Trigone ventricular meningiomas: surgical approaches.

OBJECTIVE: Report our experience with trigone ventricular meningiomas and review the surgical approaches to the trigone. METHOD: From 1989 to 2006, six patients with meningiomas of the trigone of the lateral ventricles underwent microsurgical resection. Their clinical features, image, follow up, and surgical approaches were retrospectively analyzed. RESULTS: Five patients presented with large and one with small volume meningioma. Unspecific symptoms occurred in three patients; intracranial hypertension detected in three patients; homonymous hemianopsy in three; and motor deficit present in o

Thoracic findings of systemic diseases at high-resolution CT in children.

Pulmonary involvement in systemic diseases is common, but the radiographic appearance of early-stage pulmonary changes is often subtle. Computed tomography (CT) has higher sensitivity and specificity than radiography, and high-resolution CT is the method of choice for accurate assessment of diffuse parenchymal lung disease. Even with reductions in the peak voltage and tube charge to minimize the exposure of pediatric patients to radiation, CT performed with a meticulous acquisition technique can provide detailed information. In some cases, high-resolution CT may depict clinically silent lung l

Role of Selective Management of Penetrating Injuries in Mass Casualty Incidents.

Terrorist violence has emerged as an increasingly common cause of mass casualty incidents (MCI) due to the sequelae of explosive devices and shooting massacres. A proper emergency medical system disaster plan for dealing with an MCI is of paramount importance to salvage lives. Because the number of casualties following a MCI is likely to exceed the medical resources of the receiving health care facilities, patients must be appropriately sorted to establish treatment priorities. By necessity, clinical signs are likely to prove cornerstones of triage during MCI. An appropriate and effective appl

[The first gender-changing operation from female to male in The Netherlands, 1959/'60].

The first sex-change operation in a transsexual patient, i.e. the construction of a penis (so-called phalloplasty), took place in the Netherlands in Arnhem in 1959-1960. The operative technique had already been described in 1938 and was perfected after World War II. The discussion of this controversial procedure during a staff meeting in Arnhem Municipal Hospital was published at that time in the Nederlands Tijdschrift voor Geneeskunde [Dutch Journal of Medicine] and this resulted in a large number of protesting letters to the editor. Although the operation was carefully planned and proved to

[New vaccines and new veterinary therapies derived from biotechnologies: examples of applications].

The last ten years have seen the development of vaccines derived from deoxyribonucleic acid (DNA) recombination, which, when used in association with appropriate diagnostic kits, make it possible to distinguish vaccinated from infected animals. This article describes the general principles behind these vaccines, provides examples of different applications, e.g. vaccines against Aujeszky's disease, classical swine fever, rabies, avian diseases and rinderpest, and discusses the role of this type of vaccine in certain control plans. Deoxyribonucleic acid vaccines constitute a revolution in the co

Long-term care of the patient with a tracheostomy.

An increasing number of technology-dependent patients are sent home for long-term home-management of stable chronic illness. With a patient who is going to undergo tracheotomy, patient-education (for the patient and his/her caregivers) should begin early (before the tracheostomy, if possible), should be individualized to the patient, and should include basic airway anatomy, medical justification for the tracheostomy, tube description and operation, signs and symptoms of respiratory and upper-airway distress, signs and symptoms of aspiration, suctioning technique, tracheostomy tube-cleaning and

'Thinking for two': a case study of speech and language therapists working through assistants.

BACKGROUND: Many speech and language therapists (SLTs) in the UK work with speech and language therapy assistants, and the numbers of SLT assistants is expected to grow. There has been very little empirical investigation of how SLTs feel about this situation or the effect on working practices of working indirectly. AIMS: To investigate SLTs' opinions on working with SLT assistants in a small-scale research case study. METHODS AND PROCEDURES: The study design was a case study of five SLTs delivering intervention within a research intervention project. Prepared questionnaire and formal intervi

Management of subclavian and axillary artery injuries: spanning the range of current therapy.

Injuries of the subclavian and proximal axillary arteries are potentially devastating but account for a minority of vascular injuries presenting to trauma centers in the United States. We have reviewed our recent experience with management of subclavian and axillary artery injuries in a state-designated level 1 academic trauma center and report four cases that illustrate the typical arterial injury patterns and the entire therapeutic armamentarium in its current iteration. Subclavian and proximal axillary artery injuries present as interesting surgical problems. A high index of suspicion for v

Congenital scoliosis.

Congenital scoliosis is caused by early embryologic errors in vertebral column formation. Defining the deformity, predicting the natural history, and applying the correct treatment can help ensure successful management. Most congenital spine anomalies can be classified, and many have a predictable natural history. Because the deformities are associated with other organ system anomalies in more than half of patients, the surgeon should look for cardiac, auditory, genitourinary, and renal anomalies. Intraspinal abnormalities are present in approximately one third of patients with congenital spin

Three-dimensional CT imaging of flexor tendon ruptures in the hand and wrist.

OBJECTIVE: The purpose of this study is to determine the applicability of 3D CT imaging in the evaluation of flexor tendon rupture in the hand and wrist. DESIGN: Twenty-four digits in 22 patients (18 adults, 4 children) with a spectrum of finger flexion disturbances were investigated by a multidetector-row CT scanner, and diagnosed by 3D CT image with the volume rendering technique. The accuracy of the image diagnosis was confirmed operatively in 20 digits. RESULTS: 3D CT imaging gave a precise analysis in all adult cases. It clearly depicted the location of rupture and tendon stump. All of

Myelomeningocele: prenatal diagnosis, pathophysiology and management.

