Preemptive nalbuphine combined with subcostal transversus abdominis plane block for multimodal analgesia in laparoscopic liver resection: a randomized controlled trial of multimodal analgesia for laparoscopic hepatectomy.

BACKGROUND: Postoperative pain control is crucial for Laparoscopic Liver Resection (LLR) patients, as poor analgesia may hinder recovery and elevate complication risks. METHODS: 180 LLR patients were randomized into three groups (n = 60 group): Control (C), subcostal TAPB alone (TAPB), and Nalbuphine + subcostal TAPB (N+TAPB). Outcomes included NRS, BCS, Hemodynamics (MAP, HR), PCIA metrics, QoR-15 scores and complications. RESULTS: Across the postoperative time points, the NRS scores in Group N+TAPB were significantly lower than those in Group C and Group TAPB (p < 0.05), whereas BCS scores

Transthoracic single-port-assisted laparoscopic gastrectomy versus laparoscopic transhiatal approach for Siewert type II adenocarcinoma of the esophagogastric junction: a single-center retrospective study.

BACKGROUND: The surgical approach for patients with Siewert type II AEG remains controversial. Several studies have described a new laparoscopic radical resection approach of Siewert type II AEG through the left diaphragm. However, the technical safety and feasibility of the new surgical approach compared with the transhiatal approach have not yet been tested. STUDY DESIGN: We retrospectively reviewed patients with AEG who underwent TSLG and LTH operations in the Guangdong Provincial Hospital of Chinese Medicine between January 2017 and April 2021. Histologically confirmed AEG and D2 lymphade

Laparoscopic intragastric surgery and laparoscopic roux-y gastric bypass were performed simultaneously on a morbidly obese patient with a gastric submucosal tumor: a report of a case and review.

BACKGROUND: Obesity patients sometimes have gastric submucosal tumors (SMTs). Surgical excision is recommended in cases of SMTs with suspected malignancy or other reasons making follow-up difficult. Laparoscopic intragastric surgery (LIGS) has recently achieved good results in the treatment of gastric SMTs, especially near the esophagogastric junction (EGJ). This procedure has enabled either preservation of the stomach or minimization of the extent of partial resection. METHODS: The patient is a 43-year-old female with BMI of 48.2 kg/m(2) who is a candidate for LRYGB. A small SMT was found ju

The use of laparoscopy in gynaecology.

Diagnostic laparoscopy is of most value in the evaluation of infertility and for completing the gynaecological investigation of obscure cases of chronic pelvic pain. Its use to exclude ectopic pregnancy, salpingitis and pelvic cancer is more controversial. The management of some cases of primary amenorrhoea is aided by laparoscopic gonadal biopsy. The excessive demand for female sterilization will ensure that the laparoscope continues to be used for this purpose and is leading to a compromise. The surgical techniques which have been proved to be most effective are being replaced by methods fav

Laparoscopic ureterolithotomy and retrograde flexible ureteroscopy-assisted transperitoneal laparoscopic ureteroureterostomy for a huge ureteropelvic junction stone and multiple small renal stones: A CARE-compliant case report.

RATIONALE: Concurrent kidney and ureteral stones are always complicated and a clinical challenge. Improvements in endoscopic equipment have led to the widespread adoption of retrograde intrarenal surgery, which has a good stone clearance rate. On the other hand, laparoscopic ureterolithotomy (LUL) has been reported to be non-inferior to retrograde flexible ureteroscopy in stone-free rate and the need for axillary procedures, and to have a significantly lower rate of post-operative sepsis compared to retrograde flexible ureteroscopy. We describe a case managed with LUL followed by laparoscope-a

Preperitoneal analgesia under direct visualization: laparoscopic-guided deep rectus sheath block improves postoperative pain control after laparoscopic cholecystectomy: a randomized controlled trial.

BACKGROUND: Conventional abdominal wall blocks primarily target somatic afferent nerves and may therefore provide limited relief for the peritoneal component of postoperative pain. This study aimed to investigate whether a laparoscopic-guided deep rectus sheath (preperitoneal) block (RSB) could reduce postoperative opioid consumption and improve postoperative pain in patients undergoing laparoscopic cholecystectomy. METHODS: Patients were randomly assigned to either the control group or the deep RSB group. In the deep RSB group, the block was performed intraoperatively under direct laparoscop
