A coronary artery-to-left ventricular fistula is a rare finding; to the best of our knowledge, a total of only 35 cases have been reported. Only 5 cases of a generalized arterio-systemic fistula with three vessel involvement have been reported in the literature. We describe another case involving all major coronary arteries. A review of the literature is presented and the data of the reported cases are analyzed. A 55 year old woman was examined because of recurrent chest pain which had persisted for 2 years. On physical examination, the only abnormal finding was a fourth heart sound. Exertiona

The Doppler echocardiographic features of coronary arteriovenous fistula were investigated in eight patients with left or right coronary arteriovenous fistulas who had a continuous heart murmur in the upper precordial area and whose diagnoses were confirmed by coronary angiography. In four patients the dilated lumen of the coronary arteriovenous fistula was visualised by cross sectional echocardiography. Of these, three showed abnormal unidirectional continuous flow signals with broad velocity spectra in the fistula. Abnormal, powerful, unidirectional or bidirectional continuous Doppler signal

BACKGROUND: Coronary artery disease is among the leading causes of death worldwide. The method of choice for coronary artery imaging is catheterization using real-time fluoroscopic images of coronary anatomy and pathology. The aim of the study was to create an anatomically accurate cardiac and coronary artery phantom suitable for learning and practicing image projections in invasive coronary angiography by using computed tomography (CT) image segmentation and 3D printing.
MATERIALS AND METHODS: The development of an anatomically accurate heart phantom was carried out in several phases. In the

Coronary angiography has limitations in accurately assessing the coronary microcirculation. A new comprehensive invasive hemodynamic assessment method utilizing coronary flow reserve (CFR) and the index of microvascular resistance (IMR) offers improved diagnostic capabilities. This study aimed to present early real-world experience with invasive hemodynamic assessment of the coronary microvasculature in symptomatic patients with nonobstructive coronary artery disease (CAD) from the Coronary Microvascular Disease Registry, which is a prospective, multi-center registry that standardized the eval

BACKGROUND: Coronary embolism is a rare and potentially fatal phenomenon that occurs primarily in patients with valvular heart disease and atrial fibrillation. There is a lack of consensus regarding the diagnosis, treatment, and management of coronary embolism, leaving management at the discretion of the treating physician. Through this review, we aim to establish a better understanding of coronary embolism, and to identify treatment options - invasive and non-invasive - that may be used to manage coronary embolism.
METHODS AND RESULTS: Our systematic review included 147 documented cases of c

OBJECTIVES: The present study describes an isolated defect of the coronary vasodilation in response to adenosine in five patients examined for clinically suspected coronary microangiopathy.
BACKGROUND: Coronary microangiopathies can be defined functionally as dysregulation of the microcirculatory vasomotion.
METHODS: The five patients were compared with 24 control subjects. Coronary flow velocity was measured with an intracoronary Doppler guide wire (0.018 in. [0.046 cm], 12 MHz) at rest and during intracoronary administration of adenosine (80 micrograms/min and 160 micrograms/min over 3 min

Kounis syndrome is an underrecognized condition characterized by the concurrence of acute coronary syndrome (ACS) and a hypersensitivity reaction mediated by mast cell activation. Diagnosis can be challenging, and treatments for anaphylaxis and ACS may have competing effects. We report a case of a 58-year-old male who presented with anaphylaxis after seafood ingestion, complicated by anterior ST-segment elevation myocardial infarction (STEMI). Coronary angiography revealed plaque rupture with thrombus in the mid-left anterior descending artery, consistent with type II Kounis syndrome. This cas

A 53-year-old man presented with dyspnea, orthopnea, and paroxysmal nocturnal dyspnea 1 week after an episode of severe chest pain. His electrocardiogram demonstrated inferior Q waves. He was hemodynamically stable, with clinical heart failure and a loud pansystolic murmur. Echocardiography showed a large ventricular septal defect on the basal inferior wall, with a thin septal membrane. This membrane had a small defect with left-to-right flow. Coronary angiography showed an occluded right coronary artery. A delayed patch repair and bypass graft were performed, with good success. No presurgical

Myocardial bridging (MB), a congenital coronary anomaly, can lead to dynamic systolic compression of the coronary arteries, resulting in myocardial ischemia and refractory angina. Although MB is often benign, significant cases require tailored interventions. This case report describes a unique approach using thermal imaging to guide surgical unroofing in a 46-year-old man with Behçet disease and refractory angina. The integration of thermal angiography facilitated precise localization and treatment, resulting in significant symptom improvement postoperatively. This innovative application of th

