Injectable neuromuscular blockade in the treatment of spasticity and movement disorders.

Neuromuscular blockade via injection of alcohol, phenol, or botulinum toxin reduces the tone of overactive muscles in order to restore the appropriate balance between agonists and antagonists. Such a restoration allows improved stretch and increased resting length and can reduce the likelihood of contracture. Alcohol or phenol, injected onto the motor nerve, denatures proteins and promotes axonal degeneration. The onset of action is within hours, whereas the duration of action is variable, ranging from 2 weeks to 6 months and beyond. The advantages of alcohol or phenol chemodenervation lie in

From learned helplessness to motor recovery: integrating intensive neurorehabilitation in poststroke spastic paresis-clinical insights from over 10 years of practice.

BACKGROUND: Poststroke spastic paresis represents a dual pathology combining neurological impairment and secondary muscle contracture, often perpetuated by learned non-use. Current rehabilitation approaches frequently address these components separately, limiting functional recovery potential. METHODS: This perspective synthesizes over 10 years of clinical experience treating poststroke spastic paresis across European and sub-Saharan African settings, integrating constraint-induced movement therapy with progressive muscle-lengthening protocols. Clinical insights are drawn from over 300 patien

Case Report: Frostbite in the tropics: abdominal pedicled flap reconstruction to prevent the metacarpal hand.

The reconstructive salvage of a severely frostbitten hand in a tropical setting presents a unique challenge. We report a 59-year-old male who sustained extensive frostbite to the face, ears and bilateral hands following a Mount Everest ascent. Hyperbaric oxygen therapy (HBOT) was initiated during demarcation to optimise tissue viability. Following demarcation, dry gangrene of the right hand required amputation, revealing viable 1.5-2 cm proximal phalangeal stumps in all five digits. To avoid a non-functional metacarpal hand, reconstruction aimed to preserve digital length and metacarpophalange

Botulinum Toxin Type A as an Early Intervention for Traumatic Oculomotor Nerve Palsy: A Pediatric Case Report.

Traumatic third cranial nerve palsy is a rare complication of head injury, with an incidence of approximately 1% and a characteristically poor prognosis. Conventional management remains conservative, often yielding unsatisfactory outcomes. We report the case of a 13-year-old girl who developed complete right third cranial nerve palsy following a 15-meter fall, presenting with exotropia (35Δ), ptosis, complete ophthalmoplegia, and pupillary dysfunction. Brain CT revealed hemorrhage in the right cavernous sinus, and subsequent MRI demonstrated focal nerve damage. Thirty-eight days post-injury, a

A rhesus macaque model of α-dystroglycanopathy caused by a POMT1 splice altering variant.

BACKGROUND: Biallelic mutations in genes associated with α-dystroglycan glycosylation manifest in a spectrum of conditions referred to as α-dystroglycanopathies, including Walker-Warburg syndrome, which primarily disrupt brain, eye, and muscle development. While small-animal models for the disease have been developed and described, the condition has not been documented in non-human primates. METHODS: Three spontaneous cases of severe lissencephaly, microphthalmia, and congenital muscular contracture in infant rhesus macaques were investigated. Histological analysis was performed on brain, ret

Cast-rod construct for gradual correction of fixed knee flexion deformities in young children with arthrogryposis: Technical note and preliminary results.

INTRODUCTION: Fixed knee flexion deformities (FKFDs) are common in children with arthrogryposis, yet their management in younger children is underreported. MATERIAL AND METHOD: This study describes a non-invasive cast-rod construct for the gradual correction of FKFDs secondary to arthrogryposis. RESULTS: This study presents preliminary results from 18 knees in 12 patients under four years old (median age at 30 mo, 18-48). The median knee flexion contracture angle improved from 54.5 degrees (27-90) to 10 degrees (0-30) after cast removal, (p < 0.05). Treatment lasted for a median of 32 days (

Diagnosis and treatment of early eosinophilic fasciitis: a case report.

BACKGROUND: Eosinophilic fasciitis is an infrequent connective tissue disorder that often delays treatment owing to misdiagnosis or omission. In advanced stages, severe skin sclerosis can lead to joint contracture and tendon retraction, which can severely affect the quality of life. Therefore, it is important to make a clear diagnosis and intervene in the early stages of the disease. CASE PRESENTATION: We report a case of a 52-year-old Chinese patient with marked symmetric swelling of her extremities, especially forearms and calves, sclerosis of the subcutaneous tissue, limited joint mobility

Pathophysiology, clinical presentation, and management of ketamine-induced cystitis.

