IMPORTANCE: Emtricitabine and tenofovir disoproxil fumarate (F/TDF) for HIV preexposure prophylaxis (PrEP) is highly effective in cisgender men who have sex with men (MSM) when adherence is high (>4 doses/week). Real-world effectiveness and adherence with F/TDF for PrEP in cisgender women is less well characterized.
OBJECTIVE: To characterize the effectiveness of F/TDF for PrEP and its relationship with adherence in cisgender women.
DESIGN, SETTING, AND PARTICIPANTS: Data were pooled from 11 F/TDF PrEP postapproval studies conducted in 6 countries that included 6296 cisgender women aged 15 t

BACKGROUND: Bloodstream infection (BSI) caused by Klebsiella pneumoniae (KP), is a leading cause of hospital-associated childhood mortality. There are limited data on how poor outcomes of KPBSI can be predicted in poorly resourced areas. This study aimed to assess if the profile of differential counts from full blood counts (FBC) taken at two time points in children with KPBSI could be used to predict the risk of death.
METHODS: We conducted a retrospective study of a cohort of children admitted to hospital between 2006 and 2011 with KPBSI. FBC collected within 48 h (T1) of blood culture and

OBJECTIVES: Mobility (international/internal migration, and localized mobility) is a key driver of the HIV epidemic. While mobility is associated with higher-risk sexual behavior in women, a possible association with condom, modern contraceptive, and dual method use among women living with HIV (WLHIV), is unknown. In addition, HIV status and sexual behaviors such as relationship concurrency may also affect condom, modern contraceptive, and dual method use.
STUDY DESIGN: We surveyed sexually active women (N = 1067) aged 15 to 49 in 12 communities in Kenya and Uganda participating in a test-and

Exchanging money, drugs, and other goods for sex has been associated with sexual risk behaviors and increased STIs/HIV. While female sex work is well described, data on men who exchange sex for money or goods are more limited. This paper examined the prevalence and correlates of transactional sex among young men who have sex with men, especially focusing on substance use and HIV status. We conducted a cohort study of 511 participants recruited between August 2014 and December 2017 in Los Angeles, CA. Eligible participants were: (1) between 18 and 45 years of age; (2) male; and (3) if HIV-negat

IMPORTANCE: Patients with HIV infection are at increased risk for cancer. Cancer is the leading cause of death among non-AIDS-defining illnesses in these patients. Immune checkpoint inhibitor (ICI) therapy has transformed the treatment of cancer. However, clinical trials of ICIs have historically excluded patients with HIV infection. The safety and efficacy profile of ICIs is unknown in this underrepresented population.
OBJECTIVE: To summarize results on the safety and efficacy of ICI therapy in HIV-infected patients with advanced-stage cancer.
EVIDENCE REVIEW: This systematic review was con

BACKGROUND: Studies show that uptake of couples' HIV counseling and testing (couples' HCT) can be affected by individual, relationship, and socioeconomic factors. However, while couples' HCT uptake can also be affected by background HIV prevalence and awareness of the existence of couples' HCT services, this is yet to be documented. We explored the correlates of previous couples' HCT uptake among married individuals in a rural Ugandan district with differing HIV prevalence levels.
DESIGN: This was a cross-sectional study conducted among 2,135 married individuals resident in the three HIV prev

BACKGROUND: Infection with the human immunodeficiency virus (HIV) is associated both with infections of the central nervous system and with neurological deficits due to direct effects of the neurotropic virus. Seizures and epilepsy are not rare among HIV-infected patients. We investigated the frequency of acute seizures and epilepsy of patients in different stages of HIV infection. In addition, we compared the characteristics of patients who experienced provoked seizures only with those of patients who developed epilepsy.
METHODS: The database of the Department of Neurology, University of Mün

This report updates and combines into one document earlier versions of guidelines for preventing and treating opportunistic infections (OIs) among HIV-exposed and HIV-infected children, last published in 2002 and 2004, respectively. These guidelines are intended for use by clinicians and other health-care workers providing medical care for HIV-exposed and HIV-infected children in the United States. The guidelines discuss opportunistic pathogens that occur in the United States and one that might be acquired during international travel (i.e., malaria). Topic areas covered for each OI include a b

INTRODUCTION: Early identification of Primary HIV İnfection (PHI) is essential to reduce onward transmission and enable prompt initiation of Antiretroviral Therapy (ART). Despite increasing HIV diagnoses in Türkiye, data on the prevalence and characteristics of PHI remain limited. This study aimed to determine the proportion of PHI among newly diagnosed individuals and to describe and compare the sociodemographic and laboratory characteristics of individuals with PHI and CHI.
MATERIALS AND METHODS: This retrospective observational study included 225 individuals newly diagnosed with HIV at a t

