Premature rupture of the membranes: the enigma of the obstetrician.

Decisions in regard to the management of patients with premature rupture of the membranes depend on the relative risks of prematurity, infection, and hypoxia for the fetus or neonate and infection and cesarean section for the mother. These risks vary primarily with gestational age. Because of the many variables and new technologies involved, the management of these patients is extremely controversial. In this article, the controversies are reviewed and management plans are presented for several different gestational age groups.

Babesiosis in pregnancy.

Babesiosis is a malaria-like illness due to intraerythrocytic protozoan parasites. To the authors' knowledge, this unusual disease has not previously been described in a pregnant woman. Herein is reported the case of a gravid woman with an intact spleen who developed infection with Babesia microti in the fifth month of gestation. Her illness resolved following supportive care only, and evidence of transmission of disease to the fetus was not found.

Sonographic detection of polyhydramnios: a five-year experience.

Between 1975 and 1980, 16,586 sonograms, including repeat scans, were obtained on pregnant women for various obstetric indications. The diagnosis of polyhydramnios was made 78 times by an experienced sonographer as a simple observer judgment of an excess amount of amniotic fluid. In pregnancies thus diagnosed, the fetus was then carefully scanned for anomalies, and repeat scans were done. Major clinical problems encountered are discussed.

Problems in counseling the epileptic mother.

Epileptics, medical colleagues, and the public must be educated regarding the teratogenicity of anticonvulsants to the developing fetus. To prevent the serious problems of the fetal hydantoin-barbiturate syndrome, epileptic mothers must be given counseling. There are numerous problems encountered in counseling any individual with genetic disorders and dull intellect. More than the usual time and effort and cooperation of many agencies is needed to achieve success in counseling.

Diagnosis and treatment of advanced extra-uterine pregnancy.

Eighteen cases of advanced extra-uterine pregnancy are reported. The difficulty of diagnosis is discussed; the only symptoms of any help were amenorrhoea, lower abdominal pain, postmaturity and failed induction of labour. The physical findings were variable and none were absolutely reliable. In 17 cases the fetus was dead on admission and early laparotomy was performed, and in 17 patients it was possible to remove the placenta completely.

Peripheral Nerve Blockade for Trimalleolar Ankle Fracture Surgery in a Third-Trimester Pregnancy: A Case Report.

Nonobstetric surgery of pregnant patients is a challenge and concern for nonobstetric anesthesiologists. The approach to the pregnant patient for nonobstetric surgery must be multidisciplinary, including an obstetrician, an anesthesiologist, and a surgeon. We present the case of a 21-year-old female patient, in the 37th week of pregnancy, who sustained a trimalleolar fracture of the ankle joint, which required surgical treatment. The operation was performed under peripheral nerve blockade with moderate sedation. For the popliteal sciatic block, 15 mL of 0.5% levobupivacaine and 10 mL of 1.3% l

Considering the Developing Entity in an Artificial Womb as a Patient.

Artificial womb (AW) prototypes are currently being developed with the aim of improving the medical care of extremely premature infants. Despite the seemingly imminent reality of partial ectogenesis (i.e., gestation partially outside a human womb), there is persisting debate about the moral status of the fetus transferred in an AW-henceforth, the "developing entity." For some, AWs are simply another neonatal intensive cares' technology. Thus, developing entities in AWs should deserve the same protections as newborns in incubators. Others consider that AWs are fundamentally different technologi

Sonographic diagnosis of ruptured scar of previous cesarean section with secondary intra-abdominal pregnancy.

Ruptured scar of a previous caesarean section is an uncommon yet serious complication, frequently resulting in considerable morbidity and mortality. In certain instances, this rupture can lead to a secondary intra-abdominal pregnancy, a condition where the fetus is expelled from the uterine cavity following a scar rupture. The article details 5 cases that were treated in a tertiary care hospital observed over a period of 2 years from October 2022 to October 2024. The patients had previous history of cesarean deliveries and had clinical symptoms of vaginal bleeding and abdominal pain. Ultrasoun

Vertebral Bone Density Abnormalities in Fetal Ultrasound: A Distinctive Clinical Sign of Spondylocarpotarsal Synostosis Syndrome MYH3-Related.

BACKGROUND: Ultrasound diagnosis of fetal skeletal conditions remains challenging. MYH3 is a gene that encodes the embryonic myosin heavy chain; it is important for skeletal and muscular development and is strongly expressed during fetal development. Variants in MYH3 are involved in distal arthrogryposes 2A and 2B3 and in spondyocarpotarsal synostosis syndrome with contractures and pterygia, contractures of proximal and distal joints, variable spine anomalies and vertebral, carpal and tarsal fusions. KEY FINDINGS: We describe a case in which prenatal ultrasonography detected abnormal bone den

Evaluation of the health impacts and deregulation of signaling pathways in humans induced by microplastics.

