[Depression: diagnosis, treatment and course].

Depressions are very common mental diseases. The diagnosis is made by psychopathology criteria and the course of the disorder. There is a growing body to the knowledge about etiology and treatment of depression - neuro-imaging, neuroplasticity, CREB and BDNF are all modified by antidepressants treatment and/or psychotherapy. As differential diagnoses, depressive syndromes on the basis of organic diseases but also burn-out syndromes may play a role, different therapeutic efforts are necessary. The treatment of depression consists of psychopharmacological and psychotherapeutic strategies, combin

βIIPKC and εPKC isozymes as potential pharmacological targets in cardiac hypertrophy and heart failure.

Cardiac hypertrophy is a complex adaptive response to mechanical and neurohumoral stimuli and under continual stressor, it contributes to maladaptive responses, heart failure and death. Protein kinase C (PKC) and several other kinases play a role in the maladaptative cardiac responses, including cardiomyocyte hypertrophy, myocardial fibrosis and inflammation. Identifying specific therapies that regulate these kinases is a major focus of current research. PKC, a family of serine/threonine kinases, has emerged as potential mediators of hypertrophic stimuli associated with neurohumoral hyperactiv

[Lysosomal storage diseases].

Lysosomal storage diseases are a heterogeneous group of disorders caused by lysosomal enzyme dysfunction. Individually they are very rare, but this group as a whole has a prevalence of more than 1:8,000 live births. While severe phenotypes are easily diagnosed this can be a real challenge with attenuated forms. Because musculoskeletal complaints are frequently the first reason for the patient to seek medical advice, the rheumatologist plays a key role for the early recognition of these diseases. Since several of these can be treated very effectively by enzyme replacement, a timely diagnosis an

Cytoreductive surgery (cs) and hyperthermic intraperitoneal chemotherapy (hipec) in treatment of peritoneal surface malignances: report of a phase II clinical study.

Peritoneal surface malignancy is the expression of a spectrum of disease involving the peritoneum primary or secondary to gastrointestinal and gynecological neoplasms. Even if intraperitoneal therapy has now been demonstrated in multiple randomized trials to improve the outcome of chemotherapy for patients with optimally debulked or small volume ovarian carcinoma, it is believed that peritoneal carcinomatosis is considered an advanced stage of disease; for this reason, it is treated with systemic chemotherapy and surgery plays only a palliative role (1). In the last twenty years, some centres

[Prevention of stroke: what is evidence based?].

On the background of aging population an increase of patients suffering from stroke has to be expected during the next years and decades. Strokes are the 3rd leading cause of death and reason for disability in Germany and many other countries. An effective treatment of the modifiable risk factors hypertension, hyperlipidemia, diabetes mellitus, atrial fibrillation, smoking, alcohol consumption, and adipositas is of enormous impact. Due to the linear relationship between blood pressure and risk of stroke, the treatment of hypertension is the most important and effective one in primary as well a

Applications of Two-Dimensional Electrophoresis Technology to the Study of Atherosclerosis.

Atherosclerosis is a multifactorial disease in which hypertension, diabetes, hyperlipidemia and other risk factors are thought to play a role. However, the molecular processes underlying plaque formation and progression are not yet completely known. In the last years some researchers applied proteomics technologies for the comprehension of biochemical pathways of atherogenesis and to search new cardiovascular biomarkers to be utilized either as early diagnostic traits or as targets for new drug therapies. Due to its intrinsic complexity, the problem has been approached by different strategies,

Sleep disturbance in bipolar disorder: therapeutic implications.

In this review, the authors detail our current understanding of the crucial role that sleep and its disturbances play in bipolar disorder. Multiple lines of evidence suggest that impaired sleep can induce and predict manic episodes. Similarly, treatment of sleep disturbance may serve as both a target of treatment and a measure of response in mania. The depressive phase of bipolar illness is marked by sleep disturbance that may be amenable to somatic therapies that target sleep and circadian rhythms. Residual insomnia in the euthymic period may represent a vulnerability to affective relapse in

The value of diffusion-weighted imaging in the diagnosis of Marchiafava-Bignami disease: apropos of a case.

Marchiafava-Bignami disease (MBD) is characterized by demyelination and necrosis of corpus callosum encountered in chronic alcoholic patients. Etiology is the deficiency of vitamin B complex. Magnetic resonance imaging (MRI) in MBD typically reveals focal lesions of high T2 and FLAIR signal intensity in the corpus callosum. We here present a 42-year-old male alcoholic diagnosed as MBD on the basis of MRI and diffusion-weighted imaging (DWI) features. The patient totally recovered following appropriate vitamin B complex replacement therapy, despite reduced diffusion in the initial setting. This

The aldosterone receptor--new insights?

Aldosterone, or mineralocorticoid, excess plays an important role in the pathophysiology of cardiac disease. This response is mediated through the mineralocorticoid receptor (MR). Although blockade of the MR has been shown to reduce mortality in cardiac failure, therapy may be limited by the hyperkalaemia that results from blockade of the renal MR. It is now well established for a number of steroid receptors that tissue-specific antagonism and agonism can be achieved by the so-called selective receptor modulators. Recently, three groups have crystallised the ligand-binding pocket of the MR, an

Biologic poisons for pain.

Pain therapies from natural sources date back thousands of years to the use of plant and animal extracts for a variety of painful conditions and injuries. We certainly are all familiar with modern uses of plant-derived analgesic compounds such as opium derivatives from papaverum somniferum and salicylates from willow bark (Salix species). Local anesthetics were isolated from coca leaves in the late 1800s. Sarapin, derived from carnivorous pitcher plants, has been injected for regional analgesia in human and veterinary medicine, but efficacy is controversial. Biologic organisms can play importa

Nutrition in Crohn disease.

