Genetic Predictors of Early-Onset Spinal Intervertebral Disc Degeneration: Part Two of Two.

Understanding genetic indicators is a fundamental aspect to characterizing the pathophysiology of chronic diseases such as intervertebral disc degeneration (IVDD). In our previous spinal genetics review, we characterized some more common genetic influencers in the context of IVDD. In this second part of our two-part comprehensive spinal genetics review, we characterize the more infrequently studied genes that have pathophysiological relevance. In doing so, we aim to expand upon the current gene-library for IVDD. The genes of interest include: asporin, cartilage intermediate layer protein, insu

[Comparative assessment of the effectiveness of toothpastes to reduce hyperesthesia in inflammatory periodontal diseases].

The purpose of the study is a comparative assessment of hygiene products recommended for increased sensitivity of teeth and having an anti-inflammatory effect on periodontal tissue. The study involved 40 people aged 55-65 years with hyperesthesia and chronic mild and moderate periodontitis, which were divided into 2 groups (20 people each) depending on the toothpastes used based on HAP. Over time, indicators of hygiene, sensitivity, bleeding, PMA indices and the periodontal probing depth are evaluated. The study revealed an improvement of the studied indices in both groups after two weeks of u

Antihypertensive mechanisms of intra-renal dopamine.

PURPOSE OF REVIEW: This review will highlight recent findings concerning the regulation and signalling of the intrarenal dopaminergic system and the emerging evidence for its importance in blood pressure regulation. RECENT FINDINGS: There is an increasing evidence that the intrarenal dopaminergic system plays an important role in the regulation of blood pressure, and defects in dopamine signalling appear to be involved in the development of hypertension. Recent experimental models have definitively demonstrated that abnormalities in intrarenal dopamine production or receptor signalling can pr

Attitudes and practices of New Zealand anaesthetists with regard to epidural and subarachnoid anaesthesia.

A survey was conducted among 259 New Zealand specialist anaesthetists to assess attitudes and practices with regard to epidural or subarachnoid anaesthesia (ESA). Ninety-four per cent replied and virtually all of the respondents indicated that they performed ESA at some time. ESA was used by most anaesthetists for most patients undergoing major hip or knee surgery, abdomino-perineal resection, cystectomy, caesarean section or transurethral resection of the prostate, ESA was used is about half of patients undergoing abdominal aortic aneurysm repair, femoro-popliteal bypass or thoracotomy and th

Impact renaming non-alcoholic fatty liver disease to metabolic associated fatty liver disease in prevalence, characteristics and risk factors.

BACKGROUND: Recently, a group of hepatologists proposed to rename non-alcoholic fatty liver disease (NAFLD) as metabolic associated fatty liver disease (MAFLD) with modified diagnostic criteria. It is important to note, however, that there are some differences between the diagnostic criteria used for NAFLD and MAFLD. Since the research on MAFLD is just beginning, however, evidence on its incidence and prevalence in the general population and in specific subpopulations remains limited. AIM: To assess epidemiology of fatty liver in new definition and compare MAFLD with NAFLD. Exploring risk fact

[Evidence based indications for tonsillectomy].

Tonsillectomy (TE) is one of the most frequent as well as one of the most controversial surgical interventions. The objective of this paper was to assess the evidence of the most common indications for TE. For this purpose the literature of the past 25 years was analysed using the criteria of evidence based medicine. In children seventy-five percent of TEs are performed because of recurrent tonsillitis. Several randomised clinical trials (RCTs) have demonstrated the efficacy of TE in this disease. This indication is based on evidence grade I-II. No consensus has yet been reached, however, abou

Association between gut microbiota and chronic diseases in East Asian ethnicity: A two-sample Mendelian randomization study.

The etiology and pathogenesis of chronic diseases (CDs) are complex and may result in a variety of symptoms that negatively affect patients' daily lives. There is growing evidence that changes in the composition and function of the gut microbiota (GM) are linked to the onset and progression of CDs, but a causal relationship between them has not yet been demonstrated in the East Asian ethnicity. We sourced data from publicly available genome-wide association study summary statistics that focused on the composition of the GM and its association with 6 specific CDs: chronic obstructive pulmonary

Analysis of Hematological Indices in Patients of Systemic Lupus Erythematous and Its Correlation with SLEDAI-2K.