Ketamine is illegally used as a recreational drug in many Asian countries. Long-term ketamine abusers often develop irritable bladder symptoms that gradually develop into more severe urinary frequency and urgency and eventually into a painful ulcerated bladder. These patients typically have reduced functional bladder capacity, increased bladder sensation, detrusor overactivity, severe urgency, urinary incontinence, and bladder contracture. Ketamine metabolites can cause severe inflammation of the urothelium, urothelial barrier deficits, vascular endothelial fibrinoid changes, increased oxidati

Navigational Transmaxillary Endoscopic Approach for Inferomedial Tumors.

Orbital tumors encompass a heterogeneous range of histopathology and usually variable in location. Traditionally, transconjunctival medial orbitotomy is used to access the medial orbital wall. However, it creates potential risk of soft tissue sequelae such as scarring, lid contracture, or entropion/ectropion. For the lesions close to the orbital apex, increased risk of optical nerve injury should be cautious during orbitotomy procedure. Transnasal endoscopic approach to the orbital walls has been applied since 1999. Although it provides good surgical visualization and prevents the soft tissue

A novel RyR1-selective inhibitor prevents and rescues sudden death in mouse models of malignant hyperthermia and heat stroke.

Mutations in the type 1 ryanodine receptor (RyR1), a Ca2+ release channel in skeletal muscle, hyperactivate the channel to cause malignant hyperthermia (MH) and are implicated in severe heat stroke. Dantrolene, the only approved drug for MH, has the disadvantages of having very poor water solubility and long plasma half-life. We show here that an oxolinic acid-derivative RyR1-selective inhibitor, 6,7-(methylenedioxy)-1-octyl-4-quinolone-3-carboxylic acid (Compound 1, Cpd1), effectively prevents and treats MH and heat stroke in several mouse models relevant to MH. Cpd1 reduces resting intracell

Lower Urinary Tract Stenosis Following Surgery for Benign Prostatic Hyperplasia.

PURPOSE OF REVIEW: This article reviews the mechanisms, risk factors, evaluation and current management options for iatrogenic lower urinary tract stenosis, including urethral stricture and bladder neck contracture, caused by surgery for benign prostatic hyperplasia (BPH). RECENT FINDINGS: The incidence of iatrogenic stenosis following endoscopic BPH surgery ranges from 0 to 9.7%. New endoscopic techniques and technologies for treating BPH do not appear to substantially mitigate this risk. However, new advances in our understanding of urethral sphincter anatomy combined with both innovative o

Peyronie Disease as a Marker of Inflammation-Is There Hope on the Horizon?

Although the description of Peyronie disease, a connective tissue disorder involving the growth of fibrous plaques in the soft tissue of the penis, is attributed to François de la Peyronie, surgeon to Louis XV of France, there are reports previous to that time. Over the intervening 450 years, a variety of empiric treatments, varying in barbarity, have been proposed. The frequency of this condition and the etiology of the fibrosis are unknown. Quality of life for affected men and their partners is adversely impacted. In this review, the authors summarize the history of the discovery of this con

Use of a buccinator myomucosal flap and bilateral pedicled buccal fat pad transfer in wide palatal fistula repair: a case report.

Primary palatoplasty for cleft palate places patients at high risk for scarring, altered vascularity, and persistent tension. Palatal fistulas are a challenging complication of primary palatoplasty that typically form around the hard palate-soft palate junction. Repairing palatal fistulas, particularly wide fistulas, is extremely difficult because there are not many choices for closure. However, a few techniques are commonly used to close the remaining fistula after primary palatoplasty. Herein, we report the revision of a palatal fistula using a pedicled buccal fat pad and palatal lengthening

Magnetic Resonance Imaging Studies in Duchenne Muscular Dystrophy: Linking Findings to the Physical Therapy Clinic.

Duchenne muscular dystrophy (DMD) is a muscle degenerative disorder that manifests in early childhood and results in progressive muscle weakness. Physical therapists have long been an important component of the multidisciplinary team caring for people with DMD, providing expertise in areas of disease assessment, contracture management, assistive device prescription, and exercise prescription. Over the last decade, magnetic resonance imaging of muscles in people with DMD has led to an improved understanding of the muscle pathology underlying the clinical manifestations of DMD. Findings from mag