BACKGROUND: Cervical cancer is the second leading malignancy in incidence among women in sub-Saharan Africa. Early detection through screening and treatment of pre-cancerous cervical lesions is paramount in preventing cervical cancer. We sought to examine factors associated with ever being screened for cervical cancer among women attending infant immunization clinics in Kenya.
METHODS: We conducted secondary data analysis from a cross-sectional survey that enrolled mother-infant pairs attending 55 infant immunization clinics in Kenya between 2018 and 2019. Clinics were selected from high and

INTRODUCTION: Optimizing HIV prevention may require structured approaches for providing client-centred choices as well as community-based entry points and delivery. We evaluated the effect of a dynamic choice model for HIV prevention, delivered by community health workers (CHWs) with clinician support, on the use of biomedical prevention among persons at risk of HIV in rural East Africa.
METHODS: We conducted a cluster randomized trial among persons (≥15 years) with current or anticipated HIV risk in 16 villages in Uganda and Kenya (SEARCH; NCT04810650). The intervention was a client-centred

BACKGROUND: Targeting immune checkpoints for HIV treatment potentially provides a double benefit resulting from the ability to restore viral-specific CD8+ T-cell functions and enhance HIV production from reservoir cells. Despite promising pre-clinical data, PD-1 blockade alone in HIV-1-infected patients with advanced cancer has shown limited benefits in controlling HIV, suggesting the need for additional targets beyond PD-1. CD39 and PD-1 are highly co-expressed on CD8+ T cells in HIV-1 infection. However, the characteristics of CD39 and PD-1 dual-positive CD8+ T-cell subsets in chronic HIV-1

BACKGROUND: African American/Black and Hispanic persons living with HIV (AABH-PLWH) in the U.S. evidence insufficient engagement in HIV care and low uptake of HIV antiretroviral therapy, leading to suboptimal clinical outcomes. The present qualitative study used critical race theory, and incorporated intersectionality theory, to understand AABH-PLWH's perspectives on the mechanisms by which structural racism; that is, the macro-level systems that reinforce inequities among racial/ethnic groups, influence health decisions and behaviors.
METHODS: Participants were adult AABH-PLWH in New York Ci

Despite effective suppression of viremia by antiretroviral therapy, HIV-1 persists in long-lived cellular reservoirs. Novel approaches aimed at eliminating these reservoirs are therefore essential for an HIV-1 cure. Among emerging cure strategies, harnessing antibody-dependent cellular cytotoxicity (ADCC) has generated significant interest. In this mGem, we discuss how small CD4-mimetic compounds (CD4mc), by forcing envelope (Env) into more "open" conformations, thereby exposing conserved CD4-induced epitopes, can unlock the ADCC potential of non-neutralizing antibodies. We also highlight how

We evaluated immune biomarker profiles in human immunodeficiency virus (HIV)-infected adults (n = 398) from 5 African countries. Although all biomarkers decreased after antiretroviral therapy (ART) initiation, levels of C-X-C chemokine ligand 10 (CXCL10), lipopolysaccharide-binding protein, C-reactive protein, soluble CD163, and soluble scavenger receptor CD14 were significantly higher during ART than in an HIV-uninfected reference group (n = 90), indicating persistent monocyte/macrophage activation, inflammation, and microbial translocation. Before ART initiation, high HIV viral load was asso

BACKGROUND: Clustered cases of acute hepatitis C virus (HCV) infection, frequently accompanied by sexually transmitted diseases, have recently been reported among men who have sex with men (MSM) in several European cities.
METHOD: We performed a retrospective record of cases of syphilis in a large cohort of HIV-infected individuals on regular follow-up in Madrid, Spain. HCV testing was carried out in all of them.
RESULTS: A total of 53 diagnosis of syphilis were made during the previous 4 years (2002 to 2005) in 729 HIV-infected MSM on regular follow-up. However, in only 2 of them (3.7%) asy

Tumour necrosis factor-alpha (TNF-alpha), interleukin (IL)-6, -8 and-10 and soluble TNF receptor II (sTNFR II) levels were measured at baseline, and after antifungal therapy for 2 weeks and 3 months, in plasma from 75 human immunodeficiency virus (HIV)-positive and 14 HIV-negative patients with cryptococcosis, and in plasma from 14 HIV-positive controls. At baseline, TNF-alpha, IL-6 and sTNFR II levels, and cryptococcal antigen titres, were increased in patients with fungaemia compared to controls (p < 0.02). The mediator levels were not influenced by the severity of the disease or subsequent

BACKGROUND: HIV self-testing (HIVST) can increase access to and uptake of HIV testing among people underserved by other HIV testing approaches. Several countries in Africa, the region most affected by HIV, have scaled-up HIVST. However, no comprehensive analysis has yet quantified HIVST uptake trends and how HIVST kits are used. We aimed to estimate 1) country-level and regional trends in HIVST uptake among adults by sex and age and 2) the proportion of distributed HIVST kits that are used and re-testing rates with HIVST.
METHODS AND FINDINGS: Across African countries, we analyzed 1) data fro