Background: Systemic lupus erythematosus (SLE) is an autoimmune disease which is characterized by chronic multisystem inflammation and end-organ damage. In recent times there has been a need for new hematological markers to assess disease activity in SLE patients specifically, which can be easily available like eosinophil, basophil, neutrophil, monocytes, and platelet to lymphocyte ratios (ELR, BLR, NLR, MLR, and PLR, respectively). Materials and methods: The present investigation determines the use of a different peripheral hematological marker to assess SLE activity in 106 patients attended

Cellular senescence: the "hidden driver" in chronic inflammatory skin disorders.

Chronic inflammatory skin disorders are characterized by persistent skin inflammation, impaired barrier function, and frequent recurrence, all of which substantially reduce patients' quality of life. Traditional studies attribute the pathogenesis of these diseases to immune dysregulation, whereas new evidence indicates that cellular senescence is a key risk factor and a mechanism underlying disease progression. Cellular senescence, defined by irreversible cell cycle arrest, is accompanied by a senescence-associated secretory phenotype (SASP), mitochondrial dysfunction, and DNA damage. This rev

Gut microbiota in the pathogenesis and treatment of inflammatory bowel disease: a critical review of mechanisms and therapeutic advances.

Inflammatory bowel disease (IBD), comprising Crohn's disease (CD) and ulcerative colitis (UC), is a complex, recrudescent chronic gastrointestinal disease. The prevalence of IBD has increased globally year by year, and the exact pathogenesis remains incompletely understood. Evidence indicates that there is a strong correlation between dysbiosis of gut microbiota and the occurrence and progression of IBD. This review systematically describes recent advances in understanding the role of gut microbiota in IBD, with a particular focus on how dysbiosis contributes to pathogenesis. In addition, this

Adolescent Cholelithiasis: Evolving Challenges and Clinical Implications.

Gallstone disease in adolescents is no longer a rare clinical finding and has become an increasingly important concern in pediatric and adolescent healthcare. Shifts in lifestyle, rising rates of obesity, and improved survival of children with chronic conditions such as sickle cell disease have collectively altered the epidemiology and clinical profile of adolescent cholelithiasis. Compared with adult gallstone disease, adolescents exhibit distinct etiological factors, variable clinical presentations, and unique management challenges, particularly with regard to the timing and indications for

A numerical evaluation of the economic tradeoff of vaccination against chikungunya virus in Brazil.

This article uses a compartmental model describing the dynamic of the chikungunya virus in populations of humans and mosquitoes with parameters fitted to the incidence in Brazil to estimate the economic trade-off of vaccination against the virus infection. The model uses time-dependent parameters to incorporate fluctuations in the transmission and the mosquito population across the years. Using the model predictions of symptomatic infections and literature data concerning the proportions of post-acute and chronic cases, the vaccination cost is compared with the disease cost. Numerical results

Non-Invasive Serum Markers of Non-Alcoholic Fatty Liver Disease Fibrosis: Potential Tools for Detecting Patients with Cardiovascular Disease.

Non-alcoholic fatty liver disease (NAFLD), one of the most common chronic liver diseases with a prevalence of 23%-25% globally, is an independent risk factor for cardiovascular diseases (CVDs). Growing evidence indicates that the development of NAFLD, ranging from non-alcoholic fatty liver (NAFL), non-alcoholic steatohepatitis (NASH), advanced fibrosis to cirrhosis, and even hepatocellular carcinoma, is at substantial risk for CVDs, which clinically contribute to increased cardiovascular morbidity and mortality. Non-invasive serum markers assessing liver fibrosis, such as fibrosis-4 (FIB-4) sc

Gut Microbiota Alteration and Its Modulation with Probiotics in Celiac Disease.

Celiac disease (CD) is a chronic inflammation of the small intestine triggered by gluten ingestion in genetically predisposed people. Recent literature studies highlight the possible role of the gut microbiota in the pathogenesis of this disease. The gut microbiota is a complex community of microorganisms that can interact with the innate and adaptative immune systems. A condition of dysbiosis, which refers to an alteration in the composition and function of the human gut microbiota, can lead to a dysregulated immune response. This condition may contribute to triggering gluten intolerance, fav

Mineralocorticoid receptor antagonists in cardiovascular translational biology.

This review examines the role of mineralocorticoid receptor antagonists (MRAs) in cardiovascular biology and the molecular mechanisms involved in mineralocorticoid receptor antagonism. The data discussed suggest that MRAs can play an important role in decreasing the impact of inflammation and fibrosis on cardiorenal outcomes. Evidence derived from major randomized clinical trials demonstrates that steroidal MRAs reduce mortality in patients with heart failure and reduced ejection fraction. Initial positive findings observed in patients with chronic kidney disease and type 2 diabetes (T2D) indi