This review assesses the diverse health risk factors associated with microplastic (MP) exposure and their impact on cellular signaling pathways. MPs induce chronic inflammation, oxidative stress, endocrine disruption, apoptosis, and immune dysregulation. They activate signaling pathways such as NF-κB, MAPK, and Nrf2, exacerbating inflammatory responses, oxidative damage, and hormonal imbalances. Understanding the interplay between MPs and signaling pathways is crucial for elucidating the mechanisms underlying MP-induced health effects. Effective risk assessment and management strategies are es

A case report of vaginal delivery in the second trimester of severe uterine prolapse complicated with cervical incarceration.

BACKGROUND: Uterine prolapse is a rare complication of pregnancy, and there is still no consensus on the choice of delivery method. METHODS: The patient's reproductive history included an abortion and eutocic delivery of a girl weighing 3200 g; the current pregnancy was the third pregnancy. Her cervical region was outside the vaginal opening and was red in color, with evident enlargement (6 × 4 cm) and a broken surface. The cervical area also showed white discharge. According to her Transvaginal ultrasonography revealed a fetus in the uterine cavity at approximately 19 weeks of gestation. Gyn

Fetal Metabolic Alkalosis Resulting from Maternal Vomiting.

We describe a pregnant patient with severe compulsive water ingestion and vomiting that lead to metabolic alkalosis and preterm delivery. A 21-year-old patient was hospitalized multiple times throughout pregnancy for symptoms initially thought to be related to hyperemesis gravidarum. Overtime, it became apparent that the patient induced vomiting by rapidly drinking large volumes of water. At 32 weeks' gestation, rapid ingestion of water caused 3 days of vomiting with findings of hyponatremia, hypokalemia, hypochloremia, metabolic alkalosis, and compensatory respiratory acidosis. Fetal monitori

Virome and metagenomic analysis reveal the distinct distribution of microbiota in human fetal gut during gestation.

Studies have shown that fetal immune cell activation may result from potential exposure to microbes, although the presence of microbes in fetus has been a controversial topic. Here, we combined metagenomic and virome techniques to investigate the presence of bacteria and viruses in fetal tissues (small intestine, cecum, and rectum). We found that the fetal gut is not a sterile environment and has a low abundance but metabolically rich microbiome. Specifically, Proteobacteria and Actinobacteria were the dominant bacteria phyla of fetal gut. In total, 700 species viruses were detected, and Human

Cerebral meningioma during pregnancy mimicking eclampsia: About a rare case.

INTRODUCTION: Pregnancy is a physiological state whose association with any pathology gives it specific characteristics in the management of patients. Meningioma is the most frequent brain tumor but rarely develops during pregnancy. Treatment of the tumor generally follows the same principle as that proposed outside pregnancy, with priority given to the mother's life. PRESENTATION OF CASE: We report a rare case of gestational meningioma that was wrongly attributed to prepartum eclampsia, in a poorly monitored pregnancy estimated at 34 weeks' amenorrhea, which resulted in the termination of th

New pathogenic variant in DLX5: New clues for a clinical spectrum from split-hand-foot malformation to fibular aplasia, tibial campomelia and oligosyndactyly.

Introduction: FATCO (Fibular Aplasia, Tibial Campomelia and Oligosyndactyly) is a very infrequent skeletal dysplasia classified within the limb hypoplasia-reduction defects group whose genetic cause has not yet been identified. The advent of next-generation sequencing is enabling the diagnosis of diseases with no previously known genetic cause. Methods: We performed a thorough autopsy on a fetus whose pregnancy was legally terminated due to severe malformations detected by ultrasound. A trio exome was run to identify the genetic cause and risk of recurrence. Previous literature of similar case

Polyhydramnios associated with congenital bilateral vocal cord paralysis: A case report.

RATIONALE: Polyhydramnios may develop when the fetus cannot swallow amniotic fluid or the amount of fetal urine increases. Occasionally, unpredictable fetal abnormalities can be diagnosed postnatally. Bilateral vocal cord paralysis in the fetus may cause polyhydramnios, which could be related to impaired prenatal swallowing. PATIENT CONCERN: A 36-year-old multipara underwent an emergent cesarean section because of polyhydramnios and active labor at 35 + 5 weeks of gestation and gave birth to a girl. DIAGNOSIS: The neonate cried feebly and exhibited cyanosis as well as very weak response to s

Fetal pharyngeal glial heterotopia manifested as polyhydramnios: a rare case with difficult prenatal diagnosis.

BACKGROUND: Glial heterotopia is a rare congenital developmental malformation that presents as tumor-like lesions of the nerve tissue that grow outside the nervous system, but are not true tumors. At present, most cases are reported in neonates and children and are very rarely found in fetuses. The present report describes a case of fetal pharyngeal glial heterotopia and associated imaging findings to better understand the disease in the future. CASE PRESENTATION: A 32-year-old pregnant woman was admitted to the hospital with polyhydramnios. An ultrasound examination revealed a hypoechoic mas