Nutrition plays an important role in the pathogenesis, treatment, and morbidity of Crohn disease. Approximately two thirds to three fourths of hospitalized patients with active disease and one fourth of outpatients with Crohn disease are malnourished. Malnutrition, which can be present even when Crohn disease is in remission, can affect growth, cellular and humoral immunity, bone density, and wound healing. Decreased nutrient intake, malabsorption, drug-nutrient interactions, anorexia, and protein-losing enteropathy can all contribute to the protein-calorie malnutrition and other specific nutr

Successful treatment of severe orthostatic hypotension with erythropoietin.

A 71-year-old man, who was diagnosed with familial amyloidosis type I, was admitted for treatment of severe orthostatic hypotension associated with recurrent syncopal attacks. Head-up tilt testing demonstrated severe orthostatic hypotension (114/72 mmHg in the supine position and 62/34 mmHg in the upright position) with syncope or presyncope. Oral midodorine and fludrocortisone therapies failed to prevent his symptoms. After administration of subcutaneous erythropoietin, his blood pressure drop in the upright position was decreased and symptoms disappeared unassociated with improvement of anem

Oxidative damage to nucleic acids in human prion disease.

Recently, several studies proposed a physiological role for the cellular prion protein (PrP(c)) in defense against oxidative stress. Since the pathogenesis of prion disease necessarily involves a disturbance of PrP(c) homeostasis, we hypothesized that such diseases would be associated with concomitant disturbances in oxidative balance. In support of such a notion, in this study we show increased oxidative damage to nucleic acids in affected brains of patients with Creutzfeldt-Jakob disease. These data suggest that damage by free radicals is a likely cause for neurodegeneration in human prion d

The authors, o the basis of a retrospective analysis of a series of 13 cases of chronic periaortitis, after a review of the literature about the subject, discuss about the most recent theories about pathogenesis, pathological anatomy, diagnosis and treatment of this condition. At first the authors stress the importance of adopting the term chronic periaortitis to describe any idiopathic fibrotic retroperitoneal process, as a consequence of the well established relationship between the atherosclerotic abdominal aorta an the development of this condition. Using the KTP/532 laser to control epistaxis in patients with hereditary hemorrhagic telangiectasia.

Osler-Weber-Rendu disease remains a challenging clinical management problem. We have found the potassium-titanyl-phosphate laser (KTP/532) for photocoagulation of intranasal telangiectases to be useful in controlling epistaxis refractory to traditional surgical therapy. We present the case of a patient who had Osler-Weber-Rendu disease previously treated with partial septectomy, turbinectomy, and intranasal dermoplasty. A technique using the KTP laser for photocoagulation of intranasal telangiectases is described. This technique has resulted in reducing the frequency of epistaxis, decreasing t

Spinal tuberculosis with circumferential involvement of two noncontiguous isolated vertebral levels: case report.

We present a case of a 16-year-old boy with an atypical form of spinal tuberculosis, which circumferentially involved two noncontiguous vertebral levels without destruction of the adjacent vertebral bodies and intervertebral discs. The lesions caused paraplegia and loss of sphincter control, and they were confined to a single vertebra at each site; the findings more closely mimicked spinal malignancies. There was no evidence of pulmonary or other extrapulmonary tuberculous disease. The patient was successfully treated both surgically and medically using posterior decompression and a stabilizin

Physiology of endocrine skin interrelations.

Hormones influence the skin and play a role in normal biologic processes. Keratinocytes can convert and synthesize endocrine hormones. Endocrine dysregulation of the skin and abnormalities of endocrine functions of keratinocytes may produce abnormal changes in the skin. Knowledge of the cutaneous metabolism of thyroid hormones, steroids, peptide hormones, and vitamin A derivatives is being rapidly updated. Skin manifestations of endocrine disorders result from imbalance in feedback loops maintaining endocrine homeostasis. Define molecular mechanisms of hormonal action on target cells underlie

[Application of a paste composed of sodium bicarbonate, sodium chloride and hydrogen peroxide in treating of active periodontal pockets].

Close relationship existing between dentobacterial plaque and chronic inflammatory periodontal disease, as well as effectiveness of conservative treatments used to eliminate chronic gingivitis, is wordly known. However, several years ago, chronic destructive periodontal disease was only solved by surgical techniques, most of them bloody and of doubtful solution for some periodontal problems; for such reasons, a study was carried out in order to learn about the role played by a medicament formed with sodium bicarbonate, sodium chloride and hydrogen peroxide, deeply applied as a paste into activ

Pacing techniques in the treatment of tachycardias.

Pacemakers are playing an increasing role in the management of patients with tachycardia. Pacing to prevent tachycardia must be differentiated from pacing to terminate tachycardia. Pacing is of proven value in preventing ventricular tachycardia that occurs in the setting of complete heart block or the setting of drug-induced QT interval prolongation and may be valuable in some other patients with nonsustained ventricular tachycardia. Pacing termination has found its greatest use as an alternative to cardioversion in patients with atrial flutter, especially those with suspected digitalis toxici

Mitochondrial Dysfunction in Parkinson's Disease: Diagnostic and Therapeutic Advances.

Parkinson's Disease (PD) is a neurodegenerative disorder in which mitochondrial dysfunction plays a central role in pathogenesis. This review summarizes key mitochondrial abnormalities involved in PD, including respiratory chain impairment, dysregulation of mitochondrial dynamics, defective mitophagy, and mitochondrial DNA (mtDNA) damage. It further examines how these processes interact with α-synuclein (α-syn) pathology, contributing to increased vulnerability of dopaminergic neurons. In addition, current diagnostic approaches are transitioning from isolated biomarkers to an integrated Mitoch
