieved minimal weight loss (<5%) despite tirzepatide dose escalation to 15 mg weekly over 6 months. Following switch to semaglutide 2.4 mg weekly, the patient achieved 20% weight loss over 6 months, with normalization of fasting glucose, resolution of insulin resistance, and marked improvement in appetite control. This case challenges the assumption that dual glucose-dependent insulino

Effects of GLP-1 receptor agonists on body weight and cardiovascular outcomes: a review.

Obesity is a rapidly increasing global health challenge that is strongly linked to the cardiorenometabolic syndrome, cardiovascular morbidity, and mortality. Lifestyle interventions remain the cornerstone of treatment, but average long-term weight loss is modest and durable weight maintenance is often limited. Bariatric surgery is not suitable for all patients, and substantial weight regain occurs in a considerable proportion over time. Glucagon-like peptide‑1 receptor agonists (GLP-1RAs), originally developed for type 2 diabetes (T2D), have emerged as potent agents for weight reduction and ca

Dietary energy levels regulate feed intake of broilers through the brain-gut axis.

BACKGROUND: Feed intake (FI) in broilers is significantly influenced by dietary energy levels through the brain-gut axis (BGA), profoundly affecting growth performance. The mechanisms underlying dietary energy regulation of FI and intestinal health via BGA remain unclear. AIM: This study investigated the effects of low- (LED, 2,900 kcal/kg), medium- (MED, 3,200 kcal/kg), and high-energy diets (HED, 3,500 kcal/kg) on BGA function over 21 days. RESULTS: We found that increasing dietary energy reduced FI but increased body weight gain and improved feed conversion ratio. MED elevated anorexigeni

Differential associations of objective hearing loss and subjective hearing difficulty with depressive symptom profiles: A nationwide population-based study in Korea.

OBJECTIVE: To examine whether objective audiometric hearing loss and subjective hearing difficulty show differential associations with depressive symptoms in a nationally representative Korean population. METHODS: Data were obtained from 6589 participants aged ≥40 years in the Korea National Health and Nutrition Examination Survey (2020 and 2022). Hearing status was classified into four groups based on pure-tone audiometry (≥26 dB HL at 0.5-4 kHz in the better ear) and self-reported hearing difficulty. Depressive symptoms were defined as a Patient Health Questionnaire-9 (PHQ-9) score ≥ 10, wi

LEAP2 Regulates Insulin Secretion from INS-1E Beta Cells and Rat Pancreatic Islets: An In Vitro Study.

LEAP2 is an endogenous antagonist and inverse agonist of GHSR-1a, the receptor for acyl-ghrelin. Although LEAP2 is increasingly recognized as a regulator of appetite and glucose homeostasis, its direct effects on pancreatic endocrine cells remain incompletely understood. The present in vitro study evaluated whether LEAP2 modulates insulin secretion and the expression of insulin-related genes in INS-1E beta cells and isolated rat pancreatic islets. LEAP2 treatment altered selected components of the insulin-related expression profile in a model- and time-dependent manner. In INS-1E cells, LEAP2

Cardiovascular Risk Reduction With GLP-1 RA Drugs.

GLP-1 (glucagon-like peptide-1) receptor agonists (RAs) have emerged as a major therapeutic advance in cardiometabolic medicine. Initially developed as glucose-lowering therapies for type 2 diabetes, these agents have demonstrated broad benefits that extend well beyond glycemic control. GLP-1 RAs enhance glucose-dependent insulin secretion and reduce appetite, leading to improved glycemia and sustained weight loss. In addition, GLP-1 signaling exerts vascular and myocardial effects, including improved endothelial function, reduced inflammation and oxidative stress, and favorable changes in car

Optimizing Nutritional Support in Advanced Non-Small Cell Lung Cancer: Evidence and Controversies in Oral, Enteral, and Parenteral Approaches.

Malnutrition and cachexia affect the majority of patients with advanced lung cancer, driven by systemic inflammation (IL-6, TNF-α), metabolic dysregulation, and anorexia. These conditions worsen prognosis, reduce treatment tolerance, and diminish quality of life (QoL) and survival. Early recognition, utilizing tools such as the Patient-Generated Subjective Global Assessment (PG-SGA), is crucial. Nutritional support should align with patient-centered goals, prioritizing function and QoL rather than survival alone. Oral nutritional supplements (ONS), especially high-protein/EPA (eicosapentaenoic

Effect of Oral Glucose Administration on Ghrelin Levels in Normal-Height Prepubertal Children Born Small for Gestational Age (SGA).

Ghrelin is a regulator of appetite and growth hormone secretion, rising during fasting and decreasing after food intake. Children born small for gestational age (SGA) who undergo postnatal catch-up growth are at increased risk of obesity and metabolic disturbances, which may be related to impaired ghrelin regulation. This study assessed fasting and post-glucose ghrelin responses during an oral glucose tolerance test (OGTT) in prepubertal SGA children in relation to obesity and metabolic syndrome components. Ninety-eight children aged 5-9 years were included. Anthropometry, blood pressure, fast

A single dorsal vagal complex circuit mediates the aversive and anorectic responses to GLP1R agonists.

GLP-1 receptor agonists (GLP1RAs) effectively reduce feeding to treat obesity, although nausea and other aversive side effects of these drugs can limit their use. Brainstem circuits that promote satiation and mediate the physiological control of body weight can be distinguished from those that cause aversion. It remains unclear whether brainstem Glp1r neurons contribute to the normal regulation of energy balance and whether GLP1RAs control appetite via circuits distinct from those that mediate aversive responses, however. Here, we silenced Glp1r neurons in the nucleus of the solitary tract or

Cocaine- and amphetamine-regulated transcripts in two percomorphs: evolutionary conservation and energy-status dependent responses.

Cocaine and amphetamine-regulated transcript (Cart) is a pleiotropic neuropeptide involved in appetite regulation and maintenance of energy homeostasis, among other processes. Although Cart is widely studied in vertebrates, its multigenic nature in fish raises questions about which functions are affected by specific Cart peptides. This study compared two important aquaculture percomorph species with distinct habitats and feeding behaviors: Nile tilapia (Oreochromis niloticus) and gilthead seabream (Sparus aurata). Bioinformatics revealed seven cart genes in tilapia and six in seabream that sho