Myelomeningocele (MMC) is a common birth defect that is associated with significant lifelong morbidity. Little progress has been made in the postnatal surgical management of the child with spina bifida. Postnatal surgery is aimed at covering the exposed spinal cord, preventing infection, and treating hydrocephalus with a ventricular shunt. In utero repair of open spina bifida is now performed in selected patients and presents an additional therapeutic alternative for expectant mothers carrying a fetus with MMC. Early fetal intervention may improve neurologic outcome and reduce the hindbrain he

Reference values of facial features in Scandinavian children measured with a range-camera technique.

Reference values of facial features in white Scandinavian children were ascertained using a newly developed range-camera technique with a three-dimensional measuring program specifically designed for anthropometric measurements. Seven facial features in 613 healthy subjects (322 girls, 291 boys), aged 1 month to 18 years, and nine subjects with the fetal alcohol syndrome, aged 7 to 18 years, were studied. Data were analysed by multivariate multiple regression and measurements of each facial feature plotted against age and presented as curves, with 95% and 99% univariate prediction limits. All c

Histopathology of congenital subglottic stenosis.

A precise delineation of each laryngeal abnormality is critical to planning rational therapy. Since congenital cricoid cartilage anomalies are easily overlooked, a systematic approach to their identification is as important as a clear concept of their classification. The purpose of this communication is to present the pathologic findings and classification of congenital cricoid cartilage deformities and to summarize updated information derived by the whole organ macrosection technique. The classification of congenital subglottic stenosis is based on study of the English-language literature and

Helical CT and 3D reconstruction of ureteropelvic junction obstruction: accuracy in detection of crossing vessels.

PURPOSE: The purpose of our study was to evaluate the accuracy of 3D helical CT and its value in surgical planning for the treatment of ureteropelvic junction obstruction (UPJO). METHOD: CT scans and 3D reconstruction of renal vessels and the renal pelvis were performed in 20 cases of UPJO treated by open surgery. We analyzed scans in search of a vessel at the junction and its position relative to the UPJ. Helical CT scans and 3D reconstructions were correlated with macroscopic surgical findings. RESULTS: CT scans demonstrated the presence of a UPJO in all cases. Crossing vessels were presen

[Fates of bone defects in the mastoid cavity studied from the findings of two-stage tympanoplasty].

The incidence and prognosis of bone defects occurring in the mastoid cavity were investigated in 205 ears of 197 patients who underwent planned two-stage tympanoplasty by the intact canal wall technique. Bone defects found at the first stage had spontaneously closed by the second stage in 43 of 62 ears at the mastoid tegmen, in 13 of 15 ears at the posterior wall of the mastoid cavity and in 2 of 17 ears at the posterior wall of the external ear canal. Twenty two ears developed bone defects at the ear canal between the first and second operations, despite the canal wall having been primarily p

Management of post-traumatic temporomandibular joint ankylosis in children: case report.

Temporomandibular joint (TMJ) ankylosis is a distressing affliction that denies the victim the benefit of normal diet, careers that require normal speech and causes severe facial disfigurement that aggravates psychological stress. Opinions in current literature portray controversy in its management. Hurried clinicians take shortcuts and partially treat symptoms and often make the condition worse. Others ignore shortcomings and erroneously prescribe their preferred techniques as absolute indication to the peril of the affected children. Gap arthroplasty if applied in a growing maxillofacial ske

Frameless method of stereotaxic localization with DSA.

PURPOSE: To improve a method of stereotaxic localization with digital subtraction angiography that does not require use of a localization frame fixed to the patient's skull during examination. MATERIALS AND METHODS: An independent, low-cost, stereotaxic computing system was devised. Software programs used magnetic resonance images, computed tomographic scans, and digitized radiographs of plastic bone implants with head landmarks to establish reference trihedrons in each imaging system, transpose target coordinates from one system to another, adjust surgical instruments, help plan radiation su

Infertility in women: diagnostic evaluation with hysterosalpingography and other imaging techniques.

This article examines the current roles of hysterosalpingography and other imaging studies used in diagnostic assessment and treatment of infertility in women. The advantages and limitations of hysterosalpingography, transabdominal or transvaginal sonography, and MR imaging in detecting abnormalities of the uterus (congenital anomalies, intrauterine adhesions, myometrial and endometrial derangements) and tubal and peritubal disease are presented. The use of sonography to evaluate cyclical endometrial changes, to assess development of the follicle and corpus luteum, to determine whether ovulati

Surgical strategies in the treatment of pancreatic necrosis and infection.

Controversy still surrounds the management of necrotic and septic complications of acute pancreatitis. A review of the literature of the past decade dealing with the surgical treatment of pancreatic necrosis, pancreatic abscess and infected pancreatic necrosis has been undertaken. Three main patterns of management could be identified: (1) 'conventional treatment', consisting of pancreatic resection or necrosectomy with drainage; (2) 'local lavage', consisting of necrosectomy followed by regional lavage; and (3) 'open management', with resection or necrosectomy followed by planned multiple re-e

Computed tomographic staging of gastrointestinal tract malignancies. Part I. Esophagus and stomach.

The utility and limitations of CT staging of carcinomas of the esophagus, stomach, small bowel, and colon are reviewed. Routine preoperative staging of esophageal carcinoma is recommended because of CT's sensitivity of more than 90% in detecting mediastinal invasion and a specificity of more than 85% in collected series. We recommend the use of CT as a problem-solving technique rather than as a routine preoperative test in patients with gastroesophageal junction, and gastric and small bowel malignancies. Routine use of CT to stage colorectal carcinomas also does not seem justifiable, due to th
