Coexistent hairy cell leukaemia and hepatosplenic T-cell lymphoma: a case report.

BACKGROUND: Hairy cell leukaemia (HCL) is a chronic B-cell leukaemia characterized by expansion of neoplastic cells in the spleen, bone marrow and blood. Symptoms of HCL are related to pancytopenia and immune deficiency. Patients with HCL have an increased risk of second malignancy either in a form of synchronous disease or in a form of an increased incidence of a second neoplasm after the treatment of HCL. Hepatosplenic T-cell lymphoma (HSTCL) is a rare form of aggressive extranodal T-cell lymphoma. Its pathogenesis is connected to a chronic immune deficiency status and its coexistence with o

Severe congenital dyserythropoietic anaemia type I: prenatal management, transfusion support and alpha-interferon therapy.

We report a case of congenital dyserythropoietic anaemia, type I, with severe pre- and postnatal manifestations. Exchange transfusions were required for fetal anaemia (3.5 g/dl) at 28 and 30 weeks of gestation. Transfusions were administered at birth (Caesarean section at week 35) and at regular intervals thereafter. At 14 months, alpha-interferon therapy was initiated (106 units three times a week). This resulted in stabilization of the haemoglobin at or above 11 g/dl and a reduction in the percentage of erythroblasts with ultrastructurally abnormal heterochromatin. After 9 months, the dose o

Abnormal chromatin clumping in granulocytes in a case of Ph1-negative, BCR-ABL rearrangement positive, haematologically atypical chronic myeloid leukaemia (CML).

In the present study, we report the case of a patient displaying an abnormal chromatin clumping (ACC) syndrome, a rare disease which shares features with both myeloproliferative and myelodysplastic disorders. Although various non specific cytogenetic abnormalities have been observed in ACC, the presence of a Ph1 chromosome has not been reported. In our patient, despite a lack of Ph1, PCR analysis of blood and bone marrow samples revealed a BCR-ABL rearrangement. These results indicate that at least some cases of ACC syndrome could represent a form of Ph1-negative chronic myeloid leukaemia.

Wrist Joint Skeletal Changes in Children With Transfusion-dependent Thalassemia.

BACKGROUND: Arthropathies and bone deformities are well known to occur in patients with thalassemia major and have been attributed to the disease or to its therapy. Before the advent of chelation therapy, these children developed widened diploic space and "hair-on-end" pattern in skull, "cobweb" pattern in the pelvis, and the lack of the normal concave outline in the long bones because of extensive marrow proliferation. After the introduction of iron-chelation therapy, these patients were noted to develop metaphyseal abnormalities and vertebral changes resembling spondylo-metaphyseal dysplasia

Comparison of bone marrow cells harvested from various bones of cynomolgus monkeys at various ages by perfusion or aspiration methods: a preclinical study for human BMT.

Using cynomolgus monkeys, we have previously established a new method for harvesting bone marrow cells (BMCs) with minimal contamination of the BMCs with T cells from the peripheral blood. We originally conducted this new "perfusion method" in the long bones (the humerus, femur, and tibia) of cynomolgus monkeys. Here, we apply the perfusion method to obtain BMCs from the ilium of cynomolgus monkeys, since BMCs are usually collected from the ilium by the conventional aspiration method in humans. The perfusion method consists of two approaches: transverse iliac perfusion and longitudinal iliac p

Outcomes of Haploidentical Transplantation in Patients with Relapsed Multiple Myeloma: An EBMT/CIBMTR Report.

Allogeneic hematopoietic cell transplantation (allo-HCT) using siblings and matched donors has the potential for long-term disease control in a subset of high-risk patients with multiple myeloma (MM); however, the data on using haploidentical donors in this disease are limited. We conducted a retrospective analysis to examine the outcomes of patients with MM who underwent haploidentical allo-HCT within European Society for Blood and Marrow Transplantation/Center for International Blood and Marrow Transplant Research centers. A total of 96 patients underwent haploidentical allo-HCT between 2008

Extraosseous osteochondroma of the hypothenar region: a case report and literature review.

BACKGROUND: Osteochondroma (OC) is the most common benign bone tumor, yet hand involvement is uncommon and extraosseous presentations are rarer still. The hypothenar compartment is an exceptionally unusual site, and lack of corticomedullary continuity can mimic soft-tissue and parosteal lesions. We report an extraosseous OC of the hypothenar region with adjacent ulnar artery thrombosis, provide a focused literature synthesis, and advance a microtrauma-related pathogenesis. CASE PRESENTATION: A 24-year-old Afghan male construction worker presented with a 10-year history of a slowly enlarging,

Childhood-onset systemic lupus erythematosus with trisomy X and the increased risk for bone complications: a case report.