Coronary CT angiography (CCTA) and fractional flow reserve with CCTA (FFRCT) have been endorsed by the ACC/AHA Chest Pain guidelines to streamline the diagnosis of coronary artery disease (CAD), but there is still a significant lack of adherence. In our study of 673 stable chest patients without known CAD from 5 European countries, we found that CCTA is the most common noninvasive diagnostic test, but nearly 40 % of them still underwent upfront CAD. Additionally, there was no temporal improvement trend, and the integration of FFRCT is low. We highlighted the urgent need to improve diagnostic

This case report highlights the atypical presentation of coronary artery vasospasm in a 59-year-old patient presenting with syncope due to ventricular arrhythmia. Despite initially elevated troponin levels and non-significant lesions observed during left heart catheterization, the patient experienced recurrent chest pain and dizziness, prompting further evaluation. Ultimately, coronary vasospasm was identified as the likely differential diagnosis, supported by various diagnostic modalities including electrocardiogram, Zio patch monitoring, transthoracic echocardiogram, cardiac MRI, and CT angi

A 67-year-old male presenting with an anterior ST-segment elevation myocardial infarction (STEMI) underwent stent placement in the left anterior descending coronary. The patient was discharged on an appropriate medical regimen containing dual antiplatelet therapy (DAPT). Four days later, the patient presented with repeat acute coronary syndrome symptomatology. Electrocardiogram demonstrated ongoing STEMI in the previously treated artery distribution. Emergency angiography revealed restenosis and total thrombotic occlusion. Post-intervention stenosis was 0% after aspiration thrombectomy and bal

A 79-year-old man presented with impaired consciousness, fever with a body temperature of 41.1°C, and an electrocardiogram showing significant ST segment elevation in leads V1-6, coved-type ST elevation in leads V1-3, and partial right bundle branch block. Echocardiography revealed notable left ventricle dysfunction with apex-based akinesis. Coronary angiography confirmed severe obstructive lesions but left ventriculography displayed the distinct apical ballooning of takotsubo cardiomyopathy (TC). This case highlights the presence of atypical electrocardiographic patterns in TC influenced by c

Spontaneous coronary artery rupture (SCAR) is a rare, life-threatening disease, and the diagnosis is often challenging. We herein report a 70-year-old man who suffered sudden cardiac arrest due to SCAR with pericardial fluid. At first, emergent coronary angiography (CAG) failed to detect abnormalities. The emergent operation revealed that the presence of pericardial fluid was caused by bleeding that had spontaneously occurred at the left circumflex artery (LCx). A careful retrospective CAG review showed slight contrast spillage from the distal LCx. SCAR should be suspected in patients with unk

Acute inferior myocardial damage can induce transient bradycardia and hypotension-the Bezold-Jarisch reflex, which is explained by the preferential distribution of vagal nerves in the inferior wall of the left ventricle. We report a 76-year-old man who showed a perfusion defect in the inferior wall with redistribution on exercise scintigraphy with thallium-201. Of note, during exercise at an intensity of 100 watts, the patient's heart rate transiently decreased from 122 to 95 bpm in sinus rhythm, accompanied by ST-segment depression. A diagnosis of coronary spastic angina was made since no ste

We present the case of an 18-year-old man with aborted sudden cardiac death. His initial echocardiogram suggested an anomalous origin of the left coronary artery from the pulmonary artery. Diagnosis was confirmed with coronary angiography. He underwent Takeuchi procedure and fully recovered. A two-year follow-up echocardiogram showed a moderate supravalvular pulmonary stenosis related to the transpulmonary baffle. The presence of extensive collateral circulation should raise suspicion of ALCAPA. Postoperative surveillance in this group of patients needs to be oriented in finding complications

A 73-year old male developed syncope during a bicycle race. Exercise stress testing demonstrated non-sustained ventricular tachycardia (NSVT) and ischemic changes. Coronary angiography revealed a 99% occluded right coronary artery which was stented; repeat stress testing demonstrated normal perfusion and NSVT. An electrophysiology study demonstrated left posterior fascicular ventricular tachycardia, which was ablated at two lower turnaround points. NSVT was observed during subsequent stress testing, prompting a repeat electrophysiology study. The inferoseptum and inferior wall were extensively

Vasospastic coronary artery disease (CAD) usually occurs during the percutaneous interventions and responds to conventional medical treatment. However, in rare conditions, it may be resistant to medical treatment, resulting in lethal complications, including acute myocardial infarction, ventricular arrhythmia, cardiopulmonary arrest, cardiogenic shock, and acute pulmonary edema. In this case report, a 44-year-old woman was admitted to the hospital with a diagnosis of non-ST-segment elevation myocardial infarction. During a diagnostic coronary angiography and in-hospital stays, multiple catastr