BACKGROUND: Systemic lupus erythematosus is a multi-organ inflammatory autoimmune disease; immune complexes are part of the pathogenesis, but not entirely responsible. Trisomy X is the most common female chromosomal abnormality and the role of an additional X chromosome in the development of systemic lupus erythematosus is well recognized. However, the potential complications and optimal management of childhood lupus with trisomy X remain unclear. Herein, we describe a case of childhood-onset systemic lupus erythematosus associated with severe bone complications presumably secondary to trisomy

Exogenous vitamin C regulates osteoclast differentiation kinetics, cytoskeletal maturation, and bone resorption through ERK1/2-DAPK1-caspase-3 signaling.

BACKGROUND: Vitamin C (VC) plays essential but incompletely defined roles in osteoclast (OC) development. We examined how exogenous VC and AKR1A-dependent endogenous VC regulate OC differentiation, cytoskeletal organization, resorptive activity, and apoptosis. METHODS: Murine bone marrow-derived macrophages (BMDMs) from wild-type and AKR1A-deficient mice, together with RAW264.7 cells, were used to assess the effects of exogenous and endogenous VC on OC differentiation, actin ring formation, resorption, apoptotic signaling, and ERK1/2 activation. RESULTS: AKR1A1 deficiency altered osteoclasto

Quercetin attenuates osteoarthritis through SIRT1-dependent regulation of synovial macrophage polarisation.

Excessive M1 polarisation of synovial macrophages contributes substantially to the dysregulated immune environment that drives osteoarthritis (OA) progression, although the role of silent information regulator 1 (SIRT1) and its therapeutic relevance remain uncertain. We investigated whether quercetin protects against OA by modulating synovial macrophage polarisation and SIRT1-related signalling. SIRT1 expression and M1-associated markers were examined in synovial tissues from patients with OA. Myeloid-specific SIRT1-deficient mice were generated, and OA was induced by destabilisation of the me

Optimizing diagnosis and management of ONFH, FNSF, and SIF.

Osteonecrosis of the femoral head (ONFH), femoral neck stress fracture (FNSF), and subchondral insufficiency fracture (SIF) are distinct yet clinically overlapping causes of non-arthritic hip pain. All three conditions threaten femoral head viability via distinct pathophysiologic mechanisms and may lead to fracture displacement (FNSF) and irreversible collapse (ONFH, SIF, and displaced FNSF), ultimately necessitating arthroplasty if not identified and managed early. These conditions frequently present with groin pain and unremarkable radiographs in their early stages. Imaging may be normal or

Intra-articular Osteoid Osteoma of the Femoral Neck Mimicking Inflammatory Hip Disease: A Case Report and Review of the Literature.

Osteoid osteoma is a benign osteogenic tumor that predominantly affects the long bones of young adults. Intra-articular involvement of the femoral neck is uncommon and may present with atypical clinical features, often leading to a delayed diagnosis. We report the case of a 30-year-old woman presenting with chronic right hip pain of two years' duration, characterized by nocturnal exacerbation and partial relief with nonsteroidal anti-inflammatory drugs (NSAIDs). Initial evaluation raised suspicion of inflammatory hip disease; however, laboratory investigations were unremarkable. Subsequent CT

The emerging role of lactate in skeletal homeostasis and disorders: Integrated mechanisms and translational opportunities.

UNLABELLED: Lactate has evolved from a metabolic byproduct to a central regulator of skeletal homeostasis and disorders, integrating metabolic flux with epigenetic reprogramming. Within the skeletal system, it modulates the processes of osteogenesis, osteoclastogenesis, chondrogenesis, and matrix synthesis by regulating the physiological activities of bone marrow mesenchymal stem cells, osteoblasts, osteoclasts, chondrocytes, and intervertebral disc cells, thereby maintaining skeletal homeostasis. Dysregulated lactate signaling is increasingly recognized as a pivotal pathological mechanism in

Activating transcriptional coactivator with PDZ-binding motif by (R)-PFI-2 attenuates osteoclastogenesis and prevents ovariectomized-induced osteoporosis.

Excessive osteoclast activation is a leading cause of osteoporosis. Therefore, identifying molecular targets and relevant pharmaceuticals that inhibit osteoclastogenesis is of substantial clinical importance. Prior research has indicated that transcriptional coactivator with PDZ-binding motif (TAZ) impedes the process of osteoclastogenesis by engaging the nuclear factor (NF)-κB signaling pathway, thereby suggesting TAZ activation as a potential therapeutic approach to treat osteoporosis. (R)-PFI-2 is a novel selective inhibitor of SETD7 methyltransferase activity, which prevents the nuclear tr

Mogrol Attenuates Osteoclast Formation and Bone Resorption by Inhibiting the TRAF6/MAPK/NF-κB Signaling Pathway In vitro and Protects Against Osteoporosis in Postmenopausal Mice.

Osteoporosis is a serious public health problem that results in fragility fractures, especially in postmenopausal women. Because the current therapeutic strategy for osteoporosis has various side effects, a safer and more effective treatment is worth exploring. It is important to examine natural plant extracts during new drug design due to low toxicity. Mogrol is an aglycon of mogroside, which is the active component of Siraitia grosvenorii (Swingle) and exhibits anti-inflammatory, anticancer and neuroprotective effects. Here, we demonstrated that mogrol dose-dependently inhibited osteoclast f
